  • Purpose: Here we present tardy ulnar nerve palsy cases that were caused by the anconeus epitroch-learis muscle. Materials and Methods: A 37 year old female patient presented with tardy ulnar nerve palsy findings for the elbow. During surgery, an atypical anconeus epitrochlearis muscle was observed, which originated from the triceps aponeurosis and inserted into the medial intermuscular septum. Muscle resection, nerve decompression and ulnar nerve anterior transposition were done. A 35 year old male patient presented with tardy ulnar nerve palsy findings for the elbow. During surgery, a typical anconeus epitrochlearis muscle was found. Muscle resection, nerve decompression and ulnar nerve anterior transposition were done. Results and Conclusion: Tardy ulnar nerve palsy can be associated with and caused by either an atypical or a typical anconeus epitrochlearis muscle.

  • We evaluated biceps pulley as a stabilizing sling for the long head of biceps tendon in the rotator interval. We present 3 cases of detached biceps pulley which impinges on posterosuperior glenoid labrum in the position of late cocking. Pulley impingement related to anterior shoulder instability was relieved after Bankart repair. In another two cases, impingement symptoms were disappeared after resection of the detached biceps pulley.

  • Lesions of the long head biceps tendon pulley are frequent causes of shoulder dysfunction and pain. These lesions cause instability of the long head of the biceps tendon (LHB), and intra-articular tear of the subscapularis and the supraspinatus tendon might result from them. The arthroscopic repair of these lesions has not gained widespread acceptance as an effective procedure. Predictable results can be obtained by treating these lesions more definitively with tenotomy or tenodesis. The purpose of this article is to review the anatomy and properties of the LHB pulley and to provide treatment strategies for alleviating pulley lesions.

  • Purpose: The purpose of this study is to evaluate the relationship between trochlear medial facet osteophyte (TMFO) and elbow flexion in the elbow joints without trauma history. Materials and Methods: Twenty five patients, who underwent computed tomography without elbow trauma history, were reviewed. Patients were checked for osteophyte or loose bodies in the coronoid and olecranon sides. The height and length of TMFO were measured. Results: The average elbow flexion contracture was $18.6^{\circ}$, and further flexion was $112.1^{\circ}$. The TMFO height and length was 2.2 mm and 4.7 mm, respectively. The average elbow further flexion was $105.1^{\circ}$ in the coronoid block group (n=14) and $119.1^{\circ}$ (p=0.011) in the coronoid free group (n=11). The relationship between further elbow flexion and TMFO was significant with a partial correlation coefficient of 0.687(p<0.000) in the TMFO length. Conclusion: Elbow joints with longer TMFO length decrease further flexion.

  • Purpose : The role of biceps pulley is stabilizing sling for the long head of the biceps tendon against anterior shearing stress in the rotator interval. The purpose of this study was to classify arthroscopic findings of biceps pulley and to evaluate the relationship with shoulder pathology. Materials and Methods : From January 2002 through July 2002, we observed biceps pulley in 49 cases of shoulder pathology treated with arthroscopically. There were 22 cases of anterior instability, 12 cases of rotator cuff tear, 5 of impingement syndrome, 6 of frozen shoulder, 2 of superior labral injury and 1 of each scapulothoracic bursitis and biceps dislocation. We classified biceps pulley as four types according to the arthroscopic appearance. Type I its stretched type. type II as sling type, type III at detached sling type, and type IV as concealed type. Results : We observed stretched type in 24 cases $(49\%)$, sling type in 5 cases $(10\%)$, detached sling type in 2 cases, concealed type in 1 case, and unidentified cases in 17 cases $(35\%)$. Conclusion : Development and variation of biceps pulley may have symptomatic correlation according to the degree of shoulder motion or pathologic status.

  • We evaluated outcomes of dogs surgically treated for grade 2 or 3 medial patellar luxation (MPL) with and without trochleoplasty. A retrospective study of 63 dogs with grade 2 or 3 MPL surgically treated with or without trochleoplasty was performed. Results of radiographic evaluation were expressed numerically as degenerative joint disease score and were compared between the groups with and without trochleoplasty. The study included 7 (7/68, 10.3%) cases of reluxation and 1 case of tibial tuberosity transposition implant failure. Reluxation and complications requiring additional surgery were confirmed in 3 cases (3/47, 6.4%) in trochleoplasty group and in 1 case (1/21, 4.8%) in nontrochleoplasty group. The nontrochleoplasty group showed a shorter recovery time than the trochleoplasty group (P < 0.05). There was a significant difference in degenerative joint disease scores over time between the groups, with nontrochleoplasty group having lower scores (P < 0.05). This study suggests that surgical treatment without trochleoplasty results in favorable outcomes compared to treatment with trochleoplasty. It is not mandatory to exclude trochleoplasty when performing surgery for grade 2 or 3 MPL, but we propose that surgical treatment without trochleoplasty is one option when choosing a combination of surgical techniques.
