Self-care Status of the Aged Diabetic Patients with Noncompliance (일개 종합병원에서의 치료 미순응 노인 당뇨병 환자의 자가관리 실태)

  • Objective: The purpose of this study was to describe and understand self-care status of the aged diabetic patients with noncompliance after hospital discharge. Methods: A qualitative research method was used for the study design. The participants were 15 diabetic patients aged 65 or older who had been admitted more than two times for hyperglycemia in the past 1 year in a general hospital in Daegu. Data were collected from November 1 2015 to March 1 2016 through in-depth personal interview. Results: Patients' knowledge level on causes and symptoms of diabetes was low. Most participants rarely and irregularly checked blood sugar at home. They were under a lot of stress from their family. Due to old age and illness they did not have sufficient physical activity and they have had rarely regular meals. They considered the self-care education program unnecessary and there was limitations of accessibility for education. Conclusions: The aged diabetic patients who had the repeat admission did not perform self-care activities properly and had problems especially in blood glucose check, support from their family, and health education after returning home. More personalized and community-based self-management education programs will be necessary.

Comparison of Dose Distribution between the Techniques of Non-small Cell Lung Cancer (비소세포폐암의 방사선 치료기법간의 선량분포의 비교)

  • Comparison of the dose aspect that radiation therapy treatments using IMRT, tomotherapy, mArc (modulated arc therapy). The experimental subject is non-small cell lung cancer patient. The prescription dose is 58.0 Gy to the volume of PTV(planning target volume). and spinal cord, esophagus, and liver organ is the normal organ(OAR, organ at risk). Average PTV value is 57.60 Gy in mArc and 61.04 Gy in tomotherapy and 58.95 Gy in IMRT. The average dose of the Esophagus is 2.84 Gy in m-Arc, 5.14 Gy in tomotherapy, 1.84 Gy in IMRT. The average dose of the Liver is 19.44 Gy in m-Arc, 12.22 Gy in tomotherapy, 21.97 Gy in IMRT. The average dose of the Spinal cord is 5.72 Gy in m-Arc, 7.08 Gy in tomotherapy, 6.15 Gy in IMRT. Results of this study is no significant difference between mArc and tomotherapy and Linac based IMRT in dose study and also, mArc's dose coverage and dose volume histogram is better than IMRT and tomotherapy. but, This study is limited to a disease of cancer. in addition, fewer number of groups. The wide range the more research can be developed patient-specific treatment techniques and be applied to the patients

Clinical Importance of the Resection Margin Distance in Gastric Cancer Patients (위암환자에서 위절제술 시 근위부 절제연거리의 임상적 중요성)

  • Purpose: The way in which the resection margin distance for gastric cancer patients who undergo a gastric resection influences the recurrence rate, aspects of recurrence, and the prognosis according to the characteristic of the tumor is not known. We aim to find a standard for tailor-made treatment after selecting patients in this point of view who need a more sufficient resection margin. Materials and Methods: A retrospective study was done on 1,472 patients who underwent a gastrectomy due to gastric cancer at our hospital from 1992 to 2005. The median follow-up period was 37 months. Results: There were no significant differences in the recurrence rate, the aspects of recurrence, and the 5-year survival rate between early gastric cancer (EGC) patients with a resection margin distance of less than 2 cm compared with EGC patients with a resection margin distance of greater than 2 cm. However, significant differences in the survival rate were found in advanced gastric cancer (AGC) patients when the patients were classified into groups with resection margin distances less than or greater than 3 cm (P=0.02). Significant differences were noted especially in cases of diffuse histologic-type tumors located in the lower third of the stomach and in cases with Borrmann type-3 and -4 tumors. Conclusion: The distance between the tumor resection margin and the proximal gastric resection margin has no significant influence on the survival rate in EGC patients if the resection margin is negative. However, to improve a patient's survival rate, it is important to guarantee a resection margin of more than 3 cm in AGC patients, especially when the tumor is a diffuse histologic type located in the lower third of the stomach or a Borrmann type 3 and 4.

A Case Report of Phantogeusia Managed by Postural Yinyang Correction of Temporomandibular Joint with Customized TMJ Balancing Intraoral Appliance (맞춤형 턱관절 균형 구강내 장치를 활용한 턱관절 자세 음양교정술의 환상미각증 치험 1례)

  • Objectives: The purpose of this study was to report the effect of Postural Yinyang Correction of the Temporomandibular joint (Functional Cerebrospinal Therapy, FCST) for phantogeusia. Methods: A patient with phantogeusia was treated at the Department of Acupuncture & Moxibustion, Dong-Eui University Korean Medicine Hospital from Oct 10th, 2018 to Dec 4th, 2018. A patient received 24 times of FCST with Customized TMJ Balancing Intraoral Appliance, upper cervical manipulation. To estimate the efficacy, Visual Analogue Scale (VAS), Oral Health Impact Profile-14 (OHIP-14) and questionnaire. Results: The patient showed a certain degree of improvement in phantogeusia and other subject symtoms within 8 weeks. Adverse effects were not reported. Conclusions: These results showed that FCST using an Customized TMJ Balancing Intraoral Appliance, may be useful for reducing the symptoms of phantogeusia.

  • The purpose of this study was to investigate the effect of a new designed splint on the hand function. A new design splint that enlarges the area between thumb and index finger was manufactured using 3D printing. After wearing a new splint the patient was possible to open thumb and hold a small object. She showed improvement in overall hand function and could move eight blocks in box and block test. But grasping a cup without a handle has become more difficult than before. Also there is a disadvantage that it could not be fixed by water. This study is meaningful in that it is the first splint application study using 3D printing. In the future, we expect that various patient-specific splints will be developed through 3D printing in the field of occupational therapy.

Effect of Dental Treatment Fear and Distrust of Dentists with Dental Caries Experience among Middle and High School Students in Daegu City, Korea (일부 지역 중·고등학생들이 치과치료공포감 및 치과의사불신감이 치아우식증에 미치는 영향)

  • The purpose of this study is to analyze factors impacting on the middle & high school students' dental treatment fear and distrust of dentists with dental caries experience to provide empirical data for the development of students oral health. The subjects of this study consists of 720 middle & high school students in Daegu area. The data were collected from July 1, 2009 to September 30, 2009 by self-administrative questionnaires. Among sub-scales of Dental Fear Survey(DFS), stimulate-response factor was the highest ($3.42{\pm}0.98$), and patient distrust toward dintist factor was the highest score($3.07{\pm}0.912$) among sub-scales of Dentist Distrust Survey(DDS). Mean score of DFS were DDS were higher in female than male student. The students who have higher DFS and DDS score, the also have high DMFT index. The higher DDS, the higher DFS, and it was statistically significant(p<0.001). The results suggest that oral health status of adolescent can improve diminishing dental fear and dentist distrust if patient-specific medical service may be provided.

  • The technology of 'Virtual Reality' has placed in advanced tools for human beings' joy and anger together with sorrow and pleasure in our generation. It has recently tried in a variety ways to use as an implication for treatment in the field of Cognitive Psychology. Especially, it widely approaches to human in terms of that a sense of reality in a virtual world through the five senses should reinterpret the meaning of cognition in the real world. Based on this paradigm shift, it allows for new treatment using the technology of virtual reality. A typical example is a field of Therapy in order to overcome panic disorder. It has advantages that in particular development of flexible interaction technologies in a virtual space can lead patients to experience psychological environments rather than physical one. the interaction technology provides environments in which users' five senses can be actively stimulated, it is very useful that information from the experiences in the virtual world allows people to learn through real experiences by renewing potential energies, advantages of Virtual Reality Therapy can be customized treatment by depending on symptoms in patients with panic disorder and are capable of differentiate application for the cure at each stage. It is to treat by leading patients to get accustomed to environments and situations in real world through care process with each symptom and stage. It is helpful that based on A Human-Sensibility Ergonomics, technologies like immersive virtual reality equipment, force-relative feedback and stereophonic sound, and like stimulating the sense of smell make people to induce experiences by stimulating human's five senses. There are many advantages of immersion in virtual world in that the phenomenon such as challenge, interaction, reality, illusion, and cooperation is expanded. As an application for therapy by growing such augmented reality, virtual space and sharing of data through the Internet and also inexpensive its availability have recently expanded the base. There are other benefits of Virtual Reality Therapy offering active interaction environments for cognitive experience which can provide appropriately adjusted environments for patients who are hard to overcome the real situation because of phobia. In addition to that it is safe and economical and patients' confidentiality is assured. Moreover, due to the principles of applying real-time navigation the Virtual Reality Therapy makes modification and supplementation easier and also it can reduce cybersickness because of the supply of Lenticular allowing people to see stereoscopy without eyeglasses, which makes sense of presence clearer. On top of that due to the development of interactive technologies, it is becoming close to sense of reality similar to real world by leading users to navigate by themselves and to operate objects in a virtual space. This paper will therefore examine, although it is of limited, characteristics of application of virtual reality technology based on A Human-Sensibility Ergonomics used for treatment for a disorder. this paper will analyse a range of its application and problems and it will suggest the future possibilities.

Efficacy and Accuracy of Patient Specific Customize Bolus Using a 3-Dimensional Printer for Electron Beam Therapy (전자선 빔 치료 시 삼차원프린터를 이용하여 제작한 환자맞춤형 볼루스의 유용성 및 선량 정확도 평가)

  • We develop a manufacture procedure for the production of a patient specific customized bolus (PSCB) using a 3D printer (3DP). The dosimetric accuracy of the 3D-PSCB is evaluated for electron beam therapy. In order to cover the required planning target volume (PTV), we select the proper electron beam energy and the field size through initial dose calculation using a treatment planning system. The PSCB is delineated based on the initial dose distribution. The dose calculation is repeated after applying the PSCB. We iteratively fine-tune the PSCB shape until the plan quality is sufficient to meet the required clinical criteria. Then the contour data of the PSCB is transferred to an in-house conversion software through the DICOMRT protocol. This contour data is converted into the 3DP data format, STereoLithography data format and then printed using a 3DP. Two virtual patients, having concave and convex shapes, were generated with a virtual PTV and an organ at risk (OAR). Then, two corresponding electron treatment plans with and without a PSCB were generated to evaluate the dosimetric effect of the PSCB. The dosimetric characteristics and dose volume histograms for the PTV and OAR are compared in both plans. Film dosimetry is performed to verify the dosimetric accuracy of the 3D-PSCB. The calculated planar dose distribution is compared to that measured using film dosimetry taken from the beam central axis. We compare the percent depth dose curve and gamma analysis (the dose difference is 3%, and the distance to agreement is 3 mm) results. No significant difference in the PTV dose is observed in the plan with the PSCB compared to that without the PSCB. The maximum, minimum, and mean doses of the OAR in the plan with the PSCB were significantly reduced by 9.7%, 36.6%, and 28.3%, respectively, compared to those in the plan without the PSCB. By applying the PSCB, the OAR volumes receiving 90% and 80% of the prescribed dose were reduced from $14.40cm^3$ to $0.1cm^3$ and from $42.6cm^3$ to $3.7cm^3$, respectively, in comparison to that without using the PSCB. The gamma pass rates of the concave and convex plans were 95% and 98%, respectively. A new procedure of the fabrication of a PSCB is developed using a 3DP. We confirm the usefulness and dosimetric accuracy of the 3D-PSCB for the clinical use. Thus, rapidly advancing 3DP technology is able to ease and expand clinical implementation of the PSCB.

Multiple fixed implant-supported prosthesis using temporary denture and scannable healing abutment: a case report (임시의치와 스캔가능한 치유지대주를 이용한 고정성 임플란트 보철 수복 증례)

  • The use of digital technology in fixed prosthetic treatment using implants enables predictive treatment through diagnosis and virtual surgery by integrating clinical and radiological information of patients. Existing digital scanning methods require several components to be removed, such as removing the healing abutment and connecting the scan body. In the scannable healing abutment developed in consideration of this point, scanning is performed directly on the healing abutment, maintaining soft tissue sealing and simplifying scanning. Digital technology can also be used when obtaining the intermaxillary relationship. Recently, various digital technologies have been reported to acquire the intermaxillary relationship of edentulous patients using surgical guides, patient-specific scanning devices, or scans of the inside of temporary dentures. In this case, the implant-supported fixed prosthesis treatment was performed through scanning the scannable healing abutment and the inner side of the temporary denture to obtain the intermaxillary relationship, thereby simplifying the treatment process and obtaining aesthetically and functionally excellent clinical results.

Factors Influencing on Burden of Family Members of Intensive Care Patients : Planned Transfer to General Ward after Intensive Care (일반병동 전실예정의 중환자실 환자 가족의 부담감 영향 요인)

  • The aim of this study was to identify the levels of family members' burden and to describe factors influencing on famiCX*//8ly caregiver's burden when they are planned to transfer to general ward from the intensive care units (ICU). A descriptive survey was used with a convenience sampling of 101 family members of ICU patients at S university hospital in C city, Korea from August to November, 2010. The data were analyzed with SPSS 18.0 program. The level of object burden in family members had statistically significant differences with level of consciousness, number of patients' monitoring devices, and tracheostomy tube. The level of subject burden had statistically significant differences according to patients' gender, patients' education level, and relationship with patients. Transfer anxiety was showed statistically positive correlation with objective and subjective burden. We found transfer anxiety was statistically significant predictor of family caregiver's objective burden with explanatory power 12.7%. Family members' subjective burden were statistically influenced by patients' gender and transfer anxiety. These factors explained 23.8% out of total variance of family members' objective burden. The structured individualized method of transfer is recommended with further research of ICU families to further examine the dimension of transfer anxiety and how it affects family members' burden and patient outcomes.
