119구급대원에게 시행한 일개 응급의료정보센터의 직접의료지도에 관한 분석 (Analysis of direct medical control conducted to 119 emergency medical technicians in an emergency medical information center)

  • Purpose : The goal of the present study is to provide the basic information to medical control which is the most important improving factor of pre-hospital medical treatment. Method : A total of 749 records of direct medical control were collected from 119 EMTs in emergency medical information center of Daejeon, Chungcheongnam-do and Chungcheongbuk-do from March 1, 2010 to February 28, 2011. Results : The 119 EMTs should record the level of qualification of EMT and general patient history taking precisely when they receive direct medical controls. The doctors should take medical controls within the task range of qualification of EMTs. Conclusion : It is necessary to establish the guideline of medical direction and protocol of prehospital emergency care. The quality improvement of pre-hospital emergency services will be possible by the guideline and protocol.

119 상황 접수 요원의 대응능력 향상을 위한 업무 및 훈련 개선방안 연구 (A study on the improvement of work and training to make better Korean 119 dispatcher's response ability)

  • 본 논문에서는 한국의 119 상황 접수 요원들의 업무능력 향상을 위하여 업무와 교육방식의 문제점과 개선방안을 제시하였다. 먼저, 한국 및 미국의 응급전화 접수에 대한 매뉴얼과 교육 방법을 조사 및 비교분석 하였다. 조사내용을 바탕으로 경기도 119상황실 상황접수 요원 95명을 대상으로 설문조사를 실시하여 구체적인 개선방안을 검토하였다. 그 결과 119상황 접수와 관련된 인력 및 장비는 지속적으로 보완되고 있지만, 접수요원들에 대한 사전교육 등 훈련은 부족하여 실무에서는 요원들의 개인적인 경험과 역량에 의존하고 있는 실정이었다. 또한 업무에 비하여 전체적으로는 여전히 인력이 부족한 실정이었다. 따라서 119상황접수 요원에 대한 체계적인 교육 프로그램 마련 및 적정한 훈련과 테스트 기준 마련을 통한 상황접수요원의 전문성 강화, 대응 업무의 표준화 및 적정인력을 배치를 통하여 상황 판단의 오류 발생을 방지하고 업무를 체계화할 필요가 있는 것으로 검토 되었다.

HD047703, a New Promising Anti-Diabetic Drug Candidate: In Vivo Preclinical Studies

  • G-protein coupled receptor 119 (GPR119) has emerged as a novel target for the treatment of type 2 diabetes mellitus. GPR119 is involved in glucose-stimulated insulin secretion (GSIS) from the pancreatic b-cells and intestinal cells. In this study, we identified a novel small-molecule GPR119 agonist, HD047703, which raises intracellular cAMP concentrations in pancreatic ${\beta}$-cells and can be expected to potentiate glucose-stimulated insulin secretion from human GPR119 receptor stably expressing cells (CHO cells). We evaluated the acute efficacy of HD047703 by the oral glucose tolerance test (OGTT) in normal C57BL/6J mice. Then, chronic administrations of HD047703 were performed to determine its efficacy in various diabetic rodent models. Single administration of HD047703 caused improved glycemic control during OGTT in a dose-dependent manner in normal mice, and the plasma GLP-1 level was also increased. With respect to chronic efficacy, we observed a decline in blood glucose levels in db/db, ob/ob and DIO mice. These results suggest that HD047703 may be a potentially promising anti-diabetic agent.

119와 1339에 접수되는 중독 상담 정보의 변화 비교: 응급의료정보센터(1339) 통합 이후의 소방구급상황센터(119)에서의 병원 전 독성 물질 노출자료 현황 분석 (Changes of Poison Data Characteristics Collected from Telephone Response in 1339 and 119: Discrepancy in Characteristics of Post-toxin Exposure Data Obtained through Telephone Counselling Provided by 1339 and 119)

  • Purpose: The aim of this study was to compare the toxicologic profiles and outcome of poisoned patients by comparing the data obtained through telephone counselling, each provided by emergency medical information center (1339) and emergency dispatch center (119). Methods: We analyzed the telephone-based poison exposure data before and after Seoul 1339 merged to 119. We compared the Seoul 1339 call response data in 2008 with Seoul and Busan 119 call response data between 2014 and 2016. We analyzed the changes in the trend and quality of data obtained, as well as the quality of service provided by each center before and after this reallocation, by comparing the data each obtained through telephone counselling. Results: The data was collected for a total of 2260 toxin exposure related calls made to Seoul 1339 in 2009, and 1657 calls to 119 in Seoul and Busan between 2014 and 2016. Significant difference was observed for age, sex, and reason for exposure to toxic substance between the two groups. Conclusion: After the integration of 1339 with 119, 119 focused on role of field dispatch and hospital transfer, lacking the consulting on drug poisoning. Moreover, data on exposure to toxic substances at the pre-hospital stage indicate that drug information and counseling are missing or unknown. In addition, first aid or follow-up instructions are not provided. Thus, systematic approach and management are required.
