119구급대원의 아나필락시스 환자에 대한 병원 전 응급처치 현황 및 개선 방안 (Prehospital care status and improvement plan of 119 emergency medical technician to anaphylaxis patients)

  • Purpose: This study aims to analyze the current state of emergency care for patients with anaphylaxis and to identify problems and areas for improvement in prehospital care. Methods: This study was conducted using 119 emergency running sheets and medical records of 109 patients diagnosed with anaphylaxis. Questionnaires were also given to emergency medical technicians (EMTs) and emergency physicians. The data were analyzed using SPSS 21.0. Results: Prehospital emergency care included oxygen administration in 64.2%, an intravenous line in 15.6%, and medication injection in 11.0%. The most commonly administered medications were antihistamines in 66.7% and epinephrine in 8.3%. Of EMTs surveyed, 47.0% suggested an epinephrine injection and using direct medical control, while 53.8% of the emergency physicians suggested an epinephrine injection and using indirect medical control. Most emergency physicians 88.8% responded that epinephrine could be administered by EMTs. Conclusion: The data support epinephrine injection of patients with anaphylaxis by EMTs, but a larger sample size of EMTs is required. Education about the treatment of anaphylaxis should be improved for EMTs. The scope of paramedic responsibilities should also be redefined.

119 구급대원의 곤충교상환자에 대한 응급처치 현황 및 개선방안 (Prehospital care and improvement of 119 emergency medical technician for the insect bite patients)

  • Purpose : This study aimed to improve quality of prehospital emergency care for the insect bite patients by figuring out its current situations and problems. Methods : This study was conducted to 219 insect bite patients who were transferred to the hospital by 119 ambulance in D-City from July 1, 2009 to June 30, 2011. This study was a descriptive study by 119 run sheets. A total of 171 emergency medical technicians(EMTs) working in D-city and C-province completed the questionnaire. All of data were analyzed by SPSS 18.0. Results : Among the 219 patients, bee sting accounted for 62.6% and wasp sting accounted for 20.1%. Some patients complained of dizziness, headache, dyspnea, and disturbance of consciousness. The 119 EMTs initially treated the patients with oxygen supply to 57 patients (31.1%). Knowledge toward insect bite was statistically significant (p=.000) between advanced EMTs and basic EMTs. The obstacle to the prehospital care, many EMTs answered that the obstacle was due to the lack of 119 EMTs. Conclusion : It is necessary to differentiate the patients' symptoms and provide the better prehospital emergency care to verify the exact symptoms and signs.

119구급대원에게 시행한 일개 응급의료정보센터의 직접의료지도에 관한 분석 (Analysis of direct medical control conducted to 119 emergency medical technicians in an emergency medical information center)

  • Purpose : The goal of the present study is to provide the basic information to medical control which is the most important improving factor of pre-hospital medical treatment. Method : A total of 749 records of direct medical control were collected from 119 EMTs in emergency medical information center of Daejeon, Chungcheongnam-do and Chungcheongbuk-do from March 1, 2010 to February 28, 2011. Results : The 119 EMTs should record the level of qualification of EMT and general patient history taking precisely when they receive direct medical controls. The doctors should take medical controls within the task range of qualification of EMTs. Conclusion : It is necessary to establish the guideline of medical direction and protocol of prehospital emergency care. The quality improvement of pre-hospital emergency services will be possible by the guideline and protocol.

  • Purpose : This study intended to improve the quality of prehospital emergency care for patients with dyspnea by figuring out the current situations and problems based on run-sheets of 119 emergency medical technicians (EMTs). Methods : A total of 336 dyspnea patients were transferred to G university hospital by 119 ambulances from January 1, 2011 to December 31, 2011. Results : The data were analyzed by SPSS 19.0. Patients over 60 years old accounted for 57.2% and 48.4% had the history of heart disease. Among the heart disease patients, 31.7% complained of dyspnea and chest pain. The vital sign measurement performance by 119 EMTs increased compared to previous study. The number of prehospital treatment by 119 EMTs significantly increased depending on the mental state of the patients. Conclusion : 119 EMTs should remind the heart disease and other past history of the patients when they perform the dyspnea patient care.

119구급대원의 감정노동과 직무스트레스가 피로에 미치는 영향 (The effects of emotional labor and job stress on the fatigue of 119 emergency medical technicians)

  • Purpose: This is a descriptive research study that aims to identify factors affecting fatigue by examining the degree of emotional labor, job stress, and fatigue of 119 emergency medical technicians (119 EMTs). The correlation between emotional labor, job stress, and fatigue was also confirmed. Methods: A total of 139 EMTs working in the Daejeon area participated in this study. Data were collected from July 6-15, 2021. Results: When the number of dispatches was high and the job was dissatisfying, the degree of emotional labor was high. In addition, the shorter the work experience and the lower the job satisfaction are, the higher the job stress is. The degree of fatigue was significantly higher in female than male 119 EMTs, and when they had short work experience and were dissatisfied with their job, the degree of fatigue was higher. Moreover, there was a significant correlation between emotional labor, job stress, and fatigue of the 119 EMTs. Conclusion: To reduce emotional labor, job stress, and fatigue of 119 EMTs, appropriate interventions for each factor and institutional improvements are required.

119구급대원의 외상환자에 대한 병원 전 응급처치와 업무수행현황 (Prehospital Care of 119 Emergency Medical Technician to Trauma Patients)

  • Purpose: This study intended to improve quality of prehospital emergency care for trauma patients by figuring out its current situations and problems based on run-sheets and questionnaires of 119 emergency medical technicians (EMTs). Methods: This study conducted a research of 425 trauma patients transferred to the 3rd hospital in G-city by 119 ambulances from July 1, 2008 to June 30, 2009. We aslo utilized 114 copies with questionnaires of 119 EMTs working in J-province. The data were analyzed with SPSS 18.0. Results: There were 425 trauma patients including 272 men and 137 patients with traffic accident. When it comes to types of 119 EMTs who delivered cares to patients, there were 206 (48.5%) advanced EMTs, 101 (23.8%) basic EMTs, 50 (11.8%) nurses and 43 (10.2%) rescue education receivers. The most frequent measured vital sign was pulse rate (54.1%). Regarding assessment of systolic blood pressure, pulse rate and respiration rate, there were some significant differences in accordance with type of 119 EMTs. Among the 317 patients evaluated 'emergency' in field, 137 patients returned to their home. Prehospital emergency cares accounted for 861, around 2.0 treatments per a patient. In view of questionnaire, the 74.6% of 119 EMTs hoped supplement of man power for proper prehospital care to trauma patients. Conclusion: This study suggested that it is necessary to develop detailed guidelines for trauma patients so as to improve quality of trauma patient evaluation and prehospital care. Furthermore, improvement of emergency care systems will reduce mortality of trauma patients and lead to their good outcome.

보호자의 응급처치 수용의도에 관한 연구: 정교화 가능성 모델 중심 (Influential processes for the acceptance of protectors toward emergency care for patient based on an elaboration likelihood model)

  • Purpose: This study validated the influential relations between the effects of emergency care quality, credibility of 119 emergency medical technicians (119 EMTs), and perceived usefulness and attitude of emergency care, focusing on the moderating effect of protectors' characteristics (education, experience, age, and recognition of patient severity). Methods: This study was based on elaboration likelihood and technology acceptance models. In total, 172 protectors with experience in utilizing prehospital service were surveyed from April 1 to July 31, 2011. Results: The results showed that the emergency care quality and the credibility of 119 EMTs were the main determinants of the perceived usefulness and attitude of emergency care, irrespective of the protector's characteristics (p <.001). In addition, the findings showed that the protector's intention of emergency care had a moderating role. The impact of the quality of emergency care on its perceived usefulness was greater for high-level protectors (p <.001). By contrast, the impact of the credibility of 119 EMTs on the perceived usefulness of emergency care was greater for low-level protectors (p <.001). Conclusion: The protectors' characteristics have different influences on the relations between the effects of emergency care quality, the 119 EMT credibility, and the perceived usefulness and attitude of emergency care.

119구급대 응급구조사에 대한 법의학교육의 필요성 (The Educational Need of Forensic Medicine for Emergency Medical Technicians in 119 Rescue Service)

  • 사건 사고현장에서 가장 먼저 접근하는 초동조치 활동은 원인을 규명하는데 결정적인 역할을 한다. 법의학적 지식이 부족한 자가 최초현장에 접근하여 결정적인 사건해결 증거를 멸실케 한다면 억울한 죽음은 물론이고 무고한 혐의자를 발생시키며 유사범죄의 발생을 막을 수 없을 것이다. 본 연구는 119구급대 응급구조사에 대한 법의학교육의 필요성을 제시하고자 전국 대학(교) 응급구조학과 및 2급 양성기관의 법의학교육 실태분석을 위한 조사를 시행하였으며, 119구급대원 중 1 2급 응급구조사 307명(1급 43%, 2급 57%)을 대상으로 설문조사를 실시하였다. 그 결과 업무 중 범죄와 관련된 경험 94.8%, 죽음(변사체)의 경험이 91.9%였고, 이러한 현장에 92.8%가 경찰보다 먼저 현장에 도착하였다. 응답자의 72%는 법의학지식이 부족하여 업무 중에 어려움을 겪은 경험이 있었고, 수사경찰관과 문제, 법적증언, 유가족과의 문제 등을 경험하였다. 90.9%는 응급구조사의 업무수행에 법의학관련 지식이 도움이 된다고 긍정적으로 밝혔으며, 법의학교수에 의해 정기적인 교육을 희망하고 있었다. 응급구조사들의 법의학적 검시관련 지식을 알아보기 위해 국가 및 지방자치단체가 억울한 죽음이 없도록 감시하고, 정책적 차원에서 법의학적인 검시가 필요한 것에 대한 질문에 1급 2급 응급구조사 두 군 모두 20개 항목 중 각각 8.86개와 7.99개만 인식하고 있어, 법의학교육이 절실히 필요한 것으로 나타났다. 따라서 법의학은 응급구조사에게 단순히 범죄와 관련된 상황에서만 필요한 것이 아니라 각종 대량재해 교통사고 보험관련 사고 산업재해 등의 죽음과 관련된 법률적 문제를 해결하는 데에도 연관되어 있다. 신고체계가 다원화 되어 있는 우리나라 현실에서 119구급대의 응급구조사들은 의학적 판단에 기초한 신속하고 정확한 응급처치와 더불어 현장을 보존하고 결정적 사건해결의 물적 증거 보존을 위한 법의학적 지식을 가지기 위해서는 체계적인 법의학교육이 시행되어져야 할 것이다.

소방 1급응급구조사의 병원임상수련 술기 실태 조사 (The Actual State of Hospital-based Skill Training in Level 1 Emergency Medical Technician)

  • 이 연구는 소방 119구급대에서 근무하는 1급 응급구조사의 병원임상수련 실태를 조사한 연구이다. 조사대상은 소방에 근무하는 1급 응급구조사 42명으로 하였으며, 2012년 5월 11일부터 2012년 5월 12일까지 구조화된 무기명 자기기입식 설문지를 이용한 설문조사에 의하였다. 연구결과 1급 응급구조사의 업무범위를 한 번 이상 직접 경험한 경우는 56.5 %였고, 이들 중 기관내삽관 66.9 %, 후두마스크 8.4 %, 정맥로확보 76.6 %, 니트로글리세린 설하투여 61.2 %, 기관지확장제 흡입 50.0 %, 수액투여 73.8 %, 인공호흡기 사용 57.6 %, 포도당 주입 57.6 %를 경험하였으며, 1급 응급구조사 업무범위를 16개 의료기관별로 살펴본 결과 58.0 %가 한 번 이상 직접 경험하였다. 향후 1급 응급구조사의 교육 요구도에 대한 지속적 연구가 필요하며, 병원임상수련의 문제점 분석, 임상실습지침서의 보완과 실무중심의 교육프로그램 운영 및 관리에 대한 규정과 지속적인 평가가 필요하다.

119 구급대원의 법의학 지식에 대한 연구 (Assessing the forensic knowledge of 119 emergency medical technicians)

  • Purpose: It is crucial that 119 emergency medical technician (EMT) have adequate knowledge regarding forensic science. This study aimed to assess the forensic knowledge of EMTs in the Republic of Korea. Methods: This study was conducted on EMTs of five fire stations in D metropolitan city between August 6 and August 13, 2018. A questionnaire consisting of items on crime-related incident recognition (i.e., mechanical asphyxiation, trauma, sex crimes, and child abuse), forensic evidence management (i.e., evidence collection, preservation, and recording) was administered to the participants. Finally, 119 questionnaires were analyzed. Results: The ratio of correct answers for crime-related incidents recognition was 55.82%, which was lower than that for forensic evidence management(84.5%). In particular, the scores for the type of neck compression(16.5%) and wound assessment(44.0%) were low. The percentage of correct answers for forensic evidence management was 78.4% for evidence collection, 84.4% for evidence preservation, and 90.6% for evidence recording. Previous forensic education experience was not a significant variable. Conclusion: Specific and practical forensic science education on perception of crime-related incident, including asphyxia and wound identification, and forensic evidence collection is needed.
