A Literature Review for Fall-Prevention Nursing Program Development based on the Fall Information of a Rehabilitation Hospital (일 지역 재활병원의 낙상 정보에 근거한 낙상예방 간호중재 프로그램 개발을 위한 문헌분석 연구)

  • This study is to design an effective fall-prevention nursing program. Researchers investigated the current condition and risk factors for falls in the small-medium sized rehabilitation hospital with descriptive research. We conducted integrative literature review to reflect the current approach of fall-prevention nursing program. We found that the risk factors for falls are related to the characteristics of clinical setting of the hospital. We suggested the nursing intervention with applying fall risk assessment scale that is sensitive to the elderly population and the intervention that shows maximum effect. In conclusion, this study proposed the way of selecting an effective fall-prevention nursing program based on a clinical setting and prospective of its application. This will contribute to the improvement in nursing practice with the critical view in hospital fall.

The Effect of a Convergence Approach of Home Environment Modification and Task-oriented Intervention on Fall Efficacy and Activity Level of Person with Stroke in the Community (가정환경 수정과 과제기반 중재의 융복합 접근이 지역사회 뇌졸중 환자의 낙상 효능감 및 활동 수준에 미치는 영향)

  • The purpose of this study is to determine the effect of a convergence approach, which applies both home environment modification applied with assistive technology and task-oriented intervention using a fall safety checklist, on the fall efficacy and activity level of stroke patients. 41 persons with stroke(21 experimental groups and 20 control groups) were divided into experimental and control groups. The experimental group was applied environmental modification and the task-oriented intervention using the fall safety checklist for 4 weeks. The control group was provided only the environmental modification. Before and after the intervention, the Korean Activity Card Sort (KACS) and the Korean Falls Efficacy Scale-Korean version (FES-K) were used for evaluation. There was no significant difference in fall efficacy and activity level change between the experimental group and the control group. Both the experimental group and the control group significantly improved the fall efficacy and activity level after the intervention. By applying the results of this study to the clinical field, it will be possible to support stroke patients to live safely and actively participate in the community.

Circumstances, Risk Factors, and the Predictors of Falls among Patients in the Small and Medium-sized Hospitals (중소병원 입원환자의 낙상발생 시 현황, 낙상위험요인 및 낙상발생 예측요인)

  • Purpose: This study examined the circumstances, risk factors, and the predictors of fall incidents among patients in the small and medium-sized hospitals. Methods: Fifty patients with any fall experiences were matched by gender, age, and medical departments with 100 patients without fall incident at the same hospital. Data were collected from 5 small and medium-sized hospitals. Data were analyzed using descriptive statistics, a Chi-square test, a Fisher's exact test, and a logistic regression with the SPSS/WIN 21.0 program. Results: In the patients with falls, the largest number of falls occurred during the day shift, in the patients' rooms, and while they were walking. Further 74.0% of the patients had physical injuries, and 34.0% had to take further medical diagnostic tests. Significant differences were found between the patients with falls and the others on 14 variables (cardiovascular disease, anemia, sedative-hypnotics, vasodilators, narcotic analgesics, dizziness, general weakness, unstable gait, walking aids, anger, anxiety, depression, orientation, and fear of fall). Narcotic analgesic use, dizziness, walking aids, and cardiovascular disease were identified as the predictors of fall incidents. Conclusion: These findings are hoped to be used in developing a fall risk assessment tool and fall prevention nursing programs for small and medium-sized hospitals.

  • 본 연구는 와상환자에게 자주 발생하는 낙상, 욕창, 불면증을 영상기술을 통해 인식하여 섬망의 조기진단 모형을 구현한다. 실시간 모니터링을 통해 섬망 잠재환자를 선별하고 집중적인 관리와 치료로 이어질 수 있도록 간호인력을 보조하는 데 주된 목적을 두고 있다. 과활동형 섬망은 파생위험 중 하나인 낙상과, 저활동형 섬망은 원인 요소인 욕창과 묶어 자세인식을 통해 판정한다. 또한 주로 밤에 악화되는 섬망의 특성을 고려해 눈 깜빡임을 통한 불면증 검사를 추가로 반영하였다. 낙상과 욕창을 섬망과 묶어 융복합적인 위험예측 시스템을 구축함과 동시에, 기존의 섬망 사정도구들이 지니는 시공간적 제약을 개선함으로써 간호인력의 부담을 덜어줄 것으로 기대된다.

A Study on Environment, Care Process, and Patient-related Factors Associated with Patient Falls - A Retrospective Study of Inpatient Falls in the Unit of General Medicine in the United States (환자낙상에 영향을 주는 환경, 의료과정, 그리고 환자 관련 요소에 관한 연구 - 미국의 일반내과병동 환자낙상 데이터 분석을 통한 후향성 연구)

  • Purpose: The main objective of this paper is, to assess environment, care process, and patient-related factors associated with patient falls. The study also aims at identifying various factors that would affect inpatient falls and, therefore, helping both caregivers and designers contribute to better prevent inpatient falls in their own areas of expertise. Methods: A retrospective analysis of inpatient falls that occurred in the unit of General Medicine in the United States has been conducted and environment, care process, patient-related factors associated with those falls have been analyzed at the same time. Results: The study identified several factors associated with inpatient falls. They range from environmental factors to care process- and patient-related factors. Patient visibility and patient accessibility can matter to patient falls and where those falls occur, along with patient days per room, the percentage of patient days with high fall risk patients per room, the percentage of high fall risk patients per room. Implications: The findings of the study can provide design implications that can be incorporated into design process and design decisions to promote fall prevention in inpatient care units. Inpatient falls can be effectively reduced when caregivers and designers work together to understand the complex nature of inpatient falls and the importance of multidisplinary efforts among various experts in the areas of healthcare.

The Effect of Patient-related and Environment-related Characteristics on the Risk of Falling in Inpatient Care Unit - A Case-Control Study to identify Inpatient Fall Risk Factors (환자의 개별 특성 및 병동의 환경 특성이 환자낙상 위험도에 미치는 영향 - 환자낙상 위험인자 파악을 위한 사례-통제 연구)

  • Purpose: The study aims at identifying patient-related and environmental factors associated with an increased risk of falling and, therefore, both caregivers and designers can be aware of fall risk factors and can contribute to prevent inpatient falls in their own areas of expertise. Methods: A case-control study has been conducted, utilizing patient data and physical environmental data in the unit of General Medicine in the United States. The case-control study investigated data about patients who had suffered falls as well as patients with similar characteristics (e.g., age, gender, and diagnosis) who did not suffer falls. Results: The study identified both patient-related and physical environmental factors associated with inpatient falls. Morse fall risk score, patient visibility, and patient accessibilityB were identified as significant predictors to inpatient falls, when controlling for other significant variables. Implications: The findings of the study can provide implications to both caregivers and healthcare and hospital environment designers. Caregivers should give special attention to patients with high Morse Fall Risk Scores to prevent inpatient falls. Designers also need to examine and to fine-tune the unit layout of inpatient care units to maximize each patient room's patient visibility from the rest of the unit and patient accessibilityB from working areas of nurses.

Meta-analysis of the Effects of Untact Convergence Exercise Programs on Balance, Gait, and Falls Efficacy of Parkinson's Disease Patients (비대면 방식을 융합한 운동 프로그램이 파킨슨 환자의 균형, 보행, 낙상 효능감에 미치는 효과에 관한 메타분석)

  • In this study, a meta-analysis was conducted to find out the effects of convergent "untact" exercise programs on the balance, gait, and falls efficacy of Parkinson's patients. In this study, the PICO were Parkinson's disease, virtual reality program and an e-exercise program intervention, a comparative intervention with the contact interventions applied, and without any interventions applied, outcomes were balance, gait, and fall efficacy. The survey period was between January and February 2021, and five papers were selected and subjected to meta-analysis. Risk of bias, a tool commonly used for randomized control trial studies, was used. Furthermore, RevMan program was used to investigate effect size of untact exercise programs. The result of the meta-analysis showed that the effect size of balance of the untact exercise program group was 1.27 (SMD=1.27; 95% CI 0.72 to 1.83) (Z=4.51, p<0.001), the effect size of fall efficacy was 0.52 (SMD=0.52; 95% CI -0.000 to 1.03) (Z=1.96, p=0.05), and the effect size of gait was -0.40 (SMD=-0.40; 95% CI -1.00 to 0.10)(Z=1.32, p>0.05). A total of 5 literature analysis showed that untact exercise program is more effective in improving balance and falls efficacy than contact exercise program, but no difference in effectiveness was shown on gait.

Influence of Fall Risk, Fear of Falling, and Social Support on Fall Prevention Behaviors among Older Inpatients (입원한 노인환자의 낙상위험도, 낙상두려움 및 사회적 지지가 낙상예방행위에 미치는 영향)

  • Purpose: This study aimed to investigate the influence of fall risk, fear of falling, and social support on older inpatients' fall prevention behaviors. Methods: The participants were 105 older inpatients. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis using the SPSS program. Results: The mean fall risk, fear of falling, social support, and all prevention behaviors were 58.05±28.27, 2.47±0.67, 3.58±0.63, and 2.93±0.48, respectively. There were significant differences in fall prevention behaviors by sex, and fall related to education experience. Fall prevention behaviors were associated with fall risk, and fear of falling. The factors influencing the fall prevention behaviors of the older inpatients were sex, fall related to educational experience, fall risk, and fear of falling. It was found that 21% could explain fall prevention behaviors. Conclusion: The results indicate a need to develop a fall prevention education program, including intervention, to reduce the fear of falling among older inpatients.

The Effect of Whole-Body Vibration Exercise for Life Care on Balance and Walking Ability in the Stroke Patient with Risk of Fall (라이프케어 증진을 위한 전신진동운동이 낙상위험 뇌졸중 환자의 균형 및 보행능력에 미치는 영향)

  • The purpose of this study was to examine the effects of whole-body vibration exercise for life care on balance and walking ability in the fall risk stroke patients. 30 stroke patients were divided into two groups, Otago exercise group(n=15) and whole-body vibration exercise group(n=15). Walking ability were measured by 10m walking and Tinetti mobility test with stroke patients and balance ability were measured by BBS and TUG on comparative analysis of pre, post exercise. To compare differences before and after the program, a paired t-test was used and to compare differences between both groups, an independent samples t-test was used. As a result of the test, it was discovered that BBS and Tinetti mobility test of the both groups were increased statistically significantly and TUG and 10m walking test of the both groups were decreased statistically significantly. balance ability and walking ability of the whole-body vibration exercise group were improved statistically significantly in comparison with those of the Otago exercise group. The results of this study showed that whole-body vibration exercise for the improvement of life care had significant effects on improving BBS, TUG, 10m walking and Tinetti mobility test of fall risk stroke patients. Therefore, it is recommended to apply whole-body vibration exercise to improve life-care through improving balance ability and walking ability of fall risk stroke patients.

  • 노인 및 거동 불편 환자들이 목욕 중 낙상 사고 등을 당하는 비율은 매년 증가하고 있다. 특히 국내 65세 이상 1인 가구 197만 가구는(22년 기준) 가족의 돌봄 없이 혼자 생활하고 있어, 이들이 목욕 중에 사고를 당하는 경우, 빠른 구호가 어려운 상황이다. 이에 본 논문에서는 독거노인의 증가와 낙상 사고의 위험성에 초점을 맞추어 거동이 불편한 환자라도 혼자서 목욕이 가능한, 모든 사용자로부터 접근성을 높인 샤워 부스를 개발 및 연구한다. 요양보호사 두 명과 인터뷰를 하여 실제 상황에서의 문제점을 파악한다. 인터뷰 내용을 바탕으로 사용자의 안전을 1순위로 고려하고, 사용 편의성 및 청결을 유지하는 방안을 고려한다.
