Research of IoT concept implemented severity classification system (IoT개념을 활용한 중증도 분류 시스템에 관한 연구)

  • The following research has focused and implemented on designing a system that classifies the severity of mass casualty situations across both normal and disaster levels. The system's algorithm has implemented requirements such as accuracy as well as user convenience. The developed e-Triage System has applied various severity classification algorithms implemented from IoT concepts. In order to overcome flaws of currently used severity classification systems, the e-Triage System used electronic elements including the NFC module. By using the mobile application's severity classification algorithm the system demonstrated quick and accurate assessment of patient. Four different LED lamps visualized the severity classification results and RTS scores were portrayed through FND(Flexible Numeric Display) after a two wave classification.

A Study on the Medical Services Satisfaction of Cancer Patients in Busan Area (부산지역 암환자의 의료서비스 만족도에 관한 연구)

  • Hospital out-shopping by local patients has become more popular. In particular, many cancer patients have visited hospitals in the capital area to get better medical services regardless of how far the hospitals are from their home. However this kind of hospital out-shopping may discontinue medical treatment and cause a waste of medical resources including manpower. In addition, it creates additional economic and social expenses such as caretakers' transportation and lodging expenses. It is necessary to study patients who would take the medical treatment in Busan go to Seoul for better medical services are more satisfied based on objective data. For this, a questionnaire survey has been performed against 223 cancer patients who had a surgery in Busan and 187 patents who had an operation in Seoul. According to the survey, the patients who had an operation in the capital area(Group A) were more satisfied than the patients who had a surgery in Busan(Group B) in terms of convenience facilities, doctors' competence. However the Group A was very dissatisfied in terms of accessibility and satisfaction on overall medical services.

Effectiveness of MDCT for the Followup of CABG Patients with LIMA to LAD and Saphenous Veins to Others (좌내흉동맥과 복재정맥편을 사용한 관상동맥우회로술 환자에서의 추적조사에서 MDCT의 유용성)

  • There are several options for choosing a graft in CABG, we routinely chose LIMA for LAD and great saphenous vein for other target vessels. To evaluate the posoperative graft patency, we have studied the results using a 16 slices multi-detector computed tomography. Material and Method: From 1995 to 2003, 80 CABG patients who did not complain any event of MACE have been examined by 16-MDCT, mostly in an out patient clinic. Result: There were 61 men and 19 women. MDCT was used as early as 7 days to 9 years post-operatively with a median follow-up period of 6.5 years, and mean follow-up peiod of $31.5\pm25.4$ months. Mean age was $58.4\pm12.6$ years old in men and $61.5\pm17.2$ years old in women. 72180 patients received LIMA to LAD, and all other patients received vein grafts for bypass. The target vessel of vein grafts were 8 in LAD, 47 in RCA, 60 in diagonals, and 61 in obtuse marginals. Among them 42 sequential anastomoses were performed. The mean graft number was $3.1\pm1.8$ grafts. 5 year graft patency rate of each grafts was as followings; $93.1\%$ in LIMA to LAD, $94.9\%$ in vein to diagonals, $92.1\%$ in vein to obtuse marginals, and $79.2\%$ in vein to RCA. Sequential grafting showed better graft patency than the isolated grafting $(95.2\%\;vs\;78.7\~95.0\%)$. Conclusion: In this study, CABG with LIMA and saphenous veins showed satisfactory longterm results. 16-MDCT provided good images for follow-up study after CABG. Additionally, as radiologic tools (64-MDCT, MRI) improve more in the future, they can be used for diagnosing preoperative anatomical coronary disease as well as cardiac functions.

  • 'Personal Healthcare Records' (PHR) is a service for providing individual clinical data to patients. PHR service should be useful for the patient and healthcare service providers. This study has aimed at not only providing patients' clinical data but also developing optimized healthcare service for every patients. The research has been conducted as 3 phases: formal case analysis, caregivers interviews and patients interviews. The patients interviews were limited to cancer patients. As results, 3 key functions have been developed. First, it offers patient's clinical pathway as a personalized medical treatment scheduler. Second, it supplies Question & Answer board on online. Last, it supports patients to input their healthcare record. This Myongji PHR service has 3 months of pilot test on web and mobile application(android version). For further commercialized PHR service, the standardization for clinical pathway registration and user convenience need to be considered.

  • An ubiquitous healthcare system for the home care of elderly persons was designed and implemented using wireless sensor network technology. The wireless technology for home-care purpose gives new possibilities for monitoring of vital parameter with wearable biomedical sensors, and will give the patient the freedom to be mobile and still be under continuously monitoring and thereby to better quality of patient care. Emphasis is placed on recent advances in wireless ECG system for cardiac event monitoring with particular attention to arrhythmia detection in patient. This paper presents a diagnostic system for cardiac arrhythmias from ECG data, using wireless sensor technology. The system also provides an application for recording activities, events and potentially important medical symptoms. The hardware allows data to be transmitted wirelessly from on-body sensor to the base station and then to PC/PDA. Data is also transmitted to a back-end server for analysis using wireless internet connection. Experiments were conducted using the system for activity monitoring, exercise monitoring and medical screening tests and present preliminary data and results.

Development of Supplemental Equipment to Reduce Movement During Fusion Image Acquisition (융합영상(Fusion image)에서 움직임을 줄이기 위한 보정기구의 개발)

  • Purpose: Patients' movement during long image acquisition time for the fusion image of PET-CT (Positron Emission Tomography-Computed Tomography) results in unconformity, and greatly affects the quality of the image and diagnosis. The arm support fixtures provided by medical device companies are not manufactured considering the convenience and safety of the patients; the arm and head movements (horizontal and vertical) during PET/CT scan cause defects in the brain fundus images and often require retaking. Therefore, this study aims to develop patient-compensation device that would minimize the head and arm movements during PET/CT scan, providing comfort and safety, and to reduce retaking. Materials and Methods: From June to July 2012, 20 patients who had no movement-related problems and another 20 patients who had difficulties in raising arms due to shoulder pain were recruited among the ones who visited nuclear medicine department for PET Torso scan. By using Patient Holding System (PHS), different range of motion (ROM) in the arm ($25^{\circ}$, $27^{\circ}$, $29^{\circ}$, $31^{\circ}$, $33^{\circ}$, $35^{\circ}$) was applied to find the most comfortable angle and posture. The manufacturing company was investigated for the permeability of the support material, and the comfort level of applying bands (velcro type) to fix the patient's head and arms was evaluated. To find out the retake frequency due to movements, the amount of retake cases pre/post patient-compensation were analyzed using the PET Torso scan data collected between January to December 2012. Results: Among the patients without movement disorder, 18 answered that PHS and $29^{\circ}$ arm ROM were the most comfortable, and 2 answered $27^{\circ}$ and $31^{\circ}$, respectively. Among the patients with shoulder pain, 15 picked $31^{\circ}$ as the most comfortable angle, 2 picked $33^{\circ}$, and 3 picked $35^{\circ}$. For this study, the handle was manufactured to be adjustable for vertical movements. The material permeability of the patient-compensation device has been verified, and PHS and the compensation device were band-fixed (velcro type) to prevent device movements. A furrow was cut for head fixation to minimize the head and neck movements, fixing bands were attached for the head, wrist, forearm, and upper arm to limit movements. The retake frequency of PET Torso scan due to patient movements was 11.06% (191 cases/1,808 patients) before using the movement control device, and 2.65% (48 cases/1,732 patients) after using the device; 8.41% of the frequency was reduced. Conclusion: Recent change and innovation in the medical environment are making expensive medical image scans, and providing differentiated services for the customers is essential. To secure patient comfort and safety during PET/CT scans, ergonomic patient-compensation devices need to be provided. Therefore, this study manufactured a patientcompensation device with vertically adjustable ergonomic ROM according to the patient's body shape and condition during PET Torso scan. The defects in the basal ganglia images due to arm movements were reduced, and retaking was decreased.

Effectiveness of Patient and Family-Centered Care interventions: A Systematic Review (환자-가족중심 의료서비스의 융합적 성과: 체계적 문헌고찰)

  • This review aimed to analyze characteristics and performances of patient and family-centered care interventions and evaluate the convergence effectiveness. Randomized controlled trials were searched, selected, data extracted and quality-assessed using the Risk of Bias in 15 databases. Characteristics suggested from 21 studies were provision of information, education, communication and family and friend participation. There were 89 measurement variables of performances. Patient outcome was measured by mortality, length of hospitalization, etc., indicated as significantly improved in 18 studies. This review has provided evidence that patient and family-centered care improved experience and performance of diverse patients, families and health-care providers. There is need to convergence adopt patient and family-centered care and conduct evidence-based studies for improvement of quality of healthcare and patient safety in the future.

A Study on the Nutrition Contents and Blood Glucose Response Effect of Diabetic-Oriented Convenience Food prepared Medicinal Plants and Chicken (생약재와 닭고기를 이용하여 개발된 편의 당뇨식사의 영양성분 및 혈당반응)

  • This study was carried out to develop a diabetic-oriented convenience flood using 7 medicinal plants (Schisandra chinensis, Coix lachryma-jobi, Dioscorea batatas, Ophipogon japonicus, Lyicium chinense, Houttuynia cordata, Polygonatum sibiricum) and chicken. Portion size was 310g, total calorie was 551.6 kcal and carbohydrate, lipid and protein were consisted of 53.0%, 20.9% and 26.1%, respectively. Calcium, zinc and iron content were 268.9mg, 5.4mg and 6.1mg, respectively. Crude fiber content was 22.9g. In sensory evaluation, the scores of taste, color, texture and overall acceptability were higher than normal diabetic meal. Hypoglycemic effect of the device meal for diabetic persons was excellent compared to that of normal diabetic meal. The above results indicate that the 7 medicinal plants can be used as functional ingredients fur diabetic-oriented convenience flood industry. Also, device meal can be used as ready-prepared food for weight control.

  • In recent years, the number of foot disease patients is increasing due to bad walking habits and wearing shoes. With the development of shoes and living environment, no one can be relieved from foot disease. A study on the effect of shoe design characteristics on satisfaction and purchase intention when a patient with foot disease purchases orthopedic shoes. The shoe design characteristics consisted of functionality, convenience, and aesthetics. This study is conducted from various perspectives, such as frequency of participation in social activities, purpose of wearing, and status and characteristics of each foot disease. It is expected that the research results will lead to the development of orthopedic shoe designs that can be usefully worn by patients with orthopedic mild foot disease in their daily life, and to lead to practical use and satisfaction.

Comparison of Multidetector Computed Tomography with Coronary Angiography for Evaluation of Coronary Artery Bypass Grafts (관상동맥조영술과 MDCT를 이용한 관상동맥 이식편의 비교평가)

  • Background: The new Multidetector Computed Tomography (MDCT) is useful in visualization of complex coronary artery anatomy. We investigated usefulness comparing of invasive coronary angiography with noninvasive MDCT in judgment of functional degree of coronary arteries grafts after coronary artery bypass graft operation. Material and Method: We analyzed the patency of 52 conduits from 15 patients whom consented to take both 32 Channel MDCT and coronary angiography from November 2003 to November 2004. Comparisons were performed for sensitivity, specificity, positive prediction value and negative prediction value between coronary angiography and 3 dimensional reconstruction image using MDCT. Result: The average graft used was 3.4 $\pm$ 0.8 per patient. Average heart rate during MDCT was 86/minute (Range, 60$\∼$110/minute) without administration of $\beta$-blocker. All patients could hold breath as much as necessary. The average graft patency obtained through corollary angiography was 96.2$\%$. In MDCT group, the sensitivity, the specificity, the positive predictive value and the negative predictive value for diagnosis was 100$\%$, 98.0$\%$, 100$\%$ and 66.6$\%$ respectively. Conclusion: The effectiveness of 32 Channel MDCT may be compared to coronary angiography in grasping about patency and bloodstream of graft conduits after coronary artery bypass graft. Also MDCT has the advantage of noninvasiveness and inexpensiveness compared to coronary angiography.
