Effects of Isometric Contraction Training by Electrostimulation on Type I and II Hindlimb Muscles in Cerebral Ischemia Model Rats (전기자극을 이용한 등척성 수축훈련이 뇌허혈 유발 쥐의 환측 Type I, II 근육에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of cerebral ischemia on Type I(soleus) and Type II(plantaris, gastrocnemius) muscles, and to determine the effects of isometric contraction training by electro- stimulation on Type I and II muscles in cerebral ischemia model rats. Method: Twenty-five male Sprague-Dawley rats were randomly divided into four groups: ST(stroke), STES(stroke+electrostimulation), SH(sham) and SHES (sham+electrostimulation). The ST and STES groups received a transient right middle cerebral artery occlusion operation. The SH and SHES groups received a sham operation. The STES and SHES groups had daily isometric contraction training by electrostimulation(100Hz, 45mA, 7.5V) on hindlimb muscles for 7days. Result: Plantaris and gastrocenmius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the muscle fiber cross-sectional area of gastrocnemius in the ST group significantly decreased compared with the SH group. Soleus, plantaris, gastrocnemius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the Type I muscle fiber cross-sectional area of soleus and the Type II muscle fiber cross-sectional area of gastrocnemius in the STES group significantly increased compared with the 57 group. Conclusion: Hindlimb muscle atrophy occurs after acute stroke and isometric contraction training by electrostimulation during early stages of a stroke attenuates muscle atrophy of Type I and Type II muscles.

A Systematic Review on Measurement Instruments of Bilateral Upper Extremity Function (양측 상지 기능 평가도구에 관한 체계적 고찰)

  • Objective : This study was conducted to review about instrument for bilateral upper extremity assessment. Methods : We searched published papers in Medline database. The keywords used in the search were 'upper extremity' and 'motor activity', 'activities of daily living' and 'assessment', 'instrument', 'disability evaluations'. In total of 68 papers, 44 assessment instruments was extracted. We analyzed about numbers, subjects, methods, reliability, validity, responsiveness of assessment instruments about bilateral upper limb function comparing unilateral, combined bilateral and unilateral instruments. Results : The numbers of bilateral measurement tool were 2 in a total of upper extremity measurement tools. Also, subjects were patients with stroke and measurement was based on performance. The reliability, validity and responsiveness of tools were high. Conclusions : Futher study will be needed to development and research about instrument of bilateral upper extremity.

A Review of the Modified Constraint Induced Movement Therapy in Stroke Patients (뇌졸중 환자에게 적용된 수정된 강제유도 운동치료에 대한 고찰)

  • Constraint Induced Movement Therapy(CIMT) is intense in that patient's unaffected arm is restrained for 90% of waking hours during a two-week period while they also participate in activity sessions using the affected arm for 6 hours/day. However CIMT showed that an issue for applying it to clinics of patients with stroke, and then modified constraint induced movement therapy(mCIMT) was designed to minimize the issue. Application on mCIMT for the patients has been studied in various ways. As a result, it has proved the effect on functional improvement of patients with stroke through methods such as MAL, FMA, WMFT, ARAT, FIM, SIS and so forth. It's considered that modified constraint induced movement therapy can be useful applied on clinical experiments of occupational therapy, as it is a way of treatment of upper extremity function, activities of daily living and an improvement of the quality of life for stroke patients.

Effects of Passive Upper Arm Exercise on Range of Motion, Muscle Strength, and Muscle Spasticity in Hemiplegic Patients with Cerebral Vascular Disease (입원초기에 적용된 상지수동운동이 뇌혈관질환자의 환측 관절가동범위, 근력 및 상지 경직에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of passive upper arm exercise on range of motion, muscle strength, and muscle spasticity in hemiplegic patients with cerebral vascular disease. Methods: A quasi-experimental design with nonequivalent control group was utilized. According to inclusion criteria, 25 patients were assigned to the control group with routine care, followed by 25 to the intervention group with passive exercise for 30 minutes per session, twice a day for 2 weeks. Eighteen patients in the intervention group and 17 in the control group completed the posttest measurement, including range of motion for upper arm joints, manual muscle test, and Modified Ashworth Scale for muscle spasticity. Results: The intervention group had a significantly improved range of motion in the shoulder and wrist joints. No interaction effect was found for the elbow joint. No significant differences were found in muscle strength or muscle spasticity between the groups. Conclusion: Results of the study indicate that passive exercise safely applied for two weeks improves range of motion in joints of the upper arm in these patients. Further study with long-term follow-up is needed to verify the role of passive exercise in preventing muscle spasticity in this population.

Effect of Dehydroepiandrosterone on Affected and Unaffected Hindlimb Muscles in Rats with Neuropathic Pain Induced by Unilateral Peripheral Nerve Injury (DHEA 투여가 일측성 말초신경 손상에 의해 유발된 신경병증성 통증 쥐의 환측과 정상측 뒷다리근에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of DHEA (Dehydroepiandrosterone) on muscle weight and Type I and II fiber cross-sectional area of affected and unaffected hindlimb muscles in rats with neuropathic pain induced by unilateral peripheral nerve injury. Methods: Neuropathic pain was induced by ligation and cutting of the left L5 spinal nerve. Adult male Sprague-Dawley rats were randomly assigned to one of two groups: The DHEA group (n=10) had DHEA injections daily for 14 days, and the Vehicle group (n=10) had vehicle injections daily for 14 days. Withdrawal threshold, body weight, food intake and activity were measured every day. At 15 days all rats were anesthetized and soleus, plantaris and gastrocnemius muscles were dissected from the both hindlimbs. Body weight, food intake, activity, muscle weight and Type I, II fiber cross-sectional area of the dissected muscles were measured. Results: The DHEA group showed significant increases (p<.05), as compared to the vehicle group for muscle weight of the unaffected plantaris, and in Type II fiber cross-sectional area of the gastrocnemius muscle. The DHEA group demonstrated a higher pain threshold than the vehicle group whereas total diet intake and activity score were not significantly different between the two groups. Conclusion: DHEA administration for 14 days attenuates unaffected plantaris and gastrocnemius muscle atrophy.

Analysis of Movement Time and Trunk Motions According to Target Distances and Use of Sound and Affected Side During Upper Limb Reaching Task in Patients With Hemiplegia (편마비 환자의 팔 뻗기 과제 수행 시 목표거리와 건·환측 사용에 따른 운동시간과 체간의 움직임 분석)

  • The aim of this study was to investigate effects of reaching distance on movement time and trunk kinematics in hemiplegic patients. Eight hemiplegic patients participated in this study. The independent variables were side (sound side vs. affected side) and target distance (70%, 90%, 110%, and 130% of upper limb). The dependent variables were movement time measured by pressure switch and trunk kinematics measured by motion analysis device. Two-way analysis of variance with repeated measures was used with Bonferroni post-hoc test. (1) There were significant main effects in side and reaching distance for movement time (p=.01, p=.02). Post-hoc test revealed that there was a significant difference between 110% and 130% of reaching distance (p=.01). (2) There was a significant main effect in side and reaching distance for trunk flexion (p=.01, p=.00). Post-hoc test revealed that there were significant differences in all pair-wise reaching distance comparison. (3) There was a significant side by target distance interaction for trunk rotation (p=.04). There was a significant main effect in target distance (p=.00). Post-hoc test revealed that there were significant differences between 70% and 110%, 70% and 130%, 90% and 110%, 90% and 130% of target distance. It was known that trunk flexion is used more than trunk rotation during reaching task in hemiplegic patients from the findings of this study. It is also recommended that reaching training is performed with limiting trunk movement within 90% of target distance whereas reaching training is performed incorporating with trunk movement beyond 90% of target distance in patients with hemiplegia.

Physical Function and Fatigue in Mastectomy Patients (유방절제술 환자의 신체기능과 피로)

  • Purpose : The purpose of this study was to provide information for developing a rehabilitation intervention that improves adaptation and the quality of life after mastectomy by investigating the level of physical function and fatigue in mastectomy patients. Methods : The subjects were 63 patients selected from St. Mary's Kangnam hospital Data were collected from March to June, 2003. Physical function was measured by the range of motion of the shoulder joint, shoulder function, and physical symptoms. fatigue was measured by using the Brief Fatigue Inventory (BFI). Results : The range of motion on the affected side was significantly lower than that in the healthy side. Shoulder function score was highest in the item of 'pull on pants', and lowest in the item of 'back zipper'. Physical symptom score was highest in the item of 'numbness', and lowest in the item of 'Itching sensation'. The highest item interfered by fatigue was 'normal work (includes both work outside the home and daily chores)'. Conclusion : The result of this study suggests that effective rehabilitation intervention for mastectomy patients should be needed to improve physical function, and reduce fatigue.

Influence of Standing Balance With External Rotation Angle of the Hemiplegia Leg (편마비환자의 환측 하지 외회전 정도가 기립균형에 미치는 영향)

  • The two groups each consisting of 28 people who had an adult hemiplegia due to a brain injury received two different methods of exercises to reduce the angle to which the affected lower limb rotates externally. The comparisons between the two groups were made for the changes of the angle in external rotations measured between pre and post test. The static balance index values taken during pre and post tests were also compared. In addition, the correlations of the angle to which the affected lower limb rotates externally with static balance index values were analyzed. The result were as follows: 1. A statistical analysis indicated that in group one having taken simple R.O.M exercises, the angle to which the affected lower limb rotates externally and the static balance index values were both significantly different between pre and post test (P<.01). 2. A statistical analysis indicated that in group two having taken a pattern movement, both the angle to which the affected lower limb rotates externally and static balance index values were significantly different between pre and post test (P<.01). 3. A significant difference between simple R.O.M exercises group and pattern movement exercises groupwas shown only for the angle to which affected limb rotates externally not for static balance index values (P<.01), 4. Pearson correlation coefficient for the angle to which the affected limb rotates externally with static balance index values was found to be significant only in pattern movement exercises group (P < .05).

Comparison of Radiologic Parameters between Weight Bearing Affected Single Ankle Anteroposterior View and both Ankle Anteroposterior View in Ankle Osteoarthritis (족관절 관절염이 있는 환자에서 체중 부하상태의 양측 족관절과 환측 족관절 전후면 방사선 사진의 방사선학적 측정치의 비교)

  • Purpose: This study was performed to analyze the results of radiologic parameters between weight bearing affected single ankle anteroposterior (AP) view and both ankle AP view in ankle osteoarthritis (OA). Materials and Methods: Between January 2009 and August 2010, 41 patients (50 ankles) who visited our institution to treat ankle OA were reviewed retrospectively. In radiographic assessment, weight bearing affected single ankle AP view and both ankle AP view were checked, and measured tibial anterior surface angle (TAS), tibial medial malleolar angle (TMM), talar tilting angle (TT), maximum and minimun joint space width (JSW) of ankle, width between articular surface of medial malleolar and medial articular surface of talus as radiologic parameters. Results: On weight bearing both ankle AP view, TAS was $85.4{\pm}4.1^{\circ}$, TMM was $33.1{\pm}9^{\circ}$, TT was $5.4{\pm}6.1^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.8{\pm}1.8$ mm and on weight bearing affected ankle AP view, TAS was $85.3{\pm}3.9^{\circ}$, TMM was $34.3{\pm}10.9^{\circ}$, TT was $5.4{\pm}6.5^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.6{\pm}1.7$ mm. Conclusion: There is no statistical significance in radiologic parameters between weight-bearing affected single ankle AP view and both ankle AP view in ankle OA.

Effect of Chronic Foot Disease to Bone Mineral Density of the Affected Lower Limb (만성 족부 질환이 환측 하지의 골밀도에 미치는 영향)

  • Purpose: Pain or discomfort caused by foot diseases may lead to abnormal gait, resulting in decreased bone mineral density (BMD) of the affected lower limb. We analyzed the effect of foot affection to BMD and its clinical significance. Materials and Methods: Bilateral hip BMD was evaluated in 93 patients with unilateral chronic foot disease. To minimize statistical errors, we excluded patients with medical histories that had influence on BMD. Analysis was based on the results of BMD tests at the first visit. All patients denied past medical intervention for osteoporosis. The difference in density between bilateral limbs was determined by comparing BMDs of the neck, upper neck, trochanter and total area of hip. Results: Test results revealed the decrease of BMD in the lower limb with the affected foot, compared to the unaffected side. This decrease was significant in the area of the trochanter (p <0.05). There was no marked difference of BMD in relation with duration of affection, underlying disease or age. Pertaining the location of foot affection, the hindfoot group showed significant decrease in BMD compared to the forefoot group. The group with affection in bone and joint also showed a marked decrease in BMD compared to the soft tissue group (p <0.05). Conclusion: Pain and discomfort caused by chronic foot diseases can lead to a decrease in the BMD of the affected lower limb. This may increase the risk of complications such as osteoporotic fracture and muscular atrophy.
