거울치료가 아급성기 뇌졸중 환자의 균형, 보행 및 운동기능에 미치는 효과: 예비연구 (The Effect of Mirror Therapy on the Balance, Gait and Motor Function in Patients with Subacute Stroke : A Pilot Study)

  • Purpose : We aimed to determine whether improvements in balance, gait, and motor function were different when the same exercise was performed, with and without mirror therapy, by patients with subacute stroke using the affected and unaffected lower limbs. Methods : Eight patients with subacute stroke were randomly divided into experimental groups 1, 2 and the control group. A mirror therapy program was performed with group 1 using the unaffected lower limb and group 2 the affected lower limb. The exercise lasted 30 min per session, five times weekly, for 4 weeks. The control group did not perform the exercises. BT-4, BBS, POMA, 10MWT, and BRS were used to evaluate balance, gait, and motor function before and after the intervention. Results : Post-intervention analysis showed that all three groups had higher BBS scores. After training, the postural sway in groups 1 and 2 decreased in the post eye opened and closed positions; that of the control group increased. The scores of two subjects in group 1 increased by 4 and 5 points in POMA, resulting in significant changes compared to those in the other groups. No group showed significant results in 10MWT. BRS improved in all subjects in group 1 from BRS 2 to 1 and in only one subject in group 2 there was no change in the control group. Conclusion : Static and dynamic balance and significant results are noted in POMA, BBS, but not gait velocity. Therefore, mirror therapy seems to show a positive change in subacute patients, but the research results are not clear and the difference between groups is unknown due to the small number of subjects. The effects of mirror therapy and exercise therapy should be compared using more subjects in future.

  • Purpose : The purpose of this study was to determine effect of Medical Exercise therapy on walking ability and ankle muscles activation after stroke. Method : Participants were randomly divided into either MET training group(n=12) and control group(n=12). All of participants had been receving a traditional rehabilitation program, 5 days a week. MET training group have additionally undergone for six weeks, 3 days a week, the MET program. But control group was not received any additional program except the traditional rehabilitation program. The 10 mWT, F8WT and 2 MWT to measure walking ability were carried out twice before and after training. Muscular activity of the ankle was estimated by analyzing the RMS of action potential for EMG in terms of tibialis anterior muscle(TA), soleus(SO), gastrocnemius medial head(GM), gastrocnemius lateral head(GL). Rresults : At the walking ability, MET training group demonstrated a significant improvememt in the score of the 10 mWT(p<0.001), 2 MWT(p<0.05), but F8WT was not significant improvement(p>0.05). At the ankle muscle activation, MET training group demonstrated a significant improvememt in the RMS of the TA(p<0.01) and SO(p<0.05) but GM(p>0.05) and GL(p>0.05) were not significant improvement. Conclusion : From these results of this study, MET training for 6 weeks has an effect on improvement of walking ability and ankle muscles activation after stroke.

전신진동을 결합한 스쿼트운동이 중증 만성폐쇄성폐질환 환자의 폐기능과 넙다리네갈래근 활성도에 미치는 영향 (Effects of Squat Exercise Combined with Whole-Body Vibration on the Pulmonary Function and the Quadriceps Femoris Activity of Patients with Severe Chronic Obstructive Pulmonary Disease)

  • PURPOSE: This study aimed to propose an exercise technique that helps improve the skeletal muscle function while suppressing the symptoms of respiratory distress, by mediating squat exercises in whole-body vibration for patients with severe COPD, and comparing the post intervention pulmonary function and activity of quadriceps. METHODS: Totally, 21 patients with severe COPD were randomly assigned to two groups through clinical sampling: experimental group I included 11 patients (Squat exercise combined with whole-body vibration exercise), and experimental group II included 10 patients (Only squat exercise). Before intervention, we measured pulmonary function using a pulmonary function tester, muscle activity of quadriceps using surface EMG, and gait ability using the 6MWT. RESULTS: Comparison of intra-group changes in both experimental groups showed a significant increase in the activity of rectus femoris, vastus medialis, and vastus lateralis, and also in the 6MWT. Intra-group comparisons also revealed significant difference in the activity of rectus femoris, vastus medialis, and vastus lateralis (p < .05). CONCLUSION: Squat exercise combined with whole-body vibration significantly increased the activity of the quadriceps muscle, suggesting that this intervention helps maintain the function of skeletal muscles and prevent muscle atrophy. Therefore, studies to develop protocols using whole body vibration in clinical practice as an exercise method can safely be performed in severe COPD patients, as considered necessary.

A SMALL MODULAR REACTOR DESIGN FOR MULTIPLE ENERGY APPLICATIONS: HTR50S

  • HTR50S is a small modular reactor system based on HTGR. It is designed for a triad of applications to be implemented in successive stages. In the first stage, a base plant for heat and power is constructed of the fuel proven in JAEA's $950^{\circ}C$, 30MWt test reactor HTTR and a conventional steam turbine to minimize development risk. While the outlet temperature is lowered to $750^{\circ}C$ for the steam turbine, thermal power is raised to 50MWt by enabling 40% greater power density in 20% taller core than the HTTR. However the fuel temperature limit and reactor pressure vessel diameter are kept. In second stage, a new fuel that is currently under development at JAEA will allow the core outlet temperature to be raised to $900^{\circ}C$ for the purpose of demonstrating more efficient gas turbine power generation and high temperature heat supply. The third stage adds a demonstration of nuclear-heated hydrogen production by a thermochemical process. A licensing approach to coupling high temperature industrial process to nuclear reactor will be developed. The low initial risk and the high longer-term potential for performance expansion attract development of the HTR50S as a multipurpose industrial or distributed energy source.

심부전 환자의 신체활동과 규칙적 운동에 대한 측정의 일관성 및 운동역량과 삶의 질과의 관련성 (Consistency in Measuring Physical Activities and Regular Exercises and Their Relationship with Exercise Capacity and Quality of Life in Patients with Heart Failure)

  • Purpose: In order to examine whether daily physical activity (DPA) can be considered the same as a regular exercise (RE) in patients with Heart Failure (HF), we examined the relationship between RE and DPA, and we explored the relationship of RE and DPA with exercise capacity and quality of life (QOL) in patients with HF. Methods: This cross-sectional, correlational study utilized pretest data (N=136) of a long-term intervention study. Data of the parent study were collected from May 1st to September 26th, 2013. Measures included questions about regular exercise, International Physical Activity Questionnaire, a 6-Minute Walk Test (6MWT), and the Kansas City Cardiomyopathy Questionnaire (KCCQ). The data were analyzed using descriptive analysis, Chi-square test, and ANOVA using the SPSS 21.0 program. Results: There were inconsistencies in measures between RE and DPA. About 42.0% of the participants engaging in health enhancing physical activity did not consider themselves to be regular exercisers. Depending on the levels of RE and DPA, significant differences in 6MWT and QOL were observed. Conclusion: RE is more likely to be related to the health status of patients with HF. Deliberate assessment and cautious interpretation of DPA in patients with HF is needed.

가상현실 운동프로그램이 파킨슨병 환자의 균형, 보행 및 낙상 효능감에 미치는 영향 (The Effect of Virtual Reality-Based Exercise Program on Balance, Gait, and Falls Efficacy in Patients with Parkinson's Disease)

  • PURPOSE: The purpose of this study was to determine if virtual reality-based exercise was effective in balance, gait, and falls efficacy in patients with Parkinson's disease (PD). METHODS: Thirty patients with PD were assigned randomly to the experimental (n=15) or control groups (n=15). The experimental group performed virtual reality-based exercise and the control group underwent conventional physical therapy for 30minutes, five times per week for four weeks. A force platform system, the Korean version of the Berg Balance Scale (K-BBS), the six-minute walking test (6MWT), and the Korean Version of the Falls Efficacy Scale (K-FES) were used to evaluate balance, gait, and falls efficacy. Wilcoxon signed-rank test and Mann-Whitney U test were used to examine the within- and between-group differences after training, respectively. RESULTS: Changes in the K-BBS score (p<.001) and fall efficacy (p<.01), following the intervention were significantly greater in the experimental group than in the control group whereas significant group difference were not observed for the anterior-posterior and mediolateral postural sway lengths. The change in the ground reaction force (p<.001) and 6MWT values (p<.05) were significantly greater after intervention in patients in the experimental group than in the control group, whereas a significant group difference was not observed for the step and stride lengths. CONCLUSION: This study indicates that virtual reality-based exercise is an effective intervention for improving balance, gait, and fall efficacy in patients with PD.

만성 뇌졸중 환자의 균형 및 보행 증진을 위한 체간 자가 훈련 프로그램 -예비연구- (Self-Training Trunk Program for Improving Balance and Walking Ability in People with Chronic Stroke -A Preliminary Study-)

  • Purpose: Balance and walking ability are important elements of functional independence for people with stroke and are major goals in rehabilitation. These abilities require trunk performance, but most chronic stroke patients reach a plateau in their rehabilitation. Therefore, the purpose of this study was to investigate the effects of a self-training trunk program to improve balance and walking ability in people with stroke, and to suggest such a self-training program for integrated rehabilitation of people with stroke. Methods: The study recruited 7 people with stroke. The subjects performed trunk training for 30 min per day, 6 days per week, for 3 weeks. Participants were measured on the Trunk Impairment Scale (TIS), the Berg Balance Scale (BBS), the Korean Activities-specific Balance Confidence (K-ABC) scale, the Falls Efficacy Scale (FES), the Functional Gait Assessment (FGA), the 6 Minute Walk Test (6MWT) and a gait analysis to measure the training effects. Statistical analysis used the Wilcoxon signed-rank test as a non-parametric statistical test. Results: TIS was not significantly different after the self-training trunk program, but BBS (p>0.05), K-ABC, and FES were significantly improved after the training program (p<0.05). Furthermore, the 6MWT, stride/height %, and one-leg stance were significantly improved after the training program (p<0.05), but cadence, stance, and swing duration were not significantly different after the training (p>0.05). Conclusion: These results suggest that a self-training trunk program should be integrated into stroke rehabilitation to improve balance and walking ability, and further research is needed to develop the program to be more effective for chronic stroke patients.

Hyperlipidemia as a predictor of physical functioning for stroke

  • Objective: Elevated cholesterol levels contribute to changes of the arterial endothelial permeability. Hyperlipidemia promotes atherosclerosis and is associated with an increased risk of stroke incidence. The purpose of this study was to investigate the effects of having a history of hyperlipidemia prior to a stroke incidence on postural balance, anticipatory dynamic postural control, gait endurance and gait performance in individuals with hemiparetic stroke. Design: Cross-sectional study. Methods: Fifty-two adults who were diagnosed with stroke 6 months ago or more were enrolled in this study. They were divided into two different groups according to hyperlipidemia history before stroke. All participants were assessed with the Activities-specific into Balance Confidence (ABC) scale, Berg Balance scale (BBS), Dynamic Gait Index (DGI), Timed Up and Go test (TUG), and the 6-minute walk test (6MWT). An independent t-test was used to analyze the difference between the hyperlipidemia group and non-hyperlipidemia group. Results: After analysis, the BBS, TUG, and 6MWT scores were significantly different between the hyperlipidemia and non-hyperlipidemia group, but not the ABC and DGI scores. Conclusions: The results of this study show that having a history of hyperlipidemia before stroke affects static and dynamic postural balance performance, anticipatory dynamic postural balance, and gait endurance in individuals with chronic hemiparetic stroke. Based on the results of this study, we also suggest treatment for hyperlipidemia should be implemented throughout the therapeutic interventions, such as pharmacological or exercise programs, in order to restore the physical function of stroke survivors.

흉곽확장을 병행한 흡기근훈련이 뇌졸중 환자의 폐기능과 최대흡기압력 및 보행지구력에 미치는 영향 (The Effects of Inspiratory Muscle Training with Chest Expansion on Pulmonary Function, Maximal Inspiratory Pressure, and Gait in Individuals with Stroke)

  • Purpose: The purpose of this study was to evaluate and compare the effects of inspiratory muscle training with chest expansion exercises on pulmonary function, maximal inspiratory pressure, and gait in individuals with stroke. Methods: The participants in this study included 36 stroke patients. These patients were randomly divided into three groups: an inspiratory muscle training (IMT) with chest expansion (CE) group (n=12), an IMT group (n=12), and a control group (n=12). Participants in the IMT with CE group underwent IMT and CE exercises 5 times per week for 30 minutes over 4 weeks, whereas those in the CE group and the control group received IMT and conventional physical therapy, respectively, for the same duration. The investigator measured the patients' pulmonary function, maximal inspiratory pressure, and gait endurance. Results: After the intervention, the change values for the forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), maximal inspiratory pressure (MIP), and six-minute walk test (6MWT) in the IMT with CE group and the control group were significantly greater than those of the control group (p<0.05). Similarly, after the intervention, the change values of the FVC, FEV1, PEF, MIP, and 6MWT in the IMT with CE group were significantly greater than those in the IMT group (p<0.05). Conclusion: These findings suggest that IMT with CE could be used to increase pulmonary function, maximal inspiratory pressure, and gait endurance in stroke patients.

낙상예방 운동 프로그램이 치매 노인의 균형, 보행과 하지 근력, 심리사회적 특성에 미치는 영향 (Impact of a Fall-Prevention Exercise Program on Balance, Gait, Lower Limb Strength, and Psychosocial Characteristics in Older Adults with Dementia)

  • PURPOSE: This study measured the impact of a 12-week fall-prevention exercise program on balance, ambulatory ability, lower limb strength, and psychosocial characteristics in older adults diagnosed with dementia. METHODS: The participants comprised 31 older adults (9 men, 22 women) diagnosed with Alzheimer's or vascular dementia at a long-term care hospital located in Gunsan City. A fall-prevention exercise program was provided to the experimental group, while the control group was only provided with instruction and materials related to the fall-prevention exercise program. The participants were evaluated before the intervention, 6 weeks after the intervention, and 12 weeks after the intervention on static and dynamic balance abilities (using Timed Up and Go test: TUG, Tinetti-balance scale, one-leg standing test: OLS), gait (Tinetti-balance scale, 6-minute walk test: 6MWT), lower limb strength (sit to stand test; STS), and psychosocial characteristics (Short Form 36 Health Survey-Korean version, Korean Mini-Mental State Exam). RESULTS: An independent samples t-test and repeated measures analysis of variance were used for the statistical analysis. There were statistically significant improvements after the intervention (p<.05) in dynamic balance abilities (TUG and OLS using the left foot), gait (6MWT), and lower limb strength (STS) for the experimental group, but not for the control group. No difference was seen in psychosocial characteristics. CONCLUSION: Older adults with dementia who participated in the fall-prevention exercise program showed significant improvements in their static and dynamic balance abilities, lower limb strength, and ambulatory ability after the intervention.
