Effect of Low Frequency Electroacupuncture on the Chronic Monoarthritis and the Abundance of mRNA Encoding Substance P and Trk A mRNA levels at the Spinal Level in Rats (저빈도 전침이 만성 단발성 관절염 흰쥐의 관절염 치료효과 및 척수에서의 P 물질과 trk A mRNA 발현조절에 미치는 영향)

  • 목적 : 본 연구는 (a) 저빈도 전침의 만성 단발성 관절염에 대한 치료 효과와 (b) 척수에서 동통과 관련되는 지표인 P 물질과 trk A mRNA의 발현조절에 미치는 영향을 연구하기 위해 실행되었다. 방법 : Complete Freund's adjuvant를 실험동물의 우측 경족근관절에 주입하여 관절염을 유발시키고 9주 동안 관찰하였다. 만성 관절염의 최적의 치료조건을 찾기 위하여 각각 세 종류의 경혈(환도, 용천, 족삼리)과 전침자극강도(0.5, 1, 2 mA)를 사용하여, 행동학적 지표로서 동통을 측정하였다. 측정 결과 가장 큰 효과를 나타낸 군을 선택하여 RT-PCR 방법을 사용하여 P 물질 및 trk A mRNA의 변화를 척수 후각과 후근신경절에서 측정하였다. 결과 : 관절의 굴신을 통한 통증 검사와 족과관절 둘레 길이 측정을 통하여 관찰한 결과 원위부에 위치한 환도에 저강도의 자극을 시행한 경우(환도, 0.5 mA)가 가장 우수한 치료 효과를 나타내었다. RT-PCR을 시행한 결과, 환측 후근신경절의 P물질이 관절염 유발 대조군의 경우 정상군에 비해 증가하였고 환도-0.5mA 군에서 다시 감소하는 것을 관찰하였으며, trk A는 관절염 유발군의 경우 척수 후각에서 양측성 증가를 나타내었고 이는 전침치료에 의해 다시 감소되는 것을 관찰하였다. 결론 : 환도 0.5mA의 저빈도 전침은 관절염을 효과적으로 감소시킬 뿐 아니라 관절염으로 증가된 후근신경절의 P 물질과 척수후각의 trk A mRNA 발현을 효과적으로 감소시켰으며, 이는 원위취혈로 선혈된 환도혈의 만성관절염에 대한 치료효과를 분명히 보여준다고 할 수 있다.

Intraarterial Catheter-directed Urokinase Infusion for Femoral Artery Thrombosis after Cardiac Catheterization in Infants and Children (심도자술후 발생한 대퇴동맥 혈전증 환아에서 동맥내 Urokinase 국소 주입요법의 효과)

  • Purpose : One of the major complication of arterial catheterization is the thrombosis of the iliac or femoral arteries. Tissue loss following femoral artery catheterization is rare. However longterm sequelae such as impaired limb growth and future impairment of vascular access, are also important in pediatric cardiac patients. But standard methods to treat thrombotic complication of arterial catheterization in infants and children is not established. The present study was performed to assess the efficacy of intraarterial catheter-directed urokinase infusion in infants and children with limb ischemia due to arterial thrombosis after cardiac catheterization. Methods : From January 1994 to August 2002, 12 patients with thrombotic femoral artery occlusion after arterial catheterization were treated with catheter-directed urokinase infusion in Dong-A University Hospital. Retrospective analysis of the medical records and angiograms was conducted. Results : The incidence of femoral artery thrombosis after retrograde arterial catheterization, which had not responded to systemic infusion of heparin and/or urokinase, was 2.8 percent. The doses of urokinase were 1,000-4,400 unit/kg/hr and duration of infusion was $50.6{\pm}29.2$ hours(18-110 hours). Clot resolution was complete in all patients who started to receive the intraarterial urokinase infusion within four days after catheterization. Only partial thrombolysis was seen in two patients who were treated with intraarterial urokinase on the 12th and 19th days after thrombus formation. Balloon angioplasty was done for these two patients with partial success. Bleeding complications were seen in two cases. Conclusion : Early use of catheter-directed intraarterial infusion of urokinase is safe and effective in thrombolysis of femoral artery occlusion after cardiac catheterization in infants and children.

Arthroscopic ACL Reconstruction Using Quadrupled Hamstring Tendon with Tibial Remnant-preserving Technique (슬괵건을 이용하여 경골부 잔류 조직을 보존한 관절경적 전방십자인대 재건술)

  • Purpose: We evaluated the clinical results of arthroscopic ACL reconstruction using quadrupled hamstring tendon with tibial remnant-preserving technique. Materials and Methods: Thirty-five cases were evaluated from Feb, 2003 to May, 2006. The average interval from injury to surgery was $2.6{\pm}1.6$ months. The cause of injury was mostly sports-related trauma. The average follow-up period was 17 months. Tibial remnant was preserved as much as possible and caution was taken not to damage the remnant during ACL reconstruction. Postoperative rehabilitation was the same as the usual rehabilitation method after ACL reconstruction, except for delaying motion for 2 weeks with an extension locking brace. Clinical evaluation was performed using ROM; Lachman test; pivot-shift test; anterior displacement measurement using KT-2000 arthrometer; Lysholm score and proprioception measured by single limb standing test. Results: There was no limitation of knee motion without contracture. The Lachman test and pivot-shift test were both negative. The side-to-side difference of anterior displacement measured using KT-2000 arthrometer was improved from 6.7 mm to 2.2 mm. The average Lysholm score improved from 81 to 96. The single limb standing test for proprioceptive evaluation showed no significant difference from a normal leg. Conclusion: ACL reconstruction with tibial remnant-preserving technique can preserve mechanoreceptors with prorioception and expect good functional recovery.

Effective Frequency of External Feedback for Increasing the Percentage of Body Weight Loading on the Affected Leg of Hemiplegic Patients (편마비환자의 환측하지 체중부하율 향상을 위한 효과적인 외적 되먹임 빈도)

  • In motor learning, the relative frequency of external feedback is the proportion of external feedback presentations divided by the total number of practice trials. In earlier studies, increasing the percentage of body weight loading on the affected leg of hemiplegic patients, external feedback was continuously produced as the patient attempted to perform a movement. This feedback was produced to enhance the learning effect. However, recent studies in nondisabled populations have suggested that compared with 100% relative frequency conditions, practice with lower relative frequencies is more effective. My study compared the effect of 100% relative frequency conditions with 67% relative frequency conditions to determine what effect they exerted on motor learning for increasing the percentage of body weight loading on the affected lower limbs of patients with hemiplegia. Twenty-four hemiplegic patients were randomly assigned to one of two experimental groups. Each group practiced weight transfer motor learning on a machine. During practice, visual feedback was offered to all subjects. The experiment was carried out with full visual feedback for patients in group one but only 67% visual feedback for patients in group two. The percentage of loading on the affected leg was recorded four times: before learning (baseline value), immediately after learning, 30 minutes after learning, 24 hours after learning. The results were as follows: 1. In the 100% visual feedback group, the percentage of loading on the affected leg increased significantly in all three testing modes over the baseline value. 2. In the 67% visual feedback group, the percentage of loading on the affected leg increased significantly in all three measurements. 3. Immediately after learning, the learning effect was not significantly different between the two groups, but was significantly greater after both the 30 minutes delay and the 24 hours period. These results suggest that the 33% reduction in the provision of visual feedback may enhance the learning effect of increasing the percentage of body weight loading on the affected leg in patients with hemiplegia.

Comparison of Warm-Needling and Acupuncture for Knee Osteoarthritis: A Randomized Controlled Trial (퇴행성 슬관절염에서의 온침과 침의 효능 비교 연구)

  • Objectives : The aim of this study was to investigate whether warm-needling is more effective than acupuncture in relieving the pain and improving the symptoms of knee osteoarthritis(OA). Methods : 76 volunteers with knee OA participated in the study. The subjects were randomly assigned to one of two groups. One group received warm-needling(n=38), while the other group received acupuncture(n=38). Sixteen sessions of warm-needling or acupuncture were conducted on the pain region of each problematic knee over a period of 8 weeks. The Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC) scores, physical health score based on the 36-Item Short-Form Health Survey(SF-36) and the Global Assessment(PGA) was measured. Results : Compared to the acupuncture group, the warm-needling group showed a significant decrease in pain, function, and total WOMAC scores according to the Mann-Whitney U-test. The PGA scores of the warm-needling group also showed a significant improvement compared to the acupuncture group. Conclusions : Warm-needling showed a greater pain relief effect on knee OA compared to the acupuncture group. These findings suggest that warm-needling may be a promising alternative therapy for treating knee OA.

Immediate Effect of Patterned Sensory Enhancement (PSE) on Upper Limb Function after Stroke (패턴화된 감각 증진(PSE)이 뇌졸중 환자의 상지 기능에 미치는 즉각적 영향)

  • The purpose of this study was to investigate the immediate effect of Patterned Sensory Enhancement (PSE) technique on the motor function of the affected upper limb in hemiplegic stroke patients by comparing the use of PSE and simple rhythmic cue. A total of 16 stroke patients were recruited from rehabilitative hospitals. The participants were assigned to the experimental group (n = 8) and control group (n = 8). While performing six different upper limb motions, musical stimuli applying the PSE technique was presented for the experimental group and simple rhythmic cue using the metronome was applied for the control group. The results showed that while the significantly increased range of motion (ROM) was found in the experimental group with the immediate use of PSE (p < .05), the control group did not show no significant change. This study implies that the use of musical elements in cueing for upper limb motion immediately leads to significant improvement in ROM by providing sufficient temporal, spatial, and dynamic information for expected motor performance.

A Case Report of Meralgia Paresthetica (Meralgia Paresthetica 치료(治療) 1례에(例) 대한 증례보고)

  • Objective : The purpose of this case is to report the improvement after treatment about a patient with meralgia paresthetica. Methods : We treated the patient with acupuncture therapy and Herbal medication from 11th October 2004 to 15th October 2004 by evaluating femoral function with VAS score and R.O.M of femur joint. Results : After 5 times of treatment, this patient achieved excellent outcome following the technique, showing that clinical symptom as like heating sense, edema and pain was disappeared, VAS changed from 10 to 1 and there was no limitation of ROM of femur. Conclusions : Meralgia paresthetica is a symptom complex that includes numbness, paresthesias, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve(LFCN). Oriental medical treatment for meralgia paresthetica resulted in satisfactory results by diminishing the symptoms progressively during the five days of treatment. Differential diagnosis was based on careful physical examination. More research of meralgia paresthetica is needed.

Clinical and Bacteriological Observation of 128 Cases of Chronic Otitis Media (중이수술 128례에 대한 임상적 세균학적 고찰)

  • A Clinical and bacteriologcal observation was performed 128 cases of chronic otitis media who had taken middle ear surgery at Dept. of otolaryngology of St. Benedict Hospital during the period of Feb. 1981 to Feb. 1983. The following results were obtained. 1) Among total 128 cases, male were 60 cases (46.9%) and female were 68 cases (53.1 %) and age distribution showed 48 cases (37.5 %) in 3rd decade, 37 cases (28.9 %) in 2nd decade and 27 cases (21.1 %) in 4th decade. 2) Site distribution were 71 cases (55.5%) in right, 57 cases (44.5 %) in left. 3) Degree of preoperative hearing loss were 64 cases (50%) in moderate, 32 cases (25% ) in mild, 27 cases (21.1 %) in severe and 5 cases (3.9%) were normal. 4) Central perforation were observed in 65 cases (50.8 %), total perforation in 44 cases (34.4 %), attic perforation in 12 cases (9.4 %) and marginal perforation in 7 cases (5.4 %). 5) Pathologic changes of middle ear and mastoid antrum showed granulation in 81 cases (63.3 %), cholesteatoma in 47 cases (36.7%). 6) The route of approach were 123 cases (96.1%) in postauricular, 3 cases (2.3%) in transmeatal and 2 cases (1.6 %) in endaural. 7) Type of operation were 53 cases (41.4 %) in intact canal wall tympanoplasty with mastoidectomy, 42 cases (32.8 %) in tympanoplasty without mastoidectomy, 23 cases (18%) in modified radical mastoidectomy and 10 cases (7.8%) in radical mastoidectomy. 8) Type of anesthesia were 95 cases (74.2 %) by local anesthesia, 33 cases (25.8 %) were by general anesthesia. 9) Among 93 specimens of culture, proteus (31.2%), staphylococcus (28.7%), pseudomonas (23.7 %), streptococcus (7.5 %) and etc. (8.7%) in order of frequency.

The Effects of Modified Constraint-Induced Movement Therapy on Hand Functions of Children With Hemiplegic Cerebral Palsy (수정된 강제유도 운동치료가 편마비를 가진 뇌성마비 아동의 상지 기능에 미치는 영향)

  • Objective : The aims of this study was to investigate the effects of modified constraint induced movement therapy(CIMT) on upper extremities function of children with hemiplegic cerebral palsy. Methods : The participants of this study are 4 children with hemiplegic cerebral palsy aged between 8 to 11 years old. During the modified CIMT period, the unaffected hand of the subjects was restrained by a hand splint for 8 weeks, five days per week, five hours a day. And the affected upper extremity was strongly trained by performing functional tasks, which were individually structured use of the affected arm. Measurements used to assess hand function are Jebsen Hand Function Test, and 3D Motion Analysis. The Jebsen Hand Function Test was performed repeatedly every two weeks. The 3D Motion Analysis was performed before and after the 8 weeks of modified constraint induced movement therapy. Results : After the modified CIMT, there was a significant improvement in completed time for the 6 tasks of Jebsen Hand Function Test(p<.05). 3D Motion Analysis was that the finger tapping and the hand tapping has been significantly decreased (p<.05), and the pronation-supination movement has been significantly increased as well(p<.05). Conclusion : In the results of this study, it is evidenced that modified CIMT is effective treatment for upper extremities function of children with hemiplegic cerebral palsy. For future research, it is recommended to examine various periods and protocol of modified CIMT including impact of long periods application.

Coronoid view: A New Radiograph for the Evaluation of the Coronoid Fractures (Coronoid view: 구상돌기 골절을 평가하는 새로운 방사선 촬영법)

  • Purpose: It is very important to evaluate and fix coronoid process fractures because they are a critical element for a stable, effective elbow function. The lateral view of the elbow joint is used for a radiographic evaluation of the coronoid but an understanding of the fracture pattern is often difficult because of overlap of the radial head and obliquity of the fracture line. We developed the coronoid view, which is a new radiograph for an evaluation of the coronoid process fracture, and discuss its advantages for a postoperative follow-up. Materials and Methods: The coronoid view was designed for an evaluation of the anteromedial fragment of the coronoid process. After the patient sat on his side, the shoulder was abducted $45^{\circ}$ and the elbow was flexed $90^{\circ}$. The X-ray beam was shot perpendicular to the table. Since shoulder was abducted $45^{\circ}$ the fracture line of the coronoid process can be parallel to the X-ray beam, and the radial head can be cleared. Conclusion: The coronoid view can be a good alternative radiograph for an evaluation of a coronoid process fracture because the beam is parallel to the fracture line. The coronoid view can be particularly useful in postoperative patient follow-up where computed tomography is impractical due to metal implants and cost.
