고발코너: 환자가 만드는 의료서비스 불합리한 치료를 받았다면

  • 환자도 소비자이다. 또한 의료행위도 서비스 중의 하나이다. 하지만 다른 서비스업에 비하면 소비자인 환자가 중심이 되지 못하고 있는 실정이다. 물론 요즈음 많은 병원들이 더 나은 의료서비스를 제공하고자 노력하고 있지만, 아직 환자의 목소리가 반영됨을 실감하지 못한다. 앞으로 보다 나은 양질의 의료서비스가 정착되길 기대하며 불합리한 의료서비스를 개선하고자 환자의 의견을 들어보기 위해 <고발코너>를 마련했다.

  • Medical services should be provided not only to patients but also to medical staff who are the main actors of medical treatment. This paper is based on Ethnography, observing the behavior of medical staff performing medical actions and finding what is the most frequent of physical medical treatment. In particular, I observed the need for recognition and performance of the clinical workforce for invasive medical treatment. Based on the Insight from the observation and analysis of medical staff's behavior on intravenous fluid therapy and in-depth interview, I defined four design value factors. Plus, I suggest a design prototype with a proposal for designing a medical consumable kit for intravenous fluid therapy to validate the design elements. Kitting medical consumables simplify the preparation and disposal process throughout the treatment, enabling rapid medical action and increasing work efficiency.

The Effect of Stress and Sense of Coherence on Health Behavior Compliance in Outpatients with Stroke (뇌졸중 환자의 스트레스와 통합성이 건강행위 이행에 미치는 영향)

  • The purpose of this study was to identify the effect of stress and sense of coherence on health behavior compliance in patients with stroke. The subjects were 124 outpatients with stroke at a general hospital in Ulsan from May to July, 2019. The data were analyzed using descriptive statistics, t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient and hierarchical multiple regression analysis by using SPSS 25.0. In the results, the health behavior compliance showed a significantly positive correlation with sense of coherence(r=.71, p<.001) and a negative correlation with stress(r=-.53, p<.001). The predictors of model 1 were gender(t=4.07, p<.001), education level(t=4.18, p<.001), and monthly income(t=-2.34, p=.021) and the statistical power was 39.6%. The predictors of model 2 were gender(t=4.65, p<.001), education level(t=43.24, p<.001), and sense of coherence(t=4.33, p<.001) and the statistical power was 60.4%. Thus, a nursing intervention program which is focused on a sense of coherence could promote health behavior compliance in patients with stroke.

Relationship among Resilience, family support and health promotion of hospitalized cancer patients in an advanced general hospital (일 상급종합병원에 입원한 암환자의 극복력, 가족지지 및 건강증진행위간의 관계)

  • Purpose: The aim of this study was to investigate the relationship among resilience, family support and health promotion of hospitalized cancer patients in an advanced general hospital. Methods: Data were collected from 131 hospitalized cancer patients and analyzed SPSS 18.0 program. Results: Resilience showed significant differences according to gender, education level, occupation, family income, family help. and recurrence. Family support showed significant differences according to education level, occupation, care giver, family help, progress of disease and recurrence. Resilience and family support are positive correlations. Conclusion: It is needed to develop various program including cancer patient and their family for the promoting resilience.

Factors Affecting Health Promotion Behaviors in Patients with Reflux Esophagitis (역류성 식도염 환자의 건강증진행위에 미치는 영향요인)

  • This descriptive survey aimed to investigate the health promotion behaviors of patients with reflux esophagitis and identify the factors influencing it using the health promotion model. The survey was conducted with 178 outpatients diagnosed with reflux esophagitis at C university hospital. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation, and multiple regression using SPSS win ver 20. We found that alcohol consumption, BMI, perceived benefit, internal control level, and perceived health status of the patients are significant factors influencing health promotion behavior. The explanatory power was 49%. Therefore, this study is consistent with previous studies that elucidated that changing lifestyle behaviors and obesity control are necessary to alleviate reflux esophagitis. Based on these findings, we propose to develop and apply an educational program for evaluation.

Factors Influencing Health Promotion Behavior in Patients with Pulmonary Tuberculosis (폐결핵 환자의 건강증진행위와 관련요인)

  • This study was conducted to identify the health promotion behavior of patients with pulmonary tuberculosis and examine the factors influencing this behavior. The subjects were 152 outpatients who were treated for pulmonary tuberculosis at the national tuberculosis hospital located in M City. Data were collected from July to September, 2016 and analyzed using the t-test, ANOVA, Duncan's test, Pearson's correlation coefficient, and multiple regression analysis. Health promotion behavior was negatively correlated with physical symptoms, perceived barriers, and depression, but positively correlated with self-efficacy. The most powerful variable influencing health promotion behavior was depression and the model showed an explanatory power of 63.5%. The results of this study suggest that it is necessary to employ a variety of strategies to practice and maintain the health promotion behavior of pulmonary tuberculosis patients to decrease their depression and provide psychological support.

Self-Efficacy, Self-Care Behavior, Posttraumatic Growth, and Quality of Life in Patients with Cancer according to Disease Characteristics (질병관련 특성에 따른 암환자의 자기효능감, 자가간호행위, 외상 후 성장, 삶의 질)

  • Purpose: This study examined self-efficacy, self-care behavior, posttraumatic growth, and quality of life in cancer patients and their levels by disease characteristics groups to identify patient groups that require psychosocial intervention. Methods: We surveyed 107 patients using a structured questionnaire about the four factors and analyzed the factors by stratifying the patients by the period after the cancer diagnosis, by stage and by current treatment status. Results: The mean score for self-efficacy was 37.78, and that for self-care behavior 49.96. Patients who were diagnosed less than one year ago scored higher on medication, a sub-category of self-care behavior, than the post-diagnosis period of 1~2 year group. The score was higher in the currently-treated group than the follow-up and distant metastasis groups. For posttraumatic growth, the mean was 56.17, and the factor was higher in the 1~2 year post-diagnosis group after than the less than one year group. The score was higher in the follow-up group than the currently-treated group. With regard to quality of life, the mean score was 25.79, and no significant correlation was found with disease characteristics. Conclusion: A shorter post-diagnosis period increased self-care behavior, and the greatest posttraumatic growth was reported by the 1~2 year post-diagnosis group. It may be necessary to provide cancer patients with an education program and other strategies less than one year after the diagnosis to improve self-efficacy and self-care behavior. To promote post-traumatic growth, it may be helpful to provide patients with psychosocial intervention within two years after the diagnosis.

Convergence Relationships among Nutrition Knowledge, Health Beliefs, Self-efficacy, and Diet Management Behaviors in Persons with Hypertension in Sri Lanka (스리랑카 고혈압 환자의 영양지식, 건강신념, 자기효능감, 식이조절행위의 융복합적 관계 연구)

  • This study aimed to identify the convergence relationships among nutrition knowledge, health beliefs, diet self-efficacy, and diet management behaviors among Sri Lankans with hypertension (HTN). 91 adults diagnosed with HTN for more than one year participated in this study. A set of interviewer-administered questionnaire was used to assess general characteristics, nutrition knowledge, health beliefs, diet self-efficacy, and diet management behaviors. A stepwise multiple regression analysis was conducted to identify the factors that might influence diet management behaviors. There were statistically significant positive correlations between nutrition knowledge (r=0.26, p=.013), health beliefs (r=0.35, p=.001), diet self-efficacy (r=0.34, p=.001) and HTN diet management behaviors. Three variables, nutrition knowledge, health beliefs, and diet self-efficacy, explained 19.9% of the variance in HTN diet management behaviors (F=8.36, p=.001). Diet self-efficacy was the most significant influencing factor (${\beta}=.29$, t=2.75, p=.007), followed by nutrition knowledge (${\beta}=.25$, t=2.50, p=.014). The findings of this study can be used as valuable data for developing effective interventions to improve diet management behaviors of persons with HTN.

Analysis for Oral Health Behavior of Some Residents in Gyeongsangnam-do Area (경남권 지역 일부 주민들의 구강건강행위 분석)

  • The purpose of this study was to analyzed the major factor variables against oral health behaviors and oral health state and quality of life (QOL) for some residents in Gyeongsangnam-do area. This study was based on the PRECEDE model as a theoretical model for diagnosis in health education needs. Total data 697 were analyzed by PASW 18.0 program. Results for this study were as follows: Firstly, t-test results in sex had statistically significant in predisposing, reinforcing, oral health behavior, health${\cdot}$QOL factor, education had statistically significant in predisposing, reinforcing, health${\cdot}$QOL factors, also, number of teeth had statistically significant in health${\cdot}$QOL factor. Secondly, ANOVA results in age had statistically significant in predisposing, enabling, health${\cdot}$QOL factor, area had statistically significant in health${\cdot}$QOL factor, also, economic had statistically significant in reinforcing, oral health behavior. Thirdly, in CATREG analysis, oral health behavior as the response variable results in had statistically significant in sex, predisposing, reinforcing, enabling factor and its explanation was 28.3%. Lastly, in CATREG analysis health${\cdot}$QOL as the response variable results in had statistically significant in age, number of tooth, enabling, oral health behavior factor and its explanation was 17.9%. So, results from this data we could contribute to identify oral health behavior patterns in Gyeongsangnam-do area residents.
