Purpose: To resolution of A hospital-handmade modification double tilt angle immobilization system (DTAB immobilization system) and to report the clinical results of it. Material and Methods: It was developed in conjunction with the breast board for patients unable to achieve and maintain the desired uncomfortable respiration and position of set-up needed in the treatment of RT (This custom design provides an alternative to accomplishing this desired head angle needed to relax position treatment area, realizing that the lenses totally protected eye-ball out) By using the angled breast board and SBDD(small bowel device), reproducibility of set-up and patient comfort were addressed throughout the simulation, computed tomography planning and treatment process. Results: Usually patients the error range-within 5 mm. When use of Aqua patients error range-within 3 mm. Conclusion: It was constructed in tandem with a unique custom-built double tilt angle board (DTAB). It was designed to eliminate clinical set-up problems with head immobilization and instability during treatment, thus providing for a more comfortable head rest for the patient.
Purpose : In contrast with traditional time-cycled, pressure-limited ventilation, during volume-controlled ventilation, a nearly constant tidal volume is delivered with reducing volutrauma and the episodes of hypoxemia. The aim of this study was to compare the efficacy of pressure-regulated, volume controlled ventilation (PRVC) to Synchronized intermittent mandatory ventilation (SIMV) in VLBW infants with respiratory distress syndrome (RDS).Methods : 34 very low birth weight (VLBW) infants who had RDS were randomized to receive either PRVC or SIMV with surfactant administration : PRVC group (n=14) and SIMV group (n=20). We compared peak inspiratory pressure (PIP), duration of mechanical ventilation, and complications associated with ventilation, respectively with medical records. Results : There were no statistical differences in clinical characteristics between the groups. After surfactant administration, PIP was significantly lower during PRVC ventilation for 48hrs and accumulatevive value of decreased PIP was higher during PRVC ventilation for 24hrs (P<0.05). Duration of ventilation and incidence of complications was no significant difference. Conclusion : PRVC is the mode in which the smallest level of PIP required to deliver the preset tidal volume in VLBW infants with RDS, adaptively responding to compliance change in lung after surfactant replacement.

Many cellular phone volunteer studies have been conducted since such a social issue is raised that the long time usage of cellular phone by teenagers may increase health risk. While there were various adult volunteer studies using GSM cellular phone on heart rate and blood pressure, very few teenager studies using CDMA phone were conducted. In this study, two volunteer groups of 21 teenagers and 21 adults were exposed to 300 mW CDMA wave for half an hour, and the physiological parameters such as blood pressure, heart rate, respiration rate, and skin impedance were investigated. All the parameters fur both groups were unaffected during exposure except the skin impedance of teenager group.
Background : Pressure-controlled ventilation (PCV) is frequently used recently as the initial mode of mechanical ventilation in the patients with respiratory failure. Theoretically, because of its high initial inspiratory flow, pressure-controlled ventilation has lower peak inspiratory pressure and improved gas exchange than volume-controlled ventilation (VCV). But the data from previous studies showed controversial results about the gas exchange. Moreover, the comparison study between PCV and VCV with various inspiration : expiration time ratios (I : E ratios) is rare. So this study was performed to compare the respiratory mechanics and gas exchange between PCV and VCV with various I : E raitos. Methods : Nine patients receiving mechanical ventilation for respiratory failure were enrolled. They were ventilated by both PCV and VCV with various I : E ratios (1 : 2, 1 : 1.3 and 1.7 : 1). $FiO_2$, tidal volume, respiratory rate and external positive end-expiratory pressure (PEEP) were kept constant throughout the study. After 20 minutes of each ventilation mode, arterial blood gas, airway pressures, expired $CO_2$ were measured. Results : In both PCV and VCV, as the I : E ratio increased, the mean airway pressure was increased, and $PaCO_2$ and physiologic dead space fraction were decreased. But P(A-a)$O_2$ was not changed. In all three different I : E ratios, peak inspiratory pressure was lower during PCV, and mean airway pressure was higher during PCV. But $PaCO_2$ level, physiologic dead space fraction and P(A-a)$O_2$ were not different between PCV and VCV with three different I : E ratios. Conclusion : There was no difference in gas exchange between PCV and VCV under the same tidal volume, frequency and I : E ratio.

Measuring a biosignal during sleep is an important part of diagnosis and treatment of sleep disorder and also used to determine the general quality of sleep. As in current polysomnography, Contact method, which requires the attachment of electrodes to the skin, is the typical method to measure a biosignal during sleep. The procedure of this test is often considered to be inconvenient and tiresome because it requires attaching the device to the skin for each observation, and also limits free movement throughout the test. For this reason, the research on the acquiring the biosignal information without any attachment of a fixture on the skin is being conducted actively these days. In this study, it is suggested to check the heart rate per minute and the presence of breathing by placing a Piezo, which is a film type of pressure sensor, on the bed.

The purpose of this study was to determine the effect of gait training based on the way in a speed pattern on the respiratory function of general adults. A total of 37 people were divided into three groups to conduct fast, standard, and interval gait training. For gait training, a treadmill was used. Three groups were trained for 60 minutes, three times per week, for a period of 6 weeks. Inspiration pressure, maximum inspiration volume, and the size of diaphragm movement were measured. Repeated Measures ANOVA was used to compare times, groups, and interactions. For inspiratory pressure, maximum inspiration volume, and size changes in diaphragm movement, there were significant differences depending on the time and interaction between times and groups. For size changes in diaphragm's movement, there was a significant difference between interval gait training group and standard gait training group. Therefore, interval gait training had effects on size changes in diaphragm movement.
Purpose The present study aimed at assessing the effectiveness of the respiratory gating method used in the flow mode and additional localized respiratory-gated imaging, which differs from the step and go method. Materials and Methods Respiratory gated imaging was performed in the flow mode to twenty patients with lung cancer (10 patients with stable signals and 10 patients with unstable signals), who underwent PET/CT scanning of the torso using Biograph mCT Flow PET/CT at Bundang Seoul University Hospital from June 2016 to September 2016. Additional images of the lungs were obtained by using the respiratory gating method. SUVmax, SUVmean, and Tumor Volume ($cm^3$) of non-gating images, gating images, and additional lung gating images were found with Syngo,bia (Siemens, Germany). A paired t-test was performed with GraphPad Prism6, and changes in the width of the amplitude range were compared between the two types of gating images. Results The following results were obtained from all patients when the respiratory gating method was applied: $SUV_{max}=9.43{\pm}3.93$, $SUV_{mean}=1.77{\pm}0.89$, and $Tumor\;Volume=4.17{\pm}2.41$ for the non-gating images, $SUV_{max}=10.08{\pm}4.07$, $SUV_{mean}=1.75{\pm}0.81$, and $Tumor\;Volume=3.56{\pm}2.11$ for the gating images, and $SUV_{max}=10.86{\pm}4.36$, $SUV_{mean}=1.77{\pm}0.85$, $Tumor\;Volume=3.36{\pm}1.98$ for the additional lung gating images. No statistically significant difference in the values of $SUV_{mean}$ was found between the non-gating and gating images, and between the gating and lung gating images (P>0.05). A significant difference in the values of $SUV_{max}$ and Tumor Volume were found between the aforementioned groups (P<0.05). The width of the amplitude range was smaller for lung gating images than gating images for 12 from 20 patients (3 patients with stable signals, 9 patients with unstable signals). Conclusion In PET/CT scanning using the respiratory gating method in the flow mode, any lesion movements caused by respiration were adjusted; therefore, more accurate measurements of $SUV_{max}$, and Tumor Volume could be obtained from the gating images than the non-gating images in this study. In addition, the width of the amplitude range decreased according to the stability of respiration to a more significant degree in the additional lung gating images than the gating images. We found that gating images provide information that is more useful for diagnosis than the one provided by non-gating images. For patients with irregular signals, it may be helpful to perform localized scanning additionally if time allows.

The capnograph system for determining the partial pressure of carbon dioxide in the blood of a patient was developed based on the NDIR(non-dispersive infrared) absorption technology. NDIR gas analyzing method requires an optical absorption chamber and signal processing hardware. In this paper, we have designed and implemented NDIR type $CO_2$ gas chamber in consideration of the time response characteristics and lamp chopping frequency. And we have implemented signal processing hardware using two infrared sources to reduce the thermal background effect. The implemented gas chamber and signal processing hardware were tested in the temperature variation experiment and human expiratory experiment. The results showed that the system could produce a stable output signal and a good $CO_2$ gas concentration curve like a typical capnogram.
Background: Pressure support ventilation(PSV) is a new form of mechanical ventilatory support that assists spontaneous inspiraory effort of an intubated patient with a clinician-selected amount of positive airway pressure. Low level pressure support during inspiration can overcome the resistive component of inspiratory work imposed by an endotracheal tube. However the clinical efficacy of PSV as a weaning method has not been established yet. Object: The aim of study was to evaluate the efficacy of PSV when it is added to intermittent mandatory ventilation(IMV) in facilitating weaning precess compaired to IMV mode alone. Method: When the subject patients became clinically stable with their arterial blood gas analysis in acceptable range, they underwent weaning process either by IMV alone or by IMV plus PSV. The level of pressure support was held constant throught the weaning period. For the patients who required mechanical ventilation for less than 72 hr, 2h weaning trial was performed with IMV rate starting from 6/min. For the patients who required mechanical ventilation more than 72 hr, 7 hr weaning trial was performed with IMV rate starting from 8/min. For the patients who failed three consecutive trials of weaning, retrial of weaning was attempted over 3 days with IMV rate starting from 8/min. Clinical characteristics, APACHE II score and nutritional status were compared. For all patients, heart rate, mean blood pressure and respiratory rate were mornitored for 48 hrs after weaning trial started. Results: The total number of weaning trial was 37 in 23 patients(18 by IMV, 19 by IMV+PSV). Total ventilation time, APACHE II score and nutritional status were not statistically different between the two groups. The weaning success rate were not statistically different(38.3% by IMV, 42.1% by IMV+PSV) and the changes of mean blood pressure, heart rate, respiratory rate during first 48 hours were not different between the two groups. Conclusion: Low level PSV when added to IMV for weaning trial does not seem to improve the success rate of weaning from mechanical ventilation. PSV at 10cm $H_2O$ did not induce significant physiologic changes during weaning process.
Coated filters were developed to replace the glass impinger methods that use reagent solutions. The purpose of this study was to simultaneously measure nitrogen dioxide ($NO_2$) and respirable suspended particles (RSP) by a filtration method with a cyclone connected to a pump. A first pre-filter for RSP and second filter for $NO_2$, which was soaked in a TEA (Triethanolamine) solution, were loaded into a filter cassette with a pump flow rate of 1.7 l/m. After sampling, the TEA soaked filter was removed from the cassette, placed in a large test tube (10 ml), mixed, and allowed time to develop. The absorbance (abs) of the diazo compound of the $NO_2$ and N-(1-Napthylethylenediamine dihydrochloride) in the color reagent was measured at 545 nm on a spectrophotometer. The collection efficiency(%) of $NO_2$ by each 3 filter soaked in TEA solution and used in the cyclone with a pump flow rate 1.7 l/m was $89{\pm}3%$ and the correlation coefficient between the true $NO_2$ concentration and that determined by the TEA soaked filters was 0.993(p<0.001).
