Development and evaluation of training protocols for mass casualty incidents during disaster response (다수사상자사고 대응 실습교육 프로토콜 개발 및 효과성 검증)

  • Purpose: The purpose of this study was to develop a training protocol to standardize the management of mass casualties as part of the disaster response, and to verify the effectiveness of the training protocol. Methods: The study was conducted as a quasi-experimental study with a non-equivalent control group and pretest-posttest design. The protocol was divided into 5 parts, the first for the advance party, the second for the rescue team, the third for the paramedic team, the fourth for the ambulance team, and the fifth for the 119 EMS team. This study was conducted on November 15, 2021 and consisted of 21 subjects in the final experimental group and 23 subjects in the control group. In this study, the prior homogeneity test was analyzed using the χ2-test, intragroup comparisons were analyzed using the paired t-test, and intragroup comparisons were analyzed using the independent t-test. Results: The protocol was developed in five parts: advance party, rescue team, paramedics team, ambulance team, and 119 EMS team. In verifying the effectiveness of the protocol, it was found that there were significant differences in self-efficacy (t=-0.941, p=0.001) and self confidence within the group (t=-0.025, p=0.001) after the implementation of the mass casualty incident response training program. However, there was no significant difference between the experimental and control groups. Conclusion: Based on the findings of this study, it is believed that disaster response personnel can experience lower levels of anxiety and tension in disaster situations if they receive practical and realistic education and training. In the future, it is necessary to enhance protocol based practical education that can improve the knowledge and skills of each team and individual.

Effectiveness of the Trauma Team-Staffed Helicopter Emergency Medical Service (헬리콥터 응급의료서비스의 외상팀 탑승 여부와 외상환자의 생존율)

  • Background: Whether there is a difference in outcomes for trauma patients transferring to the helicopter emergency medical service (HEMS) according to their previous team composition is controversial. The purpose of this study is to evaluate the effectiveness of trauma team-staffed-HEMS (TTS-HEMS) when transferring to a trauma center. Methods: A retrospective comparison was conducted on patients transported to a trauma center over a 6-year period by the TTS-HEMS and paramedic-staffed-HEMS (119-HEMS). Inclusion criteria were blunt trauma with age ${\geq}15years$. Patient outcomes were compared with the Trauma and Injury Severity Score (TRISS) (30-day mortality) and the Cox proportional hazard ratio of mortality (in hospital). Results: There were 321 patients of TTS-HEMS and 92 patients of 119-HEMS. The TTS-HEMS group had a higher Injury Severity Score and longer transport time but a significantly shorter time to emergency surgery. The prehospital data showed that the trauma team performed more aggressive interventions during transport. An additional 7.6 lives were saved per 100 TTS-HEMS deployments. However, the TRISS results in the 119-HEMS group were not significant. In addition, after adjusting for confounders, the hazard ratio of mortality in the 119-HEMS group was 2.83 times higher than that in the TTS-HEMS group. Conclusion: HEMS was likely to improve the survival rate of injured patients when physicians were involved in TTS-HEMS. Survival benefits in the TTS-HEMS group appeared to be related to the fact that the trauma team performed both more aggressive prehospital resuscitation and clinical decision making during transportation.

Pre-hospitalization Advanced Airway Management Using The KING LTS-DTM and the LMA ProSealTM (LTS와 LMA를 이용한 병원 전 전문 기도 관리 연구)

  • It is very critical to promptly maintain airway of cardiac arrest cases or serious traumatic cases. The purpose of this study is to compare intubation rate, successful rate and pre-/post-education self-confidence between laryngeal tube (LTS) and proseal laryngeal mask (PLMA), so that it may contribute to improving the ability of 119 emergency team - a pivotal role in pre-hospitalization process - to carry out advanced airway management. In order to achieve the purpose, total 60 paramedics (Class I) who worked for incumbent 119 emergency team were asked to join a practical experiment and were also divided into two groups (LTS group: 30 people, PLMA group: 30 people). In details, they were all asked to take 3 sessions of practice (5 minutes per session) using dummy model to quantitatively measure the time and success/failure of intubation and the change of their self-confidence. As a result, it was found that there were statistically significant differences in the time of intubation between LTS and PLMA group (p=.000), but there was no significant difference in the rate of successful intubation between these two groups, and self-confidence of two groups tended to significantly increase after experiment (p=.000). Conclusively, if it is necessary to promptly apply intubation to traumatic cases with immobilized cervical vertebral and lingual edema, LTS can be useful as an effective means of intubation. And it is expected that continuing intubation training using LTS will improve the ability of emergency team to perform advanced airway management for traumatic cases.

  • This study is a descriptive research study to determine the effect of job stress and job satisfaction of 119 paramedics on their job performance ability. The subjects were 119 paramedics working at the fire department who agreed to participate in the study, and data collection was conducted from July 1st to September 10th, 2020. Data analysis was performed using the SPSS 23.0 program, with frequency and percentage, mean and standard deviation, correlation analysis, t-test, and ANOVA. As a result, the average job stress, job satisfaction, and job performance ability of 119 paramedics were 2.85±0.29 points, 3.35±0.30 points, and 3.53±0.89, respectively. A higher job satisfaction indicated higher job performance ability. An examination of the characteristics of the variables according to the general characteristics showed that the job performance ability was related significantly to the marital status, age, and first aid experience. Based on this, to establish a positive professional intuition to increase the job performance ability of 119 paramedics, lower job stress, and increases job satisfaction, it is important to raise awareness of job satisfaction, establish a job view, and promote job satisfaction through performance improvement. An Analysis of Prehospital Care for Major Trauma Patients depending on the number of 119 Ambulance Crews (119 구급대 편성 인원에 따른 중증외상환자의 병원 전 응급처치 실태 분석)

  • This study analyzed current prehospital emergency care for severe trauma patients administered by different number of 119 EMS (emergency medical services) teams. Out of 1,067 severe trauma patients transferred by 119 EMS teams in J province from January 1st to December 31st 2015, 438 were evaluated in this study. IBM SPSS Statistics 21.0 was used to analyze collected data. The number of patients with severe trauma during the period of study was higher in male with two and three ambulance crews of 119 EMS teams with 242 patients (70.6%) and 66 patients (69.5%) respectively. The percentage of transfers made to local emergency medical centers was highest in those two groups, being 44.0% (151 patients) and 49.5% (47 patients), respectively. Total time spent at the scene did not differ between ambulance crews of two and three, nor did the frequency of practicing advanced airway management and IV (intravenous) cannulation, or the success rate of IV cannulation (p=0.253, p=0.362, p=1.000). Overall, the results indicated that merely increasing the number of paramedics does not improve the quality of prehospital care for severe trauma patients. Measures such as securing professional paramedics, simplifying direct medical oversights, activating indirect medical oversights, and expanding the legally allowed work scope are required.

A Study on the Factors of Safety Enhancement in Mass Casualty Incidents (다수 환자 발생 시 안전 증진 요인에 관한 연구)

  • Korea is experiencing various disasters both natural and artificial. This study is descriptive research designed to examine the perception of the disaster response ability of fire-paramedics during the response stage of disaster management. The subjects of this study were EMT-P's who had more than 2 years of experience in the field at a fire station in G Province. The questionnaire, including the items for the sub-factors of the field response ability, were prepared and 161 final questionnaires were collected and analyzed with the SPSS program. The mean scores of triage ability, patient treatment ability, patient transfer ability, disaster support ability, and disaster response speed were 3.53, 3.68, 3.66, 2.95, and 3.44, respectively. As a result of multiple regression analysis, variables affecting the speed of disaster response were in the order of patient treatment ability, patient transfer ability, and disaster support ability. In conclusion, fire-paramedics will have to consider the ability to treat patients, transfer patients, and disaster support to improve disaster response speed, and, ultimately, disaster response guidelines should be developed to improve disaster response capabilities.
