Pain Disorder Associated with Psychological Factors - Case Report and Review - (심리적 요인과 관련된 통증장애 - 증례 및 고찰 -)

  • Recently many researches support the use of traditional psychiatric treatments in the management of chronic pain. Chronic pain is a significant public health problem and frustrating to everyone affected by it. Psychiatrists offer skills with treatments now recognized as effective in the management of chronic pain. In addition to the diagnosis and treatment of psychiatric co-morbidity, the application of psychological treatments to chronic pain, and the development of interdisciplinary efforts to provide comprehensive health care to the patient disabled with chronic pain, psychiatrists have particular skill in pharmacological treatment that have proven efficacy for a variety of chronic pain conditions. With their expertise in the use of psychoactive medication plus their interest in the personal and family dynamics of patients, psychiatrists have the capacity to be involved in the treatment of patients with chronic pain. So, the author reported three cases of patient with pain disorder associated with psychological factors, and reviewed to propose that psychiatrists in Korea should take an active role in the care of these patients.

A Theoretical Structural Model for Caring Persons with Early Psychosis: A systematic review and qualitative meta-analysis (조기 정신증 돌봄 모형 개발: 질적 메타분석을 통한 이론적 접근)

  • This study was aimed to construct a theoretical model for managing persons with early psychosis. We systematically searched SCOPUS, PubMed, RISS, Web of Knowledge, and KoreaMed for related factors to manage persons with early psychosis. Significant terms or phrases for a preliminary structural model were obtained from 19 studies since 2000 regarding the management of persons with early psychosis. Moreover, the content validity for the model was evaluated by 22 professionals. The study results revealed 51 factors for managing persons with early psychosis extracted by meta-analysis, and all factors were divided into 21 medium classification and 8 categories. A theoretical model was constructed with 8 categories, by using the middle range theory of Peplau's Interpersonal Relationships in Nursing. The results suggest that the theoretical model for the management of persons with early psychosis provides a sound theoretical basis for future study.

Secure Healthcare Data Management and Sharing Platform Based on Hyperledger Fabric (하이퍼레저 패브릭 기반의 안전한 헬스케어 데이터 관리 및 공유 플랫폼 개발 연구)

  • In this paper, we present a healthcare data integration management and sharing platform based on a permissioned blockchain-based system called the Hyperledger fabric. The Hyperledger fabric allows patients to easily access their data, share the data with agencies that need it, and also reward participants. The healthcare data is stored in the blockchain by a de-identification process. Privacy is protected by setting detailed access rights to the stored data. The proposed model provides higher security than other models using a public blockchain. This study confirms that patient data can be stored more securely, by comparing the data stored in the blockchain with that from existing information storage methods.

  • As the variety of scientific technology has grown repeatedly since the 19th century, innovative technology is developing high-level in healthcare field. The system to improve patient's satisfaction for silver generation introduced in domestic medical process result from promotion of convergence technology. But utilization of small and medium hospital is inevitable with maintain limited performance around existing large hospitals and high cost service system. Therefore phenomenon that weakness of patient's satisfaction and service accessibility for silver generation occurs. This study propose the design method that Android-based low-cost smart medical treatment information service system to improve accessibility to user of small and medium hospital for effective patient's satisfaction services management and medical services.

  • Currently, medical data is basically confidential and difficult to access because it is protected under the Medical Protection Act. For the practical education of the students who are in the process of education, the researcher or the faculty members create and upload simulated data (chart of the question bank) close to the actual data. In this paper, the maintenance and repair easier on the basis of node.js and Ajax, mysql, jquery to a web-based research and enables users to easily approach the problem of the chart and the easy to the difficult access to patient contact respectively.

Endovascular Revascularization for Aortoiliac Occlusive Disease (대동맥-장골동맥 폐쇄성 질환의 혈관 내 재개통술)

  • Peripheral arterial disease is an occlusive condition commonly involving the lower extremity vessels. When the aortoiliac region is affected by this disease, conventional management involves surgical bypass and endovascular treatment has been mainly recommended for patients with focal and simple lesions. It has been common strategy to perform endovascular treatment for selected patients with high surgical risk due to its minimally invasive nature. However, recent advances in the devices and techniques for endovascular treatment have resulted in its utilization for treating patients with various disease status and its clinical outcomes are comparable to those of conventional surgery. This review discusses the current diagnostic strategies for peripheral artery disease in the aortoiliac region, followed by the introduction of techniques and devices, and the role of endovascular treatment.

Comparison of Stress Perception and Depression between Gastric Cancer and Gastritis Patients (위암 환자들과 위염 환자들 간의 스트레스지각 및 우울의 비교)

  • A comparison was made between gastric cancer and gastritis patients regarding stress Perception and depression, using Global Assessment Recent Stress(GARS) scale and Beck Depression Inventory(BDI). 50% of gastric cancer patients and 38% of gastritis patients were found to be depressed on scores of BDI scale, respectively. There was no significant difference in scores of stress perception between both the groups. However, gastric cancer patients tended to be more depressed than gastritis patients, although the difference is statistically not significant. In the gastric cancer patients, severity of psychic distress showed significantly positive correlation with depression, whereas in the gastritis patients, severity of physical symptoms showed significantly positive correlation with depression. It suggested that depression of gastric cancer patients was more likely to be related to the extent of psychic distress than that of physical symptoms. In each of both the groups, female patients showed significantly higher stress perception than male patients, and age was found to have significantly negative correlation with stress perception. In conclusion, severity of pathology of the same organ was not related to extent of stress perception and of depression in which denial of gastric cancer patients might play a role. Thus, it is emphasized that psychosocial approach is more needed for gastric cancer patients than for gastritis patients.

Assessment and Treatment of Depression in the Medically III (신체질환 환자들에서 우울증의 평가 및 치료)

  • Depression in the medically ill is a common clinical problem that primary physicians and psychiatric consultants encounter. Treatment of such patients begins with a careful evaluation of the patient's medical and psychiatric conditions. The assessment of depression in the medical patients requires a multidimensional approach. Psychological instruments are also used as a method of assessment in these patients. First of all, what the therapists have to do is to find and remove organic causes. Psychosoical treatment includes dealing with the patient's resistance and despondency relevant to physical diseases. For biological treatment, it is important to select appropriate antidepressants. Therapists should be familiar with the side effects of the antidepressants as well as the patient's primary depressive symptoms, pharmacokinetics and pharmacodynamics of the available agents. In addition, special attention should be paid to the potential for drug-illness and drug-drug interactions. Tricyclic antidepressants can be still effectively used for patients with pain disorder, although a variety of new antidepressants such as selective serotonin reuptake inhibitors (SSRI), bupropion and venlafaxine could have more benefits in depression of the medically ill. However, electroconvulsive therapy can be recommended for refractory cases of depression in patients with medical illness.

A Secure Personal Health Record System for Handling of Emergency Situations (응급 상황 처리를 위한 안전한 개인건강기록 시스템)

  • In recent years, Personal Health Record (PHR) has emerged as a patient-centric model of health information exchange. The Personal Health Record (PHR) owners enjoy the full right of accessing their records anywhere and anytime making storage and retrieval more efficient. Due to the sensitivity and confidential nature of the PHR, however, the PHR is maintained in a secure and private environment with the individual determining rights of access. In this paper, we propose a system which enables access to the user's PHR in the event of emergency. In emergency situation where the user is unconscious, the emergency staff can use the PHR information to request a emergency access to the PHR server based on the predefined rights of access for PHR. Under the proposed system, the PHR owner can specify a fine grain access control policy during emergency situations.

  • Recently hospitals have divided into many divisions, specialized the medical service, and shown organic cooperation, all to provide patients with various and high quality medical service. They have also showed improvement in information protection by introducing an information protection system to regulate the access to patients' medical and personal information. The purpose of this paper is to present a case study to implement of a limited hospital medical information system that can regulate the access to medical information. For this, a router-based virtual network applying an ACL(: Access Control List) to regulate access to information was made using a packet tracer.
