  • As patient satisfaction is a crucial component of improved health care outcomes, there is a need to evaluate the relationship between physicians' attitude and patients' satisfaction. The objective of this study is to examine the relationship between patients' satisfaction and physicians' friendly and caring attitudes by using recent physician ratings by patients. Data from a cross-sectional survey using a convenience sampling was utilized to examine the relationship between physicians' attitudes and patients' satisfaction. The independent variable was the physician's attitude, and the dependent variables were patients' satisfaction with the physician and the office setting. A total of 273,994 patients it the US were included. The patients' average (standard deviation, SD) satisfaction with the physician was 78.08 (0.14), and the average (SD) satisfaction with the office setting was 78.62 (0.12) out of 100. Physicians' attitude was a significant predicting factor impacting the patients' satisfaction with the physician and the office setting (p < 0.001). To facilitate patients' satisfaction with healthcare, a continuous effort to develop physicians' ability to communicate in an empathetic manner should be undertaken so that patients perceive their physicians as empathetic.

Results : 1.

  • The purpose of this study was to define and clarify the concept of Shared decision-making (SDM) in patients with chronic disease. Walker and Avant's concept analysis process was used to analyze interdisciplinary convergence in SDM. SDM in patients with chronic disease can be defined by the following attributes: acknowledgment patients as 'self-care experts', the rights of self-determination, reversible negotiation, and patient-centered care. The antecedents of SMD consisted of situations where there is a need to make a decision from several treatment options of similar efficacy, decisional conflict, patient, family, and health provider's willingness to participate in the decision-making process, enough time and opportunity for SDM. The consequences occurring as a result of SMD were decrease decisional conflict, improvement health outcome, satisfaction, quality of life, enhancement self-management and self-efficacy with long-term, and living acceptably with the illness. Based on these results, a scale measuring SDM in patients with chronic disease is needed.

  • This study aimed to identify factors influencing the preferences for end-of-life (EOL) care among undergraduate nursing students. In this cross-sectional study, data were collected from December 2017 to February 2018. This study included 217 undergraduate nursing students. Factors influencing the preference for 'autonomous physiological decision-making' were the following: education level(by grade), having biomedical education, attitude towards death, and attitude towards life-sustaining treatments (LSTs). Preference for 'decision-making by healthcare professionals' was related to having a religion. Factors influencing the preference for 'spirituality' were education level, having a religion, and academic major satisfaction. Preference for 'pain control' was associated with education level, experience with dying patients, bad self-rated health, attitude towards death, and attitude towards LSTs. The study findings suggest that education regarding LSTs, EOL care, and EOL decision-making in nursing curricula is essential.

  • Purpose: This study was performed to explore the current state of end-of-life (EoL) care education provided to new interns at two university hospitals. Methods: A questionnaire was given to incoming interns (N=64). The levels of acquired knowledge and experience of clinical observation were measured. Seven areas for self-assessment questions were identified and used to analyze the interns' attitudes towards EoL-related education and practice. Results: On average, participants learned five elements (nine in total) from EoL-related classes and two (seven in total) from clinical observation. The most frequently educated element was how to deliver bad news (96.9%) in the classroom setting and how to control physical symptoms (56.5%) in clinical observation. Less than 20% received training on EoL care communication, including discussion of advanced directives. Compared with participants who had no EoL training, those who had EoL training showed positive attitudes in all seven categories regarding overall satisfaction, interest and preparedness in relation to EoL-care classes and practice. Conclusion: Although interns are responsible for caring of dying patients, their EoL training in classroom and clinical settings was very insufficient. Further research should be conducted to establish an education system that provides sufficient knowledge and training on EoL care.
