Arthroscopic Stabilization for Displaced Lateral Clavicular Fractures: Can It Restore Anatomy?

  • Background: The purpose of our study was to evaluate the accuracy of reduction based on postoperative computed tomography (CT) images after arthroscopic stabilization using tightrope system for unstable distal clavicle fracture. Methods: Twelve patients with distal clavicle fracture combined with coracoclavicular (CC) ligament injury (type II, V) who received arthroscopically assisted fixation using a flip button device were evaluated for accuracy of reduction using 3-dimensional postoperative CT scan by measuring the degree of distal clavicular angulation and clavicular shortening. Results: Immediate postoperative plain radiograph confirmed restoration of the CC distance (CCD) in 10 patients. At final follow-up, the CCD remained reduced anatomically on plain radiographs in these patients. All patients showed excessive posterior angulation and shortening compared to the opposite side. The average Constant score recovered to 94.8 at final follow-up. Conclusions: Indirect reduction and arthroscopic subacromial approach with flip button fixation of unstable distal clavicle fractures demonstrated favorable clinical results despite unavoidable posterior angulation of distal clavicle and shortening the total length of clavicle.

Combining Hough Transform and Fuzzy Unsupervised Learning Strategy in Automatic Segmentation of Large Bowel Obstruction Area from Erect Abdominal Radiographs

  • The number of senior citizens with large bowel obstruction is steadily growing in Korea. Plain radiography was used to examine the severity and treatment of this phenomenon. To avoid examiner subjectivity in radiography readings, we propose an automatic segmentation method to identify fluid-filled areas indicative of large bowel obstruction. Our proposed method applies the Hough transform to locate suspicious areas successfully and applies the possibilistic fuzzy c-means unsupervised learning algorithm to form the target area in a noisy environment. In an experiment with 104 real-world large-bowel obstruction radiographs, the proposed method successfully identified all suspicious areas in 73 of 104 input images and partially identified the target area in another 21 images. Additionally, the proposed method shows a true-positive rate of over 91% and false-positive rate of less than 3% for pixel-level area formation. These performance evaluation statistics are significantly better than those of the possibilistic c-means and fuzzy c-means-based strategies; thus, this hybrid strategy of automatic segmentation of large bowel suspicious areas is successful and might be feasible for real-world use.

Non-osseous Coalition of the Third Metatarsal-Lateral Cuneiform Joint: A Case Report (제 3중족-외측 설상 관절에서 발생한 비골성 족근골 결합: 증례 보고)

  • This paper reports a rare case of the symptomatic third metatarsal (MT3) - lateral cuneiform (LC) in a 55-year-old male who presented with complaints of severe intermittent pain in his right foot. Plain radiographs and computed tomography scans revealed sclerosis and irregularity at this joint. The intraoperative findings demonstrated a fibrocartilaginous coalition. The pain had improved one year after removing the MT3-LC joint by en bloc and arthrodesis.

Comparison of Subchondral Fracture on Radiographs with MR Images in Evaluation of Osteonecrosis in LCP Patients (LCP 환아에서 골괴사의 평가방법으로 연골하 골절선과 자기공명영상의 비교)

  • Purpose : To compare the subchondral fracture on plain radiography and MR image as a method for assessing osteonecrosis in Legg-Calve-Perthes(LCP) pateients. Materials and methods: We retrospetively reviewed 15 hip joint MR images and plain radiography which visualized subchondral fracture. With basis of the Salter-Thompson classification, extent of necrosis was graded group A to B, as follows; Group A = < 50%, B = > 50%. On PACS workstation, necrotic area of each MR image was measured to calculate the volume of necrotic portion: volume = necrotic area $\times$ slice thickness. Necrotic areas on MR images were graded group A to B and results were compared with that measured in Salter-Thompson classification. On follow up, bone resorption was measured and the extent was compared with subchondral fracture representing necrotic area and that on MR volume method respectively. Results : In 9 joints of 15 hip joints (60%), the degree of necrosis in Salter-Thompson classification on plain radiographs was different from that on MR volume method. Based on plain radiographs by Salter-Thompson classification, the degree of necrosis was overestimated in 6(67%) joints, and underestimated in 3(33%) joints compared with MR volume method. On follow up study, bone resorption was not correlated with necrotic extent of subchondral fracture and MR volume method. Conclusion : The extent of femoral head necrosis measured by subchondral fracture was different from that measured by MR and was not correlated with bone resorption on follow up. Therefore, usefulness of subchondral fracture as a prognostic factor may be limited.

  • We experienced a rare case of parosteal lipoma, which located on the periosteum of the foot 4th metatarsus. A 22-year-old woman visited the hospital with painful mass in her foot. Based on the assessment of plain radiographs, computed tomography scan, and magnetic resonance image, it was suspected as lipoma. Marginal excision was performed and parosteal lipoma was confirmed histologically. Any local recurrence and complications were not observed in 2 years after surgery.

Performance Evaluation of Pixel Clustering Approaches for Automatic Detection of Small Bowel Obstruction from Abdominal Radiographs

  • Plain radiographic analysis is the initial imaging modality for suspected small bowel obstruction. Among the many features that affect the diagnosis of small bowel obstruction (SBO), the presence of gas-filled or fluid-filled small bowel loops is the most salient feature that can be automatized by computer vision algorithms. In this study, we compare three frequently applied pixel-clustering algorithms for extracting gas-filled areas without human intervention. In a comparison involving 40 suspected SBO cases, the Possibilistic C-Means and Fuzzy C-Means algorithms exhibited initialization-sensitivity problems and difficulties coping with low intensity contrast, achieving low 72.5% and 85% success rates in extraction. The Adaptive Resonance Theory 2 algorithm is the most suitable algorithm for gas-filled region detection, achieving a 100% success rate on 40 tested images, largely owing to its dynamic control of the number of clusters.

Color Doppler Ultrasonographic Evaluation in Observing the Affects of $Osteoset^{(R)}$ on Radial Defects in Beagle Dogs (개에서 $Osteoset^{(R)}$의 요골 결손부 충진에 대한 색도플러초음파검사의 유용성 평가)

  • This study is conducted to compare ultrasonographs with plain radiographs in monitoring bone regeneration during reconstruction of critical sized radial defects in dogs. A 15 mm bony defect was taken on each of the eight dog's radius using an electrical saw and an external fixator was applied. The experimental groups were divided into non-treated group(group 1) and $Osteoset^{(R)}$-treated group(group 2). Each fracture site was evaluated using plain radiography and ultrasonography. Radiographic callus formation occurred after $11.50{\pm}1.12$ days in group 1 and $11.50{\pm}0.5$ days in group 2. Neovascularized flow signal could be seen $6.50{\pm}1.5$ days and the vascular signal disappeared after $45.00{\pm}6.16$ days after operation in group 1. Neovascularized flow signal was observed $6.75{\pm}1.78$ days and vascular signal disappeared $23.25{\pm}3.03$ days after surgery which was caused by acoustic shadowing in group 2. Early stages of regeneration were observed more clearly with color Doppler ultrasonography than with plain radiography. Also from the results it is concluded that color Doppler ultrasonography are useful in observing initial stages of bone repair.

MR Findings of Pigmented Villonodular Synovitis : Case Report (색소융모결정성 활막염의 자기공명영상 : 증례 보고)

  • Pigmented villonodular synovitis is a rare proliferating process of the synovium, tendon sheaths and bursae usually affecting the bone and joints. The disease can be localized or diffuse. Patients with this condition typically present with symptoms of mild discomfort and associated swelling of the involved joint. However, the spectrum of presentations is broad. Diagnosis of Pigmented villonodular synovitis can be clinically difficult, and plain radiographs are usually nonspecific. Magnetic resonance imaging is a highly diagnostic modality in characterizing pigmented villonodular synovitis when it contains hemosiderin deposits exhibiting low signal intensity on all pulse sequences. Magnetic resonance imaging is recommended for accurate preoperative staging of the disease and for follow up after treatment. I report a case of pigmented nodular synovitis in the knee joint, with review of literatures.

  • A 4-month-old, intact male, Tosa with a history of a regurgitation, vomiting, and weight loss for three weeks was presented to Animal Medical Center, Chonbuk National University. In Serial plain radiographs, a severely distended stomach was seen and ultrasonogram revealed a nonfunctional pylorus with normal layer comparable with an obstruction of pyloric region by pyloric achalasia. An esophagram and endoscopy revealed normal peristalsis with failure of the lower esophageal sphincter to open, supporting the diagnosis of esophageal achalasia. Megaesophagus was observed on reradiograph and esophagram 11 days later. The clinical signs and esophageal dilation were resolved without resorting to any treatment.

  • Naviculocuneiform coalition is one of uncommon tarsal coalitions and especially symptomatic cases which need operative treatment are rare. Authors report 2 cases of pediatric naviculocuneiform patients who showed symptomatic condition as mainly pain. Plain radiographs, computed tomography or magnetic resolution imaging study showed bony bridge in naviculo-medial cuneiform joint. After over six months conservative treatment, excision of coalition and interposition $Tisseel^{(R)}$ was performed for motion preservation and relief of pain.
