Vocal sulcus accompanying with benign vocal cord disease (양성 성대질환을 동반하는 성대구 (Vocal Sulcus))

  • A retrospective review of 158 patients who had microlaryngeal surgery for benign vocal cord disease, at the Department of Otolaryngology, Kang-Nam St. Mary Hospital, over 2-year period from Mar. 1991 to Feb. 1993, was completed. Of 158 patients, 16 patients(10.1%, male 8 & female 8, most common in fifth decade) had concomitant vocal sulcus unilaterally (10) or bilaterally (6). Vocal sulcus was accompanied with 7.5% (7/93) of vocal polyp, 15.8% (6/38) of vocal nodule, 11.1% (1/9) of Reinke's edema and 8.3% (1/12) of vocal cyst. Vocal sulcus located above the associated disease in 13(59.1%), below the disease in 7(31.8%) and in the lesion in 2(9.0%). Vocal sulcus was found mainly on the midportion of the membranous vocal cord and the length was less than half of membranous vocal cord in 16(72.7%), more than half in 6(27.3%). In 12 cases (54.5%), vocal sulcus was deep enough to attach to the vocal ligament. We thought it is important to consider the concomitant vocal sulcus during the microlaryngeal surgery for benign vocal cord disease.

양성 성대질환의 콜라겐 발현 및 분포양상

  • 성대결절, 폴립, 부종 등은 성대의 남용이나 과용등의 성대손상이 그 공통된 주된 원인으로 거론되고 있다. 하지만 음성치료를 비롯한 보존적 치료에 대한 반응이 서로 상이하며, H&E 염색을 이용한 병리조직학적인 감별이 곤란하여 진단에 혼돈이 있으며, 치료의 방침을 결정하거나 예후를 예측함에 있어서도 어려움이 있다. 양성성대질환은 기저막부 위와 세포외 간질에 주된 변화가 발생함이 알려져 있고, collagen type IV의 발현양상이 성대결절과 폴립에서 서로 다름에 대하여는 보고된 바 있으나 기타 점막하층의 골격유지를 주기능으로 하는 대표적 세포외간질인 collagen subtype에 대하여는 아직 보고된 바가 없는 실정이다. Collagen 발현의 차이를 연구하는 것은 상기질환의 병인을 이해하고 질환분류의 guideline을 제시하며 나아가 적절한 치료방범을 제시하는 데에 큰 의미가 있을 것으로 기대된다. Paraffin에 고정되어 있는 5례 이상씩의 성대결절과 성대폴립, 육아 종 및 라인케씨 부종 조직을 collagen type I부터 VII에 대하여 peroxidase kit를 사용하여 염색한 후 각 군간에 collagen 분포양상과 발현정도에 차이가 있는가 비교하였다.

  • Variable systemic diseases affect larynx and vocal fold and result in voice change. Asthma and chronic obstructive pulmonary disease make increase of intra-abdomimal pressure followed by reflux of gastric acid, which stimulate vagal-bronchopulomary reflex aggravating cough and respiratory disturbance. Fungal laryngitis in the general population is extremely rare, but can occur in immunocompromised AIDS patients. Although, initially, empirical antifungal therapy for candidiasis is often given without biopsy, diagnostic direct laryngoscopy and biopsy is imperative if a substantial clinical response is not rapidly achieved. In the highly active anti-retroviral therapy era, HIV-positive patients are living longer and are at higher risk for developing non-AIDS-defining malignancies. The incidence of head and neck cancer (HNC) which is related with human papilloma virus infection has increased. The survival is significantly lower among the AIDS-HNC patients with CD4 counts ${\leq}200cells/{\mu}L$. Rheumatoid arthritis (RA) cause voice disturbance by developing cricoarytenoid joints fixation or nodule on vocal fold. Post-menopausal voice disorder (PMVD) is caused by decreased secretion of estrogen-progesterone resulting in decrease of fundamental frequency (F0). Hormonal replacement therapy is helpful to reduce F0 decrease. RA and PMVD result in slight voice change, but it could crucial in professional voice user.

  • MIP was significantly increased in singers, compared to the untrained group. MIP in patients showed decreasing tendency compared to the untrained group, but were significantly lower than that in singers. MEP in singers was higher than that in the untrained group. MPT increased significantly in singers, but diminished in patients compared to the untrained group. MIP, MEP, and MPT in male singers were 50.8%, 61.0%, and 28.7 % higher than those in female singers. MIP, MEP, and MPT in the untrained male were more increased 32.3%, 25.0%, and 28.7%, respectively than those in the untrained female. There was no correlation between MPT and MIP or MEP. Regression analysis of the data set showed that weight and vocal cord dysfunction was a positive predictor of MPT. Factors affecting MIP were male, singers and weight. Factors affecting MEP were male, singers, vocal cord dysfunction and weight.

  • Substantial confusion exists regarding the nomenclature of benign vocal fold lesions (BVFL), because there were no pathologically diagnostic findings and deep understanding of pathogenesis in the past. There is no consensus on specific labels for BVFL, nor are the most commonly used terms defined. There is a need for a defined nomenclature for the purpose of improving communication among clinicians and with patients. Furthermore, precise definitions of BVFL will facilitate clinical research of voice disorders and may lead to a better understanding of outcomes for BVFL treatment. Laryngoscope, stroboscope, voice evaluation are used to diagnose BVFL. The objective of this review article was to develop a new paradigm of BVFL nomenclature using patient's history, stroboscope, voice therapy results, and operative findings. Furthermore, precise definitions of BVFL will facilitate clinical research of voice disorders and may lead to a better understanding of outcomes for BVFL treatment.

Videostrobokymographic Analysis of the Benign Vocal Folds Lesions (양성 성대 질환에서의 Videostrobokymography 소견)

  • Objectives : Videostrobokymography(VSK) has been recently developed and reported by Sung et at. We aimed to analyze vibratory patterns and objective parameters in various benign vocal fold lesions using VSK, and examine the efficacy of VSK in clinical application. Materials and Methods : Using VSK, we analyzed the vibration pattern of normal vocal fold and various benign lesions, such as nodules, polyps, cysts, Reinke's edema and unilateral vocal fold paralysis. We also calculated objective parameters, open quotient and asymmetric index, and compared them with mean values of parameters in normal controls. Results : In nodules, polyps, and cysts, the open quotient on the site of the lesion was similar to the mean value in normal controls, however, on the other part of the vocal folds it was much larger than normal mean value. In Reinke's edema, irregular and asymmetric vibration was observed. The posterior portion of the vocal folds showed larger open quotients than the anterior portion. In the unilateral vocal fold paralysis, irregular vocal folds vibration and incomplete closure of the vocal folds were documented. Much larger asymmetric indices were calculated in the unilateral vocal fold paralysis than in normal controls and other lesions. The asymmetric index could be a good quantitative parameter of vibration from a patient with vocal fold paralysis. Conclusion : This study demonstrated that VSK could generate clear quantitative documentations of fine vibrations of vocal folds in many different benign lesions. VSK has a potential as an effective tool for quantitative analysis of vibratory patterns of the vocal folds iii clinical settings.

  • The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered These effects are also graded according to the dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dystonia, and blepharospasm, vocal tics and stuttering, cricopharyngeal achalasia, various tremors and tics, hemifacial spasm, temporomandibular joint disorders and a number of cosmetic applications. Botulinum toxin treatment has recently begun to show some benefit in the control of pain from migraine and tension headache. It may also prove useful in the control of autonomic dysfunction, as in Frey syndrome, sialorrhea, and rhinorrhea. In over 20 yews of use in humans, botulinum toxin has accumulated a considerable safety record, and in many cases represents relief for thousands of patients unaided by other therapy.

The Efficiency of Voice Therapy on Various Laryngeal Disorders (각종 후두질환에서 음성치료의 효과)

  • Vocal hyperfunction is considered to be the most significant characteristic in larynx disorders which is found among many patients presenting hoarseness Primarily as chief complaint. In Pusan National University Hospital, we executed the voice therapy to 28 patients being 17 female and 11 male patients who visited the Voice & Speech Therapy Clinic, due to the voice disorder, and then compared and analysed the voice before and after its therapy using acoustic and aerodynamic test. The obtained results were as follows. In the analysis by the local findings, it was improved to 88% in the patients of vocal nodule, 75% in mutational falsetto, 75% in the functional dysphonia, 75% in the vocal cord palsy, 50% in the vocal polyp and 50% in dysphonia plica ventricularis. For the acoustic analysis, Fo, litter, Shimmer and NHR were measured. In the patients of mutational falsetto, Fo, Jitter and NHR were shown to be improved significantly and in the patients of vocal nodule, Shimmer was shown to be improved significantly. In the patients of vocal polyp, Fo was significantly improved. In the patients of vocal cord palsy in litter and NHH were significantly improved. In the patients of dysphonia plica ventricularis, Shimmer and NHR were significantly improved and the patients of functional dysphonia were more improved in Fo, litter and Shimmer. For the aerodynamic analysis, MPT was measured. In particular, it was shown to be improved significantly in the patients of vocal nodule, improved in the vocal polyp, vocal cord palsy, functional dysphonia patients.

Dogma of Extraesophaghgeal Reflux (식도 외 역류의 도그마)

  • Laryngopharyngeal reflux (LPR) disease is an extraoesophageal variant of gastro-esophageal reflux disease that can affect the larynx and pharynx. LPR is associated with symptoms of laryngeal irritation such as throat clearing, coughing, and hoarseness. The main diagnostic methods currently used are laryngoscopy and pH monitoring. The most common laryngoscopic signs are redness and swelling of the throat. However, these findings are not specific of LPR and may be related to other causes or can even be found in healthy individuals. Furthermore, the role of pH monitoring in the diagnosis of LPR is controversial. A therapeutic trial with proton pump inhibitors (PPIs) has been suggested to be cost-effective and useful for the diagnosis of LPR. However, the recommendations of PPI therapy for patients with a suspicion of LPR are based on the results of uncontrolled studies, and high placebo response rates suggest a much more complex and multifactorial pathophysiology of LPR than simple acid reflux. Laryngoscopy and pH monitoring have failed as reliable tests for the diagnosis of LPR. Empirical therapy with PPIs is widely accepted as a diagnostic test and for the treatment of LPR. However, further research is needed to develop a definitive diagnostic test for LPR.

Surgical Treatment of Sulcus Vocalis Using KTP Laser (성대구증에서 KTP Laser를 사용한 수술적 치료 1례)

  • Sulcus vocalis remains a surgical challenge despite many recent advances in laryngomicrosurgeries. We previously reported that 585-nm Pulsed dye laser (PDL) exerts favorable outcome in treatment of sulcus vocalis due to its therapeutic effects of collagen rearrangement and improved wound remodeling. In spite of the usefulness of PDL glottoplasty for treating sulcus vocalis, the device is no more available in the country. It prompted us to focus another angiolytic laser ; 532-nm KTP laser which has similar mechanisms of action and has been used for treatment of other laryngeal lesions elsewhere. Herein, we present a case of sulcus vocalis successfully treated with KTP laser. A patient underwent laryngomicrosurgery with angiolytic KTP laser (KTP glottoplasty) by the same surgical procedure with PDL glottoplasty. After the surgery, the patient presented improved voice outcome in time without complications.
