Emergent Recanalization with Stenting for Acute Stroke due to Athero-Thrombotic Occlusion of the Cervical Internal Carotid Artery : A Single Center Experience

  • Objective : The purpose of this study is to demonstrate the technical feasibility and clinical efficacy of emergent carotid angioplasty and stenting (CAS) for acute stroke due to athero-thrombotic occlusion of the cervical internal carotid artery (ICA). Methods : Review of medical records identified 17 patients who underwent emergent CAS for treatment of athero-thrombotic occlusion of the cervical ICA with acute stroke between 2009 and 2013. Eleven patients (64.7%) presented with concomitant intracranial artery occlusion, which was treated primarily by mechanical thrombectomy after CAS. Results : Successful revascularization of the cervical ICA with emergent CAS was achieved in all patients. After CAS, intracranial recanalization with Thrombolysis in Cerebral Infarction ${\geq}2b$ flow was achieved in four of the 11 patients (36.4%). The overall recanalization rate (cervical ICA and intracranial artery) was 10 of 17 patients (58.8%). Symptomatic intracranial hemorrhage occurred in two patients (11.8%), resulting in death. Ten patients (58.8%) showed improvement (decrease in NIHSS score of ${\geq}4$ points) at seven days after recanalization. Nine patients (52.9%) showed a favorable outcome ($mRS{\leq}2$) at the last follow-up. A favorable outcome ($mRS{\leq}2$) was obtained in four of the six patients with isolated cervical ICA occlusion (4/6, 66.7%) and five of 11 patients with intracranial tandem occlusion (5/11, 45.5%). Conclusion : Emergent CAS for acute stroke due to athero-thrombotic occusion of the cervical ICA showed a good technical feasibility and favorable clinical outcome.

간장질환의 임상에 대한 관찰 (The Clinical Evaluation of Hepatic Disease)

  • The function of the liver is so complicated and important that various Hepatic diseases are occured due to functional retardation of liver. Above all, the incidence of Hepatitis and liver cirrhosis among Koreans has shown an increasing tendency recently. 113 cases of Hepatitis and 125 cases of liver cirrhosis which had been admitted, more than 10 days, to KOREA Hospital between November, 1968 and July,1972, were studied through clinic charts. (A) Hepatitis 1) of the 113 cases; 54 cases (47.7%) were Infectious hepatitis; 40 cases (35.4%) were chronic hepatitis 2) of the 113 cases ; 80 cases (70.8%) were male and 33 cases (29.2%) were female; the sex ratio was 2.4 : 1 The ages of the onset of the disease was as follows; 34 cases (30.1%) were among $30{\sim}40$. 3) Patients had abdominal pain (77.9%) anorexia (66.4 %) and general weakness(82.3%) as symptom and jaundice (94.7%) hepatomegaly (76.1%) as sign 4) 57 cases of all had complication 26 cases (45.6%) were parasite, 12 cases (21.1%) were diabeties mellitus. 5) 99 casea (87.6%) of all were improved and recovered. (B) Liver Cirrhosis 1) Etiologic factors are hepatitis (56cases) and alcoholics (28 cases). 2) of the 125 cases, 84 cases (67.2%) were male and 41 cases (32.8%) were female; the sex ratio was 2 : 1 The age of the onset of the disease was as follows; 47 cases (37.6%) were among 41-50. 3) Patients had symptoms: indigestion (64.8%), Abdominal pain (60%), general wenkness (35.2%) and signs; Hepatomegaly (61.6%) Ascites (59.2%), Jaundice(56.8%). 4) 107 cases of all had complications; Hepatic coma was 20 cases (18.7 %), Ascites was 16 cases (15.0%). 5) 69 cases (55.2%) were improved. (c) Treatment of Hepatitis and cirrhosis. 1) Absolutely (bed) rest. 2) A well-balanced diet adequate in calorie value, showed be given (High Carbohydrate, High Protein, High vitamin diet) if the patient's appetite is good and easily digested. 3) Drugs; (1) Vitamins (2) Digestants (3) Tranquize

Carotenoid Intake and Esophageal Cancer Risk: a Meta-analysis

  • This meta-analysis was conducted to evaluate the association between intake of carotenoids and risk of esophageal cancer. A systematic search using PubMed, Cochrane Library, Web of Science, Scopus, CNKI, and CBM (updated to 6 May 2012) identified ten articles meeting the inclusion criteria with 1,958 cases of esophageal cancer and 4,529 controls. Higher intake of beta-carotene, alpha-carotene, lycopene, beta-cryptoxanthin, lutein, and zeaxanthin reduced esophageal cancer risk with pooled ORs of 0.58 (95% CI 0.44, 0.77), 0.81 (95% CI 0.70, 0.94), 0.75 (95% CI 0.64, 0.86), 0.80 (95% CI 0.66, 0.97), and 0.71 (95% CI 0.59, 0.87), respectively. In subgroup analyses, beta-carotene showed protective effects against esophageal adenocarcinoma in studies located in Europe and North America. Alpha-carotene, lycopene, and beta-cryptoxanthin showed protection against esophageal squamous cell cancer. This meta-analysis suggested that higher intake of carotenoids (beta-carotene, alpha-carotene, lycopene, beta-cryptoxanthin, lutein, and zeaxanthin) is associated with lower risk of esophageal cancer. Further research with large-sample studies need to be conducted to better clarify the potentially protective mechanisms of carotenoid associations risk of different types of esophageal cancer.

Effect of Ketoconazole, a Cytochrome P450 Inhibitor, on the Efficacy of Quinine and Halofantrine against Schistosoma mansoni in Mice

  • The fear that schistosomes will become resistant to praziquantel (PZQ) motivates the search for alternatives to treat schistosomiasis. The antimalarials quinine (QN) and halofantrine (HF) possess moderate antischistosomal properties. The major metabolic pathway of QN and HF is through cytochrome P450 (CYP) 3A4. Accordingly, this study investigates the effects of CYP3A4 inhibitor, ketoconazole (KTZ), on the antischistosomal potential of these quinolines against Schistosoma mansoni infection by evaluating parasitological, histopathological, and biochemical parameters. Mice were classified into 7 groups: uninfected untreated (I), infected untreated (II), infected treated orally with PZQ (1,000 mg/kg) (III), QN (400 mg/kg) (IV), KTZ (10 mg/kg)+QN as group IV (V), HF (400 mg/kg) (VI), and KTZ (as group V)+HF (as group VI) (VII). KTZ plus QN or HF produced more inhibition (P<0.05) in hepatic CYP450 (85.7% and 83.8%) and CYT b5 (75.5% and 73.5%) activities, respectively, than in groups treated with QN or HF alone. This was accompanied with more reduction in female (89.0% and 79.3%), total worms (81.4% and 70.3%), and eggs burden (hepatic; 83.8%, 66.0% and intestinal; 68%, 64.5%), respectively, and encountering the granulomatous reaction to parasite eggs trapped in the liver. QN and HF significantly (P<0.05) elevated malondialdehyde levels when used alone or with KTZ. Meanwhile, KTZ plus QN or HF restored serum levels of ALT, albumin, and reduced hepatic glutathione (KTZ+HF) to their control values. KTZ enhanced the therapeutic antischistosomal potential of QN and HF over each drug alone. Moreover, the effect of KTZ+QN was more evident than KTZ+HF.

Quantitative Determination of Compounds from Akebia quinata by High-Performance Liquid Chromatography

  • To provide the scientific corroboration of the traditional uses of Akebia quinata (Thunb.) Decne., a detailed analytical examination of A. quinata stems was carried out using a reversed-phase high performance liquid chromatography (RP-HPLC) method coupled to photodiode array detector (PDA) for the simultaneous determination of four phenolic substances; cuneataside D (1), 2-(3,4-dihydroxyphenyl)ethyl-O-${\beta}$-D-glucopyranoside (2), 3-caffeoylquinic acid (3) and calceolarioside B (4). Particular attention was focused on the main compound, 3-caffeoylquinic acid (3), which has a range of biological functions. In addition, 2-(3,4-dihydroxyphenyl)ethyl-O-${\beta}$-D-glucopyranoside (2) was considered as a discernible marker of A. quinata from its easy confuse plants. The contents of compounds 2 and 3 ranged from 0.72 to 2.68 mg/g and from 1.66 to 5.64 mg/g, respectively. The validation data indicated that this HPLC/PDA assay was used successfully to quantify the four phenolic compounds in A. quinata from different locations using relatively simple conditions and procedures. The pattern-recognition analysis data from 53 samples classified them into two groups, allowing discrimination between A. quinata and comparable herbs. The results suggest that the established HPLC/PDA method is suitable for quantitation and pattern-recognition analyses for a quality evaluation of this medicinal herb.

Aspergillus속 균주를 이용한 콩 발효물의 이화학적 특성 (Physicochemical Characteristics of Fermented Soybean Products with Aspergillus Strain)

  • In this study, the properties of the substances fermented soybean producted by manufacturing with several Aspergillus strains were investigated. The five soybean cultivar that includes miso, jinpung, pyeongwon, cheonga and chamol were used in this experiment. The pH and total acidity were 6.30~6.66%, and 0.27~0.48%, respectively with differences depending on the samples. The color values for L value, a, and b value were 60.28~69.80, 0.64~3.68, and 11.48~31.21, respectively. The amylase activities, protease activities, and amino-type nitrogen of the jinpung samples were the highest among all cultivars. The mold counts the fermented soybean products by cultivars were 6.18~9.14 log CFU/g, and miso was the highest. Free amino acid and organic acid contents were highest in the jinpung and showed different composition depending on each sample. A total of 18 volatile aroma-compounds, including two acids, four alcohols, four ketones, three phenols, one furan, three pyrazines, and one miscellaneous compounds. In conclusion, it is expected that manufacturing A. oligze inoculation fermented soybean products using jinpung cultivar will improve quality.

곰취 추출물의 항돌연변이성 및 유전독성억제효과 (Antimutagenic and Antigenotoxic Effects of Ligularia fischeri Extracts)

  • The antimutagenic and antigenotoxic effects of ethanol, methanol, water and non-heating ethanol extract of Ligularia fischeri were investigated using Ames test and micronucleus test. Four solvent extracts by themseleves did not induce mutagenesis. The four extract of 200㎍/plate showed approximately 84.7%, 77.1%, 72.5% and 71% inhibitory effect on the mutagenesis induced by N-methyl-N'-nitro-N-nitrosoguanidine(MNNG) and 67.9%, 66.8%, 64.6% and 56% inhibition on the mutagenesis by 4-nitroquinoline-1-oxide(4NQO) against TA100 strain, whereas 70.2%, 60.9%, 61.9% and 52.8% inhibitions were observed on the mutagenesis induced by 3-amino-1,4-dimethyl-5H-pyrido[4,3-b]indol(Trp-P-1) in the presence of 200㎍/plate. TA100 strain was more sensitive than TA98 strain by four kinds of extracts on antimutagenesis. The effects of Ligularia fischeri extracts on the frequencies of micronucleated poly chromatic erythrocytes(MNPECs) induced by MNNG were investigated in the bone marrow. Ten, 20, 40 and 80mg g/kg of each extract were administered to animals immediately after injection of MNNG and the exposure time was 36 hours. Inhibitory effects of Ligularia fischeri ethanol extracts were 12%, 35.3%, 58.8%, and 57%, in the presence of 20, 40, 60 and 80mg/kg, respectively whereas methanol extracts showed 15.5%, 32.7%, 50.8%, and 57.9% inhibitory effects, respectively. Both extracts showed enhanced antimutagenic and antigenotoxic effects. These results showed a good correlation between antimutagenic effects in in vitro and in in vitro assay.

강원 지역정보화 실태 및 확산전략에 관한 연구 (Recent Trends of Informatization and Ks Diffusion Strategy of Kang Won Locals)

  • 강원지역 정보화 실태분석을 한 결과, 첫째, 강원도민은 64.7%이상이 정보화에 대해 높은 관심을 보였으며, 특성별로는 남자가 여자보다, 40대가 20대보다, 고학력자가 저학력자보다, 고소득층이 저소득층보다 정보화에 대한 관심이 높은 것으로 나타났다. 둘째, 정보처리, 정보전달 방법, 정보획득 방법에 있어서 정보통신의 이용도가 증가하는 추세에 있지만 아직까지 전통적인 방법에 의존하는 경우가 지배적인 것으로 나타났다. 셋째, 정보화 홍보에 대해 도민들의 관심은 전시회, 학술행사, 강연회 순으로 나타났으며, 정보화추진 사업의 연중계획 입안시 고려되어야 한다. 넷째, 지금의 속도로 정보화를 추구한다면 21세기 초에는 선진국과의 격차를 줄일 수 있다고 낙관적인 견해를 가진 사람이 많은 것으로 인식하고 있지만 상대적으로 강원도민들은 타시도에 비해서 정보화 정책 추진속도가 늦다고 인식하는 것으로 조사되었다. 이상의 분석결과를 토대로 고도정보화 사회로 가기 위한 확산전략 목표는 다음과 같이 제시할 수 있다. (1) 지역정보화의 가장 기본 목표인 주민생활을 풍요롭고 윤택하게 하기 위한 정보화를 실현시켜야 한다. (2) 도내의 산업활성화를 촉진시킬 수 있도록 정보화를 추진해야 한다. (3) 도내 정보중추기능의 거점도시 형성과 도농간, 지역간 균형과 조화를 이룬 지역정보화를 추진해야 한다.

갑천류역(甲川流域)의 농업개발(農業開發)에 관(關)한 조사연구(調査硏究)(I) -하천부지(河川敷地) 농지확대(農地擴大)를 중심(中心)으로 (Research on Development of Farm Land of Gab River Basin(I) -Enlargement of Farm Land River-Site)

  • This study was conducted to investigate the development of Gab river-basin which was a part of the farm land enlargement to contribute to the increased production of food. The results were as follows; 1. Gab river has the upper stream which occupy 50 percent in moumtains and the mid-stream in the Daejeon city area, and the downstream in a field which is about 22.9 percent in which farming area per household is 0.82 ha., agricultural population is 76 percent except of Daejeon city. Also, urban enlargement of mid-stream basin and development of industrial area in the lower stream diminish farm land. Consequently, this area should be developed to farm land to increase farming size. 2. There is no possibility to develop farm land in mountains of which (64.9 percent) is forests and in midstream which was constructed river-improvement. But Weonjeong area and Yongcheon area will be effective area. 3. If river banks of Weonjeong area will make straight with cost of construction 195,000,000 won, bank length 6 km will be useless, water will flow smoothly, flood will be prevented, farm land will develop 21.66 ha in which rice will produce annually 81.698 M/T which is about 10,860,000 won. 4. This area has good conditions of development. that is, investment efficiency (B/C) is 1.47 more than 1.00. 5. This area is a multiple purpose development district. The reasons are that there are beautiful mountains and a reservoir to be expected to construct, so it will be a sight seeing district in the vicinity of Daejeon city. 6. If Honam railway double line and river straight construction had executed simultaneously, cost of construction 50,000,000 won would have saved.

Heavy metal assessment of marine sediment in selected coastal districts of the Western Region, Ghana

  • A preliminary investigation to establish the status of contamination of trace metals in the Western coast of Ghana was conducted prior to the commercial production of crude oil in the area. The study revealed the presence of heavy metals such as Pb (4.00-79.64 mg/kg), As (8.81-236 mg/kg), Cu (12.86-108.06 mg/kg), V (28.07-953.32 mg/kg), Zn (7.08-264.25 mg/kg), Cr (101.69-1366.62 mg/kg), Ni (42.41-451.43 mg/kg), Mn (16.77-1890.45mg/kg), Br (7.66-142.78 mg/kg), Ti (542.03-19960 mg/kg) and Fe (7472.88-97120 mg/kg) at six sites sampled along the coast. With the exception of Ti and Fe which showed no variation in metal concentration, the rest of the metals varied significantly among the sampled locations. Potential ecological risk of metals particularly of Co, As and Br which exhibited extreme enrichment of the sediments indicates considerable metal pollution in the studied areas. The degree of contamination is of particular concern especially to benthic biota that inhabit this environment for survival.
