Effects of COVID-2019 on plastic surgery emergencies in Korea

  • Background: To fight the coronavirus disease 2019 (COVID-19) pandemic, many countries have implemented social distancing and lockdowns. We investigated the changes in the trauma patterns of emergency plastic surgeries in the midst of a pandemic. Methods: A retrospective review of the medical records of all patients treated for emergency plastic surgeries was performed at our hospital in Seoul. We conducted the analysis between March 1 and June 30, 2020, and compared the data obtained with that of the same period in 2019. We also investigated changes in trauma patterns according to the social distancing level from July 2020 to February 2021. Results: There was a total of 800 emergency plastic surgery patients from March to June 2020, which was less than the 981 in the corresponding period in 2019. The proportion of patients aged 7-17 years and ≥ 80 in 2020 showed a significant decrease. In 2020, patients presenting with facial trauma decreased and hand trauma, markedly laceration, increased significantly. In 2020, more injuries happened at home, whereas significantly fewer injuries happened on the streets. In 2020, slipping and sports injury decreased, whereas penetrating injury increased significantly. In the changes observed according to the social distancing level, there were significant differences in age classification, facial open wound, and the mechanism of injury. Conclusion: Social distancing has caused a change in emergency plastic surgeries. To ensure safe and appropriate treatment, strict epidemiologic workup and protective equipment are required.

Improvement of EIA Associated with Greenhouse Gases Subject Matter for the Preparedness of Post-2020 (Post-2020에 연계한 온실가스 항목의 환경영향평가 개선 방안)

  • In orderto cope with the post-2020 in accordance with the Paris Agreement, greenhouse gas (GHG)reduction in Environmental Impact Assessment (EIA) and its contributions to post-2020 were discussed. The 26 Environmental Impact Statement (EIS) administered by Geum-River Basin Environmental Office from 2010 to 2019 were analyzed for reviewing GHG mitigation measures. From the case study, it was found that the assessment of GHG emissions reduction and climate change adaptation were not appropriately performed. In this study, the following measures are proposed to improve the inappropriate assessment of 'GHG subject matter' associated with EIA according to post-2020, 1) allotment of enforced charge on GHG emission during the EIA process, 2) addition of the 'GHG subject matter' in 'establishing permissible discharge standards' which is based on "Act on the Integrated Control of Pollutant-discharging Facilities", and 3) the participation of stakeholders in early EIA stage for governance. A Study on Modern Korean Menu Food Styling through Web Image Analysis - Focusing on the Michelin Guide Seoul 2020 Star Restaurant - (웹이미지 분석을 통한 모던 한식 메뉴 푸드스타일링 연구 - 미쉐린가이드 서울 2020 스타레스토랑을 중심으로 -)

  • Tis study was an attempt to systematically analyze the characteristics of modern Korean food styling using the menu image photos of the Michelin Guide Seoul 2020 restaurants. The first sampling was conducted on the 27th of March 2020 and the second on the 27th of October 2020. A collection of images on the web and 442 photographic cases obtained by a search through theoretical background literature and research papers were studied by a qualitative analysis method. First, the food styling contents were analyzed and based on that, the food styling characteristics of the menus of 11 restaurants in Korea including the contemporary restaurants were considered. The analysis revealed several aspects of Korean food styling. First, food styling appeared to have three major characteristics: color, shape, and container styling. Color styling was further subdivided into single color/similar color, color contrast, source type, and accent color type. The shape/formative styling was classified into figure type, shape type, accessory type, and garnish type, and container styling was categorized as container color type, container shape type, and container material type. Second, the modern Korean food color styling characteristics of Michelin restaurants were categorized in the order of monochromatic/similar type, sauce type, accent color type, and color contrast. In the formative styling category, it was categorized in the order of shape type, small piece type, garnish/garnish type, and figure type. In container styling, container material type and formative type accounted for the major portion of the category. The food styling characteristics of the modern Korean menu were systematized and image examples were presented visually. Epidemiology of Facial Bone Fractures During the Coronavirus Disease 2019 Pandemic: A Single Korean Level I Trauma Center Study

  • Purpose: The medical community has been heavily impacted by the coronavirus disease 2019 pandemic. The management of facial trauma patients has been affected by the patient capacity of emergency rooms. In this study, we share our experiences of facial trauma management during the social lockdown period and investigate the epidemiological changes in facial bone fractures. Methods: A total of 997 patients who presented to Ajou University Hospital Emergency Center and were evaluated by plastic or maxillofacial surgeons for facial trauma were included in this retrospective study. Our study design was a comparative study of two groups: the 2019 group (control) and the 2020 group (the experimental group that experienced social lockdown). Results: The total number of emergency room inpatients reflected the national pandemic trends with three peaks in patient numbers. According to these trends, facial bone fractures had two different low points in August 2020 and December 2020. A comparison of the 2019 and 2020 facial bone fractures did not show a statistically significant difference in the total number of patients. An analysis of the causes of trauma showed that domestic accidents increased in 2020 (30.92%; p<0.001). Among the anatomical sites of facial injury in surgical patients, the frontozygomatic complex fracture increased the most in 2020 (p=0.018). Facial injuries with two separate sites of injury or with three or more involved sites also showed a significant increase in 2020 (p<0.001). Conclusions: We demonstrated that the incidence of facial trauma patients correlated with the incidence of patients presenting to the emergency department and that facial trauma is inextricably related to multi-trauma cases. Domestic accidents and facial trauma with multiple anatomically involved sites are increasing trends that need more attention.

An Analysis of Delivery/Transport Documents Content in Relation to the Contract of Carriage under Incoterms 2020 Rules

  • Purpose - The purpose of this study is to review and analyzes the contract of carriage and delivery/transport document in light of the major changes made to the Incoterms® 2020 rules forced into effect on January 1st, 2020. Design/methodology - This study analyzed responsibility for the loading and unloading of goods under the contract of carriage in Incoterms 2020® rules forced into effect by the ICC from January 1, 2020, and what document must be presented as evidence of delivery by the seller. Findings - A review revealed that in Rule C, the costs of unloading at the place of destination are determined by the terms of the contract of carriage, and in the DAP and DDP rules, if the seller bears the unloading costs, such unloading costs cannot be recovered from the buyer. To settle this issue, the seller needs to make a contract of carriage by sea with the carrier on FI terms. Furthermore, in the case of containerized goods that the FCA should be used, FOB was misused because the seller could not present an on-board bill of lading in the L/C transaction. However, it was confirmed that in FCA, the parties can use an optional mechanism to issue an on-board bill of lading. Originality/value - Incoterms 2020® rules are still widely used in international trade by parties to contract sales around the world, just like Incoterms 2010® rules. This study attempts to reduce or eliminate disputes that may arise from interpretative misunderstandings between the parties in the contract of sales concluded by the seller and the buyer.

Maternal, infant, and perinatal mortality statistics and trends in Korea between 2018 and 2020

  • Purpose: This study aimed to identify maternal, infant, and perinatal mortality using the national population data of South Korea between 2018 and 2020, and to analyze mortality rates according to characteristics such as age, date of death, and cause of death in each group. This study updates the most recent study using 2009 to 2017 data. Methods: Analyses of maternal, infant, and perinatal mortality were done with data identified through the supplementary investigation system for cases of death from the Census of Population Dynamics data provided by Statistics Korea from 2018 to 2020. Results: Between 2018 and 2020, a total of 99 maternal deaths, 2,427 infant deaths, and 2,408 perinatal deaths were identified from 901,835 live births. The maternal mortality ratio was 11.3 deaths per 100,000 live births in 2018; it decreased to 9.9 in 2019 but increased again to 11.8 in 2020. The maternal mortality ratio increased steeply in women over the age of 40 years. An increasing trend in the maternal mortality ratio was found for complications related to the puerperium and hypertensive disorders. Both infant and perinatal mortality continued to decrease, from 2.8 deaths per 1,000 live births in 2018 to 2.5 in 2020 and from 2.8 in 2018 to 2.5 in 2020, respectively. Conclusion: Overall, the maternal, infant, and perinatal mortality statistics showed improvements. However, more attention should be paid to women over 40 years of age and specific causes of maternal deaths, which should be taken into account in Korea's maternal and child health policies.

  • 최근들어 하천내 식생이 차지하는 비율이 크게 증가하고 있으며, 이러한 현상은 하천의 규모나 위치, 댐 유무에 관계없이 전국적으로 발생하고 있다. 식생 면적 비율의 증가는 하천의 육역화로 이어질 뿐만아니라 홍수위를 상승시켜 홍수 안전에 영향을 미칠 수 있다. 본 연구에서는 2020년 8월 섬진강에서 발생한 홍수를 대상으로 식생이 홍수위에 미치는 영향을 분석하였다. 섬진강에는 초본류 뿐만아니라 버드나무류의 목본류가 상당히 분포하고 있으며 이와 같은 식생이 홍수흐름에 큰 영향을 미친 것으로 보인다. 분석을 위해 홍수 이후 섬진강 현지 답사를 통해 식생분포 현황 및 홍수로 인한 영향을 조사하였다. 섬진강의 식생분포 조사를 위해서는 2020년 4월 조사된 유럽 우주국 Sentinel-2 위성영상을 사용하였으며, 정규식생지수(NDVI)와 정규수분지수(NDWI)를 이용하여 하천내 식생밀도를 단계별로 구분하였다. 군집된 식생지수에 따라 USGS의 매닝계수 산정표를 기본으로 식생분포 군집별 조도계수를 산정하여 대상 구간내에 2차원으로 분포시켰다. 수치모형은 NAYS2D 모형을 사용하였으며 대상구간은 섬진강 고달교에서 구례교까지 21.5km이다. 계산조건은 2020년 8월 홍수중 구례교 수위관측소를 기준으로 최대수위가 발생한 시점의 자료를 활용하였다. 고달교에서는 홍수통제소에서 제공하는 홍수량을 상류경계 조건으로 입력하였고 구례교에서는 해당 시각의 수위를 하류경계조건으로 입력하였다. 모형의 검증을 위해 대상구간의 중간에 있는 압록수위관측소 수위를 활용하였다. 식생 유무에 따른 홍수위변화는 조도계수 값에 의해 반영되도록 하였는데 식생이 있는 경우는 현재 상태, 식생이 없는 경우는 모든 지점에 모래와 자갈이 분포하는 것으로 가정하여 계산된 홍수위를 비교하였다. 분석 결과 대상구간에는 전체 면적중 약 56%를 식생이 차지하고 있으며 이로 인해 0.5~1.0m의 홍수위 상승이 발생하는 것으로 분석되었다. 수목으로 인해 2020년 8월과 같은 큰 홍수시에도 홍수위가 크게 상승하는 것으로 분석되었는데 이와 같은 점을 고려하면 하천내 식생이 홍수위에 미치는 영향을 감안하여 적극적인 식생관리 방안이 시행될 필요가 있을 것으로 판단된다.

Quality Reporting of Systematic Review and Meta-Analysis According to PRISMA 2020 Guidelines: Results from Recently Published Papers in the Korean Journal of Radiology

  • Objective: To evaluate the completeness of the reporting of systematic reviews and meta-analyses published in a general radiology journal using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. Materials and Methods: Twenty-four articles (systematic review and meta-analysis, n = 18; systematic review only, n = 6) published between August 2009 and September 2021 in the Korean Journal of Radiology were analyzed. Completeness of the reporting of main texts and abstracts were evaluated using the PRISMA 2020 statement. For each item in the statement, the proportion of studies that met the guidelines' recommendation was calculated and items that were satisfied by fewer than 80% of the studies were identified. The review process was conducted by two independent reviewers. Results: Of the 42 items (including sub-items) in the PRISMA 2020 statement for main text, 24 were satisfied by fewer than 80% of the included articles. The 24 items were grouped into eight domains: 1) assessment of the eligibility of potential articles, 2) assessment of the risk of bias, 3) synthesis of results, 4) additional analysis of study heterogeneity, 5) assessment of non-reporting bias, 6) assessment of the certainty of evidence, 7) provision of limitations of the study, and 8) additional information, such as protocol registration. Of the 12 items in the abstract checklists, eight were incorporated in fewer than 80% of the included publications. Conclusion: Several items included in the PRISMA 2020 checklist were overlooked in systematic review and meta-analysis articles published in the Korean Journal of Radiology. Based on these results, we suggest a double-check list for improving the quality of systematic reviews and meta-analyses. Authors and reviewers should familiarize themselves with the PRISMA 2020 statement and check whether the recommended items are fully satisfied prior to publication.
