Clinical Analysis of Pulmonary Resection Using Staplers (자동 조직 봉합기를 사용한 폐절제술의 임상적 고찰)

  • From 1991 to 1994, We performed 75 cases of pulmonary resection. These were divided into two groups according to the method of bronchial stump closure : 51 cases automatic staplers were a plied in 49 patients (Group 1), 24 patients were closed with manual interrupted suture (Group II). Disease entities of the patients were malignant tumor in 33 patients(Group I: Group II, 22· II, bronchiectasis in 23(18:5), benign tumor in 5(3:2), aspergilloma in 5(2:3), tuberculosis(2:1) in 3, bronchogenic cyst in 2 (0 : 2) and so on. Surgical Procedure% for these Patients were 21 Pneumonectomies(18:3), 13 bilobectomies(11:2), 26 lobectomles (14:12), 11 segmentectomies (6:5) and 4 lobectomy with segmentectomies (4:0). In conclusion, the Amount of tube drainage was smaller and the removal of chest tube after surgery was shorter than manual bronchial closure group by means of statistical significance (p=0.047, p=0.005). Although there were no statistical significance, the duration of air leakage was reduced and incidence of bronchopleural rstula was reduced in the stapler used group compared with manual bronchial closure.

Analysis of urine β2-microglobulin in pediatric renal disease (소아 신장질환에서 요 β2-microglobulin검사의 분석)

  • Purpose : There have been numerous researches on urine ${\beta}_2$-microglobulin (${\beta}_2$-M) concerned with primary nephrotic syndrome and other glomerular diseases, but not much has been done in relation to pediatric age groups. Thus, our hospital decided to study the relations between the analysis of the test results we have conducted on pediatric patients and renal functions. Methods : Retrospective data analysis was done to 102 patients of ages 0 to 4 with renal diseases with symptoms such as hematuria, edema, and proteinuria who were admitted to Chung-Ang Yongsan Hospital and who participated in 24-hour urine and urine ${\beta}_2$-M excretion test between January of 2003 and January of 2006. Each disease was differentiated as independent variables, and the statistical difference of the results of urine ${\beta}_2$-M excretion of several groups of renal diseases was analyzed with student T-test by using test results as dependent variables. Results : Levels of urine ${\beta}_2$-M excretion of the 102 patients were as follows : 52 had primary nephrotic syndrome [MCNS (n=45, $72{\pm}45{\mu}g/g$ creatinine, ${\mu}g/g-Cr$), MPGN (n=3, $154{\pm}415{\mu}g/g-Cr$), FSGS (n=4, $188{\pm}46{\mu}g/-Cr$], six had APSGN ($93{\pm}404{\mu}g/g-Cr$), seven had IgA nephropathy ($3,414{\pm}106{\mu}g/g-Cr$), 9 had APN ($742{\pm}160{\mu}g/g-Cr$), 16 had cystitis ($179{\pm}168{\mu}g/g-Cr$), and 12 had HSP nephritis ($109{\pm}898{\mu}g/g-Cr$). IgA nephropathy (P<0.05) and APN (P<0.05) were significantly higher than in other renal diseases. Among primary nephrotic syndrome, FSGS with higher results of ${\beta}_2$-microglobulin test had longer treatment period (P<0.01) when compared to the lower groups, but no significant differences in Ccr, BUN, or Cr were observed. Conclusion : IgA nephropathy and APN groups showed significantly higher level of ${\beta}_2$-M excretion value than other groups. Although ${\beta}_2$-microglobulin value is not appropriate as an indicator of general renal function and pathology, it seems to be sufficient in the differential diagnosis of the UTI and in the prediction of the treat-ment period of nephrotic syndrome patients.

Study of the Positive and Negative Caregiving Experiences in the family members who care for the psychiatric mentally ill relatives (정신장애인 가족의 긍정적, 부정적 돌봄의 경험에 관한 연구)

  • The caregiving experiences of 100 family menbers of outpatients with schizophrenia and schizoaffective disorders were investigated for the presence of positive(positive family-patient relationship. patient' contribution to the family) and negative caregiving experience(objective and subjective burden) and their predictors. This study attempts to make the analysis of caregiving experience more useful by expanding the focus to incoporate these positive aspects of the experience of family caregiver. Objective burden consists of two elements: 'disruption of family life', 'care'(amount of caregiving related to activity of daily living). Subjective burden is defined as emotional reactions to the care giving and it comprised of 6 emotional subdimensions such as 'stigma', 'grief'. 'worry', 'pity', 'fear', 'despair'. Also we investigate the severity of patients' disturbing behaviors into two categories, positive and negative disturbing behaviors and patient' contribution to the family as a predictors of positive and negative caregiving experiences. This study use Pearson's correlation coefficient, Hierardhical regressions in the SAS Program. The results are as follows: 1. Respondents reported moderate level of objective burden 'disruption of family life' (mean = 2.48, range = 1-4), and 'care' (mean=2.54, range = 1-4), and slightly high level of total subjective burden(mean = 2.19, range = 1-4). Mean scores for the measure of the severity of behavioral disturbance indicated that the caregiver experienced negative disturbing behaviors around almost 'somtimes'(mean=2.28, range = 1-4), and positive disturbing behaviors 'almost not frequent'(mean=2.78. range=1-4). So they reported that they perceived patient's negative disturbing behaviors more than positive disturbing behaviors. Mean scores for the measure of the patient' contributions (mean = 1.99. range = 1-4) indicated that caregivers experienced these contributions a little. It means that there should be a positive aspect of possibilities of patient' family roles that can be developed in the daily life. Mean scores for the measure of the positive family-patient relationship indicated that caregivers experienced moderate level of positive family-patient relationship(mean=2.52, range = 1-4). 2. Hierardhical regression analysis 1) Hierardhical regression of 'disruption of family life' showed that the interaction between positive disturbing behaviors and patient' contributions (B = .20. p = .022) and caregiver's educational level(B=.06. p=.000) were 'significant and Hierardhical regression of 'care' showed that 'negative disturbing behaviors'(B= .35. p= .007). 'patient' contributions'(B= .28, p= .019). 'family income'(B=-.l1. p=.096) were significant. 2) Hierardhical regression of 'total subjective burden', 'stigma', 'grief', 'worry', 'pity'. 'fear', 'dispair' showed that 'positive disturbing behaviors'(B=.51. p=.000). 'negative disturbing behaviors' (B = .17, p = .026), 'caregiver's educational level'(B = .03. p=.036), 'family income'(B=.08. p=.041) were significant predictors of 'total subjective burden': 'positive disturbing behaviors'(B=.32. p=.066). 'negative disturbing behaviors'(B=.24, p=.096) 'durations of illness'(B= .03. p= .079) were significant predictors of 'stigma' 'negative disturbing behaviors'(B=.28. p=.005). 'patient sex'(B=-.32. p=.022). 'positive disturbing behaviors'(B=.28. p=.020), 'patient age'(B=.02. p=.010), 'caregiver age'(B=-01, p= .002) were significant predictors of 'grief' 'negative disturbing behaviors'(B= .28, p= .005). 'patient sex'(B= -.32. p=.039), 'caregiver age'(B=-.02, p= .023). 'caregiver's educational level'(B= .04, p = .044) were significant predictors of 'worry' 'patient sex'(B=-.46. p=.005). 'negative disturbing behaviors'(B= .28. p=.018), 'caregiver age'(B=-.01, p=.037) were significant predictors of 'pity' 'positive disturbing behaviors'(B=.83. p=.000). 'patient' contributions' (B = .22, p =.017). 'family income'(B=.09. p=.65) were significant predictors of 'fear' 'positive disturbing behaviors'(B=.49, p=.001). Study on Biological Effect of Multi-Herbal Drug KOCO-P1 on Human Hepatocytes (HepG2) (인간 간조직세포(HepG2 Cells)에 대한 한약조성물 KOCO-P1의 효과 연구)

  • Objectives : The purpose of this study is to investigate the biological Effect of multi-herbal drug 'KOCO-P1' on human hepatocyte HepG2 cells. Methods : Multi-herbal drug 'KOCO-P1' was composed of Ginseng Radix, Astragali Radix, Polygonati Rhizoma, Liriopis Tuber, and Scrophulariae Radix. Cytotoxicity and cytoprotective activity of KOCO-P1 was verificated by MTT assay. And antioxidative effect of KOCO-P1 against EtOH, Nicotine was inspected by Hydroperoxide assay. Results : KOCO-P1 showed no cytotoxicity on HepG2 cells for 24, 48 hours. KOCO-P1 at 50 ${\mu}g/mL$ reduced the production of H2O2 in HepG2 cells by EtOH. KOCO-P1 at 50 ${\mu}g/mL$ reduced the production of $H_2O_2$ in $HepG_2$ cells by Nicotine. Conclusions : KOCO-P1 at the low concentration could be supposed to have antioxidative effect on human hepatocyte with no cytotoxicity.

Effects of Hormone and Na-Pyruvate on the In Vitro Maturation of Canine Oocytes (개 난자의 체외성숙에 미치는 호르몬과 Na-Pyruvate의 영향)

  • This study was conducted to examine the effects of hormone and sodium pyruvate on in vitro maturation of canine oocytes. Canine oocytes were collected from the ovaries of dogs and cultured in NCSU-37 medium with hormones and sodium pyruvate for 72 hr. Oocytes matured to the metaphase II (MII) stage were observed only from estradiol $17{\beta}\;(E_2)$, and the presence of gonadotropin did not improve the nuclear maturation. No oocytes were developed to the MII stage when $E_2$ was added to medium during the first 6 and 24 hrs of culture period. The presence of $E_2$ during the whole culture period enhanced the nuclear maturation to the MII stage (6.0%, P<0.05). High concentration of sodium pyruvate (2.5 mM) slightly enhanced the nuclear maturation to the metapahse I (HMI) stage, but not the MII stage. the result of the present study shows that the presence of $ E_2$ during the whole culture period of 72 hr enhances the maturation of canine oocytes to the M stage, but sodium pyruvate does not affect the nuclear maturation of the canine oocytes.

The association between dietary sodium intake and obesity in adults by sodium intake assessment methods: a review of systematic reviews and re-meta-analysis

  • BACKGROUND/OBJECTIVES: The scientific evidence of a sodium-obesity association is limited by sodium intake assessments. Our specific aim is to synthesize the association between dietary sodium intake and obesity across the sodium intake assessments as evidenced by systematic reviews in adults. SUBJECTS/METHODS: A systematic search identified systematic reviews comparing the association of dietary sodium intakes with obesity-related outcomes such as body mass index (BMI), body weight, waist circumference, and risk of (abdominal) obesity. We searched PubMed on October 24, 2022. To assess the Risk of Bias in Systematic Reviews (ROBIS), we employed the ROBIS tool. RESULTS: This review included 3 systematic reviews, consisting of 39 unique observational studies (35 cross-sectional studies and 4 longitudinal studies) and 15 randomized controlled trials (RCTs). We found consistently positive associations between dietary sodium intake and obesity-related outcomes in cross-sectional studies. Studies that used 24-h urine collection indicated a greater BMI for those with higher sodium intake (mean difference = 2.27 kg/m2; 95% confidence interval [CI], 1.59-2.51; P < 0.001; I2 = 77%) compared to studies that used spot urine (mean difference = 1.34 kg/m2; 95% CI, 1.13-1.55; P < 0.001; I2 = 95%) and dietary methods (mean difference = 0.85 kg/m2; 95% CI, 0.1-1.51; P < 0.05; I2 = 95%). CONCLUSIONS: Quantitative synthesis of the systematic reviews has shown that cross-sectional associations between dietary sodium intake and obesity outcomes were substantially different across the sodium intake assessments. We need more high-quality prospective cohort studies and RCTs using 24-h urine collection to examine the causal effects of sodium intake on obesity.

Application of array comparative genomic hybridization in Korean children under 6 years old with global developmental delay

  • Purpose: Recent advancements in molecular techniques have greatly contributed to the discovery of genetic causes of unexplained developmental delay. Here, we describe the results of array comparative genomic hybridization (CGH) and the clinical features of 27 patients with global developmental delay. Methods: We included 27 children who fulfilled the following criteria: Korean children under 6 years with global developmental delay; children who had at least one or more physical or neurological problem other than global developmental delay; and patients in whom both array CGH and G-banded karyotyping tests were performed. Results: Fifteen male and 12 female patients with a mean age of $29.3{\pm}17.6months$ were included. The most common physical and neurological abnormalities were facial dysmorphism (n=16), epilepsy (n=7), and hypotonia (n=7). Pathogenic copy number variation results were observed in 4 patients (14.8%): 18.73 Mb dup(2)(p24.2p25.3) and 1.62 Mb del(20p13) (patient 1); 22.31 Mb dup(2) (p22.3p25.1) and 4.01 Mb dup(2)(p21p22.1) (patient 2); 12.08 Mb del(4)(q22.1q24) (patient 3); and 1.19 Mb del(1)(q21.1) (patient 4). One patient (3.7%) displayed a variant of uncertain significance. Four patients (14.8%) displayed discordance between G-banded karyotyping and array CGH results. Among patients with normal array CGH results, 4 (16%) revealed brain anomalies such as schizencephaly and hydranencephaly. One patient was diagnosed with Rett syndrome and one with $M{\ddot{o}}bius$ syndrome. Conclusion: As chromosomal microarray can elucidate the cause of previously unexplained developmental delay, it should be considered as a first-tier cytogenetic diagnostic test for children with unexplained developmental delay.

Extraction of protein from defatted sesame meal using the enzyme from Bacillus sp. CW-1121 (Bacillus sp. CW-1121이 생성하는 단백 분해 효소를 이용한 참깨박 단백질의 용출)

  • To extract insoluble proteins of sesame meal residue by using microorganism, the sesame meal residue was treated with crude enzyme solution from Bacillus sp. CW-1121. It was found that the solubility reached to maximum at pH 7.5, $45^{\circ}C$. Under optimum condition, the nitrogen solubility with the enzyme solution from Bacillus sp. CW-1121 reached to 60% in 2 hours. Nitrogen solubility of protein from sesame meal showed minimum value at pH 4.5 and significantly increased above pH 6.0. When the protein from sesame meal extracted with Bacillus sp. CW-1121 was subjected to sodium dodecyl sulfate polyacrylamide gel electrophoresis, water soluble protein was showed 4 bands and salt soluble protein was showed 2 bands. The amino acid composition of water soluble protein, salt soluble protein and free amino acid indicated relatively high contents of serine (17.24 mg/g), glutamic acid (10.77 mg/g) and glutamic acid (6.55 mg/g). Specially, the contents of essential amino acids were high.

Biomedical Ethics Education for Nursing Students: The Effect on Awareness and Application of Nursing Code of Ethics, Consciousness of Biomedical Ethics and Moral Sensitivity (생명의료윤리 교육 프로그램이 간호 대학생의 간호사 윤리강령인식, 간호사 윤리강령 활용 정도, 생명의료 윤리 의식 및 도덕적 민감성에 미치는 효과)

  • Purpose: This study examines the effectiveness of an educational program of nursing biomedical ethics for nursing students to increase awareness of the Code of Ethics for Nurses, moral sensitivity, and application of the Code of Ethics for Nurses. Methods: A quasi-experimental research design and t-test were used. The experimental group (n=31) and control group (n=32) were second-year university nursing students in two major cities. The experimental group received 15 hours of nursing ethics education during the 15-week semester. The training was not provided to the control group. Results: The application of the Code of Ethics for Nurses increased significantly in the experimental group (t=-1.06, p=.017), nurses and patient (t=-2.23, p=.029), obligation of nurse as an expert (t=-2.08, p=.042), nurse and cooperator (t=-2.54, p=.014). The consciousness of biomedical ethics increased significantly in the experimental group (t=4.28, p=.021), newborn's right to live (t=-2.61, p=.011), euthanasia (t=-2.36, p=.021). Conclusion: The results of the study show that providing a nursing biomedical ethics program to nursing students is an effective method to enhance the application of the Code of Ethics for Nurses and Consciousness of Biomedical Ethics. Implementing an intervention program of the Code of Ethics for Nurses in the regular nursing curriculum may reduce conflicts involving ethical decision making by nurses.

Postprandial Hypotension and Heart Rate Variability in Older Adults (노인의 식후 저혈압과 심박동변이)

  • Purpose: A study was designed to compare heart rate variability (HVR) of older adults with postprandial hypotension (PPH) with older adults with no PPH. Methods: The samples were community-dwelling older adults (${\geq}$ 60 years old) with PPH (n=35) and without PPH (n=35). HRV was measured by 10 minutes of electrocardiography using Physiolab 400 and analyzed in time domain (SDNN) and with power spectral analysis in frequency domain (TP, LF, HF, LF/HF). Blood pressure was measured every 15 minutes before and until 120 minutes after the meal. Results: All HRV parameters were lower in the PPH group compared to those with no PPH. Specifically the measures of SDNN, TP, LF, and LF/HF of the PPH group were significantly lower (U=385.0, $p$=.008; U=361.0, $p$=.003; U=375.0, $p$=.005; U=368.0, $p$=.004, respectively). Further, the LF and LF/HF of SBP (${\geq}140$ mmHg) group and the LF/HF of DBP (${\geq}$ 90 mmHg) group were significantly lower ($x^2$=6.33, $p$=.042; $x^2$=33.10, $p$ <.001; $x^2$=11.03, $p$=.004, respectively). Conclusion: These findings indicate that the autonomic nervous functions in persons with PPH are less activated than those persons with no PPH. The findings contribute to an understanding of HRV and the association of autonomic nervous function with the mechanism of PPH.
