Analysis of Consumer's Recognition and Satisfaction for the Improvement of the Doctor-Designation System (선택 진료 제도 개선을 위한 소비자 인식도와 만족도)

  • The purpose of this study was to examine the recognition of citizens in Busan on the doctor-designation system, the awareness of medical consumers with experience of using this system and their satisfaction in an effort to seek ways of improving this system. The subjects in this study were the selected citizens in Busan who were at the age of 20 and up. As a result, it's found that the largest group of the respondents was female, in their 20s, received college or higher education, students and in the service industry, and that the most common monthly mean income was between two and 2.99 million won. 27.7 percent were aware of the doctor-designation system, and 23.7 percent became cognizant of the system through others who had used it. The rate of knowing the rules of the doctor-designation system (the right answer) stood at 66.3 percent. They got to know about the system through mass media(31.9%), and used it since it offered highly specialized treatment(57.5%). The respondents who had used it intended to reuse it(76.3%), and the reason was that they were provided with high-class medical services (35.2%). The respondents who had used this system got a mean of 2.96 in satisfaction level, which was not high in general. They mentioned more publicity efforts(91.2%), offering information in a conspicuous place (96.7%) and cutting doctor-designation treatment fee as a means of improving this system. As for how to ensure the operating efficiency of the system, sustained publicity seem to be necessary to raise awareness of the system among patients, and it's required to take measures to relieve patients of financial burden caused by medical bills.

Dental hygienist's leisure activities and job satisfaction

  • This study was conducted to identify the relationship between the willingness to leisure activity and job satisfaction of dental hygienists and to suggest the need to expand opportunities for the will to leisure activity. The degree of job satisfaction was 3.84 points, and the willingness to leisure activity was analyzed to prioritize exchange with the community. As a result of the analysis of the correlation between job satisfaction, turnover intention, and willingness to leisure activity, all three items show a strong correlation. If job satisfaction is high, then the willingness to leisure activity is high and the intention to turn over is low, and if the will to leisure activity is high, then job satisfaction is high and turnover intention is low. Results of regression analysis to establish measures to increase job satisfaction, turnover intention (β=-0.551), leisure use (β=0.368), health status (β=0.139), current working years at work (β=0.069), number of patients visiting the hospital during the day (β=-0.056) were analyzed in order of effect. It is judged that there is a need to establish a plan to provide an optimal working environment in consideration of the intention to turnover, leisure activity, health status, working years at work, and the number of patients visiting the hospital during the day.

Needle Thoracoscopic Sympathectomy for Essential Hyperhidrosis (2 mm 흉강내시경을 이용한 흉부교감신경 절제술)

  • Excessive sweating of the palms, axillae, and face has a strong negative impact on the quality of life for many people. The existing non-operative therapeutic options seldom give sufficient relief and have a transient effect. But a definitive cure can be obtained by upper thoracic sympathectomy. From June 1997 to October 1997, 117 cases of the needle (2 mm) thoracoscopic thoracic sympathectomies were performed in the Department of Thoracic and Cardiovascular Surgery in the Respiratory Center Yong-dong Severance Hospital in Seoul, Korea. We have followed up on 94 cases which include palmar hyperhidrosis (n=85), facial hyperhidrosis(n=5) and axillary hyperhidrosis(n=4). There were 42 males and 52 females whose ages ranged from 14 to 63 years(median:23 years). The T2 ganglia and T3-4 ganglia were excised by electrocuting with a hook and endoscissors and were removed for histologic examination. There have been no mortalities or life-threatening complications. The surgical results were classified as excellent(much improvement,very dry) in 93.6%, good(some improvement, minimally wet) in 2.1%, and fair(slight improvement, still wet) in 4.2%. Five patients(5.3%) required closed thoracostomy drainage because of pneumothorax in the immediate postoperative day. Horner's syndrome occurred in one case. The compensatory sweating occurred in 67 cases(71.2%) and was embarrassing in 21 cases(22.3%) and disabling in 9 cases(9.6%) of these cases. Primary failure occurred in one case. The patient with primary failure underwent successful operation. Fifty-one patients had concomitant hyperhidrosis. Our experiences indicate needle thoracoscopic sympathectomy is a very effective, safe, and time- saving procedure for essential hyperhidrosis.

The Relationship between Job Stress and Job Satisfaction of Caregivers (요양보호사(療養保護司)의 직무(職務)스트레스와 직무만족도(職務滿足度)의 관계(關係))

  • 본 연구는 요양보호사들의 직무스트레스가 직무만족도에 미치는 영향을 알아보고자 하는 연구로 연구대상은 대전지역의 요양시설에 근무하는 요양보호사 302명을 대상으로 2008년 10월 6일부터 10월 30일 까지 구조화된 설문지를 이용하여 다음과 같은 결론을 얻었다. 연구 대상자의 직무스트레스에 대한 요인은 총 4개로 요인별 평균값은 환자관련이 2.38(표준편차 1.27), 업무관련 2.27(표준편차1.23), 대인관계 2.04(표준편차 1.01), 경제적인면 2.09(표준편차 1.17)로 나타났다. 연구 대상자의 직무만족에 대한 요인은 총 2개로 요인별 평균값은 업무자체에 대한 만족여부가 3.36(표준편차 0.690), 이직의사 2.36(표준편차 0.796)으로 나타났다. 업무에 대한 만족여부는 환자관련(P=0.021) 스트레스와 업무관련(P=0.014)스트레스가 유의하게 나타났고, 이에 대한 영향으로는 연령에서 50대(P=0.002), 40대(P=0.008), 20대(P=0.030), 종교를 가지고 있는 그룹(P=0.041), 근무동기로는 일에 대한 보람(P=0.042), 경력은 3년이상(P=0.002), 6개월이상 1년미만(P=0.006), 6개월미만(P=0.002)인 그룹, 소유자격증에 관해서는 간호사(P=0.009)면허 소지 그룹, 돌보는 환자수로는 1~5명(P=0.034), 6명~10명(P=0.001), 근무시간은 24시간(P=0.000)근무와 8시간 교대근무(P=0.026)가 유의한 영향을 주는 것으로 나타났다. 5. 이직의사 여부에서 요인2.업무관련(P=0.002)스트레스 유의하게 나타났고, 이에 대한 영향은 무교(P=0.010)인 그룹, 근무동기로는 미래에 대한 준비(P=0.012), 월급여로는 100만원~149만원(P=0.030), 돌보는 환자수로는 6명~10명(P=0.020)이 유의한 영향으로 나타났다.

The Validity and Reliability of a Korean Version of the Satisfaction with Simulation Experience Scale for Evaluating Satisfaction with High-Fidelity Simulation Education for Nursing Students (간호대학생의 고성능 인체 환자 모형 시뮬레이션 교육 평가를 위한 한국판 시뮬레이션 만족도 경험 도구의 타당도와 신뢰도 연구)

  • The purpose of this study was to test the validity and reliability of the Satisfaction with Simulation Experience (SSE) scale for evaluating high-fidelity simulation education for nursing students. Participants were 174 nursing students, seniors enrolled in two colleges in two different regions. Collected data were analyzed using SPSS / WIN 22.0 and tested for construct validity (factor analysis, group comparison test) and reliability (internal consistency). Factor analysis revealed 17 items and 3 factors explaining 71.581% of the variance. Group comparisons showed that satisfaction with simulation training differed significantly across satisfaction to a college life and school record. Internal consistency reliability for all items was .945. For each sub-domain, the reliability coefficient was .929 for 'Debrief', .908 for 'Clinical learning and reflection', and .860 for 'Clinical reasoning'. Nursing students' mean satisfaction with simulation using the high-fidelity simulator was 3.92. Results of this study are expected to be used for evaluating the satisfaction of nursing college students receiving high-fidelity simulation education, and to serve as groundwork for the development and application of nursing simulation education.

Structure Model for Job Satisfaction and Service Quality of Staff of Geriatric Hospital (요양병원 직원들의 직무만족도, 서비스 질에 대한 구조모형)

  • This study has been implemented to explore the method to improve job satisfaction and service quality of its staff by figuring out the effect of job satisfaction of geriatric hospital staff on service quality. For this reason, we conducted a questionnaire survey on April 2 to May 22, 2013 to target 300 people ten geriatric hospital workers in Busan Metropolitan City area, was utilized to study the final total of 276 parts. Firstly, It was found that work satisfaction, wage satisfaction and colleague satisfaction have an effect on job satisfaction, and Secondly, satisfaction and fellow satisfaction of business, were analyzed to affect, but had no effect conservative satisfaction Thirdly, for the effect of job satisfaction on the service quality, it was found that job satisfaction has an effect on service quality. Therefore, it can be concluded that service quality for patients can be improved only if job satisfaction of staff is improved, and by reducing stress caused by overwork of staff and boosting their morale through reasonable wage and compensation.

Effects of Nurses' Teamwork on Job Satisfaction at Hospital: Mediating Effect of Self-efficacy (간호사의 팀워크가 직무만족에 미치는 영향: 자기효능감의 매개효과)

  • This study aimed at (a) examining the effects of teamwork, demonstrating with other nurses at a care unit on nurses' job satisfaction and self-efficacy, and (b) identifying if there is the mediating effect of self-efficacy on the relationship between teamwork and job satisfaction at a hospital. A descriptive study was conducted with a sample of 304 nurses, who were caring for patients with more than one nurse at a university hospital located in Southern area of Korea. The degree of teamwork demonstraing by registered nurses was 3.78(${\pm}0.59$) in the range from 1 to 5. Teamwork at caring units affected significantly nurses' job satisfaction(F=58.26, p<.001), and explained 42.80% in the variance of job satisfaction. The degree of self-efficacy that nurses perceived played as a mediator on the relationship between teamwork and job satisfaction at significant level(Z=5.25, p<.001).

Job stress and job satisfaction of the nurses in comprehensive nursing service units and the nurses in general units (간호간병통합서비스병동 간호사와 일반병동 간호사의 직무스트레스와 직무만족)

  • This study was conducted to compare and analyze occupational stress and satisfaction levels of the nurses in two different units to provide data for effective nursing resource management. Survey was conducted in four hospitals in June 2019 and data was obtained from 62 nurses in comprehensive nursing service units (CNSU) and 62 nurses in general units (GU). CNSU nurses had higher level of job satisfaction than GU nurses. Contributing factors included pay, professional status, administration, autonomy, task requirements, and interaction. Job stress level was not significantly different between groups. GU nurses had higher stress levels from nursing practice, conflict with doctors, and patients and caregivers, while CNSU nurses had higher stress levels in working condition. Negative correlation was found between job satisfaction and stress in GU nurses, whereas not in CNSU nurses. Clear definition of task, smaller work load, appropriate reward, and educational support is suggested to enhance job satisfaction.

The Change in Far Distance and Near Distance of Exophoria Patients (외사위 환자의 원거리와 근거리 때의 변화)

  • A36 patients of a total of 60 persons show $2{\Delta}$ and less, and the rest of 24 patients show $6{\Delta}$ and less. The patient, who increase more in near distance than far distance in exophoria amount are 27 persons. The unchanging patients are 13 persons, and the patients who phoria amount decreasing are 20 persons. Like this, the patients who phoria amount unchange and decrease show more than the half. That is why convergence increases in near distance. The expected value of horizontal phoria in near distance is $3exo{\pm}3$. Only 13 patients get rid of its value, 47 satisfy its value. In AC/A value, 35 persons show under 3(1ow), 21 persons show 3~5(normal), and 4 persons show more than 5(high). By Percival's theory, 36 patients don't feel a slight asthenopia without correcting phoria. Finally, in near point of convergence 27 patients show 10cm and less, 24 patients show 11~15cm, 3 patients show 15~20cm, and 6 patients show 21~25cm. Most of them show high accommodation.

Convergence Effects of Nursing Professionalism and Critical Thinking Disposition on Patient Safety Attitude in Nursing Students (간호대학생의 간호전문직관과 비판적 사고성향이 환자안전에 대한 태도에 미치는 융 복합적 영향)

  • This study examined convergence effects of nursing professionalism and critical thinking disposition on patient safety attitude of nursing students. The subjects were 172 senior nursing students in S city. The data were collected from May 15 to May 25, 2017. The data collected were analyzed using SPSS Statistics version 23.0. Nursing students showed 3.76, moderate level in patient safety attitude. Nursing professionalism and critical thinking disposition were positively correlated with patient safety attitude. The variables that have significant influence on the patient safety attitude were 'recommendation' among motivation of admission, academic score, satisfaction with nursing major and nursing professionalism. These factors explained 17.8% of variance. In conclusion, to improve the patient safety attitude of nursing students, it is necessary to systematic management for developing motivation of admission, academic score, satisfaction with nursing major and nursing professionalism.
