Clinical analysis of febrile infants and children presenting to the pediatric emergency department (소아응급실에 내원한 발열 환아에 대한 연구 분석)

  • Purpose : We studied clinical patterns of the febrile infants and children presenting to the Pediatric Emergency Department(ED) in the general pediatric population. Method : We analyzed 1,764 infants and children who had a history of measured body temperature of $38.3^{\circ}C$ or higher, before or after arrival at Pediatric ED of Ilsan Paik Hospital from September 2004 to August 2005. We analyzed their clinical patterns, such as the frequency of febrile illness, age distribution, monthly distribution, diagnosis. Result : The frequency of the febrile illness at Pediatirc ED was 19.9 percent (1,764/8,881 cases). The percentage of admission into hospital was 26.4 percent. The most common age group presenting to Pediatric ED with febrile illness was the 12-23 months group(22.8 percent) followed by the 4-11 months group and the 2 years group. In May, the most numerous patients presented to the Pediatric ED with febrile illness. The frequency of febrile illness was significant higher in the 'Spring & Summer group' compared to the 'Autumn & Winter group'. The ratio of male to femal was 1.4:1. The most common diagnosis was acute pharyngitis(42.5 percent), followed by acute gastroenteritis (15.3 percent), pneumonia(7.0 percent). Conclusion : The frequency of the febrile illness at Pediatirc ED was 19.9 percent. The most numerous patients visited Pediatric ED in May. And, the most common age group was 12-23 months. The proportion of non-urgent disease, such as acute pharyngitis, acute gastroenteritis, acute nasopharyngitis and acute bronchitis, was high. There is a need to educate parents and improve the medical system in Korea.

How Effective Are Radiation Reducing Gloves in C-arm Fluoroscopy-guided Pain Interventions?

  • Background: The physician's hands are close to the X-ray field in C-arm fluoroscopy-guided pain interventions. We prospectively investigated the radiation attenuation of Proguard RR-2 gloves. Methods: In 100 cases, the effective doses (EDs) of two dosimeters without a radiation-reducing glove were collected. EDs from the two dosimeters-one dosimeter wrapped with a glove and the other dosimeter without a glove-were also measured at the side of the table (Group 1, 140 cases) and at a location 20 cm away from the side of the table (Group 2, 120 cases). Mean differences such as age, height, weight, radiation absorbed dose (RAD), exposure time, ED, and ratio of EDs were analyzed. Results: In the EDs of two dosimeters without gloves, there were no significant differences ($39.0{\pm}36.3{\mu}Sv$ vs. $38.8{\pm}36.4{\mu}Sv$) (P = 0.578). The RAD ($192.0{\pm}182.0radcm^2$) in Group 2 was higher than that ($132.3{\pm}103.5radcm^2$) in Group 1 (P = 0.002). The ED ($33.3{\pm}30.9{\mu}Sv$) of the dosimeter without a glove in Group 1 was higher than that ($12.3{\pm}8.8{\mu}Sv$) in Group 2 (P < 0.001). The ED ($24.4{\pm}22.4{\mu}Sv$) of the dosimeter wrapped with a glove in Group 1 was higher than that ($9.2{\pm}6.8{\mu}Sv$) in Group 2 (P < 0.001). No significant differences were noted in the ratio of EDs ($73.5{\pm}6.7%$ vs. $74.2{\pm}9.3%$, P = 0.469) between Group 1 and Group 2. Conclusions: Proguard RR-2 gloves have a radiation attenuation effect of 25.8-26.5%. The radiation attenuation is not significantly different by intensity of scatter radiation or the different RADs of C-arm fluoroscopy.

The Radiation Exposure of Radiographer Related to the Location in C-arm Fluoroscopy-guided Pain Interventions

  • Background: Although a physician may be the nearest to the radiation source during C-arm fluoroscope-guided interventions, the radiographer is also near the fluoroscope. We prospectively investigated the radiation exposure of radiographers relative to their location. Methods: The effective dose (ED) was measured with a digital dosimeter on the radiographers' left chest and the side of the table. We observed the location of the radiographers in each procedure related to the mobile support structure of the fluoroscope (Groups A, M and P). Data about age, height, weight, sex, exposure time, radiation absorbed dose (RAD), and the ED at the radiographer's chest and the side of the table was collected. Results: There were 51 cases for Group A, 116 cases for Group M and 144 cases for Group P. No significant differences were noted in the demographic data such as age, height, weight, and male to female ratio, and exposure time, RAD and ED at the side of the table. Group P had the lowest ED ($0.5{\pm}0.8{\mu}Sv$) of all the groups (Group A, $1.6{\pm}2.3{\mu}Sv$; Group M, $1.3{\pm}1.9{\mu}Sv$; P < 0.001). The ED ratio (ED on the radiographer's chest/ED at the side of the table) of Group A was the highest, and the ED radio of Group P was the lowest of all the groups (Group A, $12.2{\pm}21.5%$; Group M, $5.7{\pm}6.5%$; Group P, $2.5{\pm}6.7%$; P < 0.001). Conclusions: Radiographers can easily reduce their radiation exposure by changing their position. Two steps behind the mobile support structure can effectively decrease the exposure of radiographers by about 80%.

A Study on Characteristics of Dyspnea in Patients with Chronic Obstructive Pulmonary Disease in the Emergency Department Visits (응급실에 내원한 만성 폐쇄성 폐질환 환자의 호흡곤란 속성에 관한 연구)

  • Purpose: The purpose of this study was to examine descriptors of dyspnea and clinical characteristics in patients with COPD at ED visit. Method: The study design was a descriptive survey, and 46 patients with COPD complaining of dyspnea were participated during their visits to the ED of C university hospital in G city, from October 1, 2004 to April 8, 2005. Result: The qualitative characteristics of dyspnea at the time of decision to the ED visit were short of breath (71.7%), tight (32.6%), hard to breathe (15.2%), smothering or suffocating (10.9%), gasping (8.7%), couldn't breathe (6.5%), constricted (2.2%), and hunger for air (2.2%). In the item of dyspnea checklists, My chest felt tight was the most common description among subjects. As the result of factor analysis of dyspnea checklists, the first factor was characterized rapid and shallow, the second was suffocating/smothering, the third was hunger for air, and the fourth was constricted. Conclusion: This study suggests that a checklist of dyspnea descriptors based on this findings would be utilized as a tool of initial and ongoing assessment for dyspneic patients with COPD in the ED after identifying the validity and reliability of the checklist.

Detection of Anticancer Activity from the Root of Angelica gigas In Vitro

  • Anticancer activity of a fraction of the ethanol extract from the root of Korean angelica (Angelica gigas Nakai) was recognized in human cancer cell lines HeLa $S_3$, K-562, and Hep $G_2$. The extract blocked the phorbol ester-inducing megakaryocytic differentiation of K-562 cells, which indicated the modification of protein kinase C (PKC) activity. In vitro assay showed the activation of PKC by the extract. An effective fraction of the Angelica gigas extract, of which $R_f$ value was 0.64 in a thin layer chromatography, was a different component from those of European angelicas. The $ED_50$ value of the fraction was 8, 9, and $16\;\mu\textrm{m}/ml$ against HeLa $S_3\;Hep\;G_2$, and K-562 cells, respectively, while the fraction showed higher $ED_50$ values against normal cell lines.

  • Sn-Zn계 solder에서 Zn함량의 변화에 따른 납땜성을 납땜분위기 및 용제를 달리하여 연구하였다. 계면에서의 미세조직 관찰은 열처리온도를 8$0^{\circ}C$와 10$0^{\circ}C$로 달리하여 100일간 열처리한 후 관찰하였다. 젖음성 측정 결과, Zn함량이 증가함에 따라 젖음성은 감소하였고 RMA-용제를 사용한 경우가 R-용제를 사용한 경우에 비해 젖음성이 향상되었다. Sn-9Zn의 접촉각은 약 45도이고, 공기중에서 보다 질소 분위기에서 납땜한 경우가 젖음성 개선을 나타냈다. Sn-9Zn땝납과 Cu기판에서의 계면반응을 XRD, EDS로 분석한 결과 계면화합물은 r상(Cu$_{5}$Zn$_{3}$)으로 구성되어 있음을 알 수 있으며, 시효처리에 따라 접합부의 solder쪽에는 Zn상의 고갈이 나타남을 확인할 수 있었다.

Ruminal Degradability of Tropical Feeds and Their Potential Use in Ruminant Diets

  • The objective of this study was to determine the degradability of cassava chip (CC), cassava waste (CW), yellow sweet potato (YP), white sweet potato (WP), purple sweet potato (PP), corn meal (CM), and rice bran (RB) using in situ technique. Two ruminally fistulated steers with an average weight of $303{\pm}10kg$ were used to determine in situ degradabilities of DM and OM. Seven feed sources were weighted in nylon bags ($38{\mu}m$ pore size) and incubated ruminally for 1, 2, 4, 6, 8, 12, 24, and 48 h. The results showed that asymptote (a+b) and effective degradability (ED) of DM of energy sources ranked from the highest to the lowest; CC, YP, WP, PP, RB, CW, and CM (99.3, 92.5; 97.6, 87.9; 97.5, 87.9; 97.2, 87.8; 87.5, 63.6; 78.6, 63.0 and 81.7; 59.3, respectively) and for OM asymptote (a+b) and effective degradability (ED) were similar to those of degradation of DM (99.4, 93.4; 98.8, 89.8; 98.5, 89.4; 98.4, 88.1; 92.4, 65.8; 85.1, 66.9 and 83.6, 63.3, respectively). It was concluded that disappearance characteristic of CC was the highest and it may potentially facilitate the achievement of optimal ruminal availability of energy: protein especially with NPN for microbial protein synthesis.

Anti-cell Adhesive Effect of Phenylacetylshikonin Analogues Related to their Cytotoxicity in A549 Cells

  • An attempt to estabilish the relationship between anti-cell adhesive action of phenylacetylshikonin analogues and their cytotoxicity against A549 cells was done. In the one hour incubation with A549 cells,${\alpha}$-methoxyphenylacetyl-(9), ${\alpha}$-acetoxyphenylacetyl-(13), 3,4-methylenedioxyphenylacetyl-(15) and 4-(N,N-dimethylamino)-phenylacetylshikonin (17) analogues showed a high anti-cell adhesive activity $(IC_100; value, 4-8{\mu}g/ml)$, while halophenylacetyl- and dimethoxy- or trimethoxyphenylacetyl analogues expressed no activity at $40{\mu}g/ml$, indicating that the presence of a bulky group at $ C^I-{\alpha}$ and a polar group at C-4 of phenylacetyl moiety may be important. A similar structure activity relationship exists for the 48 hr cytotoxocity $(ED_{50})$ of phenylacetylshikonin analogues in A 549 cells, but not in either K562 or L1210 cells. Furthermore, the difference between $IC_{100}$ values for anti-cell adhesive activity and$ED_{50}$ values for cytotoxicity of potent compound in A549 cells was not so great (1.5 to 3 times). Based on these observations, it is proposed that the anti-cell adhesive action of phenylacetylshikonins might be responsible for their cytotoxicity in A549 cells.

Studies on the Virus in Silkworm Bombyx mori L. -Resistance to Virus Disease- (가잠 Virus에 관한 연구 -저항성에 관한 기초조사-)

  • 1. Objectives and Importance. Many silkworms have been damaged by nuclear polyhedrosis virus diseases thoughout the country every year causing a decease in cocoon production by approximately ten per cent per year. The damage caused by the infections virus has occured in spite of complete disinfection. In this respect, it is well known it is impossible, at the present time, to protect the silkworm from these virus infections through chemical and physical control methods. Therefore, this author has attempted to solve this urgent problem from the view point of heredity and breeding, discovering the different resistances and heritabilities among 120 stains collected from throughout the country, and selecting the ones with highest resistance for the basic materials in the silkworm breeding. 2. Results of work 1) The strains with strong resistance to the nuclear polyhedrosis virus diseases are N$_4$, N$\sub$15/, N$\sub$48/, C$\sub$55/ and Em. the log ED$\sub$50/ values of them vary between 0.799 and 1.611. The susceptible strains are N$\sub$20/, C$\sub$62/, N$\sub$76/, N$\sub$79/ and C$\sub$108/, the log ED$\sub$50/ values of them vary between 5.159 and 7.258. (Reference Table 4) The Japanese strain with a log ED$\sub$50/, value of 3.770 is the strongest, followed by the Chinese strain with a log ED$\sub$50/ value of 3.564. The weakest is the European strain with a log ED$\sub$50/ value of 3.3381. The direction coefficient of the regression equation of the susceptibility varies between 0.1 and 0.6, the uniformness of the resistance of the preserved strains of this country is comparatively low. The hereditary henomena of the resistance of each strain and the conerete method of its application for silkworm breeding main the subjects for later studies. 2) The content of water and ash in silkworm has not been correlated with the capability for resistance to the virus diseases(Reference. Table 8). but it is very significantly correlated with mortality rate (in common reaning). In the case of the silkworms which have just completed the fourth moulting the content of water and ash is not related to the mortality rate. In the case of the silkworms which have just completed the third moulting, however, the water( +0.326) and ash (+0.362) registered a high significance. The ash content in the first ($\div$0.520) and second ($\div$0.386) moults is highly significant but water content in both cases is not significant (Reference Table 7). 3) The No. 205 strain proved to be the best in character among the various F$_1$ hybrids. No. 204 was very good in strength but a little lower in cocoon character than the control. No. 212 was a little low in coccon character and mortality was average, but the cocoon harvest was the best among all the varieties offered for (Reference Table 9). 4), In short, the above mentioned strains which are known to have strong resistance to the virus disease are expected to provide basic data for breeding strong varieties. CRESTIVE-DX: Design and Implementation of Distrusted Concolic Testing Tool for Embedded Software (CRESTIVE-DX: 임베디드 소프트웨어에 대해 테스트케이스 생성을 지원하는 분산 Concolic 테스팅 도구)

  • This paper presents the design and the implementation of CRESTIVE-DX, a concolic testing tool that distribute the concolic testing process over the embedded target system and the host system for efficient test generation of a target embedded program. CRESTIVE-DX conducts the execution of a target program on the target embedded system to consider possible machine-dependent behaviors of a target program execution, and conducts machine-independent parts, such as search-strategy heuristics, constraint solving, on host systems with high-speed computation unit, and coordinates their concurrent executions. CRESTIVE-DX is implemented by extending an existing concolic testing tool for C programs CREST. We conducted experiments with a test bed that consists of an embedded target system in the Arm Cortex A54 architecture and host systems in the x86-64 architecture. The results of experiments with Unix utility programs Grep, Busybox Awk, and Busybox Ed show that test input generation of CRESTIVE-DX is 1.59 to 2.64 times faster than that of CREST.
