The Correlation Analysis of BMD in Proximal Femur and Spine with Dual Energy X-ray Absorptiometry (이중에너지 X-ray 흡수법을 적용한 근위 대퇴골 및 요추부 골밀도 검사의 상관관계 분석)

  • To analogize the result of the test through explaining the correlation of bone mineral density (BMD) test value between proximal femur and lumbar spine at age. It is based on 62 cases who visited the out-patient department. They were classified into three groups by age, group I (30~40 yr) and II (40~60 yr) and III (60~80 yr). Then we evaluated the average and the degree of correlation between the three groups and analyzed the correlation of the three sites according to the BMD result and T & Z-score through the t-test analysis. The results are listed below, first, if rise in age T-score is lower and over 60age occurred osteopenia, second, compared three groups, the correlation of BMD results was some of the highest between Lumbar spine and Proximal femur of T-score & Z-score. the correlation of BMD results is very high. It is regarded that there will be a clinical availability which can analogize the result of a part by using the result of the other part.

Correlation of Unmet Healthcare Needs and Employment Status for a Population over 65 Years of Age (65세 이상 인구의 고용형태와 의료요구 미충족 경험률의 관련성)

  • The present study aimed to investigate the rate of unmet healthcare needs for elderly over the age of 65 years, as well as analyze the relevance between employment status and unmet healthcare needs due to financial reasons. With regard to the study method, a logistic regression analysis was performed to investigate the correlation between employment status and unmet healthcare needs due to financial reasons, targeting 5,528 subjects over the age of 65 years. The results showed that the rate of unmet healthcare needs was 18.9%, in which the rate of unmet healthcare needs due to financial reason was 8.1%. The rate of unmet health needs was higher for temporary workers(ORs=1.75) than for retirement workers. However, the rate of unmet healthcare needs caused by financial reasons was higher among day workers(ORs=1.92). In conclusion, in order to prevent unmet healthcare needs for senior Korean patients, it is necessary to not only improve the income security system for the elderly, but also improve the occupational form and level of income of these economically active citizens, considering the increase in average life expectancy. Moreover, it is also necessary to reinforce health insurance coverage systems for settling medical expenses.

Chronic Dialysis in Infants and Children Under 2 Years of Age (2세 미만 만성 신부전 환아에서의 만성 투석)

  • Purpose : Dialysis in children with chronic renal failure presents with many difficulies. The purpose of this study is to find an improved method in chronic dialysis in infants and children less than 2 years of age by analyzing the experience with 10 cases. Methods : A retrospective review of the medical records of 10 patients(6 boys and 4 girls) was conducted. The patients had chronic renal failure and underwent chronic dialysis at Samsung medical center from March 1999 to February 2007. Results : At Initiation of dialysis, the median age was 3 months old(22 days-20 months), the median body weight was 3.75 kg(2.2-10.3 kg), and the median serum creatinine level was 4.3 mg/dL(2.0-11.4 mg/dL). The median duration of dialysis was 29.5 months(3-62 months). Dysplastic kidney disease was the most common underlying renal disease. Two patients were treated with hemodialysis, 4 patients with peritoneal dialysis, and 4 patients eventually switched dialysis modality. Nine of the 10 patients took erythropoietin and anti-hypertensive drugs. At the end of the follow up period, 1 patient received kidney transplantation, 2 patients died due to sepsis, and 5 patients were treated with peritoneal dialysis. Two patients were lost to follow up. The most common complication of dialysis was infection. Achieving vascular access and maintaining proper catheter function were the most important factors in treating patients with hemodialysis. The growth status of patients was aggravated after 6 month of dialysis but improved after 1 year of dialysis. Patients showed better growth on peritoneal dialysis than hemodialysis. Conclusion : Chronic dialysis can be performed successfully in infants and children under 2 years of age. Vascular access was the main limitation of hemodialysis, and infection was the common problem in both hemodialysis and peritoneal dialysis. To improve the patients survival rate and quality of life, major efforts should be directed toward the prevention of infection and preservation of catheter function. ASSESSMENT OF DENTAL ANXIETY IN THE CHILD PATIENT BY THEIR DRAWINGS (환아의 그림을 통한 치과치료시 불안도 평가)

  • The aim of the present study was to establish validity for Dental Anxiety Scale in children by their drawings. Dental treatment was provied for 35 children in the ages of 4 to 8, using the quadrant approach. The children's anxiety arising during dental treatment was measured by Modified Sheskin's Criteria(Dental Anxiety Scale, DAS). After reliability analysis of Dental Anxiety Score, it was compared with Faces Pain Scale and was tested for their correlation. Thereafter the several factors having an effect on Dental Anxiety Scale were examined. A significant correlation(r=0.2610) was found between the two scales and Dental Anxiety Scale was high significantly in 4-6 score of Faces Pain Scale. The findings suggested that the Dental Anxiety Scale is a valid means of assessing child dental anxiety status in a clinical context and the Dental Anxiety Scale can be affected by 'Age', 'Gender' and 'Reaction in the past'.

Impact of COVID-19 on Dental Trauma in Korea (국내에서 발생한 치과적 외상에 대한 코로나 바이러스 감염증-19의 영향)

  • The study was conducted to analyze the impacts of COVID-19 on the number of dental trauma patients. Based on the data provided by the Health Insurance Review and Assessment Service's Healthcare Bigdata Hub, dental trauma which occurred from 2010 to 2020 was analyzed. Since the outbreak of COVID-19, the incidence of dental trauma per 100,000 decreased compared to the average Incidence between 2017 to 2019. By age, it decreased by 5.4% (p = 0.017) for 0 - 4 years old, 30.3% (p < 0.001) for 5 - 9 years old, 39.5% (p < 0.001) for 10 - 14 years old, 14.5% (p = 0.002) for 15 - 19 years old, 1.3% for 20 - 29 years old, 0.2% for 40 - 49 years old, 2.7% for 50 - 59 years old, 1.2% for 60 years old or older, but it increased by 2.5% for 30 - 39 years old. Compared monthly, before and after the outbreak of COVID-19 confirmed patients, the number of dental trauma patients dropped sharply. After the outbreak of COVID-19, the incidence of dental trauma decreased significantly for under 20 years old, but the decrease was not significant for 20 years old or older.

Relationship Between Age and Pleural Fluid A denosine Deaminase Activity in Patients with Tuberculous Pleural Effusion (결핵성 흉수 환자에서 연령과 흉수 Adenosine Deaminase 활성도와의 관련성)

  • Background : ADA is an enzyme found in most cells, and is involved in purine metabolism, but its chief role concerns the proliferation and differentiation of lymphocytes, especially T-lymphocytes. For that reason ADA has been looked on as a marker of cell-mediate immunity, which is the key mechanism of the tuberculous pleural effusion. Thus, the pleural fluid ADA activity is increased in the tuberculous pleural effusion. Age associated immune decline is characterized by decreases in both B and T-lymphocyte function and the former may be largely a result of the latter. Therefore, the pleural fluid ADA activity would be lower in old rather than in young, patients with tuberculous pleural effusion. We studied the relationship between age, and pleural fluid ADA activity, in patients with tuberculous pleural effusion. Materials and Methods : In the 46 patients with tuberculous pleural effusion enroll in this study, the pleural fluid ADA activities were measured by means of an automated kinetic method. Results : The mean age of the patients was $53.0{\pm}22.0$ years, with a male to female ratio of 30:16. The patients were divided into two groups, young patients, regarded as < 65 and old regarded as ${\geq}65$ years with 28 and 18 patients, respectively. The pleural fluid ADA activity in both groups show significant differences : $99.4{\pm}22.6$ IU/L(young patients) Vs. $75.8{\pm}30.9$ IU/L(old patients)(p<0.05), but a negative correlation with age (r=-0.311, p<0.05). Conclusion : Although pleural fluid ADA activity was not adequately increased, tuberculous pleural effusion, in older patients, would have to be considered clinically suspicious tuberculous pleural effusion.

The Effect of the Intramural Marketing Activities in Dental Hygienist' on Job Satisfaction and Turnover Intention in Dental Hygienist (내부마케팅이 치과위생사의 직무만족과 이직의도에 미치는 영향)

  • The purpose of this study was to examine the impact of internal marketing on the job satisfaction and turnover intention of dental hygienists. A self-administered survey was conducted on the selected dental hygienists, and the statistical package SPSS 12.0 was employed to analyze the collected data. The findings of the study were as follows: 1. The performance of internal marketing took place more often among the dental hygienists who belonged to the other workplaces. Those who had three to less than five years of experience at their current workplaces were more aware of the importance of education and training. 2. The dental hygienists who were aged between 26 and 35 and who belonged to the other workplaces and who had five to less than 10 years of experience in total expressed the best job satisfaction. And the strongest turnover intention was found among those who had one to less than three years of experience in total or at the current workplaces. 3. As a result of analyzing the correlation of those variables, better performance of internal marketing led to better job satisfaction, and better performance of internal marketing and higher job satisfaction led to less turnover intention. 4. The factors of internal marketing performance that exerted an influence on job satisfaction were communication and welfare benefits, and the variables that affected turnover intention were education/training and the reward system that belonged to the performance of internal marketing.

Changing Patterns of Antibiotic-resistant Rates and Clinical Features in Pneumococcal Infections (폐구균의 질병양상 및 항균제 내성률의 변화)

  • Purpose : Streptococcus pneumoniae remains a leading cause of meningitis, sepsis, pneumonia, and otitis media in children worldwide. Emergence of drug-resistant organism has substantially complicated the therapy of these infections. This study was conducted to determine the clinical features and changing patterns of antibiotic-resistant rates of Streptococcus pneumoniae. Methods : We have retrospectively examined 306 cases of culture proven patients who were admitted to the Department of Pediatrics, Severance hospital, from the year 1991 to year 2000. The type of culture specimen used, presence of penicillin resistance, characteristics according to various presenting diseases and their prevailing year were also examined. Results : The mean age of cases was 7.9 year and the ratio of male to female was 1.6 : 1. The main age group of pneumococcal infection was under 2 years(42%). Systemic infections associated with pneumococci were sepsis(19.3%) and meningitis(9.5%), while local infections presented as pneumonia(29.2%), otitis media(19.3%), exudative tonsillitis(13.3%), and sinusitis(9.2%) in the order of frequency. Seasonal variation was seen in the incidence of pneumococcal infection: high incidence of infection was seen in Spring(Mar.~Apr; 32%), while the incidence was low during summer(Aug.~Sep.; 6%). Penicillin-resistant rate of pneumococci was steadily on the increase since the year 1991(65%) to year 2000(84%). Conclusion : Antibiotic-resistant pneumococci increased during the past decade. For effective prevention of pneumococcal infections, national survey of pneumococcal infections and expanded use of pneumococcal vaccination would be needed.

Nutritional Status of the Elderly Living in a Private Silver Town of Busan Metropolitan City, Korea (부산지역 실버타운 급식시설 이용 노인들의 영양섭취상태)

  • This study was performed to assess the nutritional status of the elderly residing in a silver town. A survey was conducted in April, 2005 for fifty two healthy persons (men 18, women 34) aged over 65 years living in a private silver town of Busan Metropolitan City, Korea. General data were collected using questionnaires at homes of the subjects and food consumption data for 3 days were obtained by investigating the individual leftovers from the meals served at the foodservice facilities by well-trained graduate students of the Department of Food & Nutrition. Nutrient intake was calculated from food consumption data using Can Pro (version 3.0, Korea). Average daily intake of energy was 1077.6 kcal in men and 974.2 kcal in women. Average protein intake of the men and women amounted to 85.5 g and 67.7 g, respectively. The consumption of cereals contributed 45.3% of energy intake for all subjects. Only 1.1% of calcium intake was from milk and milk products. Almost 100% of the elderly did not reach 75% of estimated energy requirement (EER) in energy intake and 75% of estimated average requirement (EAR) in folic acid intake. Percentages of the subjects who consumed protein above 125% of EAR were 100% and 91% in men and women, respectively. All subjects consumed above EAR in dietary intake of protein and iron. These data suggest that nutrient intake of the elderly living in the silver town would be inadequate, and thus the efficient management of foodservice is necessary to improve the nutritional status of the elderly living in the silver town.

A study of factors influencing precocious puberty (조기성조숙의 관련 인자 연구)

  • The purpose of this study was to evaluate the timing of precocious puberty and the factors inducing precocious puberty in elemental school girls students of low grades. The 1st, 2nd, and 3rd grade 253 elemental girls students from the Goryong province were randomly selected, G and D elemental school. sexual maturation rate was assessed physical examination by the parents. After obtaining an informed consent, a questionnaire was administered to the parents; sexual maturation rate was assessed by self physical examination, eating habits, lifestyle and home background were evaluated to determine the factors that induced precocious puberty. The data were statistically analyzed. We selected 253 girls: The total percentage of the precocious puberty was 26.1%. 7(7.8%) 6-year-old girls, 9 (13.2%) 7-year-old girls, 50 (52.6%) 8-year-old girls were in breast stage 2 and over. The main factors influencing precocious puberty were obesity scale, frequency of eating instant food, TV watching time, stress levels and broken family. A high rating on the obesity scale and high frequency of eating instant food, too much time in front of TV, high stress and broken family indicated advanced stage of puberty. This study show that prevalence ratio of the precocious puberty was 26.1%. The significant influencing factors in advanced puberty were obesity scale and frequency of instant food, TV watching time, high stress and broken family.
