Purpose: Purposes of this study were to develop a community-based respite program for family caregivers and to test the effects of the program. Methods: Focus group interviews were performed to extract meaning of respite care for family caregivers (13 participants) and a survey was done to identify respite needs of family caregivers (157 participants). The community-based respite program for family caregivers was developed based on results of the focus group interview and survey. The program was used with 41 participants (19 experimental and 22 control). Independent t-test and Mann-Whitney U-test were used to test differences between control and experimental groups for respite needs, burden of caregivers, subjective wellbeing, social support, fatigue and functional status of elders with dementia. Results: There were statistical differences in caregiver burden, subjective wellbeing, and social support after the program, but, none for respite needs, fatigue and functional status of elders with dementia. Conclusion: The results indicate that a respite program can be useful to decrease burden of caregivers and increase subjective wellbeing and perceived social support of family caregivers in community settings. Further intervention research is needed to increase the functional status of elders with dementia and decrease fatigue in caregivers.
The purpose of this study was to analyze the effects of respite care. The analysis was conducted by reviewing published intervention studies on the effects of formal respite care for caregivers of dementia patients, patients with dementia, and the prevented or delayed rate of institutionalization of the patients. Method: Two computerized databases (MEDLINE, CINAHL) were searched to find respite care-related articles published from the year of 1981 to 2000. A total of 49 published articles were identified. Of them, nine studies, which met for the inclusion criteria of this study, were included. Results: Results revealed that there was little evidence of the effect of respite care on, not only caregivers' burden, stress, depression and well-being, but also the rate of institutionalization of the patients. It was noteworthy that dementia patients reported fewer problems in behavior, although cognitive functioning and activity of daily living abilities continued to decline. However, these findings should be carefully interpreted because of methodological problems, such as non-random sampling, non random group assignment, a small sample size, uncontrolled confounding variables, limited period of services, and no specific types of services. Conclusion: It is recommended to conduct intervention studies of respite care being conducted in Korea with the corrections of methodological problems suggested from this study.
본 연구는 급변하는 의료환경에 대비하여 간호의 질 향상 및 증진을 통한 환자 만족도 증진을 위해 간호전달체계를 기능적 간호방법에서 팀간호제 방법으로 전환을 하면서 이에 따른 간호활동 및 직무 만족도를 조사하였다. 간접간호 수행률 조사를 살펴보면 기록간호, 확인업무, 물품관리, 간접소통, 전달 업무 등 모두에서 유의한 차이를 보이지 않았다. 영양간호, 전적인 식사보조 등은 간호직무만족도가 영향을 미치는 것으로 나타났다. 간호사 직무만족도는 어려움이 있을 때 동료나 상사에게 의논하는 것으로 나타났다. 안전간호, 화재예방 등이 영향을 미치는 것으로 나타났다. 간호사 업무성과 병동의 시설과 기구를 적절히 관리한다. 간호사 업무성과, 환자에게 적절한 휴식 및 안정을 위한 간호를 제공하는 것으로 나타났다. 본 연구 결과 간호사들이 역할을 명확히 규명하고 세분화하여 이를 수행하는 직무내용을 명확하게 파악하고 작업표준 지침서를 작성하여 활용한다면 간호사들의 자가 관리 증진은 물론 환자와 보호자에게도 만족감을 높여 줄 수 있는 계기가 될 것이며, 이를 통한 다양한 팀제 운영이 될 것이라 생각된다. 또한, 향후 다양한 업무 개선 활동이 가능할 것이라 생각된다.
Purpose: Respite care is not a discrete intervention, but encompasses a range of services. This research was conducted to clarify the phenomenon of respite care for family caregivers of elders with dementia from a nursing perspective. Method: The Hybrid Model of concept development was applied to clarify the concept of respite care for family caregivers of elders with dementia. The study was conducted in the following three steps, theoretical phase, fieldwork phase, and final analytic phase. Results: The definition of respite care for family caregivers of elders with dementia was delineated through integration of data analyses in theoretical and fieldwork phase, and has three dimensions; tailored supports for caregivers, tailored supports based on physical and cognitive function of elders with dementia and community interventions related to family care function. Conclusion: Through this study, the concept of respite care for family caregivers of elders with dementia is clarified and reformulated as nursing practice phenomena in the Korean context, which indicates ways to develop caring practice forms for a family living with an elder with dementia in a community setting.
Purpose: In this study rest-activity rhythm, sleep pattern and quality of life of patients with restless legs syndrome were compared with those of a normal group. Methods: The participants in this study were 36 patients with restless legs syndrome diagnosed by a neurologist and 36 participants in the normal group. An actigraph, sleep diary, Pittsburgh Sleep Quality Index and Insomnia Severity Index scale were used as measurement tools for the study. Chi-square test, Lamda test, t-test and Kendall's correlation with SPSS 12.0 program were used to analyze the data. Results: Patients with restless legs syndrome had a higher rest-activity rhythm curve of Least 5 hr's activity(L5) and Most 10 hr's activity(M10) than those of normal group and sleep problems included decreased sleep efficiency and increased sleep latency, wake time and number of awakenings. The scores for the subscales of quality of life in patients with restless legs syndrome were lower than the normal group for general health, physical functioning, role limitations due to emotional problems, role limitation due to physical problems, social functioning, bodily pain, vitality and mental health. Conclusion: The results suggest that further studies are needed to identify rest-activity rhythm according to symptom severity and to develop nursing interventions which consider rest-activity rhythm.
Purpose: The purpose of this study was to identify the time for young adults to reach resting blood pressure after walking as measured by an automatic and a mercury sphygmomanometer. Methods: Participants were 33 nursing students in their 20s. Blood pressure after walking was measured every minute on both arms simultaneously for a total of 6 times after walking for 12 minutes on a treadmill at ratings of perceived exertion (RPE) of 11. Data were analyzed using paired t-test, Bland-Altman plots and repeated measures ANOVA. Results: Systolic blood pressure after walking was the same as measurements corresponding to resting blood pressure after 3 minutes of resting if measured with the automatic sphygmomanometer and 4 minutes of resting if measured with the mercury sphygmomanometer. Conclusions: In order to measure the resting blood pressure for healthy young adults who performed low-density walking for 12 minutes on flat land, the measurement needs to be made after a resting time of at least 3 minutes in the case of an automatic sphygmomanometer and 4 minutes in the case of a mercury sphygmomanometer.
본 연구는 오마하체계를 이용하여 보건소 방문간호사의 간호업무 수행도와 필요도를 조사한 서술적 조사연구이다. 방문간호사의 간호업무는 '개인위생', '통증', '약물요법', '영양', '신체활동', '위생', '수면과 휴식양상', '구강건강', '정신건강', '인지'순으로 수행도가 높았다. '성생활', '산후', '수입', '가족계획', '임신', '영성', '학대', '생식기능', '방임', '말과 언어' 순으로 간호업무 수행도가 낮았다. '성생활', '학대' '방임', '생식기능' 문제는 문제의 사정 및 적극적 개입을 위한 교육적 지원이 필요하다. '산후', '임신', '가족계획' 등의 문제는 출산지원을 위한 지역의 정책방향에 따라 방문간호사의 역량 강화가 필요하다. '수입', '주거', '지역사회자원과의 의사소통'은 보건복지 연계 및 정부와 지방자치단체의 정책적 지원이 필요하겠다. 본 연구결과는 방문간호사의 역할규명 및 역할확대를 위한 기초자료로 활용할 수 있을 것으로 본다.

Purpose: This study was conducted to develop an integrated undergraduate course including a PBL based on a blended learning strategy, and evaluate learners' responses. Methods: The learning contents of cardiovascular, respiratory, and musculoskeletal medical systems, and nursing diagnoses of 'activity and rest' domain (NANADA's classification II, 2005) were analyzed. Six clinical scenarios with the clients in different life cycles were developed for PBL. Classical lecture and group presentation with on-line self learning were implemented in addition to PBL. The developed course was implemented on 84 junior nursing students in a university for 7 weeks with 5 hours per day, two days per week. Students were asked to complete structured questionnaires including problem solving, critical thinking, and nursing diagnosis differentiation abilities. Results: Learner's evaluation was positive in problem solving skills and in the differentiation ability of nursing diagnoses relevant to an 'activity and rest' functional health pattern. Conclusion: Development and implementation of integrated courses based on a blended learning method need to be continued to enhance students' thinking and self-directed learning abilities. Supporting strategies for individual learners should be added for successful blended learning such as individual on-line feedback and consideration of individual learning outcomes.
