A Study on the Relationship between Self Effcacy and Self Care Behavior in Patients with Pulmonary Tuberculosis (폐결핵 환자가 지각한 자기효능과 자가간호행위와의 관계에 관한 연구)

  • This study is a descriptive survey research to investicate the relation between the performance of self-care behavior and degree of self-efficacy by evaluating them respectively. The subjects were consisted of seventy nine tuberculosis patients registered in two different public health care centers in the city of Kwangju from April 1st to 30th June, 1995. The results were as follows : 1) Demorgraphically, the proportion of males was$(59.5\%)$, the highest proportion of age group was twenties with $(31.6\%)$, the higest proportion of the patients graduated high school. the married was much more than the single$(51.9\%)$. The highest proportions in each variables were as followes : middle class in economic status with $70.9%$, the jobless in job status with $26.6\%$. the duration less than 6 months in ailing duration with $63.3\%$. The patients of $60.8\%$ and $74.7\%$ showed no family history and received no health education respectively. 2) The average drgree of self-care behavior was 3.44 points (1-5 points). 3) The average dregee of recognized self efficacy was 78.18 points(10-100points). 4) The recognized self efficacy showed close relationship with the performance of self-care behavior (r=0.6378. p<0.001) 5) The points of performance of self-care nursing behavior showed statistical significance in the variables of martial status. duration of treatment and economic status. (F=3.142. p<0.05), (F=3.701. p<0.05). (F=6.213. p<0.01) 6) The point of self efficacy showed statistical significance in the variables of sex. degree of education and marital status. (F=5.438. p<0.05), (F=4.28. P<0.01), (F=2.899, p<0.05) These results suggest as follows: 1) It is necessary to develop a program to improve the ability of self health care for the family member as well as the patient. 2) It is necessary to do further study on much more variables that influence on the performance of self-care behavior by expanding the number of subjects and duration of study. and to evaluate the self efficacy with time interval.

  • In recent years, because of the development of ubiquitous technology in healthcare research is actively progress. Especially, healthcare service area is change to home for the elderly or patients from hospital. The technology to identify residents in a home is crucial for smart home application services. However, existing researches for resident identification have several problems. In this case, residents are needed to attach of various sensors on their body. Also relating private life, it is difficult to apply to resident's environment. In this paper, we used constraint-free sensor and unconscious sensor to solve these problems and we limited using of sensor and indoor environment in the way of working economical price systems. The way of multi-resident identification using only these limited sensors, we selected elements of personal identifications and suggested the methods in giving the weight to apply these elements to systems. And we designed the SABA mechanism to tract their location and identify the residents. It mechanism can distinguish residents through the sensors located each space and can finally identify them by using the records of their behaviors occurred before. And we applied the mechanism designed for applications to approve this location tracking system. We verified to the location tracking system performance according to the scenario.

Healing Environment at the General Hospital Lobby Space -By comparison of the lobby plan type- (종합병원 로비공간의 치유환경에 관한 연구 -로비 평면형태의 비교를 중심으로-)

  • In the past, hospitals were planned with a minimum of public space for the sole purpose of treatment. On the other hand, modern hospitals have more public space as the importance of the healing environment increases and are planned taking into consideration the patients, guardians, and medical staff. In addition, the lobby shape is developed from a HALL type and planned as a STREET type or CONCOURSE type, increasing the public area. Unlike past hospitals, which were used as reception, waiting, and procedure spaces, modern hospitals are used as commercial, hobby, and cultural space. The purpose of the study was to evaluate the healing environment of the hospital lobby based on the shape of the plane surface by deducting the elements of general hospitals' healing environment from preceding studies as a framework. The hospital lobby is an important place where many actions occur. Therefore, 3 types of cases were evaluated based on the plane form. As a result of the study, the STREET type showed a better healing environment than the HALL type, but the accessibility, roof garden, and resting place showed similar results. Because the case was based on a large general hospital, which had sufficient planned space, the factor has little to do with the style of the lobby. Among the STREET types, the linear atrium showed a better result in terms of the sense of space and inflow effect of natural lighting than the four-sided atrium because the linear atrium has large open volume ratio. When the lobby plane is the HALL type, a cross section of the courtyard had a large open volume ratio. Therefore, the shape of the cross section is as important as the plane form of the lobby.

Uniformity Evaluation of Elderly Hospital Outpatients' Waiting Space using Discrete Event Simulation (이산사건 시뮬레이션을 이용한 요양병원 외래부 대기공간 균일성 평가)

  • In recent years, the introduction of complex systems analysis based on various variables has become more active in order to identify and analyze complex problems of Modern Society. Prediction of patients' spatial perception and usability according to the spatial arrangement of the outpatient department is a very important factor for providing high quality hospital service. For objective analysis, the standard program procedure and analysis index for the diseases of the elderly were prepared and the uniformity of the atmospheric space was evaluated through heat map analysis and quantitative analysis. In this study, 73 cells were installed and simulated to analyze the uniformity of the four alternatives according to the change of the arrangement of the medical care space, receiving space, and consultation space using the complex system analysis method for the nursing hospitals. The resulting density was derived. The results are as follows. 1)The layout of the reception space has the greatest influence on the total spatial density of the waiting space. 2) The uniformity of the waiting space can be increased by separating the examination space and the examination space. 3)The closer the location of the receiving space is from the entrance, the greater the density of the waiting space. Finally, this study applied discrete event simulation to the evaluation of uniformity of atmosphere space, and proved that the actor - based model can be utilized for utilization and evaluation as spatial analysis methodology.

A Comparative Study on the Satisfaction of Korean and Canadian Primary Care Based on the Survey of Overseas Korean in Canada (캐나다 재외국민 설문조사에 기초한 한국과 캐나다 1차 의료기관 만족도 비교 연구)

  • This study compares and analyzes satisfaction with Korean and Canadian primary medical care based on a survey of Koreans living in Canada. Based on data collected from Koreans who live in Canada, a paired sample t-test, analysis of variance (ANOVA), and GLM were used. The main conclusions of this study are as follows. First, overall satisfaction with Korean primary care was high. Second, the results of the analysis were stable, regardless of gender, age, education, and residential area. Third, overall satisfaction with Korean medical care was higher than for medical care in Canada. Fourth, satisfaction was high in terms of treatment skill and technology, and promptness in appointments, but was low in terms of medical expense. Fifth, there was a high level of confidence in physician skills and practices, but there was no significant difference in terms of sufficient counseling and explanations. Finally, the survey found that the tendency to increase medical treatment volume in order to increase a physician's own income was large. As a result of detailed analysis, it was concluded that Korea's primary care is more competitive than in Canada, but the primary medical doctors' patient interview and explanation obligations, and incentives to control income and medical expenses, need to be improved.

A survey on Patients' Compliance with Follow-up Coronary Angiogram after Coronary Intervention (관상동맥 중재술후 추적 관상동맥조영술 실천에 대한 조사연구)

  • Coronary intervention is now a well established method for the treatment of coronary artery disease. Coronary restenosis is one of the major limitations after coronary intervention. So medical teams advise the patients to get the follow-up coronary angiogram in 6 months after coronary intervention to know if the coronary artery stenosis recurs or not. This study was done in order to know how many patients complied with the advice, and to identify the relative factors to the compliance with getting the follow-up coronary angiogram. The subjects were 101 patients (male: 58 female: 22, mean age: $61{\pm}15$), who received coronary interventions from Jan. 1st to Mar. 31st 1997, and their data were collected from them by questionnaires one year after intervention. The questionnaires consisted of family support scale, self efficacy scale and compliance with sick role behavior scale. The result may be summarized as follows. 1. The number of patients who complied with getting the follow-up coronary angiogram were 37 people(36.6%) and did not comply with it were 64 people(63.4%). All scores of family support(t=5.56, p<.0001), self efficacy (t=4.13, p<.0001) and compliance with sick role behavior(t=5.66, p<.0001) were significantly higher in the patients who got the follow-up coronary angiogram than in those who did not get it. But there was not any relative factor in demographic variables (p>.05). 2. The major motivations for getting follow-up coronary angiogram were recurrence of subjective symptom(40.5%), the advice of medical team(32.4%), and fear of recurrence (27.1%). The restenosis rate in patients who got the follow-up coronary angiogram was 37.8%. 3. The restenosis rate was higher in the patients who had subjective symptoms than in those who did not have any subjective symptom. So subjective symptom and restenosis rate showed a high positive correlation(r=39.9, p<.001). However, 27.2% of the patients who did not have any subjective symptom showed coronary restenosis. 4. The reasons why they did not get the follow-up coronary angiogram were economic burden(37.5%), improved symptom(34.4%), busy life schedule(10.9%), fear of invasive procedure(9.4%), negative reaction of family member(3.1%), no helper for patient(3.1%) and worry about medical team's mistake (1.6%). The relative fators on compliance with getting the follow-up coronary angiogram after coronary intervention were family support, self-efficacy and Compliance with sick role behavior. And the most important reason why the patients did not get the follow-up coronary angiogram after coronary intervention was an economic burden.

Effects of a Program to Promote Self-Efficacy and Hope on the Self-Care Behaviors and the Quality of Life in Patients with Leukemia (자기효능ㆍ희망증진프로그램이 자기간호 행위와 삶의 질에 미치는 효과 -백혈병 환자 중심-)

  • Most patients having leukemia suffer severely from emotional turmoil due to the generalized perception that cancer will be fatal. The conventional chemotherapy results in side effects such as severe bone marrow depression which interfere with self-care management, vital for improvement in their condition. Bandura's theory of the self-efficacy suggests that self- efficacy can be enforced by performance attainments, vicarious experiences, verbal per-suasion and the release of emotional arousal. Self-efficacy can be enforced by a program of vicarious experiences and verbal persuasion, while the emotional arousal can be relieved through a hope promotion program, If once self-efficacy increases, the patient's self-care behaviors and the quality of life will also increase. The purpose of this study was to empirically test the effects of a program, to promote self-efficacy and hope, on self-care behaviors and quality of life in patients having leukemia. In this study, three types of approaches to enhance self-efficacy and hope were used : 1) a 20-minute long slide /tape for vacarious experiences : 2) a 10-minute long telephone call coaching for verbal persuasion ; and 3) two booklets for information about the symptoms of leukemia and treatment modalities and hope promotion. Thirty one patients were recruited in the experimental group and 29 in the control group with a nonequivalent pretest-posttest design. The subjects were patients with leukemia undergoing chemotherapy. Sherer and Maddux's self-efficacy scale, Nowotny's hope scale, and Padilla's quality of life scale were employed with some modifications. A self-care behavior scale was developed by the researchers. Statistical analyses including paired t-test, Chi-square, ANCOVA and ANOVA, were used. The results are as follows : The levels of self-efficacy, self-care behavior and quality of life were higher in the experimental group than in the control group after four weeks of intervention(F=28.71, P=.0001 ; F=63.35, P=.0001 F=16.57, P=.0001). After ten weeks of intervention, all of the dependent variables(self-efficacy, self-care behavior, hope & quality of life) in the experimental group were higher than in the control group (F=74.12, P=.0001 ; F=108.34, P=.0001 ; F=13,11, P=.001 : F=43.52, P=.0001). In conclusion, self-care behavior and quality of life increased mainly through an increase in self-efficacy, while increases in hope took more time and effort.

TONGUE INJURIES BY SELF MUTILATION IN LESCH-NYHAN SYNDROME PATIENT: A CASE REPORT (Lesch-Nyhan 증후군 환아의 자해에 의한 혀의 외상)

  • Lesch-Nyhan syndrome is a rare X-linked recessively inherited disorder, caused by complete absence or decrease in activity of hypoxanthine guanine phosphoribosyl transferase(HPRT), an enzyme involved in purine metabolism. This enzyme deficiency gives rise to nephropathy symptoms, such as hyperuricosuria and hyperuricemia by excessive uric acid production and neuropathy symptoms, such as mental retardation, choreoathetosis and self mutilation behavior. Patients with Lesch-Nyhan syndrome have tendency to bite their lip, tongue and finger. In severe cases, partial or even total amputation of tongue or finger occur. Self-inflicted bites are often complicated by secondary infection to the injured site as well as pain. Furthermore tissue loss by biting results in esthetic problems. The dental management of self mutilation includes treatment with appliances such as soft mouth guard or lip bumper, extraction of all the teeth, and orthognathic surgery. We report a case of a 13 year-old boy with Lesch-Nyhan syndrome, who severely injured himself on his tongue. At first, conservative treatment using soft mouth guard was considered, but it could not prevent trauma on his tongue. Therefore, extraction of the lower anterior and posterior teeth was carried out.

The Correlation of Dental Hygienist's Educational Experience in Infection Control with the Activity Ratio of Infection Control in Health Belief Model (건강신념모형을 적용한 치과위생사의 감염관리 교육경험에 따른 수행도와의 관련성)

  • This study examined the correlation of educational experience with practical behavior in infection control. Subjects were 152 dental hygienists in Gwangju from September 15 to October 5, 2015. The ratio of dental hygienists educated on infection control was higher in those worked in dental care with 2~4 dentist for 2~5 years and lower in those worked in dental cared with 1 dentist for 2~5 years (p<0.05, p<0.001). The dental hygienists with or without educational experience in infection control exhibited the highest activity rate in hand wash and the lowest in face protection. Barrier in health belief, was lower with the need for education in infection control among the factors affecting on the activity in infection control. The activity was higher with susceptibility and cue to action (p<0.05). Based on the results, education program on infection control should be developed, and applied periodically and obligatorily for dentist and all staffs of dental care to remove susceptibility and barrier, and to enhance cue to action. This will result the effective control of infection by elevating the health belief.

Assisted Outpatient Treatment and Crisis Intervention in USA and their Implications for Korea (미국의 외래치료명령제도 및 위기대응과 국내적 시사점)

  • Since the 1960s, the United States' (U.S.) deinstitutionalization policy has reinstated people with mental illness into communities. Unfortunately, when untreated, some people with psychiatric disorders become homeless, and some commit serious crimes during a psychological crisis. Assisted Outpatient Treatment (AOT), also known as Kendra's Law in New York and Laura's Law in California, provides treatment, services and support to people with mental illness in the community. AOT has repeatedly been found effective and is recognized as an evidence-based practice. The response to the mental health crisis (crisis intervention) in the U.S. has also been successful in preventing worsening mental illness and related criminality and other issues. This paper provides an opportunity to create a platform from which to learn how to successfully apply the AOT and crisis intervention of the U.S. to South Korea within the cultural and societal context when establishing social services for people with mental illness in South Korea's communities.
