접착제 도포후 오염된 표면의 처리방법에 따른 복합레진의 전단결합강도와 미세누출 (SHEAR BOND STRENGTH AND MICROLEAKAGE OF COMPOSITE RESIN ACCORDING TO TREATMENT METHODS OF CONTAMINATED SURFACE AFTER APPLYING A BONDING AGENT)

  • The purpose of this study was to investigate the shear bond strength and marginal microleakage of composite to enamel and dentin according to different treatment methods when the applied bonding agent was contaminated by artificial saliva. For the shear bond strength test, the buccal and occlusal surfaces of one hundred twenty molar teeth were ground to expose enamel(n=60) and dentin surfaces(n=60). The specimens were randomly assigned into control and 5 experimental groups with 10 samples in each group. In control group, a bonding system(Scotchbond$^{TM}$ Multi-Purpose plus) and a composite resin(Z-100$^{TM}$) was bonded on the specimens according to manufacture's directions. Experimental groups were subdivided into 5 groups. After polymerization of an adhesive, they were contaminated with at artificial saliva on enamel and dentin surfaces: Experimental group 1 ; artificial saliva was dried with compressed air. Experimental group 2 ; artificial saliva was rinsed with air-water spray and dried. Experimental group 3 ; artificial saliva was rinsed, dried and applied an adhesive. Experimental group 4 ; artificial saliva was rinsed, dried, and then etched using phosphoric acid followed by an adhesive. Experimental group 5, artificial saliva was rinsed, dried, and then etched with phosphoric acid followed by consecutive application of both a primer and an adhesive. Composite resin(Z-100$^{TM}$) was bonded on saliva-treated enamel and dentin surfaces. The shear bond strengths were measured by universal testing machine(AGS-1000 4D, Shimaduzu Co. Japan) with a crosshead speed of 5mm/minute under 50kg load cell. Failure modes of fracture sites were examined under stereomicroscope. The data were analyzed by one-way ANOVA and Tukey's test. For the marginal microleakage test, Class V cavities were prepared on the buccal surfaces of sixty molars. The specimens were divided into control and experimental groups. Cavities in experimental group were contaminated with artificial saliva and those surfaces in each experimental groups received the same treatments as for the shear test. Cavities were filled with Z-100. Specimens were immersed in 0.5% basic fuchsin dye for 24 hours and embedded in transparent acrylic resin and sectioned buccolingually with diamond wheel saw. Four sections were obtained from the one specimen. Marginal microleakages of enamel and dentin were scored under streomicroscope and averaged from four sections. The data were analyzed by Kruskal-Wallis test and Fisher's LSD. The results of this study were as follows. 1. The shear bond strength to enamel showed lower value in experimental group 1(13.20${\pm}$2.94MPa) and experimental group 2(13.20${\pm}$2.94MPa) than in control(20.03${\pm}$4.47MPa), experimental group 4(20.96${\pm}$4.25MPa) and experimental group 5(21.25${\pm}$4.48MPa) (p<0.05). 2. The shear bond strength to dentin showed lower value in experimental group 1(9.35${\pm}$4.11MPa) and experimental group 2(9.83${\pm}$4.11MPa) than in control group(17.86${\pm}$4.03MPa), experimental group 4(15.04${\pm}$3.22MPa) and experimental group 5(14.33${\pm}$3.00MPa) (p<0.05). 3. Both on enamel and dentin surfaces, experimental group 1 and 2 showed many adhesive failures, but control and experimental group 3, 4 and 5 showed mixed and cohesive failures. 4. Enamel marginal microleakage was the highest in experimental group 1 and there was a significant difference in comparison with other groups (p<0.05). 5. Dentin marginal microleakages of experimental group 1 and 2 were higher than those of other groups (p<0.05). This result suggests that treatment methods, re-etching with 35% phosphoric acid followed by re-application of adhesive or repeating all adhesive procedures, will produce good effect on both shear bond strength and microleakage of composite to enamel and dentin if the polymerized bonding agent was contaminated by saliva.

브라켓 부착 시 실런트 레진이 법랑질 탈회에 미치는 영향 (The effects of a sealant resin on enamel demineralization in orthodontic bracket bonding)

  • 본 연구는 교정용 브라켓 부착 시 사용되는 접착제 중 two paste type의 실런트 레진 적용이 법랑질 탈회에 미치는 영향을 알아보고자 시행되었다. 건전한 표면을 가진 발치된 소의 상악절치 48개를 실험재료로 하여 법랑질 처리 방법에 따라 4개 군으로 분류하고 각 군을 Phase II (Reliance, Itasca III)를 이용하여 치면처리 하였다. 대조군으로서 아무런 처리를 하지 않은 Group 1.37% 인산으로 30초간 산부식을 시행한 Group 2, 산부식 후 실런트 A와 B를 동량으로 혼합하여 도포한 Group 3과 산부식 후 실런트를 도포하고 레진 페이스트 A와 B를 동량으로 혼합하여 도포한 Group 4로 분류하여 각 군을 인공탈회용액에서 탈회시킨 다음 공초점 레이저 주사현미경을 이용하여 각각의 탈회된 깊이를 측정한 후 비교한 결과, 탈회된 법랑질의 평균 깊이는 $47.4{\mu}m$ (Group 1). $61.8{\mu}m$ (Group 2). $13.9{\mu}M$(Group 3) $8.2{\mu}n$ (Group 4) 로 나타났다. 산부식 후 인공탈회용액에 노출되는 군 (Group 2) 은 산부식 없이 노출도는 군 (Group 1) 센 비해 탈회된 깊이가 더 깊은 것으로 나타났으며 (p<0.05) 실런트 레진을 도포한 군 (Group 3: 은 산부식 없이 노출되는 군 (Group 1)이나 산부식 후 노출되는 군 (Group 2) 에 비해 탈회된 법랑질 깊이가 유의성 있게 감소하였다. (p<0.05). 접착레진으로 부착된 군 (Group 4)은 법랑질 탈회가 거의 나타나지 않았다. 이상의 연구결과는 교정치료 시 법랑질 탈회 가능성을 줄이기 위해서 브라켓 부착 시 실런트 레진의 도포가 유용함을 시사하였다.

Nd : YAG 레이저가 상아질 및 도재와 복합레진간의 결합강도에 미치는 영향 (AN EFFECT OF ND : YAG LASER ON THE BONDING STRENGTH OF COMPOSITE RESIN TO DENTIN AND PORCELAIN)

  • The purpose of this experiment was to determine the effects of etching with a Nd : YAG Laser on dentin, or porcelain surface on the bond strength with composite resin. The dentin specimens were devided into the following 4 groups. D1 : No treatment D2 : Etched with 10% phosphoric acid D3 : Laser etchded with 1W, 20PPs D4 : Laser etched with 2W, 20PPS The procelain specimens were devided into the following 4 groups. P1 : diamond roughened P2 : etched with HF acid P3 : Laser etched with 2W, 20PPS P4 : Laser etched with 3W, 20PPS All specimens were veneered with resin. One half of the specimens were stored in $37^{\circ}C$ water for one day and the other half were thermocycled 1000 times at temperature of $5^{\circ}C\;to\;55^{\circ}C$ at 20 seconds intervals. After that, the bonding strength of composite resin to the dentin and porcelain was measured. The surface treated state and fractured state were observed with SEM. The following results were obtained. 1. In the dentin specimens, the bond strength of group D2 was highter than that of groups D1 and D3 in the case of the specimens stored in $37^{\circ}C$ water for one day, there was a statistically significant difference between group D2 and D1, D3 (P<0.05). The bonding strength of the specimens that were thermocycled decreased in the following order : group D2,D4,D3 and then D1. 2. In the porcelain specimens, the bonding strength of groups P1,P2 were higher than that of group P3 in the case of the specimens stored in $37^{\circ}C$ water for one day (P<0.05). The bonding strength of the specimens of being thermocycled decreased in the following order : group P2,P1,P4 and then P3. 3. The groups of high bond strength had a rougher surface and a high level of microporosity with SEM findings.

Bond strength of self-adhesive resin cements to composite submitted to different surface pretreatments

  • Objectives: Extensively destroyed teeth are commonly restored with composite resin before cavity preparation for indirect restorations. The longevity of the restoration can be related to the proper bonding of the resin cement to the composite. This study aimed to evaluate the microshear bond strength of two self-adhesive resin cements to composite resin. Material and Methods: Composite discs were subject to one of six different surface pretreatments: none (control), 35% phosphoric acid etching for 30 seconds (PA), application of silane (silane), PA + silane, PA + adhesive, or PA + silane + adhesive (n = 6). A silicone mold containing a cylindrical orifice ($1mm^2$ diameter) was placed over the composite resin. RelyX Unicem (3M ESPE) or BisCem (Bisco Inc.) self-adhesive resin cement was inserted into the orifices and light-cured. Self-adhesive cement cylinders were submitted to shear loading. Data were analyzed by two-way ANOVA and Tukey's test (p < 0.05). Results: Independent of the cement used, the PA + Silane + Adhesive group showed higher microshear bond strength than those of the PA and PA + Silane groups. There was no difference among the other treatments. Unicem presented higher bond strength than BisCem for all experimental conditions. Conclusions: Pretreatments of the composite resin surface might have an effect on the bond strength of self-adhesive resin cements to this substrate.

평활면 초기 우식병소의 표면처리에 따른 조직상 및 접착제의 침투 양상 비교 (HISTOLOGIC FEATURE AND INFILTRATION OF ADHESIVE RESIN ACCORDING TO PRETREATMENT ON PROXIMAL EARLY CARIES LESION)

  • 초기 법랑질 우식은 환자의 구강 위생 증진 및 국소적 불소 도포를 통해 재광화가 가능하나 이는 환자의 협조도에 전적으로 의존하게 되어 있어 임상적 효과를 확신하기 힘들다. 그 대안으로 병소 진행의 보다 초기 단계에 병소의 미세 다공구조를 광중합 레진으로 전색하는 시도가 행해져 오고 있다. 그러나 법랑질 초기 우식의 표층은 병소 본체에 비해 상대적으로 낮은 세공 용적으로 인하여 접착레진의 침투를 방해할 수 있다. 따라서 적절한 표면 처리를 통한 표층의 일부 혹은 전체의 제거가 접착레진의 침투에 중요하다. 그러나 아직까지 자연적인 법랑질 초기 우식 병소의 표면 처리에 관한 연구는 부족한 실정이다. 이에 본 연구에서는 평활면 법랑질 초기 우식에 대한 접착레진 적용 전 적절한 표면 처리 방법을 알아보고자 시행되었다. 인접면에 법랑질 초기 우식을 보이는 39개의 발거된 유구치를 각기 다른 방법의 표면처리를 시행한 바, 수세만 시행한 대조군인 1군, 15초간 15% 염산 처리한 2군, 15초간 35% 인산 처리한 3군, 30초간 35% 인산 처리한 4군, 0.5% 차아염소산나트륨으로 세척한 5군의 5개 군으로 이루어졌다. 각 군당 3개의 치아는 주사전자현미경으로 관찰하고, 나머지 24개에는 접착레진을 도포하고 그 절단 시편을 공초점 레이저 주사현미경으로 관찰하여 다음과 같은 결론을 얻었다. 1. 주사전자현미경 관찰 결과, 2군에서 가장 명백한 표층 제거가 관찰되었고, 3군과 4군에서는 일부 불규칙적인 표층 제거, 5군에서는 미약한 표층 제거가 나타났다. 2. 각 군의 평균 침투 깊이는 $6.85{\sim}23.09{\mu}m$로 측정되었으며, 침투 깊이의 군간 비교에서는 2군에서 가장 크게, 이어 4군, 3군, 5군, 1군의 순이었으며, 5군을 제외한 모든 실험군에서는 대조군에 비해 크게 나타났다.   • During bonding procedure of composite resin, the prepared cavity can be contaminated by saliva. In this study, marginal microleakage and shear bond strength of a composite resin to primed enamel and dentin treated with artificial saliva(Taliva$^{(R)}$) were evaluated. For the marginal microleakage test, Class V cavities were prepared in the buccal surfaces of fifty molars. The samples were randomly assigned into 5 groups with 10 samples in each group. Control group was applied with a bonding system (Scotchbond$^{TM}$ Multi-Purpose plus) according to manufacture's directions without saliva contamination. Experimental groups were divided into 4 groups and contaminated with artificial saliva for 30 seconds after priming: Experimental 1 group ; artificial saliva was dried with compressed air only, Experimental 2 group ; artificial saliva was rinsed and dried. Experimental 3 group ; cavities were etched with 35% phosphoric acid for 15 seconds after rinsing and drying artificial saliva. Experimental 4 group ; cavities were etched with 35% phosphoric acid for 15 seconds and primer was reapplied after rinsing and drying artificial saliva. All the cavities were applied a bonding agent and filled with a composite resin (Z-100$^{TM}$). Specimens were immersed in 0.5% basic fuschin dye for 24 hours and embedded in transparent acrylic resin and sectioned buccolingually with diamond wheel saw. Four sections were obtained from one specimen. Degree of marginal leakage was scored under stereomicroscope and their scores were averaged from four sections. The data were analyzed by Kruscal-Wallis test and Fisher's LSD. For the shear bond strength test, the buccal or occlusal surfaces of one hundred molar teeth were ground to expose enamel(n=50) or dentin(n=50) using diamond wheel saw and its surface was smoothed with Lapping and Polishing Machine(South Bay Technology Co., U.S.A.). Samples were divided into 5 groups. Treatment of saliva-contaminated enamel and dentin surfaces was same as the marginal microleakage test and composite resin was bonded via a gelatin capsule. All specimens were stored in distilled water for 48 hours. The shear bond strengths were measured by universal testing machine (AGS-1000 4D, Shimaduzu Co., Japan) with a crosshead speed of 5 mm/minute. Failure mode of fracture sites was examined under stereomicroscope. The data were analyzed by ANOVA and Tukey's studentized range test. The results of this study were as follows : 1. Enamel marginal microleakage showed no significant difference among groups. 2. Dentinal marginal microleakages of control, experimental 2 and 4 groups were lower than those of experimental 1 and 3 groups (p<0.05). 3. The shear bond strength to enamel was the highest value in control group (20.03${\pm}$4.47MPa) and the lowest value in experimental 1 group (13.28${\pm}$6.52MPa). There were significant differences between experimental 1 group and other groups (p<0.05). 4. The shear bond strength to dentin was higher in control group (17.87${\pm}$4.02MPa) and experimental 4 group (16.38${\pm}$3.23MPa) than in other groups, its value was low in experimental 1 group (3.95${\pm}$2.51 MPa) and experimental 2 group (6.72${\pm}$2.26MPa)(p<0.05). 5. Failure mode of fractured site on the enamel showed mostly adhesive failures in experimental 1 and 3 groups. 6. Failure mode of fractured site on the dentin did not show adhesive failures in control group, but showed mostly adhesive failure in experimental groups. As a summary of above results, if the primed tooth surface was contaminated with artificial saliva, primer should be reapplied after re-etching it.

도말층 존재 유무에 따른 One-step 접착 시스템의 미세인장결합강도 (MICROTENSILE BONDING OF ONE-STEP ADHESIVES TO SHEARED AND NON-SHEARED DENTIN)

  • The purposes of this study were to evaluate the microtensile bond strength of one-step adhesives accord ing to various dentin surface treatments and to observe the interface between resin(Z-100$^{TM}$) and dentin under SEM. In this study forty-five non-caries extracted human molars and three adhesive systems were used ; AlI-Bond 2(AB), One-Up Bond F(OU), AQ-Bond(AQ). ; In Group 1, 2, 3, AB was used and tooth surfaces were treated by smearing(S), ultrasonic cleansing(US), etching(E) respectively. In Group 4. 5, 6, One-Up Bond F was used and tooth surfaces were also treated as the same way above. In Groups 7, 8, 9, AQ Bond was used and tooth surfaces wet$.$e treated as the same way. Each specimen was prepared for microtensile bond testing, and were stored for 24hrs in 37$^{\circ}C$ distilled water. After that, microtensile bond strength for each specimen was measured. Specimens were fabricated to examine the failure patterns of interface between resin and dentin and observed under the SEM. The results were as follows ; 1. The results(mean$\pm$SD) of microtensile test were group 1, 25.69$\pm$4.31MPa; group 2, 40.93$\pm$10.94MPa; group 3, 47.65$\pm$8.85MPa; group 4, 35.98$\pm$9.14MPa; group 5, 39.66$\pm$8.45MPa; group 6, 43.26$\pm$13.01MPa; group 7, 25.07$\pm$4.2MPa;group 8, 30.4$\pm$4.74MPa;group 9, 33.61$\pm$7.88MPa. 2. One-Up Bond F was showed the highest value of 36.98$\pm$9.14MPa in dentin surface treatment with smearing, and there were significant differences to the other groups (p<0.05). 3. All-Bond 2 was showed the highest value of 40.93$\pm$10.94MPa in dentin surface treatment with ultra-sonic cleansing, but was no significant difference to One-Up Bond F(p>0.05) 4. All-Bond 2 was showed the highest value of 47.65$\pm$8.85MPa in dentin surface treatment with etch ing(10%phosphoric acid), and there were significant differences to the other groups(p<0.05). 5. All-Bond 2 was showed the highest value of 47.65$\pm$8.85MPa in dentin surface treatment according to manufacture's directions. but was no significant difference to One-Up Bond F(p>0.05). 6. AQ Bond was skewed the lowest microtensile bond strength with various dentin surface treatment, and the were significant differences to the other groups(p<0.05).

상아질 손상 후 흰쥐 대구치 치수의 calcitonin gene-related peptide(CGRP) 함유 신경섬유 분포에 관한 연구 (A STUDY ON THE DISTRIBUTION OF CALCITONIN GENE-RELATED PEPTIDE CONTAINING NERVE FIBERS IN RAT PULP FOLLOWING DENTINAL INJURY)

  • The purpose of this study was to investigate the distribution of calcitonin gene-related peptide containing nerve fibers in rat pulp after dentinl injury by means of immunohistochemistry and confocal laser scanning microscope. The Spague-Dawley rats weighing about 250-300gm were used. The animals were devided into normal control and experimental groups. Experimental animals were sacrified 1, 2, 4, 7, 10, 21days after dentinal injury (dentin cutting, and then acid etching with 35% phosphoric acid) on the maxillary molar teeth. The maxillary teeth and alveolar bone were removed and immersed in the 4% paraformaldehyde in 0.1M phosphate buffer (pH 7.4), then were decalcified with 15% formic acid for 10 days. Serial frozen $50{\mu}m$ thick sections were cut on a cryostat. The rabbit CGRP antibody was used as a primary antibody with a dilution of 1:2000 in 0.01M PB. The sections were incubated for 48 hours at $4^{\circ}C$, and placed into biotinylated antirabbit Ig G as a secondary anti body with dilution of 1:200 in 0.01M PB and incubated in ABC(avidin-biotin complex). The peroxidase reaction was visualized by incubating the sections in 0.05% 3,3 diaminobenzidine tetrahydrochloride containing 0.02% $H_2O_2$. For the confocal laser scanning microscopic examination, Primary antibody reaction was same as immunoperoxidase stainning, but fluorescein isothiocyanate(FITC)-conjugate antirabbit IgG as a secondary antibody was used. The confocal laser scanning microscope was used for the examination. A series of images of optical sections was collected with a 20x objective at $3{\mu}m$ intervals throughout the depth of specimen. FITC fluerescence was registrated through a 488nm and 568nm excitation filter, and images were saved on optical disk. The stereoscopic images and three dimentionnal images were reconstructed by computer software, and then were analyzed. The results were as follows : 1. In normal control group, CGRP containing nerve fibers were coursed through the root with very little branching, and then formed a dense network of terminals in coronal pulp. 2. A slight increase in CGRP containing nerve fibers at 1 and 2day postinjury was noted subjacent to the injury site. In the 4day group, there were an extensive increase in the number of reactive fibers, followed by a partial return toward normal levels at 7~10 day postinjury, and return by 21days. 3. The sprouting of the CGRP containing nerve fibers was evident within 2day after dentinal injury, and by 4days there was a maximal increased, but was decreased at 7days and returned to normal 10~21 day postinjury. 4. In confocal laser scanning microscopic exammination, the distinct distribution pattern and sprouting reaction of CGRP containing nerve fibers were observed in stereoscopic images and three dimentional images. These results suggest that CGRP containing nerve fiber can be important role in the response to dental injury and pain regulation.

Load cycling에 따른 소수성 실험용 상아질 접착제의 nanoleakage 양상 (The nanoleakage patterns of experimental hydrophobic adhesives after load cycling)

  • 본 연구의 목적은 두 가지 실험용 소수성 상아질 접착제와 전통적인 3-step 상아질 접착제의 nanoleakage 양상을 load cycling 전, 후에 비교하여 상아질 접착제의 내구성을 예측해 보고자 하는 것이다. 두 가지 실험용 소수성 상아질 접착제 즉, 에탄을 포함 상아질 접착제, 메탄을 포함 상아질 접착제를 만들었다. 대구치 30개의 치관부 3분의 1을 절단하고 임의로 3군으로 나누어 각각 Scotchbond Multi-Purpose (3M ESPE, St. Paul, MN, USA), 에탄올 포함 상아질 접착제 및 메탄을 포함 상아질 접착제를 사용하여 상아질 접착 후, 복합레진으로 치관부를 수복하였다. 각각의 접착 시스템을 Load cycling 여부에 따라 두 군으로 나누고 각각의 치아를 접착 계면에 수직으로, 약 2.0 mm의 두께가 되도록 절단하여 치아마다 2개의 시편을 얻어 각 군 당10개의 시편을 얻었다. 시편을 50 % ammoniacal silver nitrate를 이용하여 염색한 후 주사전자현미경을 이용하여 혼성층의 nanoleakage를 관찰하였다. 은 침착의 분포는 image analysis software (Scion Image Beta 4.03, Scion Corp., Frederick, MD, USA)를 이용하여 gray value로 계산하였고 이 원분산 분석법으로 통계처리 하였다. 3종의 상아질 접착제에서 모두 nanoleakage가 관찰되었으나, 에탄올 포함 상아질 접착제와 메탄을 포함 상아질 접착제에서 Scotchbond Multi-Purpose 보다 적은 양의 nanoleakage 가 관찰되었다 (p < .0001). 각각의 상아질 접착제에서 load cycling에 따른 nanoleakage 양상의 변화는 관찰되지 않았다. 이상의 결과에서 친수성기를 줄이고 소수성기를 극대화 한 실험용 상아질 접착제가 접착 계면의 내구성 향상에 도움을 줄 수 있을 것으로 사료된다.
