The Study on the Development of Self Efficacy Expectation Promoting Program and It's Effect for Breast Self Examination (유방자가검진을 위한 효능기대 증진 프로그램의 개발 및 효과)

  • Breast self-exam is easiest, safe and cost effective to be recommended as an important method for early detection of breast cancer. This experimental research with non-equal control design was to develop the efficacy expectation program for breast self-exam based on Bandura's self-efficacy information source. The study objects selected from two local churches in Busan. Twenty from S church were selected as a study group, twenty from D church as a control group. Efficacy expectation promoting program was based on Bandura's efficacy promoting source and was two hour lecture including slide, video tape, demonstration, pamphlet. After this, there were individual interviews with them and consultations through telephone f or verbal persuasion on the weekly basis between the first week and the fifth week. The effect from the experiment were measured on the first week after education and on the fifth week. Data from control group was collected during the period from Jan 11, 2 001 to Feb 15, 2001 and data from study group was collected during the period from Jan 12, 2001 to Feb 16, 2001. Measurement instrument for this study was developed by the author with the advice of specialist in order to measure self-efficacy and breast self-exam practice. Data analysis was done by using SPSS/10.0 PC program $with^2-$ test. t-test and ANCOVA. Proved results for hypothesis were as follows. 1) It was supported that "Study group educated by efficacy expectation promoting program has higher score in change rat e of self-efficacy than control group." (1 week after education F=18.395, p=.000 5 weeks after education F=28.972, p=.000) 2) It was supported that "Study group educated by efficacy expectation promoting program has higher score in change rate of exam practice than control group." (1 week after education F=37.984, p=.000 5 weeks after education F=28.972, p=.000) In conclusion, efficacy expectation promoting program for breast self-exam developed by this study may increase s elf-efficacy and breast self-exam practice.

A Chronological Study on the Transformation and the Spatial Characteristics of Ambulatory Care Facilities in the United States (미국 외래진료시설의 시대적 변천과 공간적 특성에 관한 연구)

  • Purpose: This study aims to examine spatial characteristics of ambulatory care department Chronologically through case studies; and to consider the social implication of these spatial changes. Methods: In this study, a total of eight ambulatory care departments, one or two for each period, were selected in order to analyze the spatial characteristics. Results: First, since the 1990s, the outpatient department of the US has been changed into a patient-centered space by providing high quality outpatient medical services through individual exam room planning. Second, the exam room has been changed from open/semi-open/group exam room to individual exam room since 1990, and the trend is shifting from the cluster type to the modular type consisting of universal exam rooms in order to achieve the flexibility of exam room. Third, the diagnostic/testing area has been deployed to the ambulatory clinic to enable 'one-stop shopping' for patients-centered care since 1990s, however, it has been disappearing since 2000 due to Reduce medical expenses. Fourth, the Central Nursing Station type first appeared in the outpatient department In the 2000s, followed by the type of decentralized nursing station type since 2010s. And fifth, the area of medical support has been placing to be concentrated on one place due to the Lean Design strategy since 2010. Implications: In the future, it is expected that Korea will be more focused on efficiency, visual control, and flexibility in the planning of the ambulatory care department as in the case of the United States.

  • Objective : As the meaning of collective medical examination, the purpose of this study is to investigate distribution of weak children by measurement according to the five group of weakness (Lung, Heart, Pancreas, Liver, Kidney), and the relationship between the five group of weakness and the Growth failure on primary school children. Methods : This study is composed of 1078 children who attend A primary school in ${\bigcirc}{\bigcirc}$ city. They are divided into 6 groups by grade and for the purpose of effective comparison, The five weak symptoms are studied to determine the association with growth stature among children and we standardize the weak children's height into percent according to age. Results: When each weak children controled grade, sex, body-weight, existence or nonexistence of personal history, physical exercise so on, relatively group of weakness for kidney is statistically less then opposite group and the rest of the people statistically have no significant difference. Conclusions: From the above result, it is suggested that the statistical relationship between the weakness for kidney and growth(p<0.001). It looks forward to have contributed to collective medical examination and children's health care by oriental medical treatment.

And keeping the principle of equity and the homogeneity of risk in the group of the insured, you are able to manage the company reasonably and realize the orginal ideal of life insurance, namely, the mutual assistance. And yet, the cases of the misrepresentation have been occurred frequently so far. As a result of them, the development of life insurance industry has been obstructed, and public criticism has been caused. Is the Agricultural Work a Risk Factor for Koreans Elderly Spinal Sagittal Imbalance?

  • Objective : A primary degenerative sagittal imbalance has been considered because of unique lifestyles such as the prolonged crouched posture during agricultural work and performing activities of daily living on the floor. Previous papers have reported that sagittal imbalance disease is often seen distinctly in the farming districts of "oriental" countries such as Korea and Japan. However, this finding was only evaluated with the use of X-ray, and other factors such as magnetic resonance imaging (MRI), muscle volume, compression fracture, and laboratory results were not considered. Thus, using these, we evaluate the agricultural work-associated factors for Korean elderly spinal sagittal imbalance. Methods : We recruited 103 Korean participants who had a sagittal vertical axis (SVA) of >5 cm in this Korean Elderly Sagittal Imbalance Cohort Study. The following were evaluated : radiological parameters, MRI, compression fracture, vitamin D, parathyroid hormone, C-terminal telopeptide, osteocalcin, bone mineral density and muscle fatty change, muscle volume, and health-related quality of life from patients' survey. Moreover, in this survey, the farmers' annual working hours were investigated. Subsequently, we analyzed the associated factors for spinal sagittal imbalance depending on occupation. Results : A total of 46 participants were farmers, and the others were housewives, sellers, and office workers. The farmer group had more SVA (141 vs. 99 mm, p=0.001) and pelvic tilt (31° vs. 24°, p=0.004) and lesser lumbar lordosis (20° vs. 30°, p=0.009) and thoracic kyphosis (24° vs. 33°, p=0.03) than non-farmer group. A significantly positive correlation was noted between the working hour and SVA in the farmer group (p=0.014). The visual analogue scale score for back pain (8.26 vs. 6.96, p=0.008) and Oswestry Disability Index (23.5 vs. 19.1, p=0.003) in the farmer group were higher than that in the non-farmer group, but the Short Form-36 score was not significantly different between the two groups. The Mini-Mental State Exam score was significantly lower in the farmer group than in the non-farmer group (24.85 vs. 26.98, p=0.002). Conclusion : The farmer group had more sagittal imbalance and back pain in proportion to the working hours even though the muscle and bone factors and general laboratory condition were not significantly different between the two groups. These results supported that the long hours spent in the crouched posture while performing agricultural work were a risk factor for severe sagittal imbalance.

The Effect of Music Therapy on Reduction of Anxiety before Gastroscopy (음악요법이 위내시경 대상자의 검사전 불안에 미치는 효과)

  • Purpose: This study examined the effect of music therapy on the pre-exam anxiety of gastroscopy clients. Method: The research period is from June 17 2002 to August 30 2002, and the subjects were 60 individuals, 30 for the experimental and 30 for the control group among adult clients. The data were analyzed by using the SPSS program. Result: The results of this study were as follows: The score of the experimental group was lower than the score of the control group for the situational anxiety (t=-12.56, p=0.000) and systolic blood pressure (t=-3.97, p=0.000) before the examination was supported. The vital sign was partially supported. There was no significant difference(t=-1.05, p=0.297) in the diastolic blood pressure (t=-1.05, p=0.297) and pulse rate(t=-1.30, p=0.199) before the examination of the experimental group and lower than the score of the control group. Conclusion: Music therapy could be useful in the nursing practice, and be utilized as a way of nursing intervention to reduce anxiety of the patients in specific clinical situations. Finally, it is recommended that further studies be conducted on this therapy by utilizing different musical selections.

Analysis of Dental Care Clinic Visits and Causes of Tooth Injuries by Age and Sex (외상에 의한 치아손상의 연령 및 성별에 따른 의료이용분석)

  • Objectives: Using data from the 1styear of the 7th National Health and Nutrition Survey, this study administered a questionnaire to patients with oral damage and examined the type of medical care used based on the nature of the injury, further characterized by age group and sex. Methods: Of the 8,150 respondents, this study selected those who participated in the health survey and oral exam survey, excluding surveys with missing values, and set 7,681 people as the research participants. This study used SPSS Windows version 23.0(SPSS Inc. IL, USA) as the statistics program and applied the chi-square test (p) based on a complex sample and logistic regression analysis. Results: In the analysis of the type of tooth damage, there was a significant difference (p<.001) in the number of male patients who were injured due to exercise, violence, and safety accidents compared to females. However, there was no significant difference in their type of medical security and whether or not they visited a dental clinic. The patients with private insurance showed relatively high use for tooth extraction and oral surgery (p<.001). Conclusions: The best way to prevent tooth damage is to prevent the accident. For sports or exercises which may cause tooth damage, protective gear, such as a mouthpiece, should be used to prevent tooth damage.

The Effect of Extension Exercise on the Reduction of Pain in Patients with Low Back Pain (요통환자에 있어 신전운동이 통증감소에 미치는 효과)

  • The purpose of this study was to determine whether extension exercise is effective in reducing low back pain. Nine patients with low back pain were randomly assigened to one of two groups-one was an experimental group in which the patient were treated with extension exercise(McKenzie method) and conservative therapy. The other was a control group in which the patients were treated with conservative therapy only. Treatment was performed for a period of ten days during Which we examed each patient three times: the first exam was done at the begining of treatment, the second after fives days, and the third after ten days. We used 'The Oswestry low back pain disability questionnaire'(r=.99) as the examination tool. The results were as follows: 1. Both experimental and control groups showed nonsignificant differences before and after treatment, and the between group difference was also non-significant(P>.05). 2. There was no effective reduction of low back pain by MecKenzie's extension exercise(p>.05). We admit that our study protocol had several shortcomings. If they had been accounted for, the result might have been different. Further study should be done with a better experimental design, a larger sample, and a longer experimental duration.

THE CLINICAL STUDY OF MAXILLARY SINUS SEPTA USED IN PANORAMA, CT (Panorama, CT를 이용한 상악동내 중격의 임상적 연구)

  • Purpose: The aim of this study was to examine the incidence, location and morphology of antral septa using radiographic exam.(Panorama, CT) in the dentate/non-atrophic and edentulous/atrophic maxillary segments. Material & methods: 232 sinuses were subdivided into two groups(group1 : 175 sinuses were classified as complete & partial dentate maxillary segments, group2 : 57 sinuses were classified as complete edentulous maxillary segments) and were investigated for the incidence, location and morphology of maxillary sinus septa. Results: A total of 80 septa were observed in 232 maxillary sinuses, which corresponded to 30.65% of the sinuses(71 of 232). 67.5% of the total septa was observed in the complete & partial dentate groups, but 32.5% of the total septa was observed in the complete edentulous group. Upon analysis of the anatomical location of the septa, it was seen that 14 septa(17.5%) were located in the P1 area, 15 septa(18.7%) were located in the P2 area, 19 septa(23.8%) were located in the M1 area and 32 septa(40%) were located in the M2 area & dista area of M2. we found 45 septa in the apical lesion of teeth, Group 1. However the remaining 9 septa were found in the edentulous area. Conclusion: CT is a better method than panoramic radiography for detecting the presence of the maxillary sinus septa. Maxillary sinus septa are more commonly detected in complete edentulous maxillary segments than in complete & partial dentate maxillary segments. Also maxillary sinus septa are more commonly detected posteriorly than anteriorly.

Functional Status in Lung Cancer Patients (폐암 환자의 기능상태에 관한 연구)

  • Purpose: To exam how functional status varied according to age, gender, stage, treatment protocols, and pathologic types of cancer in lung cancer patients. Method: A Cross-sectional and descriptive study was used. Functional status was measured with the Medical Outcome Study Short Form-36. A total of 106 lung cancer patients participated. Their mean age was 61 years. Majorities were male, in advanced stages, and receiving chemotherapy. Result: Functional status of the subjects were relatively low compared to the results of previous studies. Women had more severe functional limitations in Role-Emotion(t=2.17, p <.05). Generally, older patients(> 60 yrs.) had relatively more severe limitations in all subcategories, but the difference was not statistically significant. Subjects in late lung cancer stage(stage III & IV) had more severe functional limitation in all subcategories. But the statistical difference was found only in General Health(t=2.10, p<.05). In terms of treatment protocol, no-current treatment group had lower General Health than those of the chemotherapy group(F=3.42, p<.01). There were no statistical differences in functional status among pathological cancer cell types. Conclusion: The results of this study suggest that effective management may be achieved when these factors are considered on individual basis in the clinical management of lung cancer patients.
