Reduction in the incidence of biliary and other surgical complications of ascariasis according to the decrease of its national egg prevalence in Korea (전국 회충란 양성률 감소에 따른 담도 및 기타 외과적 회충증의 감소 양상)

  • While various examples of rationale or benefits of ascariasis control could justify a necessity of control activity in a community, few practical index other than the e99 Prevalence or intensity of infection has been used to evaluate the efficacy obtained by the control activity. A literature-based retrospective study was undertaken to provide some numerical data on the decreasing morbidity of surgical ascariasis as a consequence of ascariasis control in Korea. A thorough review of literatures (1959∼1990) on biliary and other surgical complications of ascariasis reported from general hospitals revealed that their incidence has been decreasing very proportionally to the national figures of its e99 Prevalence and worm burden. For example, according to the remarkable decreases of the prevalence of ascariasis, there is an evident decreasing tendency of the proportion(%) of ciliary ascariasis cases among all biliary surgical patients and the proportion of biliary ascariasis cases among all biliary stone patients in general hospitals. It is highly suggested that the Korean people have been much benefited by the control activity of ascariasis executed at a national scale since 1960s, especially in terms of incidence of biliary and other surgical complications of ascariasis.

  • A survey of ascariasis in pigs (960) at three processing plants in Seoul, Daegu and Busan, Korea demonstrated that ascariasis was a common problem in pigs. Ascaris lesions (white and generalized fibrosis) were observed on 547 livers (57%) of the pigs. The greatest occurrence of Ascaris lesions (68%) were found in pigs from the Busan area.

  • Ascariasis is the most common helminthic infection in humans. However, its prevalence has been very low in Korea since the 1990s. Recently, there have been several case reports on intestinal obstruction or pancreaticobiliary disease due to infection with Ascaris lumbricoides in adults. However, cases of ascariasis in children have rarely been reported in Korea. We report a case of ascariasis in a 2-year-old boy who experienced expulsion of an adult ascaris worm from his anus. His mother found the worm in his diaper in the morning. His medical history was nonsignificant for any previous illnesses. There were no specific symptoms, and no abnormal findings were found on physical examination. The worm was pink, elongated, and cylindrical; it was 25 cm long and 5 mm wide. Unfertilized eggs of A. lumbricoides were detected in his stool specimen. He was treated with albendazole and remained asymptomatic at follow-up. As long as the number of immigrants from endemic areas and people returning from overseas trips, and import of agricultural products keep increasing, ascariasis can still occur in Korea. Therefore, it is necessary to raise awareness regarding ascariasis.

A case of biliary ascariasis accompanied by cholelithiasis (담석증을 동반한 담도회충증 1례)

  • A 43-year-old Korean woman with billary ascariasis accompanied by cholellthiasis is reported. Her chief complaints were abdominal pain, nausea, and vomiting. She had the past history of several attacks of abdominal pain in her childhood. ciliary stones were recovered from the left hepatic duct after cholecystectomy, which contained degenerated cuticle or body wall, and numerous eggs of Ascaris lumbriooides. It is strongly suggested that the ciliary stones were formed from the dead Ascmis worm(s). This is a rare case of biliary ascariasis during the recent 5 years in Korea.

Prevalence, Worm Burden and Other Epidemiological Parameters of Ascaris lumbricoides Infection in Rural Communities in Korea (한국 농촌지역의 회충 감염율, 감염량 및 역학적 변수)

  • The epidemiological status of ascariasis was analyzed in 8 rural villages in Korea, through observation of its epidemiological parameters such as prevalence, worm burden and basic reproductive rate. Total 978 inhabitants were subjected to stool examination and recovery of worms after chemotherapy with pyrantel pamoate. The results were as follows: 1. The worm positive rate in each village was 16.5~79.5%, while the egg positive rate was 9~18% lower, 3.3~66.7%. The average worm burden (among all inhabitants) ranged from O. 21 to 8. 44 by villages and the frequency of cases with each worm burden showed negative binomial distributions with 'k' values of O. 38-0. 54. 2. The prevalence rates (worm) in each village was almost identical with the theoretical ones from Anderson and May's equation; $p=1-(1+M^*/k)^{-k}$, where 'p' is worm prevalence and '$M^*$' equilibrium average worm burden. The basic reproductive rate 'R' was calculated from 1.03 to 2.11. lt is suggested that, although 'R' in lower endemic areas is approaching to the breakpoint of reinfection (R=1), control programs of ascariasis in Korea should be continued until it becomes below the level nationwidely.

Gastric anisakiasis cases in Cheju-do, Korea (고래회충유충증 107례 보고 및 어류감염 실태 조사)

  • Human anisakiasis may occur after ingestion of raw marine flesh infected with nematode larvae of Anlsi,hidae. Anisakiasis caused by the migration of the larva into the wall of stomach, small intestine and other portion has been reported in Korea. This prospective study was made of all cases referred to parasitological laboratory in Cheju-do between. Tune 1989 and June 1992. Gastric anisakiasis was confirmed if larvae invading the gastric wall were observed by gastrofiberscopy One hundred and seven cases were diagnosed, most of which were in 30-49 years old. Most of the patients complained acute epigastric pain with history of eating raw marine fish. This symptom usually occurred about 12 hours to 1 day after ingestion of infected marine fish. Edema, erosion or ulcer of the mucosa and hemorrhage from the gastric wall were observed in the involved areas. Ninety larvae removed from the stomach were identified; the larva of Anisqkis siwlex was the most prevalent species, and the Iarva of Pseudntewanoua decipien was also detected. The important species of marine fish from which the patients became infected was demonstrated as yellow corvina, sea eel, ling, cuttle fish, yellowtail and others. Five species of marine fish as a possible source of infection were examined, and Anisakis simplex larvae and Pseunotewqnoua decipirens larvae were collected from the mackerel and rock cod. This study demonstrates that anisakiasis is recognized as a public health problem in Korea.

  • In a 43-year-old Korean man who underwent radical gastrectomy due to a malignant stromal tumor, was found to have an enlarged Iymph node at transverse mesocolon. The Iymph node exhibited histologically necrotizing eosinophilic granuloma formed around a track containing sections of a nematode larva. The well preserved nematode sections revealed polymyarian muscle cells, Y-shaped lateral cord, a large excretory gland cell, intestine and eosinophilic cuticle. The nematode sections were identified as a larva of Anisnkis species In Korea, thIns is the first case of elrtragas trio into stinal anisakiasis.

  • Ascariasis is an intestinal disease caused by Ascaris lumbricoides. Most patients with ascariasis are asymptomatic; however, the presence of many larvae in the bowel can cause gastrointestinal complications, such as intestinal obstruction, obstructive jaundice, cholangitis, cholecystitis, and pancreatitis. Herein, we report a case of ascariasis presenting as hematoma and active bleeding in the sigmoid mesocolon of a 74-year-old man on computed tomography (CT). Sigmoid colon perforation was also detected on follow-up CT. Laparoscopic low anterior resection was performed; there was a large hematoma in the sigmoid mesocolon. Roundworms were microscopically identified in the mesenteric adipose tissue. The clinical and CT findings of this unusual presentation of ascariasis revealed serial complications during parasite migration from the intestinal lumen to the peritoneal cavity.
