  • 65세 이상 노인은 퇴행성 질환과 지병으로 인한 합병증으로 고생하거나 심하면 사망에 이르기도 한다. 이를 예방하기 위해서는 뇌졸중, 치매, 순환기질환 등에 대한 검사를 정기적으로 받는 것이 좋다.

A Influencing Factors in Korea Adults Stroke (한국인의 뇌졸중에 영향을 미치는 요인)

  • This study examined the health behavior and comorbidity based on a community health survey of stroke patients in 2010. The subjects were 4,449 adults over the age of 19 years, who were diagnosed by doctors and completed a community health questionnaire. The subjects demographic characteristics, health behavior and comorbidity were analyzed. Significant differences in age, education, smoking and alcohol consumption were observed. The influencing factors for comorbidity according to gender included myocardial infarction, diabetes and depression. According to the characteristics of comorbidity of the life cycle, hypertension, myocardial infarction and depression. The influencing factors related to the 19-44 year age group included Ex-smokers, 45-64 year old ex-smokers, diabetes, hyperlipidemia, atopic dermatitis, and the 75 year and over group with diabetes, hyperlipidemia, tuberculosis, and atopic dermatitis. This study suggests that health education and a health policy approach based on the relevance between stroke and comorbidity, and the health behavior according to the life cycle and gender are needed.

  • 생명과학의 발달로 사람의 수명이 늘고 있지만 뇌졸증과 치매, 파킨슨병 같은 노인성 질환도 늘고 있어 65세 이상 노인의 14%가 이러한 질병으로 고통을 받고 있다. 미국ㆍ일본 등 선진국에서 이러한 노인성 뇌질환 연구에 박차를 가하고 있는 가운데 아주대 의과대학은 '뇌졸증 및 치매의 중재요법 연구센터'를 설립해 최근 Sulfasalazine을 비롯 신경세포를 보호하는 물질을 다수 발견해 뇌질환의 예방과 치료에 획기적인 계기를 마련하고 있다.

A Study on Relationships between Blood Pressure and Na, K, Ca and Mg in Cerebral Apoplexy Patients (입원 치료중인 뇌졸중 환자의 혈압과 Na, K, Ca, Mg과의 관계에 관한 연구)

  • 뇌졸중이라고 진단된 입원 환자 26명을 대상으로 3일동안 섭취한 식이와 24시간 뇨와 혈액을 수집하고 혈압을 측정하여 나트륨, 칼륨, 캄슘, 마그네슘의섭취 수준에 다른 뇨중 배설량과 혈중 수준을 관찰하고 이들과 혈압과의 관계를 알아봄으로써 뇌졸중 환자를 포함한 고혈압 관리의 치료식이를 제공하는데 기초자료로 사용하고자 실시간 연구결과를 요약하면 다음과 같다. 1. 대상자들의 평균 연령은 남녀 각각 52.6세, 58.5세였으며 평균 BMU는 남녀 각각 21.6, 22.4였다. 입원 당시 평균 혈압은 남녀 각각 147.3/90.9mmHg, 162.7/102.0mmHg였고, 실험 종료일의 평균 혈압은 남자의 경운 124.6/80.8mmHg 였고 여자 대상자는 137.7/91.3mmHg 였다. 2. 1일 평균 나트륨, 칼륨, 칼슘, 마그네슘의 섭취 향은 96.7$\pm$30.7mEq,64.8$\pm$19.0mEq, 431.3$\pm$227.1mg, 333.9$\pm$145.9mg이었다. 3. 1일 평균 뇨중 나트륨, 칼륨, 칼슘, 마그네슘의 배설량은 76.7$\pm$28.2mEq, 33.9$\pm$16.6mEq, 65.6$\pm$34.0mg, 96.5$\pm$58.0mg이었다. 4. 평균 혈청내 나트륨, 칼륨, 칼슘, 마그네슘의 함량은 137.8$\pm$4.3mEq/l, 3.7$\pm$0.5mEq/l, 7.6$\pm$0.8mg/dl, 2.2$\pm$0.2mg/dl로써 칼슘 농도가 정상수준에 비해 비교적 낮은 것으로 나타났다. 5. 혈압과 무기질과의 관계에서 수축시 혈압의 증가시 뇨중 칼륨 배설이 증가함으로써 수축기 혈압과 뇨중 칼륨 배설량 사이에 유의적인 부의 상관관계가 나타났다. (p<0.05). 이상과 같은 입원중인 뇌졸중 환자의 뇨중 나트륨과 칼륨의 배설률 및 혈청과 뇨중 칼슘 배설수준이 정상인 보다 낮은 수준이었으며, 칼륨은 뇌졸중 환자의 혈압관리에 효과적이 무기질로 작용할 가능성이 보여지므로 고혈압관리를 위한 식사 처방시 이들 무기질에 대한 고려가 있어야 할 것으로 사료된다.

Comparisons of Middle-, Old-, and Stroked Old-Age Drivers' Reaction Time and Accuracy Based on Multiple Reaction Time Tasks (중다 반응시간 과제에 기반한 중년, 고령 및 뇌졸중 고령 운전자의 반응시간과 반응정확성에서의 차이 비교)

  • Differences in reaction time and accuracy were compared among driver groups of middle-, old-, and stroke old-age drivers using various reaction time tasks including simple reaction task, 2-choice task, 4-choice task with different stimuli eccentricity, search task, and moving target detection task. The results can be summarized as followings. First, although overall reaction time tended to be slowed with age and stroke, stroke old drivers showed significantly slower reaction time than the other driver groups when the stimuli were presented in a large eccentricity. Second, differences in reaction time for 2-choice task and moving target detection task seemed to be determined mainly by participants' simple reaction time. Third, the search task which required temporary retention of previously presented stimuli was found to be more sensitive in discriminating difference in reaction time between middle-age drivers and old-age drivers (including stroke old drivers). Fourth, reaction accuracy of old (and stroke old) drivers decreased when more stimuli alternatives were presented and temporary retention for stimuli was required. Altogether, memory demand in reaction time task can be sensitive to evaluate performance for different age groups, whereas size of useful field of view for brain stroke.

A Study on Health Status' Factors Affecting HRQOL in the Elderly Stroke Patients With or Without Sequela (노인 뇌졸중 환자의 후유증 유무에 따른 HRQOL 요인별 중요도 분석)

  • Objective : The main purpose of this study was to analyze factors affecting a health related quality of life(HRQOL) in the elderly stroke patients with(Group 1) or without(Group 2) sequela. Methods : This survey performed secondary analysis with 'the fifth korea national health and nutrition examination survey', and investigated 91 elderly stroke patients over 65 years. Results : The results were as followed: HRQOL and self rated health had a significantly lower in Group 1(p<.05). In all groups, HRQOL was affected by self rated health(${\beta}=0.39$), mental health, and medical health significantly(p<.05). In group 1, HRQOL was affected by mental health(${\beta}=0.45$), self rated health significantly(p<.05). In group 2, HRQOL was affected by medical health(${\beta}=0.45$), functional health, mental health significantly(p<.05). Conclusion : In order to improve the HRQOL of stroke patients during rehabilitative therapy, it is important that early detection and early intervention of depression and suicide, which affect mental health status, are important in the case of severe aftereffects. If the patient continues to support his/her current status regardless of aftereffects, he/she can improve the HRQOL with functional recovery of the stroke patient, and finally have a positive effect on the independence of the stroke patients.

Factors Affecting the Stroke related Health-Promoting Lifestyle in Middle-Aged Adult (중년기 성인의 뇌졸중 관련 건강증진 생활양식의 영향요인)

  • This research is a descriptive study that aimed to identify the health knowledge related stroke, optimistic bias, and social support of middle-aged adults and the effect these had on their health-promoting lifestyle This study was conducted by collecting 220 adults aged between 40 to 60 years from C City D city and K city. Data were analyzed using descriptive analysis, ANOVA, t-test, Pearson's correlation coefficients and multiple regression with the SPSS 23.0 program. The average health-promoting lifestyle was 44.27 points. The health-promoting lifestyle of the study participants showed a positive correlation between the optimistic bias(r=.18, p=.001) and social support(r= .61, p<.000). According to the results of multiple regression analysis, perception of Necessity for Stroke Education(β=.12, p=.010), optimistic bias(β=.18, p=.040), and social support(β=.48, p<.000) were shown to be significant factors that affected the health-promoting lifestyle of the participants. These variables explained 38.5%. Therefore, an health education program to improve the health-promoting lifestyle related to stroke in adults should be considered as a way to enhance social support and reduce optimistic bias.

Effect of Virtual Reality Game on Old Patient's Depression, Relationship and Life Satisfaction (가상현실 게임이 노인 뇌졸중 환자의 우울, 대인관계, 삶의 만족도에 미치는 효과)

  • This study was conducted to investigate the effect of virtual reality game on old stroke patient's depression, relationship and life satisfaction. The subjects of this study were 40 stroke patients of over 65 years old and each 20 patients were randomly assigned in two groups. The study was conducted for 12 weeks. Nintendo Wii Sports Resort program was applied to the virtual reality game for the experimental group. In order to investigate psychological social factor (depression, relationship) and life satisfaction of the objects, BDI, RCS, LSI-Z were checked three times. The results are as followings. First, the psychological social change (depression, relationship) after the training program showed that depression is significantly decreased (p<.001) along with the intervention period in both 2 groups. Second, change of life satisfaction after the training program showed that significant statistical improvement were found both in control group(p<.05), and experimental group(p<.001). Third, depression, relationship showed the significant statistically correlation with life satisfaction(p<.05). We can conclude from the above that virtual reality games which were played by 2 persons had the significant effect on old stroke patient's psychological social factor(depression, relationship) and life satisfaction.

Effect of Arthritis and Comorbid Chronic Conditions on Health-related Quality of Life in Korean Elderly (우리나라 노인에서 관절염과 동반 만성질환에 의한 건강관련 삶의 질 감소 효과)

  • This study was designed to evaluate the effects of arthritis and comorbid chronic conditions on the health-related quality of life (HRQoL) in the Korean older population. The study subjects were 2,708 Korean adults aged 65 and older from the $3^{rd}$ Korea National Health and Nutrition Examination Surveys data: 1,357 persons with a single chronic condition, such as arthritis, herniations of the intervertebral disc, osteoporosis, asthma, peptic ulcers, stroke, or cataract; 886 persons with arthritis and a second chronic condition described above; and 465 persons with no chronic condition. An analysis of covariance was performed to compare the EQ-5D index among the groups. The effects of arthritis, second chronic condition and their interactions were analyzed by multiple linear regression analysis. The results are as follows. Compared to men with arthritis only, men with stroke only, stroke and arthritis, or cataract and arthritis had a lower age adjusted EQ-5D index. Women with a stroke only, asthma only, cataract only, osteoporosis and arthritis, peptic ulcer and arthritis, stroke and arthritis, or cataract and arthritis had a lower age adjusted EQ-5D index than women with arthritis only. Arthritis and comorbid conditions had additive effects on the HRQol in both genders except for arthritis and stroke in women. In conclusion, comorbid chronic medical conditions in older people with arthritis may reduce the HRQoL in an additive manner.

Effects of Swallowing Training Program for Nursing Home Residents with Stroke (요양시설 거주 뇌졸중 노인을 위한 연하 훈련 프로그램의 효과)

  • Purpose: This study aimed to verify the effects of a swallowing training program on swallowing function and depression for nursing home residents with dysphagia after stroke. Methods: This is a quasi-experimental study with non-equivalent control group pre-post test design. The program (oro-facial muscle strengthening exercises, swallowing exercises, expiratory muscle strengthening exercises, and brain stimulation exercises) applied to the experimental group three times per week for eight weeks, 40-45 minutes for each intervention. The final data from 42 people (21 experimental and 21 control) were analyzed by SPSS/WIN 25.0 using descriptive statistics. 𝜒2 test, t-test, Wilcoxon rank sum test, and Friedman test. Results: The experimental group was significantly improved than control group in oro-facial muscle strength, swallowing symptoms (Z=-2.22, p=.026), and oral intake function level (Z=-2.00, p=.046). However, there was no significant difference between two groups in depression. Conclusion: This study is meaningful in that it reorganized and mediated a swallowing training program as a safe, non-invasive exercise that nurses can implement in a nursing facility with limited medical personnel and it could be easily followed by older adults.
