Formation of Spheroids of Adult Rat Primary Hepatocytes in Polyurethane Foam (폴리우레탄 폼을 이용한 쥐 일차 간세포의 구상체 배양)

  • This paper is fundamental study to develope the extracorporeal liver support system for patient with fulminant hepatic failure(FHF) or being expected for orthotopic liver transplantation. The polyurethane foam, which is composed of the density of 33kg/m3, the average pore diameter of 500${\mu}{\textrm}{m}$, the closed window of 60-70%, was manufactured with the prepolymer of 15% NCO-, Hepatocytes were inoculated to form spheroids in polyurethane foam. The time of spheroid formation in BSA(Bovine Serum Albumin) coated polyurethane foam was shorter than that in raw polyurethane foam. To verify the function of hepatocyte spheroids, we measured ammonia removal rate, urea and albumin secretion rate. Polyurethane foam was suitable for culture of hepatocyte spheroids. And culture of hepatocyte spheroids in polyurethane foam has high possibility in using as an extracorporeal liver support system.

  • The purpose of his study was to analyze the environmental and the medical factors of prematurely discharged patients in a rehabilitation hospital. The subjects were 107 inpatients. The data were collected using self-report questionnaire and analyzed using the SPSS Win 19.0 program. In result, Firstly, general selection of hospitals was the 'Recommendation' 35.5%, Environmental dissatisfaction factors are Hospital facilities 37.4%. Hospital choice is the highest 'Acquaintances' 23.4%. Second, the lowest group about professionalism, kindness, and description of explanation on satisfaction in survey is group of care workers for the sick. The highest group is physical and occupational therapist. Third, the satisfaction regarding medical procedures of administrative work, waiting time and medical expenses is the highest normal. Hospital facilities in one of the highest factors in environmental dissatisfaction is can be considered as an element of patient departure because of not much number of nervous special hospitals. Medical dissatisfaction factors is the lowest satisfaction of care worker even if those spend much time. this is the focus leaving hospital. Therefore, factors care workers on service satisfaction and dissatisfaction in analysis and problem solving is considered that the need to find ways to improve the quality of service care workers.

  • 현재 정보통신의 비약적인 발전은 의료서비스 전달체계에서도 큰 변화와 진전을 야기하고 있으며 세계적으로 점차 확대되어가고 있는 추세이다. 이는 의료정보화라는 이름으로 환자에게는 질병의 진단, 치료에 있어서 보다 신속, 세밀하고 정확히 판단할 수 있게 하여 수준 높은 보건의료서비스를 제공하는 한편 의료기관 및 관련기관은 업무의 효율성을 높여가고 있다. 그 중 원격의료는 의료기관의 방문 없이 대기시간의 단축, 일률적인 고도의 의료수준을 기대할 수 있는 등의 장점이 있는 제도이다. 그러나 현행 우리 의료법에서는 이를 규정하고는 있으나 그 내부적 관계에 따르는 세부적인 법률관계의 부재와 현장에서 이루어지는 의료행위가 아닌 정보통신망을 이용한 비대면접촉에 의한 특수한 형태라는 측면에서 제도적, 시설적, 환경적 제약이 있음을 부인할 수 없다. 따라서 본 논문에서는 원격의료의 법적 문제점 및 개선점을 고찰해보고 이를 통한 원격의료를 활성화할 수 있는 활로를 모색하고자 한다.

Factors influencing consultation time and waiting time of ambulatory patients in a tertiary teaching hospital (일개 종합병원 외래환자의 진료시간 및 진료대기시간 영향요인 분석)

  • Objectives : The purpose of this study was to identify the characteristics influencing consultation and waiting time in ambulatory patients. Methods : This study was conducted in a tertiary teaching hospital. Subjects were a total of 10,383 ambulatory patients. Consultation time was measured by time spent for meeting with his/her physician per patient. Waiting time was defined as the time difference between each patient's reserved time and time to meet with his/her physician for ambulatory care. Multiple regression analyses were performed to determine the factors influencing consultation and waiting time. Results : Consultation time was different according to patient' age, previous experience of clinic visit, recent admission history, medical department, specialist care, type of reservation, and day of the week. Significant factors influencing waiting time were patient' age, residential area, previous experience of clinic visit, recent admission history, medical department, specialist care, time spent after ambulatory care begins, and day of the week. Conclusions : The medical department was the strongest factor affecting both consultation time and waiting time. The ambulatory reservation management systems should take into account patient characteristics as well as care-related features.

Scheduling Algorithms and Queueing Response Time Analysis of the UNIX Operating System (UNIX 운영체제에서의 스케줄링 법칙과 큐잉응답 시간 분석)

  • This paper describes scheduling algorithms of the UNIX operating system and shows an analytical approach to approximate the average conditional response time for a process in the UNIX operating system. The average conditional response time is the average time between the submittal of a process requiring a certain amount of the CPU time and the completion of the process. The process scheduling algorithms in thr UNIX system are based on the priority service disciplines. That is, the behavior of a process is governed by the UNIX process schuduling algorithms that (ⅰ) the time-shared computer usage is obtained by allotting each request a quantum until it completes its required CPU time, (ⅱ) the nonpreemptive switching in system mode and the preemptive switching in user mode are applied to determine the quantum, (ⅲ) the first-come-first-serve discipline is applied within the same priority level, and (ⅳ) after completing an allotted quantum the process is placed at the end of either the runnable queue corresponding to its priority or the disk queue where it sleeps. These process scheduling algorithms create the round-robin effect in user mode. Using the round-robin effect and the preemptive switching, we approximate a process delay in user mode. Using the nonpreemptive switching, we approximate a process delay in system mode. We also consider a process delay due to the disk input and output operations. The average conditional response time is then obtained by approximating the total process delay. The results show an excellent response time for the processes requiring system time at the expense of the processes requiring user time.

The Pathology of Infection in the Department of Radiology (영상의학과 검사실의 감염 실태)

  • This study was performed to understand the bacteriologic contamination level of radiological equipments which have frequent contacts with patients in the Department of Radiology of an university hospital in Busan area. Before sterilizing in-patient of the radiology rooms, MRSA, VRE, acinetobacter baumannii, candida albicans, and enterococcus sp. were detected. After sterilization, all the bacteria were not found. As examine times become longer, more bacteria were detected and after 7 hours, bacillus sp.(GPR), CNS, acinetobacter baumannii, and Enterococcus sp. were detected. After examining infected patients, bacillus sp.(GPR), VRE, enterococcus sp. CNS, and micrococcus sp. were detected and on the hands of radiological technologists, CNS, enterococcus sp. escherichia coli, and enterobacter sp. were detected. Similar species of bacteria were detected from each radiology room, but pseudomonas aeruginosa was detected on the handles of portable radiological equipments and the chair in the waiting room. Therefore, it is the most important to regularly sterilize radiological equipments and devices which have frequent contacts with patients and to sterilize them right after the use of infected patients in order to prevent the spread of infection. Also, thorough hand washing, education on infection and management for the characteristics of Department of Radiology should be performed for the systematic prevention of infection.

Smart-Telemedicine System Design and Business Model Analysis for Longitudinal Healthcare (예방의학을 위한 Smart-Telemedicine 시스템과 비즈니스 모델의 설계와 분석)

  • Recently due to the enhancement of education and lifestyle, the trend of healthcare services are changed to a more active and differentiated service in which a continuous self health care is possible. The Smart-Telemedicine system offers medical services by merging Blue-tooth and telecommunication modules to former blood pressure, blood sugar, heartbeat and temperature measuring devices. Moreover, it could analyze one's health pattern which would be helpful for the patient to prevent potential future illness. In addition, the easier accesses to various remote controllable medical check-up programs are offered to public as a number of available smart phone are rapidly escalating. The Smart-Telemedicine system provides the most ideal interactive medical service via accessible smart phones and mobile medical check-up devices at anywhere and anytime. It is very beneficial since it can save patients' time and money because people can reach to the service right at their home and be allowed to take charge of their health care process via longitudinal health data. Therefore, not only social costs that occur in elderly community would be saved, but also business in various forms of medical service field transactions could be possible. This paper will suggest the Smart-Telemedicine System for preventive medicine, its design and analysis of business models and the evaluation of those model.

Ottonello Projection Techniques using the Metronome (메트로놈을 이용한 Ottonello 촬영 기법)

  • The ottonello method is used to see a front view image of cervical 1-7 number. The study found a systematic examination method to find a better image through an experiment using metronome. The results are as follows: In an evaluation of MTF, when used 0.5 standard of MTF, oral explanation resulted 1.2 LP/mm, Moderato-100 1.4 LP/mm, Allegro-138 1.6 LP/mm, Presto-184 2.4 LP/mm. Among them presto-184 showed the highest sharpness with 2.4 LP/mm. In an image reading by a medicine doctor of radiology, when oral explanation was 17%, Moderato-100 15%, Allegro-138 18%, Presto-184 50%. Among themprest-184 was evaluated as the best image. The result of was a method to reduce an image repeat, patients radiation exposure, patients waiting time for examination, material and so on.

An Improvement of Hospital Reception System using Web Socket (웹소켓을 이용한 병원 접수시스템 개선에 관한 연구)

  • During hospital peak times of outpatients, an effective mechanism that informs the newly receipted patient information to corresponding medical departments is lacking in current hospital reception systems. Since every department repeatedly searches entire patient reception database in sequential manner to acquire its reception information, this is a significant performance degradation factor in hospital information system. To improve hospital reception system efficiency, we developed two web socket based systems, a primary key transmitting batch system and a reception information transmitting real time system. The former reduced database access time compared to sequential search system as well as kept search time low regardless of receipted patient number. The latter effectively reduced waiting list updating time in request/response patient reception system by eliminating database access.

Improving Textile Management Process in the Hospital Using Service Blueprint Analysis (서비스 청사진 기법을 이용한 병원 내 직물 관리 프로세스 개선방안)

  • Hospital textile is a necessary which the patient used everyday, that required constant management between supply and consumption. This study applied the service blue print technique to analyze how to provide and supply hospital textile from the stock to the patients in a tertiary hospital, Seoul, South Korea. There are actual or potential process problems identified such as nurses' increasing workload, patient dissatisfaction, and infection risk. After applying blueprint analysis, we suggest the modified processes to overcome theses identified problems using automatization to provide textile. Expected outcomes may include decreases in patient's waiting time, nurse's textile workload, and lower infection risk as well as increasing process efficiency via systematic supply-demand management.
