Indoor and Outdoor Air Quality and Its Relation to Allergic Diseases among Children: A Case Study at a Primary School in Korea

  • The purpose of this study is to investigate allergic diseases related to allergy caused by the exposure to indoor and outdoor sources of air pollution in primary schools. The symptoms questionnaire of allergic diseases based on the International Study of Asthma and Allergies in Childhood (ISAAC) was completed by the participants. The past and present status of asthma, allergic rhinitis, eczema, and allergic conjunctivitis were investigated by providing a questionnaire to all the participating children. Questionnaires were sent to a total of 61,350 children from 438 primary schools. A total of 40,522 children responded to the questionnaire, which represents a 66.1% return rate. Volatile Organic Compounds (VOCs), $\underline{A}$ldehydes, and Particulate Matter ($PM_{10}$) were measured and analyzed from October to December of 2006, in 82 primary schools. The final study population comprised 35,168 children with complete data which excluded incomplete questionnaire responded by 5,354 children. Based on the survey, the level of indoor air contamination did not appear to be high, but 27.2% of the schools evaluated had exceeded the $PM_{10}$ level specified by the school health guidelines ($100\;{\mu}g/m^3$). The overall mean concentration of formaldehyde was $22.07\;{\mu}g/m^3$ and 1.0% of schools (1 school) exceeded the $100\;{\mu}g/m^3$. Statistically significant relationships have been observed between indoor air quality and prevalence rate of allergic rhinitis and conjunctivitis of primary schools in Korea.

Primary Care Physicians and Residency Training Programs in Korea (일차진료의사 양성과 전문의수련제도)

  • Recent changes in the health care environment have directed increasing attention to the number and specialty mix of practicing physicians. A major concern identified in Korean health care system is the serious oversupply of specialists and a relative lack of primary care physicians. Currently only 21% of Korean physicians are primary care physicians(general practitioners and family physicians), and less than 10% of recent medical school graduates are choosing to enter primary care. More primary care physicians are needed to deal with major problems in the current health care system, such as cost and access. The infrastructure that relies on primary care physicians is needed to deliver cost-effective and efficient care. To achieve a better balance of primary care to non-primary care physicians. more medical students need to choose careers in one of the primary care specialties(family medicine. internal medicine and pediatrics). This paper suggests the necessity of reforming the Korean graduate medical education system, that is, establishing the path of training primary care physicians in internal medicine and pediatrics residency training programs.

A Study on Stress of Primary Students in Korea (우리나라 국민학생의 스트레스에 관한 연구)

  • The purpose of this study is to find out the stress causes of the primary students in Korea, through the three major methods, life event score, multiple regression about stress causes and G.H.Q. path-diagram. A survey was made of 889 students who were selected among students of six primary schools in seoul and Ka-pyeng, from Sep. 28 to Oct. 2. 1992. The results of this study are summarized as follows : 1. Life event score Among the children's life events, (deaths of family) (degrade of school record) (the increase of absence days) (divorce of parents) (trouble with relationship) are high ranking in life event score. Besides, (scolded by teacher and parents) (beginning of extracurricular work) are spread stress causes. In conclusion, these stress causes are shown by two aspects, home, school, and so children are under stress circumstance in both places. These stress causes vary with area and sex. 2. Multiple regression about stress causes and G.H.Q. This study analysed how a stress caused by home life affects the children's health of mind and body. The most influential factors are , and these factors vary with area and sex. 3. Path-diagram This study proved that socio-psychological school environment raises children to secede from school, and this secession that operates as a stress cause affects children's health of mind and body. This serial process is analysed by path analysis, and made out a path-diagram. In conclusion, as expected socio-psychological,school environment affects the children's health of mind and body. This is shown by two aspects : positive, negative. This stress causes vary with area and sex.

A Case of Primary Pancreatic Lymphoma Presenting with Obstructive Jaundice

  • A 55-year-old man was admitted to the hospital for jaundice. Computed tomography (CT) scans showed a diffuse mass in the pancreas and peripancreatic area, with infiltration to of the whole pancreas, and overall reduced enhancement compared to normal pancreas. Esophagogastroduodenoscopy revealed elevated mucosal lesion covered hyperemic mucosa at duodenal bulb and ulcerative lesion at body of stomach. Endoscopic ultrasonography revealed an irregular mass with unclear boundaries was observed within the pancreas. Abrupt narrowing of mid to distal common bile duct was seen and the stricture was caused by compression of pancreatic mass. Plastic stent was inserted and clinical improvement was achieved including resolution of jaundice. The patient is currently being treated with combination of rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone. We report a case of primary pancreatic lymphoma presenting with obstructive jaundice.

Transcriptional activity of the short gastrulation primary enhancer in the ventral midline requires its early activity in the presumptive neurogenic ectoderm

  • The short gastrulation (sog) shadow enhancer directs early and late sog expression in the neurogenic ectoderm and the ventral midline of the developing Drosophila embryo, respectively. Here, evidence is presented that the sog primary enhancer also has both activities, with the late enhancer activity dependent on the early activity. Computational analyses showed that the sog primary enhancer contains five Dorsal (Dl)-, four Zelda (Zld)-, three Bicoid (Bcd)-, and no Single-minded (Sim)-binding sites. In contrast to many ventral midline enhancers, the primary enhancer can direct lacZ expression in the ventral midline as well as in the neurogenic ectoderm without a canonical Simbinding site. Intriguingly, the impaired transcriptional synergy between Dl and either Zld or Bcd led to aberrant and abolished lacZ expression in the neurogenic ectoderm and in the ventral midline, respectively. These findings suggest that the two enhancer activities of the sog primary enhancer are functionally consolidated and geographically inseparable.

Wear of primary teeth caused by opposed all-ceramic or stainless steel crowns

  • PURPOSE. This study was conducted to evaluate the effects of full-coverage all-ceramic zirconia, lithium disilicate glass-ceramic, leucite glass-ceramic, or stainless steel crowns on antagonistic primary tooth wear. MATERIALS AND METHODS. There were four study groups: the stainless steel (Steel) group, the leucite glass-ceramic (Leucite) group, the lithium disilicate glass-ceramic (Lithium) group, and the monolithic zirconia (Zirconia) group. Ten flat crown specimens were prepared per group; opposing teeth were prepared using primary canines. A wear test was conducted over 100,000 chewing cycles using a dual-axis chewing simulator and a 50 N masticating force, and wear losses of antagonistic teeth and restorative materials were calculated using a three-dimensional profiling system and an electronic scale, respectively. Statistical significance was determined using One-way ANOVA and Tukey's test (P<.05). RESULTS. The Leucite group ($2.670{\pm}1.471mm^3$) showed the greatest amount of antagonist tooth wear, followed by in decreasing order by the Lithium ($2.042{\pm}0.696mm^3$), Zirconia ($1.426{\pm}0.477mm^3$), and Steel groups ($0.397{\pm}0.192mm^3$). Mean volume losses in the Leucite and Lithium groups were significantly greater than in the Steel group (P<.05). No significant difference was observed between mean volume losses in the Zirconia and Steel groups (P>.05). CONCLUSION. Leucite glass-ceramic and lithium disilicate glass-ceramic cause more primary tooth wear than stainless steel or zirconia.

Arctigenin Increases Hemeoxygenase-1 Gene Expression by Modulating PI3K/AKT Signaling Pathway in Rat Primary Astrocytes

  • In the present study, we found that the natural compound arctigenin inhibited hydrogen peroxide-induced reactive oxygen species (ROS) production in rat primary astrocytes. Since hemeoxygenase-1 (HO-1) plays a critical role as an antioxidant defense factor in the brain, we examined the effect of arctigenin on HO-1 expression in rat primary astrocytes. We found that arctigenin increased HO-1 mRNA and protein levels. Arctigenin also increases the nuclear translocation and DNA binding of Nrf2/c-Jun to the antioxidant response element (ARE) on HO-1 promoter. In addition, arctigenin increased ARE-mediated transcriptional activities in rat primary astrocytes. Further mechanistic studies revealed that arctigenin increased the phosphorylation of AKT, a downstream substrate of phosphatidylinositol 3-kinase (PI3K). Treatment of cells with a PI3K-specific inhibitor, LY294002, suppressed the HO-1 expression, Nrf2 DNA binding and ARE-mediated transcriptional activities in arctigenin-treated astrocyte cells. The results collectively suggest that PI3K/AKT signaling pathway is at least partly involved in HO-1 expression by arctigenin via modulation of Nrf2/ARE axis in rat primary astrocytes.

Enterobius vermicularis egg positive rate in a primary school in Chungchongnam-do (Province) in Korea

  • The egg Positive rate of Enterobius vernicularis was investigated among students of a primary school and a kindergarten located in the rural area of Tangjin-gun, Chungchongnam-do in December, 1998. Of the 189 examinees, 28 (14.8%) were found to be infected with E. vermicularis by the adhesive cellotape anal swab method . The infection rates ranged from 4.2% to 26.1% among school children, and the highest rate was observed in children attending kindergarten. Three months after treatment with albendazole, four (14.3%) out of 28 infected children still remained infected with E. vermicularis. Through this survey, we were able to determine that E. vemicularis infection is still prevalent among children in rural areas of Korea.

  • Most primary and secondary schools in Korea provide school meals to students. After the implementation of the manual in the development of foodservice facilities within schools in the Seoul metropolitan area, this study had been instituted towards the improvement on the design of the school. Through the investigation of the use of school foodservice facilities and the establishment of putting the designer's design to the users into effect, primary and secondary schools have been observed in the study to conclude the following research: Spatial Configuration and Usage Characteristics within Elementary, Middle, and High school.

  • Signet ring cell carcinoma of lung is an unique variant of mucin producing adenocarcinoma which is characterized by abundant intracellular mucin accumulation. Only a few cases of primary signet ring cell carcinoma of lung have been reported in the world wide literature. And we have, recently experienced one case of primary signet ring cell carcinoma of lung. A 55 years old man was evaluated for paralysis of lower extremities and was found to have lung cancer in the left upper and lower lobe with pleural, multiple spinal, bone and liver metastases. Signet ring tumor cells were revealed by cytologic examination of pleural fluids. And there were no evidence of signet ring cell carcinoma of other organs. Primary signet ring cell carcinoma of lung seems to have an aggressive behavior and therapeutic modalities could be different from those for signet ring cell carcinomas from other organs. Therefore it is important to separate primary signet ring cell adenocarcinoma of lung from metastatic tumors.
