Comparison of Expectation-Perception between Patient and Nurse on Nursing Care Service in Comprehensive Nursing Care Wards (간호간병통합서비스 병동의 간호서비스에 대한 환자-간호사 인식도 비교)

  • The aim of this study was to compare perception between patient and nurse on comprehensive nursing care service using SERVQUAL model. To accomplish this goal, the 5 gaps of expectation-perception on comprehensive nursing care service was analyzed. The subjects were 192 nurses and 321 patients who admitted in comprehensive nursing care service wards. As results, patient's satisfaction was higher than patient's expectation and also nurse's performance. Therefore, we found that comprehensive nursing care service would contribute to perform holistic nursing care services in the future. To develop comprehensive nursing care service system sustainably, it needs continuous measurement of patient's expectation and satisfaction using conceptual model of this study.

  • 환자도 소비자이다. 또한 의료행위도 서비스 중의 하나이다. 하지만 다른 서비스업에 비하면 소비자인 환자가 중심이 되지 못하고 있는 실정이다. 물론 요즈음 많은 병원들이 더 나은 의료서비스를 제공하고자 노력하고 있지만, 아직 환자의 목소리가 반영됨을 실감하지 못한다. 앞으로 보다 나은 양질의 의료서비스가 정착되길 기대하며 불합리한 의료서비스를 개선하고자 환자의 의견을 들어보기 위해 <고발코너>를 마련했다.

  • Due to the development of information and communication technology as health care service is popular variety utilizing bioinformatics patient information services are being provided to the patient. In particular, the healthcare utilizing bioinformatics information, and change in a variety of healthcare trends. However, healthcare services using bioinformatics information of the patient and the complexity of the disease, new diseases (SARS, AIDS, etc .) due to the emergence of increasing health care costs and health promotion services provided to patients may not be smooth. In this paper, we propose a model for low-cost health services and medical care of patients bioinformatics fast access to information. The proposed model can be so big a bioinformatics data formation by the patient's patient information anytime / anywhere providing medical services in the home or the nearest hospital for their own disease management. In particular, the proposed model of health care services is characterized improve work efficiency, reducing the burden on hospitals by passing a medical illness to easily analyze patient information.

  • 기존의 u-HealthCare 서비스는 환자에 대해서 일정한 공간에서의 센서의 on/off 타입에 대한 모니터링/환자관리의 서비스를 제공하였다. 이러한 환경하에서의 주된 서비스는 현재 환자의 상태에 대한 수동적인 형태이다. 이러한 문제점들을 해결하기 위해 센서 데이터에 대한 연속센서 데이터마이닝 기법을 이용한다. USN의 응용서비스인 u-HealthCare 서비스는 센서데이터로부터 생체정보 및 위치정보를 이용하여 환자/보호자/관련 의료진에게 필요한 정보를 제공한다. 이것은 환자에 대한 관리/모니터링뿐만 아니라 환자의 상태에 따른 센싱(sensing)된 데이터를 이용한 패턴(pattern), 예측-(prediction), 이상치(outlier)를 분석함으로써 보다 나은 서비스를 제공할 수 있다. 본 논문에서는 센서 데이터에 대해 새로운 연속 센서데이터 마이닝 기법을 적용하여 질의를 통해 지식을 추출하고 보다 지능화된 서비스를 제공할 수 있는 응용서비스 기법을 제안한다.

Effectiveness of Patient and Family-Centered Care interventions: A Systematic Review (환자-가족중심 의료서비스의 융합적 성과: 체계적 문헌고찰)

  • This review aimed to analyze characteristics and performances of patient and family-centered care interventions and evaluate the convergence effectiveness. Randomized controlled trials were searched, selected, data extracted and quality-assessed using the Risk of Bias in 15 databases. Characteristics suggested from 21 studies were provision of information, education, communication and family and friend participation. There were 89 measurement variables of performances. Patient outcome was measured by mortality, length of hospitalization, etc., indicated as significantly improved in 18 studies. This review has provided evidence that patient and family-centered care improved experience and performance of diverse patients, families and health-care providers. There is need to convergence adopt patient and family-centered care and conduct evidence-based studies for improvement of quality of healthcare and patient safety in the future.

  • 본 논문은 유비쿼터스 환경에서 효율적으로 환자 관리 서비스를 제공할 수 있는 시스템을 제안하고자 한다. 환자 관리를 위한 기본적인 환자 정보는 RFID 태그를 통하여 관리되며 RFID 리더 장비는 PDA 및 다양한 모바일 장치로 설계된다. RFID를 기반으로 한 시스템은 기본적으로 환자의 위치 관리 서비스를 제공하며 환자의 센싱 장비를 통하여 환자에 대한 정보를 수시로 체크하고 관리할 수 있다. 특히, 물리적 요소(환자)와 다양한 서비스를 지원할 수 있는 애플리케이션을 위한 프레임 워크를 구성하고, 모바일 Agent를 개발하여 모바일 단말기와 컴퓨팅 시스템에서의 환자 관리 시스템을 위한 프로토타입을 구현한다.

Convergence outpatient medical service patient experience research using data mining (데이터마이닝 기법을 이용한 융복합 외래 의료서비스 환자경험조사 연구)

  • The purpose of this study is to find out specific measures that can help the management strategy of patient-centered medical institutions by conducting research on patient experience surveys of convergence outpatient medical services using data mining techniques according to changes in patient-centered medical culture. Using the raw data of the 2018 Medical Service Experience Survey, 8,843 people over the age of 15 who had patient experience in outpatient medical services were analyzed. Decision tree analysis was performed. The determinants of satisfaction with outpatient medical services patient experience were the doctor's area and patient's rights protection area, and the determinants of intention to recommend outpatient medical services were the doctor's area and facilities comfort. Women evaluated the experience positively in overall satisfaction as compared to men, and those over the age of 60 positively evaluated the overall satisfaction and intention to recommend. It is significant that the outpatient experience decision-making model is presented, and that the doctor's area, patient's rights protection area, and facility comfort are important factors. Long-term research on the 'Medical Service Experience Survey' is needed, and research on the inpatient medical service experience is needed.

Development of a convergence inpatient medical service patient experience management model using data mining (데이터마이닝을 이용한 융복합 입원 의료서비스 환자경험 관리모형 개발)

  • The purpose of this study is to develop a convergence inpatient medical service patient experience management model(IMSPEMM) that can help in the management strategy of a medical institution to create a patient-centered medical culture. Using the original data from the 2018 Medical Service Experience Survey, 593 people with medical services inpatient(MSI) over the age of 15 were analyzed. By using the decision tree model, we developed a prediction model for overall satisfaction(OS) with the inpatient medical service experience(IMSE) and the intention to recommend patient experience(RI), and were classified into 4 and 7 types. The accuracy of the model was 68.9% and 78.3%. The OS level of IMSE was the nurse area and the hospital room noise management area, and the RI decision factor was the nurse area. It is significant that the IMSPEMM for MSI was presented and confirmed that the nurse area and the noise management area of the hospital room are important factors for the inpatient experience. It is considered that further research is needed to generalize the IMSPEMM.

The Effect of Doctor's Payment Method on Patient's Medical Care Use: Revisit of the Patient's Asymmetric Information Problem (환자의 의료이용에 대한 의사의 지불방식의 효과: 재방문 환자의 비대칭적 정보의 문제)

  • Although the patient's problem with access to health information has been improved due to rapidly developing information technologies, such as the internet, some patients still do not have enough ability to understand, interpret, and analyze the health information. Given this view on the patient's asymmetric information problem, if a doctor provides sufficient effort to help patients understand and interpret medical information, the efficiency of patient's medical care use could be improved. This paper shows firstly that the patient's inefficient use of medical care originates from his information problems, such as the misperception of the effectiveness of medical care and secondly suggests that if the doctor makes sufficient effort to correct patient's information problems, the inefficiency can be ameliorated. This paper also suggests the manipulation of a doctor's payment method can lead a doctor to provide optimal level of efforts which can in turn lead patients to use the optimal level of medical care. With an optimal level of effort, a doctor can more easily achieve a patient's compliance with the newly recommended amount of medical care.
