No-Show Related Factors for Outpatients at a Hospital (병원 외래환자의 예약부도 요인)

  • In order to maximize the efficiency and the quality of care in a hospital, hospitals in general operate by appointments. Patient no-shows or missed appointments waste the time of staffs who prepare for appointments, deprive patients of needed care, and increase burden on clinic capacity. The purpose of this study is to examine the importance and significance of factors that affect patient no-shows. The actual data of 50,000 outpatient appointments is compiled and analyzed from a VAMC in the United States with aims to identify the factors that contribute to outpatient no-shows and to draw meaningful implications. Random forest along with logistic regression analysis identify the factors affecting no-shows; appointment leadtime, show-up rate, travel distance, no-show rate in previous period, patient age, severity, complexity of disease, chronic pain, depression, drug dependence. The appointment leadtime, show-up rate, travel distance and previous no-show rate can be improved by enhancing SMS pre-notification and by improving afterwards telephone counseling. For other factors, it needs to study on the service differentiation with the characteristics of each patient group.

A System Design for Real-Time Monitoring of Patient Waiting Time based on Open-Source Platform (오픈소스 플랫폼 기반의 실시간 환자 대기시간 모니터링 시스템 설계)

  • This paper discusses system for real-time monitoring of patient waiting time in hospitals based on open-source platform. It is necessary to make use of open-source projects to develop a high-performance stream processing system, which analyzes and processes stream data in real time, with less cost. The Hadoop ecosystem is a well-known big data processing platform consisting of numerous open-source subprojects. This paper first defines several requirements for the monitoring system, and selects a few projects from the Hadoop ecosystem that are suited to meet the requirements. Then, the paper proposes system architecture and a detailed module design using Apache Spark, Apache Kafka, and so on. The proposed system can reduce development costs by using open-source projects and by acquiring data from legacy hospital information system. High-performance and fault-tolerance of the system can also be achieved through distributed processing.

  • 의약분업의 시작으로 환자의 약국에서의 대기 시간을 줄이기 위한 전자처방전이 활성화될 것이 예상되지만, 의사가 전자처방전 작성에 할애하는 노동력을 줄이기 위한 방법과 전자처방전의 신뢰성과 전송상의 환자 동의 여부를 증명하는 방식이 현재까지 나타나고 있지 않다. 본 연구에서는 이 문제를 지문인증, 음성인식과 압축기술을 이용하여 해결하는 방식을 제안하며, 실제적 구현을 통하여 그 타당성을 제시하였다. 실제 실험실 수준에서 시험한 결과 일반저긴 전자처방전에 비하여 처방전 작성시간이 20% 정도 절감되었으며, 복잡한 복약 지도의 경우에는 더욱 효과적일 것이다. 본 시험에서는 온라인 지문인증에 소요되는 시간은 제외하였다.

The Causes Analysis for the Number of Patients Waiting in General X-ray Room (Focus on Utilization of Quality Improvement (QI)) (일반촬영실의 환자대기건수에 대한 원인별 분석 - QI 활용을 중심으로 -)

  • A questionaire survey was conducted for patients who had been examined at the Department of Radiology to investigate the items that need to be improved. Brainstorming was also conducted by team members to suggest the tactics that can efficiently perform the QI activity by selecting the most frequently answered topics for the reduction of waiting time of x-ray examination. From September 2006 to November 2007, number of patients before and after conducting QI was compared for 3 months by each category differentiated by types of detailed causes. A patient case was set as one shooting for one patient. After conducting QI, the waiting cases before conducting QI were evaluated for the method of improvement for 3 month through the QI team discussion and conducted by following the improvement method for the next 1 month and the waiting cases were measured and the difference before and after the QI activity was compared in percentage. 1. When patient waiting cases were compared before and after conducting QI activity against the causes of repetition, it resulted in 3.9% of reduction effect. 2. When patient waiting cases were compared before and after conducting QI activity against the causes for the lack of guiding, it resulted in 1.1% of reduction effect. 3. When patient waiting cases were compared before and after conducting QI activity against the causes of miss-inputting prescription, it resulted in 1.1% of reduction effect. 4. When patient waiting cases were compared before and after conducting QI activity against the causes for emergency patients, patients with acute pain and discomfort patients, it resulted in 12.0% of reduction effect. 5. When patient waiting cases were compared before and after conducting QI activity against the causes for shooting overlapping of outpatients and hospitalized patients, it resulted in 4.7% of reduction effect. There are many factors to reduce the patient waiting cases in radiography. The first step is for radiology department to find these factors through QI, to improve them, which is the reason why the QI team is organized to perform the QI activities.

Patient Management Through Simulation Modeling in the Medical Center (시뮬레이션 기법을 이용한 검진센터의 환자관리방안)

  • This study aims to develop the methods for effective patient flow in the medical center through simulation modeling. To achieve this, we developed three simulation scenarios based on max/min processing time and addition of X-ray by 15 patient tracks from real hospital data. The simulation software used in this study is Flexsim HC 2.7. According to the scenario 1 on 15 patient tracks' LOS by max processing time, there is a great difference between average length of stay(LOS) and max LOS. And average LOS increases greatly depending on the number of patients by the hours. There is no need to add extra X-ray because the addition of X-ray has not much influence in average LOS. It is possible to make good decisions on patient flow management and medical equipment purchasing through simulation modeling. The concrete simulation scenario as a tool for decision support will contribute to efficiency in hospital management.

Medical Service Satisfaction on the Patients Utilizing a Day-Case Surgical Center in Korea (국내 당일수술센터 이용환자의 의료서비스 만족도분석)

  • 본 연구는 국내 당일수술센터 이용환자의 의료서비스 만족도와 관련된 요인들을 규명하기 위해 시도하였다. 설문조사는 2009년 1월 13일부터 2월 13일까지 한 종합병원에서 독립적으로 운영하는 당일 수술센터를 이용하는 환자들로부터 도출했다. 연구결과 당일수술환자는 수술과 입원경험은 적었고 본인의 건강 관심도는 높은 것으로 나타났다. 수술대기시간과 수술시간은 의료서비스 질에 유의한 차이가 있었다(P<0.05). 다중 로지스틱 회귀분석에서 당일수술환자의 인식도에 영향을 주는 것은 연령과 교육수준으로 나타났다(P<0.05). 따라서 당일수술률을 증가하기 위해서 당일수술의 이점에 대한 정보를 제공하고 수술전 교육이 필요하다.

  • 최근 의료계에서는 환자관점에서 진료 및 행정체계를 재구축하는 시도들이 진행되고 있다. 그러나 이러한 시도들의 성공적인 시행을 위해서는 팀을 이루는 다양한 직종들의 이해조정과 투입된 자원에 대한 효과성 검토가 요구된다. 이에 본 연구에서는 시뮬레이션 기법을 적용하여 새로 만들어질 환자중심적 진료센터의 시설 및 업무체계에 대한 분석을 실시하였다. 다양한 시나리오를 적용하여 환자대기공간의 적정성을 분석하였고 진료프로세스별 대기시간 분석을 통해 병목구간을 찾아 해결방안을 제시하였다. 또한 시나리오별 개선안 적용을 통해 향후 진료스케줄 운영의 적절한 대안을 모색해보았다.
