Clinical Experiences of Open Heart Surgery [50 Cases] (개심술 치험 50례)

  • Fifty cases of Open Heart Surgery due to congenital and acquired heart disease were done using the cardiopulmonary bypass in the Department of Thoracic and Cardiovascular surgery, Chosun University Hospital from November, 1980 to June, 1985. 1. The age of the congenital heart disease was from 7 to 29 years, the mean age was 14.5 years. In the acquired heart disease, the age was from 14 to 48 years, and the mean age was 22.3 years. The ratio of male to female was about 1.8:1. 2. The number of congenital cyanotic heart disease were 7 patients, congenital acyanotic heart disease were 17 patients and acquired valvular heart disease were 26 patients. All of the acquired heart disease was one or more valve disease. 3. Preoperative symptoms of the congenital heart disease were exertional dyspnea [cyanotic 100%, acyanotic 70.6%] and palpitation [cyanotic 28.6%, acyanotic 76.1%], and the acquired heart diseases were exertional dyspnea [92.3%], palpitation [34.1 %], and chest discomfort [30.8%]. 4. The method of the myocardial protection during the cardiopulmonary bypass were mild or moderate hypothermia, intermittent coronary perfusion of the cardioplegic solution, topical myocardial hypothermia with 4oC Hartmann`s solution. 5. In the cases of the valve replacement, postoperative oral anticoagulant therapy was started at oral intake of food using the warfarin and persantin, and the prothrombin time was maintained 30-50% of control value during 3-6 months for tissue valve replacement and permanently for metal valve replacement. 6. The postoperative complications were appeared in 24 cases and the complications were wound infection, occipital alopecia, hemorrhage etc. 7. The mortality after open heart surgery was 8 percents and the cause of death was low cardiac output syndrome, right heart failure, DIC, and Left ventricle rupture.

Clinical Study on Closed Thoracotomy (폐쇄식 흉관삽관술에 관한 임상적 고찰)

  • Reexpansion of the lung is the most desirable method of filling the pleural space whether it`s contents may be, and closed thoracotomy connected to a water-seal drainage remains the basic therapeutic modality in the treatment of the problems of the pleural space. We usually used rubber mushroom tubes, size No. from 16 to 34 Fr., and performed closed thoracotomy after preliminary thoracentesis to determine the exact depending position. Author reviewed 576 cases of closed thoracotomy which were performed in the Department of Thoracic and Cardiovascular Surgery, Pusan National University Hospital, since Jan., 1980 to June, 1984. The results were as follows: 1. The age distribution was 10 days to 76 years old and mean age of the patients was 36.6 year, and the most prevalent age group was twenties, and sex predisposition was male dominant, 86.9%. 2. The most common etiologic disease group was pneumothorax and the most common etiologic disease was traumatic hemothorax. 3. Sites of tubing were predominantly at posterior axillary line, 7th intercostal space and midclavicular line, 2nd intercostal space even though frequent presence of free pleural space. The two sides, right and left difference of occurrence rate was more frequent at right side, 51.2%. 4. Usually the durations of tubing was less than 10 days, 52.6%, and the number of tubes used to the same patient concomitantly was one, 73.9%, and the time of tubing to the same patient was 1st, 83.6%. 5. The common symptom and sign were dyspnea, 50.0%, chest pain, 30.7%, cough, 10.7%, fever, 6.5%. Especially, fever and cough was the most common symptom and sign in pyogenic empyema, 59.3%. 6. The common etiologic lesions of pneumothorax were blebs and bullae, 73.3%, and of pyogenic empyema was pneumonia, 69.0%. 7. The complication rate of closed thoracotomy was 26.0%. Among these complications, infection was 44.7%, and intercostal neuralgia was 25.3%. 8. 70.9% of all patients recovered with only closed thoracotomy and the rest of patient needed additional some necessary managements such as open thoracotomy [Blebectomy, Resection, Pleurodesis, Decortication, Bleeding control], open drainage, thoracoplasty and so on to have successful results.

Further induction of amylase producing mutants from a highly proteolytic mutant strain of asppergillus flavus (돌연변이에 의한 Aspergillus flavus의 아밀라아제 생성능의 개량)

  • A mutant strain having increased productivity of both enzymes, protease and amylase, was obtained from A. flavus KU 153, isolatd from South Korea for its high protease production by successive ultra-violet light irradiation, Two glucoamylases from the mutant strain selected were purified from wheat branculture by successive salting out, followed by dialysis and column chromatography, and their characteristics were compared with those of the wild strain. Glucoamylase production of the mutant selected was increased about 3.3 times compared with the wild strain, and 2.1 times compared with the parental strain, ${\alpha}-amylase$ activity of the mutant selected was about 2 times hugher than that of the wild strain or the parental strain. Protease and cellulase productivities of the muant selected were all alike compared with those of the highly proteolytic mutant, the parental strain. Therefore, it was considered that the back mutation on the protease production did not occurred in the formation process of the glucoamylase producing mutant. Total activities of glucoamylase I and II from the mutant selected were 2.86 and 3.65 times higher compared with those from the wild strain, respectively. Considering the optimal pH-thermal stability and Km-Vmax value of glucoamylase I and II from both strains, wild and mutant, it was deduced that the characteristics of glucoamylase I and II from the wild strain did not altered during the mutation process. Therefore, it was concluded that the selected mutant did not induce the formation of another glucoamylase isozyme, or the changes in the characteristics of the glucoamylase, but induce the productivity of the same glucoamylase I and II by the action of regulatory gene.

  • Nowadays, the companies use polymer materials for many purposes fur they have many advantages. The costs of these materials take up too high a proportion of the overall cost of products that use these materials as their major material. It is advantage for polymer industries to reduce these costs. The microcellular foaming process was developed in the early 1980s to solve this problem and proved to be quite successful. Microcellular foaming process uses inert gases such as $CO_2,\;N_2$. As these gases solve into polymer matrices, many properties are changed. The microcellular foaming process makes the glass transition temperature of polymers to low, and diminish the residual stress of polymer matrices. Besides, the microcellular foaming process has several merits, impact strength elevation, thermal insulation, noise insulation, and raw material saving etc. In previous research, many facts of microcellular foaming process are founded its characteristics. But previous researcher found the characteristics of microcellular foaming process with pure gas, for example $CO_2,\;N_2$ and so on, they did not found the characteristics of microcellular foaming process with one more gases. If one more gases inlet the resin, the characteristics of microcellular foaming process is changed very amazingly. In this paper, discuss on the characteristics of microcellular foaming process wi th gas cocktail about cell morphology.

Studies on Processing and Analysis of Red Pepper Seed Oil (고추씨 기름의 정제와 분석에 관한 연구)

  • An attempt was made to remove red color and capsaicin which were contaminated in crude oil extracted from red pepper seeds with n-hexane. Both clay bleachidg and deodorization were responsible for the removal of red color. Clay bleaching was also the most effective for the removal of capsaicin, which was removed completely with 4 % clay. Capsaicin in oil was adsorbed onto the clay particles exponentially and it followed Freundlich equation : ${\frac{x}{m}}=1.1\;c^{0.31}$, where x is amount of capsaicin adsorbed (mg/ml), m is amount of clay used in bleaching (%) and c is amount of residual capsaicin (mg/ml). Neutral lipid of the seed oil consisted of 95.8 % triglyceride, 1.55 % sterol ester, 1.45 % monoglyceride, 0.78 % free fatty acids, 0.2 % diglycerides and 0.15 % sterol. Major components of fatty acids in the red pepper seed oil were linoleic acid (78.79 %), palmitic acid (15.27 %) and linolenic acid content was only 0.63 % by high performance liquid chromatography.

  • Since the advent of Six Sigma in 1980's, Six Sigma is widely used in various industry areas, such as manufacturing, financial, government, and service, and proves its innovation ability and value by providing lots of success stories. Several researches have been conducted to adopt Six Sigma for Software Development and Software Quality Assurance-Software Six Sigma. Although the wide use of Six Sigma, there is no software tool to support the execution of Six Sigma Project. For this reason, the Quantitative Six Sigma Project Management is required to support for measuring and analyzing Six Sigma Projects, storing and reporting the results from Six Sigma Projects, and managing Six Sigma Projects through the whole process of, DMAIC or DMADV. In this paper, we introduce the design and implementation of a Quantitative Six Sigma Project Management Framework and Tool. The execution of Six Sigma Project based on Six Sigma Project Management Tool benefits the quantification, the systematization, and the integration.

Schematic Regeneration Strategy of Old Downtown, Myeongdong, in Cheonan (천안 구도심 명동지역 도시재생 전략수립 연구)

  • After 1970-80's rapid economic and urban development, nowadays the old downtown suffers from the deterioration of physical environment, outflow of its function and cavitation of population. And this phenomenon makes urban regeneration as a new paradigm in urban development. This is the case of Myeongdong area (Jungang-dong, Monsung-dong), old downtown of Cheonan around rail station, in the same way. City government has tried many regeneration/ redevelopment projects on this area but all failed due to lack of feasibility. This study intends to suggest new viable strategic regeneration scheme of this area through research and analysis of TOD development, existing condition and problems. As a result, the area needs to be considered as a node that links bus terminal complex and central market place with walkable street. For regeneration of this area, a strategic development scheme of this area is suggested deploying private capital invested rail station, underground shopping mall, transfer center, culture complex and nature park with Myeongdong street mall.

Human Sparganosis in Korea

  • Background: Sparganosis is a larval cestodiasis caused by the plerocercoid of Spirometra spp. Since the first description of human sparganosis in 1924, several hundred cases have been reported in Korea. However, systematic approaches for literature surveys of Korean sparganosis have seldom appeared. Methods: We searched publicly available databases such as PubMed, Research Information Sharing Service, and Korea Medical Citation Index with relevant Medical Subject Headings. Results: At least 438 Korean sparganosis cases have been described from 1924 to 2015. Preoperative diagnosis has been significantly increased since the 1980s due to popularization of serological and imaging diagnostics. Cases were largely detected from fifth decades in general, but cerebral sparganosis was detected in relatively young age groups (third and fourth decades). Sparganosis was prevalent in men (75.9%). Consumption of frog/snake and drinking unfiltered water were found in 63.4% and 16.9% of patients, respectively. Most frequently affected sites were subcutaneous tissues (49.9%), followed by the central nervous system (36.2%). Involvements of visceral organs (7.6%), ocular regions (3.6%), and muscles (2.7%) were noticed. In women, breast sparganosis constituted a large proportion (34.2%). Sparganosis associated with immunocompromised patients has recently been reported. Conclusion: Sparganosis has been continuously reported in Korea during the past 90 years, although its incidence has decreased during the last 20 years. The disease is mostly characterized by subcutaneous nodule, but infection of the worm in vital organs often results in serious illness. Continuous awareness is warranted to monitor sparganosis occurrence and associated clinical consequences.

Occupational Health Policies on Risk Assessment in Japan

  • Industrial Safety and Health Law (ISH Law) of Japan requires abnormalities identified in evaluations of worker health and working environments are reported to occupational physicians, and employers are advised of measures to ensure appropriate accommodations in working environments and work procedures. Since the 1980s, notions of a risk assessment and occupational safety and health management system were expected to further prevent industrial accidents. In 2005, ISH Law stipulated workplace risk assessment using the wording "employers shall endeavor." Following the amendment, multiple documents and guidelines for risk assessment for different work procedures were developed. They require ISH Laws to be implemented fully and workplaces to plan and execute measures to reduce risks, ranking them from those addressing potential hazards to those requiring workers to wear protective articles. A governmental survey in 2005 found the performance of risk assessment was 20.4% and common reasons for not implementing risk assessments were lack of adequate personnel or knowledge. ISH Law specifies criminal penalties for both individuals and organizations. Moreover, under the Labor Contract Law promulgated in 2007, employers are obliged to make reasonable efforts to ensure employee health for foreseeable and avoidable risks. Therefore, enterprises neglecting even the non-binding provisions of guidelines are likely to suffer significant business impact if judged to be responsible for industrial accidents or occupational disease. To promote risk assessment, we must strengthen technical, financial, and physical support from public-service organizations, encourage the dissemination of good practices to reduce risks, and consider additional employer incentives, including relaxed mandatory regulations.

Effects of Al-Surplus and Ca-Deficiency on Content and Rhythm of ATP in Plant Leaves (식물엽의 ATP함량과 그 주기에 미치는 Al과잉 및 Ca결핍의 영향)

  • Kidneybean(Phaseolus vlgaris L.) and buckwheat(Fagopyrum esculentum M$\"{O}$nch) seedlings cultured with Hoagland solution to the height of 7 to 10cm in the earthern pot containing sand were used for experimental plants. One group of the plants was irrigated with Hoagland solution composed of various Al concentrations of 0 to 1,000 ppm containing 50ppm Ca, and the other group was irrigated with Hoagland solution composed of various Ca concentrations of 0 to 1,000ppm containing 100ppm Al for a month during June and July. By Al-surplus over 100ppm, the margins of backwheat leaves were curied down and turned into yellow-brown necrosis simialr to Ca-deficiency, and the old kidney bean leaves showed marginal and veinal chlorosis in addition to the early shedding of leaves. With Al-surplus of 100ppm the content of chlorophyll a and b in both plants was decreased by 30~40%. The ratio 3/1 of chlorophyll a/b was not altered by Al-surplus and Ca-deficienty at an early stage, but it was affected markedly at a final stage of growth. Leaf respiration with 100ppm Al containing 50pm Ca was slightly affected in buckwheat, but decreased by 20% in kidneybean. With Al-surplus of 100ppm the ATP content was decreased by 32% in the kidneybean leaves and by 80% in buckwheat leaves, whereas with Ca-deficiency it was decreased by 72% in kidneybean and by 90% in buckwheat. The rhythm of ATP level showed self-relience without being affected by light or temperature under the green house condition although by Al-s rplus and Ca-deficiency the ATP level of the rhythm was lower than that of control.trol.
