Background A pneumatic tourniquet is generally used to achieve a bloodless operation field in hand surgery. However, this has changed with tumescent solution-based wide-awake surgery. This study is a preliminary prospective case series study to elaborate the formula and indications of the tumescent technique in hand surgery without a tourniquet. Methods Seven patients (age range, 4 months to 37 years) underwent hand or upper extremity surgery for conditions such as nerve palsy, electric burn defect, fingertip injury, contracture, constriction ring syndrome, or acrosyndactyly. A "one-per-mil" tumescent solution (epinephrine 1:1,000,000+20 mg lidocaine/50 mL saline) was used to create a bloodless operating field without a tourniquet. Observation was performed to document the amount of solution injected, the operation field clarity, and the postoperative pain. Results The "one per mil" epinephrine solution showed an effective hemostatic effect. The tumescent technique resulted in an almost bloodless operation field in the tendon and in the constriction ring syndrome surgeries, minimal bleeding in the flap and contracture release surgeries, and acceptable bleeding in acrosyndactyly surgery. The amount of solution injected ranged from 5.3 to 60 mL. No patient expressed significant postoperative pain. Flap surgeries showed mixed results. One flap was lost, while the others survived. Conclusions Epinephrine 1:1,000,000 in saline solution is a potential replacement for a tourniquet in hand surgery. Further studies are needed to delineate its safety for flap survival.
To investigate the response of egg production to the artificial light to induce early egg laying. and the changes of egg weight and egg shape during the egg laying cycle, 30 Korean ring-necked pullets and their 1,284 eggs layed under artificial light control were used in this study. The first egg was ohserved at the age of 31 wk, after 4 wk of stimulating light(16 hour light: 8 hour dark). Egg production rate during seven 2-wk periods from the begining of the first egg was 43.7% and a clear peak egg production(61.7%) was shown at the 4th 2-wk period. The effects of egg production period and pullet on the egg weight, egg length, egg width and egg shape index(width/length) were significant(P<0.01). In the egg production cycle. egg weight, egg length and egg width increased steadly with time. but the value of egg shape index increased up to the 3rd 2-wk period and then decreased. Repeatabilities from intraclass correlation for the egg weight, egg length. egg width and egg shape index were 0.61. 0.53, 0.49 and 0.48, respectively.

Recently, ultra high performance fiber reinforced concrete (UHPFRC) having over 180 MPa compressive strength and 10 MPa tensile strength has been developed in Korea. However, UHPFRC represents different material properties with normal concrete (NC) and conventional high performance concrete (HPC) such as a high early age autogenous shrinkage and a rapid dry on the surface, because it has a low water-binder ratio and high fineness admixtures without coarse aggregate. In this study, therefore, to propose suitable experimental methods and regulations, and to evaluate mechanical properties at a very early age for UHPFRC, setting, shrinkage and tensile tests were performed. From the setting test results, paraffin oil was an appropriate material to prevent drying effect on the surface, because if paraffin oil is applied on the surface, it can efficiently prevent the drying effect and does not disturb or catalyze the hydration of cement. From the ring-test results, it was defined that the shrinkage stress is generated at the time when the graph tendency of temperature and strain of inner steel ring is changed. By comparing with setting test result, the shrinkage stress was firstly occurred as the penetration resistance of 1.5 MPa was obtained, and it was about 0.6 and 2.1 hour faster than those of initial and final sets. So, the starting time of autogenous shrinkage measurement (time-zero) of UHPFRC was determined when the penetration resistance of 1.5 MPa was obtained. Finally, the tensile strength and elastic modulus of UHPFRC were measured from near initial setting time by using a very early age tensile test apparatus, and the prediction models for tensile strength and elastic modulus were proposed.

The age and growth of Gomphina (Macridiscus) veneriformis sampled from February 2006 to January 2007 on the Wonsan coast of East Sea were determined from 3,899 shell. Age of Gomphina (Macridiscus) veneriformis was estimated from the ring on the surface of shell. The relationship between shell length (SL) and shell height (SH) was expressed by the following equation : SH = 0.7405 SL + 0.2227 ($R^2$ = 0.9671), and shell length (SL) and shell width (SW) was highly correlated by the equation : SW = 0.3657 SL + 0.4253 ($R^2$ = 0.922). The relationship between shell length (SL) and total weight (TW) was also expressed by the following equation : $TW=0.4274{\times}10^{-3}{\times}SL^{2.7876}$ ($R^2$ = 0.9591). The main spawning periods was estimated August through fatness index analysis. Based on the monthly variations in the marginal index (MI) of the shell, it is assumed that the ring of this species was formed once a year during the period of July. Herniorrhaphy of Indirect inguinal hernia (IIH) is one of the most frequently performed surgical procedures in children. The overall incidence of inguinal hernias in childhood ranges from 0.8 to 4.4 %. The incidence is up to 10 times higher in boys than girls, especially much higher in premature infants. IIHs in children are basically an arrest of embryologic development rather than an acquired weakness, which explains the increased incidence in premature infants. In normal development, the processus vaginalis closes, obliterating the peritoneal opening of the internal ring between 36th and 40th week of gestation. This process is often incomplete, leaving a small patent processus in many newborns. However, closure continues postnatally, and the rate of patency is inversely proportional to age of the child. The presence of a patent processus vaginalis is a necessary but not sufficient variable in developing a congenital IIH. In other words, all congenital IIHs are preceded by a patent processus vaginalis, but not all patent processus vaginalis go on to become IIHs. The overall incidence of IIH in population is approximately 1 to 2 % and the incidence of a processus vaginalis is approximately 12 to 14%, clinically appreciable IIH should develop in approximately 8 to 12 % of patients with a patent processus vaginalis. Although the classic open inguinal hernia repair remains the gold standard for most pediatric surgeons, laparoscopic repair is being performed in many centers. Like open technique, laparoscopic technique is fundamentally a high ligation of the indirect hernia sac with or without internal ring ligation. The advantages of laparoscopic approach include the ease of examining the contralateral internal ring, the avoidance of access damage to vas and vessels during mobilization of cord, decreased operative time, and an ability to identify unsuspected direct or femoral hernias. Almost all groin hernias in children are IIHs and occur as a result of incomplete closure of processus vaginalis. The treatment is repair by high ligation of hernia sac, which can be done by an open or laparoscopic technique. The contralateral side can be explored by laparoscopy or left alone, open exploration is no longer indicated due to potential risk of infertility.
Purpose: The purpose of this study was to determine the essential factors for prompt arrangement of angio-embolization in patients with pelvic ring fractures. Methods: A total of 62 patients with pelvic ring fractures who underwent angio-embolization in Dankook University Hospital from March 2013 to June 2018 were retrospectively reviewed. There were 38 men and 24 women with a mean age of 59.8 years. The types of pelvic ring fractures were categorized according to the Tile classification. Patient variables included sex, initial hemoglobin concentration, initial systolic blood pressure, transfused packed red blood cells within 24 hours, Injury Severity Score (ISS), mortality rate, length of hospital stay, and time to angio-embolization. Results: The most common pelvic fracture pattern was Tile type B (n=34, 54.8%). The mean ISS was $27.3{\pm}10.9$ with 50% having an $ISS{\geq}25$. The mean time to angio-embolization from arrival was $173.6{\pm}89minutes$. Type B ($180.1{\pm}72.3minutes$) and type C fractures ($174.7{\pm}91.3minutes$) required more time to angio-embolization than type A fractures ($156.6{\pm}123minutes$). True arterial bleeding was identified in types A (35.7%), B (64.7%), and C (71.4%). Conclusions: It is important to save time to reach the angio-embolization room in treating patients with pelvic bone fractures. Trauma surgeons need to consider prompt arrangement of angio-embolization when encountering Tile type B or C pelvic fractures due to the high risk of true arterial bleeding.

Objective: To assess the effects of iterative model reconstruction (IMR) on image quality for demonstrating non-calcific high-risk plaque characteristics of coronary arteries. Materials and Methods: This study included 66 patients (53 men and 13 women; aged 39-76 years; mean age, 55 ± 13 years) having single-vessel disease with predominantly non-calcified plaques evaluated using prospective electrocardiogram-gated 256-slice CT angiography. Paired image sets were created using two types of reconstruction: hybrid iterative reconstruction (HIR) and IMR. Plaque characteristics were compared using the two algorithms. The signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the images and the CNR between the plaque and adjacent adipose tissue were also compared between the two reformatted methods. Results: Seventy-seven predominantly non-calcified plaques were detected. Forty plaques showed napkin-ring sign with the IMR reformatted method, while nineteen plaques demonstrated napkin-ring sign with HIR. There was no statistically significant difference in the presentation of positive remodeling, low attenuation plaque, and spotty calcification between the HIR and IMR reconstructed methods (all p > 0.5); however, there was a statistically significant difference in the ability to discern the napkin-ring sign between the two algorithms (χ2 = 12.12, p < 0.001). The image noise of IMR was lower than that of HIR (10 ± 2 HU versus 12 ± 2 HU; p < 0.01), and the SNR and CNR of the images and the CNR between plaques and surrounding adipose tissues on IMR were better than those on HIR (p < 0.01). Conclusion: IMR can significantly improve image quality compared with HIR for the demonstration of coronary artery and atherosclerotic plaques using a 256-slice CT.
Ecological study of the Cynoglossus joyneri was conducted based on the specimens from the western coast of Korea from 1992 to 1994. Ecological characteristics of this species such as gonadal development, occurrence of larvae, stomach contents, and environmental conditions were investigated. Few larvae of Cynoglossus joyneri occurred in Kunsan coast. This coast is influenced by the freshwater input of the Mangyong and Kum River, especially during the summer. Therefore, the larvae of C. joyneri were considered to be spawned and grown during their early life stage in the high saline water in outer bay. Spawning occur from June to September, having the peak spawning period from July to August. Biological minimum size of matured fishes containing mature oocytes is about 143.5mm in total length, which requires about 20 months after hatching. The ranges of total length by age are as follows : 0-ring group(floating stage), 30~70mm ; 1-ring group, 49.0~133.0mm 2-ring group, 128.6~167.0mm ; 3-ring group, 169.0~202.0mm ; 4-ring group, 200.7~240.0mm. The major food items of young fish under 8cm were copepods and invertebrate larvae, and those of adult fishes were annelids(polychaets) in winter, and bivalves and shrimps in summer and autumn. Therefore, it can be assumed that the population of C. joyneri could be reduced due to the decrease of food organisms caused by the continuous reclamation activities in the inner bays of the west coast of Korea.

The determination of peripheral nerve conduction velocity is an important part to electrodiagnosis. Its value as neurophysiologic investigative procedure has been known for many years but normal value of digital nerve was not reported in Korea. To evaluate of digital nerve conduction velocity of median nerve for obtain clinically useful reference value and compare difference in each fingers. 71 normal volunteers(age, 19-65 years; 142 hands) examined who has no history of peripheral neuropathy, diabetic mellitus, chronic renal failure, endocrine disorders, anti-cancer medicine, anti-tubercle medicine, alcoholism, trauma, radiculopathy. Nicolet Viking II was use for detected conduction velocity and amplitude of digital nerves in median nerve. Data analysis was performed using SPSS. Descriptive analysis was used for obtain mean and standard deviation, ANOVA was used to compare each fingers and independent t-test was used to compare between Rt and Lt side also compare between different in genders. Conduction velocity of the right thumb was 49.77m/sec, index finger was 56.80m/sec, middle finger was 56.15m/sec and ring finger was 53.38m/sec. The left thumb was 50.48m/sec, index finger was 56.76m/sec, middle finger was 55.99m/sec and ring finger was 53.23m/sec. Amplitude of the right thumb was $64.30{\mu}V$, index finger was $73.95{\mu}V$, middle finger was $77.97{\mu}V$ and ring finger was $43.92{\mu}V$. The left thumb was $74.21{\mu}V$, index finger was $85.72{\mu}V$, middle finger was $88.06{\mu}V$ and ring finger was $47.28{\mu}V$. There were significantly difference between thumb, index, middle and ring fingers(p<.01) but there were no statistically difference between conduction velocity and amplitude of index and middle fingers(p>.01). The conduction velocity of index finger are faster than other fingers and amplitude of middle finger are greater than other fingers. The present results revealed that electodiagnosis can easily perform in index and middle finger for digital nerve of median nerve study.
Eruptive volumes of three monogenetic volcanoes (Songaksan tuff ring, Biyangdo scoria cone, and Ilchulbong tuff cone) with the youngest eruption age are calculated using the model, applied to Auckland Volcanic Field in New Zealand, to investigate the volcanic eruption scale and to evaluate volcanic hazard of Jeju Island. Calculated eruptive volumes of the volcanoes are $24,987,557m^3$, $9,652,025m^3$, and $11,911,534m^3$, respectively, and the volumes include crater infill, tuff ring (tuff cone), scoria cone, and lava flow. Volcanic explosivity indices of Songaksan tuff ring, Biyangdo scoria cone, and Ilchulbong tuff cone are estimated based on the eruptive volumes to be 3, 2, and 3 respectively, and eruption type is Strombolian to Surtseyan. It is assumed that the amount of emitted sulfur dioxide gas is $2-8{\times}10^3kt/y$ according to the correlation between volcanic explosivity index and volcanic sulfur dioxide index. Recent age dating researches reveal evidences of several volcanic activities during the last 10,000 years indicating the possible volcanic eruption in Jeju Island in the near future. Therefore, it is necessary for appropriate researches regarding volcanic eruption of the island to be accomplished. In addition, establishment of the evaluation and preparation system for volcanic hazard based on the researches is required.
