Radiological Change of Kager's Triangle in Patients with Retrocalcaneal Disorders (후족부 질환이 있는 환자에서 Kager씨 삼각의 단순 방사선 상의 변화)

  • Purpose: The aim of this study was to analyze radiological change of Kager's triangle in patients with retrocalcaneal disorders. Materials and Methods: Areas of Kager's triangle and calcaneus were calculated with Heron's Formula. Average of area ratio (area of Kager's triangle / area of calcaneus) was calculated for comparison and analysis, according to disease activity and aging process. One-tailed Z-test was done with the average of area ratio. Results: Average of area ratio (area of Kager's trianlge / area of calcaneous) tends to decrease according to aging process and disease existence. Conclusion: Because area of Kager's triangle in patients with retrocalcaneal disorders tends to change according to disease activity and aging process, we should consider clinical correlation between the anatomical structure of Kager's triangle and retrocalcaneal symptoms.

Chondromyxoid Fibroma of the First Metatarsal - A Case Recurred after Curettage and Bone Graft - (족부에 발생한 재발 연골 유점액 섬유종의 치료 - 1예 보고-)

  • Chondromyxoid fibroma is a benign tumor in adolescents and young adults. The most common location of the tumor is the proximal one-third of the tibia. Chondromyxoid fibroma accounts for less then 1% of all bony neoplasms and foot is affected in about 20% of all cases. The authors have described a case of chondromyxoid fibroma of the first metatarsal which had been recurred 7 months after curettage and iliac bone graft. En bloc resection and reconstruction of the metatarsal with autogenous iliac bone graft were performed. The results were satisfactory and the patient was free of local recurrence at two years follow-up.

  • 압력측정시스템은 입각기동안 동적압력분포를 성공적으로 측정할 수 있으나 하중수용기-중간입각기, 말기입각기-전유각기의 전환기들에 대해서는 운동분석시스템을 사용하지않고는 정확한 정의가 불가능하다. 따라서 저자들은 수평자유보행 시 압력중심의 이동을 이해하기 위해서 정상적인 발을 가진 20-30대 성인남자 78명을 대상으로 동작분석과발바닥 압력측정을 동시에 수행하였다. 결과로 하중반응기-중간입각기 전환시기의 발바닥 압력중심점은 후족부와 중족부의 경계선에서 앞쪽으로 1.9$\pm$1.5frame(32$\pm$24msec)에 위치하였으며 말기입각기-전유각기 전환시기에는 중족골두 최대 압력점의 앞쪽으로 2.3$\pm$2.0 frame(38$\pm$33msec)에 위치하였다. 정상수평보행에 있어서 최초접지 순간 압력중심은 전방으로 빠르게 이동하다가 바로 급속히 감소하여 하중반응기-중간입각기의 전환시기에는 작은 속도로 이동하였다. 압력중심의 이동속도는 그 후 다시 서서히 증가하다가 전체 보행주기의 25% 전후에서 서서히 감소하여 비교적 일정하다가 말기입각기-전유각기의 전환시점에서 다시 급격한 증가를 보였다. 족부질환과 보행특성을 판단하는데 있어서 압력중심의 이동궤적은 매우 유용한 인자가 될 것으로 기대된다. 본 연구를 통해서 압력측정시스템만으로는 정의할 수 없었던 두 전환기인 하중반응기-중간입각기, 말기입각기-전유각기를 결정할 수있게 되었다.

  • 편평족은 내측 세로궁이 소실되어 족저부가 편평하게 되는 변형의 총칭으로, 소아에서 흔하 고 보호자들의 관심이 많은 질환이다. 이학적 검사상 후족부의 외반과 전족부의 외전소견을 보이며, 감별질환을 위해 정확한 이학적 검사와 방사선학적 검사를 필요로 한다. 원인으로는 유연성 편평족, 종골 외반, 선천성 수직 거골, 부주상골, 족근골 결합등이 있으며 정상적 유 아의 경우 시간경과에 따라 저절로 호전되는 유연성 편평족이 대부분이다. 따라서 병적 편평족이 아닌 경우 편평족의 원인과 양호한 자연경과를 보호자에게 설명하는 것이 중요하다. 본 논문에서는 소아 편평족의 감별질환을 위한 각 질환에 있어서의 이학적 소견과 검사소견 그리고 이들의 치료에 대해 살펴 보고자 한다.

Extracorporeal Shock Wave Therapy for Postoperative Pain after Ankle Surgery (족관절 수술 후 발생한 통증에 대한 체외충격파의 치료 효과)

  • Purpose: To evaluate the effectiveness of extracorporeal shock wave therapy (ESWT) for postoperative pain after surgery around ankle. Materials and Methods: We included 21 patients who performed an operation around ankle from 2009 to 2013. 4 times ESWT were applied to the patients who have tenderness more than visual analog scale (VAS) point 4. We evaluated the VAS at each sessions and final follow-up, and American Orthopedic Foot and Ankle Society ankle-hindfoot score (AFOAS) was checked at preoperative and final follow-up. Patient's satisfaction and complications were surveyed. Results: Preoperative VAS was mean 4.7, postoperative pain VAS was mean 6.0 at 4.5 months follow-up. The VAS after ESWT each session at 1, 2, 3 weeks and final follow-up were 4.8, 3.2, 2.3, 2.9, respectively. Mean final follow-up period was 9.4 months. Final VAS were significantly decreased (p<0.001). Final VAS were increased from VAS at last session of ESWT, however, not significant (p=0.189). AOFAS significantly improved from preoperative 60 to final follow-up 86 (p<0.001). Excellent was 12 patients (57%), good was 4 patients (19%), no change was 3 patients (14%) and poor was 2 patients (9%). 1 patient complained a dizziness and nausea during ESWT. Conclusion: ESWT for postoperative pain after ankle surgery shows satisfactory pain reduction in 76% of all patients without severe complication.

Free Flap Transplantation to the Injured Foot (족부 손상에 시행한 유리조직 이식술)

  • The aims of free tissue transplantation to the injured foot are to cover the exposed blood vessels, nerves, muscles, tendons and bones, to clear up infection, to lessen the morbidity, to shorten the hospitalization, and to prepare for further surgical procedures when no local or transpositioning flaps are available. Authors have carried out free flap transplantation in 13 cases of crushing injury, osteomyelitis and electrical burn in the foot at Chonbuk National University Hospital from June 1992 through May 1996. The results were as follows : 1. 9 cases of 13 (69%) were sustained from the traffic accident. 2. The dorsalis pedis free flap transplantation has been performed most frequently in 5 cases (38.5%), followed gracilis muscle flap in 4 cases(30.7%), rectus abdominis muscle flap in 2 cases(15.4%), latissimus dorsi muscle flap and upper arm flap in 1 each. 3. 6 muscle flaps were covered with split thickness skin graft 20 days after microsurgical anastomosis. 4. All of 13 cases were survived after microsurgical procedure and showed excellent coverage in the foot.

A Comparative Analysis on Changes of Foot Pressure by Shoe Heel Height during Walking (하이힐 굽 높이에 따른 보행 시 족저압 변화 비교 분석)

  • We aimed to determine the effect of heel height on foot pressure by comparing and analyzing data on foot pressure in shoes with different heel heights. Qn the basis of a previous study, we selected 3cm and 7cm as the shoe heel heights preferred by female college students. We divided 10 female students into forefoot and hindfoot to measure vertical force, maximum pressure, and average pressure. The average pressure on the forefoot was higher and that on the hindfoot was lower in the case of 7cm high-heeled shoes. The maximum pressure on the forefoot was significantly higher in the case of the 7cm heel height (p<.05). The vertical force, maximum pressure, and average pressure on the hindfoot were also significantly higher in the case of the 7cm heel height (p<.05). The results showed that wearing 7cm high-heeled shoes exerted greater maximum pressure on the forefoot and greater vertical force, maximum pressure, and average pressure on the Hndfoot. This leads to increase in confining pressure caused by high pressure distribution over the forefoot and increase in the pressure on the hindfoot, which may cause deformation of toes and heel pain over a long period. Therefore, female college students who wish to wear high heels are recommended to wear 3cm high-heeled shoes rather than 7cm high-heeled shoes.

Lifestyle, Diet, Self-care, and Diabetes Fatalism of Diabetic Patients with and without Diabetic Foot (당뇨병성 족부질환 여부에 따른 당뇨병 환자의 생활습관, 식습관, 자가관리 및 Diabetes fatalism)

  • Objectives: This study was to determine diabetes fatalism of diabetic patients with and without diabetic foot and its association with lifestyle, diet, and self-care. Methods: The subjects were diabetic patients with (male/female 48/21) and without diabetic foot (male/female 33/26). We administered the questionnaires which were designed to determine diabetes fatalism, lifestyle, diet, and self-care. Diabetes fatalism was determined by Diabetes fatalism scale (DFS), which consisted of total 12 items in three subscales namely, emotional stress, religiou spiritual coping, and perceived self-efficacy. Results: The patients with diabetic foot had undesirable diets more frequently (1.37 and 0.91 days/week respectively) and their desirable diets (2.74 and 3.61 days/week respectively) and foot care (4.61 and 5.53 days/week respectively) were less frequent than those without diabetic foot (p < 0.05). An item analysis of the 12 DFS items revealed a Chronbach' ${\alpha}$ of 0.614 and 0.869, respectively in diabetic patients with and without diabetic foot. Perceived self-efficacy related DFS of subjects without diabetic foot was positively associated with smoking (r=0.350, p<0.01), undesirable diet (r=0.295, p<0.05), and drinking (r=0.257, p<0.05), while its negative association with exercise (r=-0.224, p<0.088) and foot care (r=-0.247, p<0.059) did not reach to statistical significance. Conclusions: This work was the first study reporting the potential usefulness of DFS, especially perceived self-efficacy related subscale as a predictor of lifestyle, diet and self-care on the Korean diabetic patients, at least those without severe diabetic foot to screen those who should be the first target for diabetes education.

  • Hemangioma are not rare tumors. They can be found in almost any of the vascular structures of the body. Hemangiomas involving the deep structures of the extremities may produce extremely difficult therapeutic problems for the orthopedic surgeon. Pigmented villonodular synovitis (PVNS) is a rare proliferative disorder that affects synovium, tendon sheath and bursa. Although the condition can present in any joint, knee joint is the most commonly affected site and only 2.5% of cases occur in foot and ankle joint. We have experienced a patient who has of foot and report an optimal method of surgical treatment. Authors report the result of hemangioma in mid-foot which arise from removal of a pigmented villonodular synovitis that has low out break rate of benign tumor in mid-foot with literature review.

Meshed Stab Incision for Severe Swelling after Foot and Ankle Injury: A Technical Report (족부 및 족관절 손상 후 심한 부종에 대한 그물망 절개술: 술기 보고)

  • Swelling is a body's natural reaction to foot and ankle injury to mount a healing inflammatory response. To some degree, swelling is necessary for healing and is something that cannot be avoided following injury. However, post-traumatic swelling may have an adverse effect on wound healing and surgery can often be delayed due to preoperative swelling. We report on a unique technique of making multiple meshed stab incisions around the site of injury to reduce soft tissue swelling and promote wound healing.
