목 적 : 안와 주변 방사선 치료 시 수정체 피폭선량감소를 위하여 사용된 2차 차폐 block의 유용성을 알아보고자 한다. 대상 및 방법 : Human phantom(Alderson Rando Phantom, The Phantom Laboratory, USA)을 사용하여 CT(Somatom Definition AS, Siemens, Germany) 모의촬영 후 전산화치료계획시스템(Pinnacle, PHILIPS, USA)을 통해 실제 치료와 유사한 IMRT치료계획을 실시하였다. 2차 차폐를 위하여 두께 3mm 지름 25 mm의 납판과 3 mm tungsten eye-shield block(Extra small size, Radiation Products Design, Inc, USA)를 사용하였으며, TPS(Treatment Planning System) 상의 lens dose와 모의치료 상의 lens dose를 OSLD로 측정 비교하였다. 또한, 5 cm 두께의 acrylic phantom에 동일한 조건의 2차 차폐물인 3 mm 납판과 tungsten eye-shield block을 사용하여 200 MU(6 MV, SPD(Source to Phantom Distance)=100 cm, $F{\cdot}S\;5{\times}5cm$)를 조사 및 측정하였으며, 조사야 밖의 누설선 및 투과방사선 영향을 제한시키고자 8 cm 납블럭(O.S.B: Outside Scatter Block)을 적용하여 위와 동일한 실험을 시행하였다. 조사야로부터 1 cm 이격하여 phantom 끝 옆면에 OSLD(Optically Stimulated Luminescence Dosimeter)를 부착하였고, eyelid의 두께에 해당하는 bolus 3 mm를 적용하였다. 결 과 : human phantom을 이용하여 IMRT 치료계획 상의 Lens dose와 실 측정치는 각각 315.9, 216.7 cGy가 측정되었고, 3 mm 납판과 tungsten eye-shield block으로 2차 차폐 후 각각 234.3, 224.1 cGy가 측정되었다. acrylic phantom을 이용한 실험 결과는 no block, 3 mm 납판, tungsten eye-shield block을 사용했을 때 5.24, 5.42, 5.39 cGy가 측정되었으며, 조사야 밖에 O.S.B를 적용하여 no block, 3 mm 납판, tungsten eye-shield block을 실험한 결과 각각 1.79, 2.00, 2.02 cGy가 측정되었다. 결 론 : 광자선 조사 시 critical organ을 보호하기 위하여 2차 차폐를 적용할 시에는 field 외부일지라도 헤드 누설방사선 및 collimator & MLC 투과방사선이 존재하므로 치료부위와 beam 방향에 따라 금속과 같은 높은 원자번호의 차폐물질이 critical organ근처에 있다면 선량 증가의 원인이 될 수 있다는 사실을 알 수 있었다. 따라서 피폭선량 감소를 위한 2차 차폐의 시도는 분명 의미가 있었으나 미 검증된 시도는 오히려 역효과를 가져올 수 있다는 사실을 인지하여 QA를 통해 목적에 부합하는 결과가 나오는지를 사전에 알아보아야 할 것이다.
Stellate ganglion block was performed in 15 patients with intractable glaucoma by injection of 8 ml of 1y lidocaine. The followings are the changes of intraocular pressure(mmHg): Pre -block $32.26{\pm}4.44$ 10 minutes after block $36.80{\pm}4.60$ 20 minutes after block $32.61{\pm}4.41$ 30 minutes after block $28.87{\pm}4.30$ 60 minutes after block $24.73{\pm}4.37$ 120 minutes after block $26.87{\pm}3.18$ 180 minutes after block $31.40{\pm}3.76$ The pressure rose up to 10 minutes post block and significant pressure descent was noticed from 30 minutes post-block. Maximum descent was seen by 60 minutes post-block and pre-block pressure was restored by 180 minutes post-block. For the purpose of cheeking the treatment effect, similar blocks were performed in 15 patients, one time daily for 5~8 days 3 times every othor day.
Digital simulation has recently become the preferred method for designing complex and dynamic systems. Simulation packages provide interactive, block-diagram environment for modeling and simulating dynamic models. The block diagrams in simulation models are flowcharts which describe the components of dynamic systems and their interaction. This paper is the first part of the study for determining the seismic behavior of soil systems. The aim of this part is to present the constructed block diagrams for discrete-time analysis of seismic site amplification in layered media for vertically propagating shear waves. Detailed block diagrams are constructed for single and multiple soil layers by considering wave propagation with and without damping, respectively. The block diagrams for recursive filter to model attenuation in discrete-time form are also constructed. Finite difference method is used for strain calculation. The block diagrams are developed by utilizing Simulink which is a software add-on to Matlab.
This study is intended to develop the new concrete block for planting through investigating weight and basic physical properties, varying the shapes of concrete block for planting. According to the results, unit weight, void ratio and absorption water ratio of concrete block show 1625kg/$m^3M$ 30%, and 7.7% respectively, and pH is small, compared with non neutralization and pH is below 8.5 after 7 days elapses. The compressive strength of concrete block for planting shows 38kgf/$cm^2$ at the age of 7 days, and 50kgf/$cm^2$ at the age of 28 days respectively. As the number of the hole is many and the area of hollow is large, weight of developed concrete block for planting grow light. Weight of optimum scheme with 2 hollow is reduced by 25%.
AB -type block copolymers of poly($\tau$-benzyl L-glutamate) (PBLG, a segment) and polyoxypropylene (POP, Bsegment) were synthesized and they were reacted with polyoxypropylene his(6-aminohexyl) (POE) to enhance the hydrophilicity of the poly- mers. The degree of swelling of the POE -crosslinked block copolymers (CPB -2) was higher than that of the block dopolymers (PB). The amount of 5-fluorouracil re- leased from CPB-2 block copolymers was greater than that from the PB block copol- ymers when they were incubated in the same period. These results indicate that the amount of 5-fluorouracil released from the matrices is proportional to the hydrophilicity of the polymers. In this paper. a new automatic multi=block grid generation technique for general 2D regions is introduced. According to this simple and robust method, the domain of interest is first triangulated by using Delaunay triangulation of boundary points, and then geometric information of those triangles is used to obtain block topology. Once block boundaries are obtained. structured grid for each block is generated such that grid lines have $C^0-continuity$ across inter-block boundaries. In the final step of the present method, an elliptic grid generation method is applied to smoothen grid distribution for each block and also to re-locale the inter-block boundaries, and eventually to achieve a globally smooth multi-block structured grid system with $C^1-continuity$.
Background: Chemical lumbar sympathetic ganglion block could potentially be used to treat plantar hyperhidrosis; therefore, we analyzed the outcome of lumbar sympathetic ganglion block using alcohol for the treatment of plantar hyperhidrosis. Methods: Between March 1992 and June 2003, 356 patients with plantar hyperhidrosis underwent lumbar sympathetic ganglion block using alcohol. All 356 patients were followed up for 2 years and the results evaluated. There were 185 and 171 male and female patients, respectively, with a mean age of 25.1 years, ranging from 15.3 to 56.5 years old. Lumbar sympathetic ganglion block using alcohol was performed with fluoroscopic guidance under local anesthesia. Results: The recurrence rate after 2 years was 34%. Compensatory hyperhidrosis, ejaculation failure, lower back pain and genitofemoral neuritis developed as complications in 132, 4, 12 and 2 patients, respectively. Of the 356 patients, 65% were satisfied. Conclusions: Lumbar sympathetic ganglion block using alcohol is an effective and safe method for the treatment of plantar hyperhidrosis, but more information about the complications and relatively high recurrence rates should be provided to the patient.
DL-lactide의 단점을 개선할 목적으로 개환중합법으로 촉매로서 diethylzinc (ZnEt$_2$)를 사용하여, poly(ethylene oxide) (PEO)를 세그먼트로서 도입한 블록공중합체를 합성하였다. 그 결과 DL-lactide에 대한 PEO세그먼트의 첨가비가 30% 이상에서는 조성비가 급격히 증가하였으며, 이들 공중합체를 캐스트법에 의해 필름으로 제작하여 6$0^{\circ}C$에서 2.5배 연신하여 인장실험을 실시하였다. 모든 공중합체 필름은 PDLLA에 대한 PEO에스테르 첨가비의 증대에 따라 탄성율이 저하되는 것이 관찰되어 세그먼트 도입에 의한 유연성의 증대를 확인하였으며 유연성이 요구되는 의용재료로서의 기대를 높였다.
Purpose: The carrier used as delivery agent for bone morphogenetic proteins(BMPs) should also act as a scaffold for new bone formation. Moreover, bone formation should be predictable in terms of the volume and shape. This study examined the osteogenic effect of macroporous biphasic calcium phosphate (MBCP) block combined with ePTFE membrane as a carrier for recombinant human bone morphogenetic proteins (rhBMP-2). In addition, the additive effect of ePTFE membrane on bone formation was evaluated. Materials and Methods: Eight-millimeter critical sized calvarial defects were created surgically in 28 male Sprague-Dawley rats. The animals were divided into 2 groups containing 14 animals each. The defects were treated with either rhBMP-2/MBCP block (rhBMP-2/MBCP group) or rhBMP-2/MBCP block/ePTFE membrane (rhBMP-2/MBCP/ePTFE group). A disc-shaped MBCP block (3 mm height and 8 mm diameter) was used as the carrier for the rhBMP-2 and ePTFE membrane was used to cover the rhBMP-2/MBCP block. The histologic and histometric parameters were used to evaluate the defects after 2- or 8-week healing period (7 animals/group/healing interval). Results: The level of bone formation in the defects of both groups was significantly higher at 8 weeks than that at 2 weeks (P < 0.05). The ePTFE membrane has no additional effect compared with the rhBMP-2/MBCP block only. However, at 8 weeks, rhBMP-2/MBCP/ePTFE group showed more even bone formation on the top of the MBCP block than the rhBMP-2/MBCP group. Conclusion: These results suggest that the ePTFE membrane has no additive effect on bone formation when a MBCP block is used as a carrier for rhBMP-2.
Stellate ganglion block and cervical epidural nerve block are frequently practiced in pain clinics because of simple procedure and good effect. Nerve block at head and neck may produce serious complication such as loss of consciousness and cardiac arrest. Blood supply is rich in neck and inadvertent arterial injection of local anesthetics may enter directly into brain. We experienced convulsion and respiratory arrest during SGB and cervical epidural block. The patients were resuscitated successfully and recovered without any adverse effects.
