Non-rigid Registration Method of Lung Parenchyma in Temporal Chest CT Scans using Region Binarization Modeling and Locally Deformable Model (영역 이진화 모델링과 지역적 변형 모델을 이용한 시간차 흉부 CT 영상의 폐 실질 비강체 정합 기법)

  • In this paper, we propose a non-rigid registration method of lung parenchyma in temporal chest CT scans using region binarization modeling and locally deformable model. To cope with intensity differences between CT scans, we segment the lung vessel and parenchyma in each scan and perform binarization modeling. Then, we match them without referring any intensity information. We globally align two lung surfaces. Then, locally deformable transformation model is developed for the subsequent non-rigid registration. Subtracted quantification results after non-rigid registration are visualized by pre-defined color map. Experimental results showed that proposed registration method correctly aligned lung parenchyma in the full inspiration and expiration CT images for ten patients. Our non-rigid lung registration method may be useful for the assessment of various lung diseases by providing intuitive color-coded information of quantification results about lung parenchyma.

Development and Radiation Shield effects of Dose Reduction Fiber for Scatter ray in CT Exams (피폭선량저감 섬유의 개발과 CT 검사시 산란선 차폐 효과)

  • In this study, we developed and characterized the shielding properties of dose reduction fiber (DRF, Buffalo Co.) sheet during brain and chest CT examinations. The DRF sheet was composed of $1{\sim}500{\mu}m$ oxide Bismuth ($Bi_2O_3$) and 5 ~ 50 nm nano-barium sulfate ($BaSO_4$). Phantom and clinical studies were performed for characterization of the DRF shielding properties. In clinical study, we measured doses of eye, chest, abdomen and reproductive system of 60 patients in 3 hospitals during brain and chest CT examinations. We could determined the shielding effect of the DRF by comparing the doses when we used the DRF sheet or not. When we used the sheet during CT examination, the scattered dose were reduced about 20~50%. So, we suggest that the fiber should be used in radiological examinations for reducing patients doses.

Chest Radiographs and CT Findings during Healthcare Workers' Tuberculosis Screening Using Interferon- Gamma Release Assay: Retrospective Observational Study (인터페론-감마 분비 검사를 이용한 의료 종사자의 결핵 스크리닝에서 흉부 X선 사진 및 CT 소견: 후향적 관찰 연구)

  • Purpose To investigate the incidence of tuberculosis (TB) in healthcare workers (HCWs) with positive interferon-gamma release assay (IGRA) results based on chest X-ray (CXR) and CT findings and determine the role of imaging in the diagnosis of TB. Materials and Methods Among 1976 hospital personnel screened for TB using IGRA, IGRApositive subjects were retrospectively investigated. Clustered nodular and/or linear streaky opacities in the upper lung zone were considered positive on CXR. The CT findings were classified as active, indeterminate, inactive, or normal. The active or indeterminate class was considered CT-positive. Results IGRA was positive in 255 subjects (12.9%). CXR and CT were performed in 249 (99.2%) and 113 subjects (45.0%), respectively. CXR- and CT-positive findings were found in 7 of 249 (2.8%) and 9 of 113 (8.0%) patients, respectively. Among the nine CT-positive subjects, active and indeterminate TB findings were found in 6 (5.3%) and 3 (2.7%) patients, respectively. Microbiological tests, including acid-fast bacilli staining, culture, and polymerase chain reaction for TB, were negative in all nine CT-positive subjects. Empirical anti-TB medications were administered to 9 CT-positive subjects, and 3 of these nine subjects were CXR-negative for pulmonary TB.

Evaluation of the usefulness of Images according to Reconstruction Techniques in Pediatric Chest CT (소아 흉부 CT 검사에서 재구성 기법에 따른 영상의 유용성 평가)

  • With the development of technology, efforts to reduce the exposure dose received by patients in CT scans are continuing with the development of new reconstruction techniques. Recently, deep learning reconstruction techniques have been developed to overcome the limitations of repetitive reconstruction techniques. This study aims to evaluate the usefulness of images according to reconstruction techniques in pediatric chest CT images. Patient study conducted a study on 85 pediatric patients who underwent chest CT scan at P-Hospital in Gyeongsangnam-do from January 1, 2021 to December 31, 2022. The phantom used in the Phantom Study is the Pediatrics Whole Body Phantom PBU-70. After the test, the images were reconstructed with FBP, ASIR-V (50%) and DLIR (TF-Medium, High), and the images were evaluated by obtaining SNR and CNR values by setting ROI of the same size. As a result, TF-H of deep learning reconstruction techniques had the lowest noise value compared to ASIR-V (50%) and TF-M in all experiments, and SNR and CNR had the highest values. In pediatric chest CT scans, TF images with deep learning reconstruction techniques were less noisy than ASiR-V images with adaptive statistical iterative reconstruction techniques, CNR and SNR were higher, and the quality of images was improved compared to conventional reconstruction techniques.

Korean Clinical Imaging Guidelines for Justification of Diagnostic Imaging Study for COVID-19 (한국형 COVID-19 흉부영상 진단 시행 가이드라인)

  • To develop Korean coronavirus disease (COVID-19) chest imaging justification guidelines, eight key questions were selected and the following recommendations were made with the evidence-based clinical imaging guideline adaptation methodology. It is appropriate not to use chest imaging tests (chest radiograph or CT) for the diagnosis of COVID-19 in asymptomatic patients. If reverse transcription-polymerase chain reaction testing is not available or if results are delayed or are initially negative in the presence of symptoms suggestive of COVID-19, chest imaging tests may be considered. In addition to clinical evaluations and laboratory tests, chest imaging may be contemplated to determine hospital admission for asymptomatic or mildly symptomatic un-hospitalized patients with confirmed COVID-19. In hospitalized patients with confirmed COVID-19, chest imaging may be advised to determine or modify treatment alternatives. CT angiography may be considered if hemoptysis or pulmonary embolism is clinically suspected in a patient with confirmed COVID-19. For COVID-19 patients with improved symptoms, chest imaging is not recommended to make decisions regarding hospital discharge. For patients with functional impairment after recovery from COVID-19, chest imaging may be considered to distinguish a potentially treatable disease.

  • Endobronchial lipomas are rare benign tumors that arise from the lung. They partially or totally obstruct the bronchial lumen, producing a variable degree of collapse, irreversible bronchiectasis, and pulmonary damage. Although bronchoscope, CT and MR are reported to be helpful in establishing the diagnosis, CT is highly specific and sensitive in detecting fatty tumor. They may be removed by endoscope or thoracotomy or lobectomy. We present a case of endobronchial lipoma completely obstructing the right middle lobe and postobstructive irreversible pulmonary change with review of literatures.

Detection and Analysis of the liver Area and liver tumors in CT Images using Quantization Method and Fuzzy based-SOM Algorithm (양자화 기법과 퍼지 기반 SOM 알고리즘을 이용한 CT 영상에서의 간 영역과 간 종양 검출 및 분석)

  • 간은 인체의 생명을 유지하고 성장할 수 있도록 하는 영양섭취와 매우 밀접한 관계를 가진 중요한 장기이다. 이러한 간의 중요성에도 불구하고 현재 우리나라의 간암 발병률이 세계에서 가장 높은 수치를 기록하고 있으며 이에 따라 간암을 조기 진단하고 예방할 수 있는 방법의 중요성이 확대되고 있다. 따라서 본 논문에서는 영상 의학적 검사 방법 중 하나인 CT 촬영으로 획득된 조영 증강 CT 영상에서 간 영역과 간 종양 영역을 정확히 검출하고 간 종양의 악성도를 판별할 수 있는 방법을 제안한다. 흉부로부터 5mm 간격으로 약 $40\;{\sim}\;50$장 정도로 촬영한 조영 증강 CT 영상에서 명암도와 명암의 분포도를 이용한 양자화 기법과 장기들의 위치 및 형태학적 특징정보, 그리고 흉부와 복부 양방향으로 인접한 CT 영상들의 정보를 분석하여 간 영역을 검출한다. 간 종양 영역은 과혈관성 종양의 특징을 분석하고 간 영역의 검출 방법에 적용하여 추출한다. 추출된 간 종양 영역은 퍼지 기반 SOM 알고리즘을 제안하여 간 종양의 악성도를 분석하는데 적용한다. 제안된 퍼지 기반 SOM 알고리즘은 SOM의 이웃 반경을 동적으로 조정하는데 퍼지 제어 기법을 적용하여 기존의 SOM 알고리즘보다 종양의 악성 정도를 분류하는 정확성을 개선하였다. 제시된 간 영역과 간 종양 검출 및 분석 방법의 결과와 전문의가 진단한 결과를 비교 분석한 결과, 기존의 간 영역 및 간 종양 영역 검출 방법보다 정확성이 향상된 것을 확인할 수 있었다.
