Naturopathy Theory and Mechanism of Breathing Massage for Life Care (라이프 케어를 위한 호흡마사지법의 자연치유 이론기전)

  • In this study, the authors aimed to give careful consideration to the circulation of bioenergy (Qi) by using cardiorespiratory massage technique. Finding enlightenment in harmony with nature thousands of years ago, our ancestors created Qi using natural breathing and tried to achieve good health and long life by controling the up and down movement. Experiencing the importance of the abdomen during the hungry times, also, grandmothers cured diseases by rubbing the abdomen, which is the ocean of life with their soft, soothing touch of hand. Naval breathing therapy was formed, based on natural breathing, emotional recognition of grandmothers' soft, soothing touch, and the Theory of Unified Reason and Energy that is the key of Oriental medicine. Natural breathing and Yakson massage are kinds of naturopathy to change body and mind, cure diseases naturally, and maintain the improvement in physical functions by taking care of the abdomen through the creation of Qi and the up and down movement and keeping dynamic balance between the natural world and the inside and outside of human body. It is anticipated that this study could contribute to the practical spread for an active application of naval breathing therapy and be broadly used in scientific clinical researches.

Effect of Foot Reflexology on the Vital Signs, Blood Cortisol, Lymphocytes and Natural Killer Cell of Female Cancer Patients (발반사마사지가 여성암환자의 활력징후, 코티졸, 면역반응에 미치는 효과)

  • 목적 : 이 연구는 항암제를 투여받는 여성암환자에서 발반사마사지가 스트레스반응(활력징후, 코티졸)과 면역반응에 미치는 효과를 평가하기 위함이다. 방법 : 11명의 여성암 환자를 임의로 표출한 후 단일군 전후실험설계로 진행하였다. 종속변수는 스트레스 반응으로 활력징후(수축기 혈압, 이완기 혈압, 맥박수, 호흡수)와 혈중 코티졸, 면역반응으로 림프구 아군(CD3, CD4, CD8, CD19)과 자연살세포(NK cells)이며, 독립변수는 발반사마사지이었다. 중재 전후 종속변수의 변화를 보기 위해 Wilcoxon signed rank test를 실시하였다. 결과 : 중재 전에 비해 중재 후 수축기 혈압, 이완기 혈압, 맥박수, 혈중 코티졸치, CD4와 CD19는 유의한 변화를 보였으나 호흡수, CD3, CD8, 그리고 자연살세포는 유의한 변화를 보이지 않았다. 결론 : 이러한 연구결과는 발반사마사지가 항암화학요법을 받는 여성암 환자의 스트레스 호르몬과 면역관련세포에 영향을 보였지만 표본수가 작고 중재가 1회에 그쳤기 때문에 이러한 단점을 보완한 추후연구를 통해 발반사마사지의 효과를 좀더 잘 규명할 수 있을 것이다.

  • The purpose of this study is to suggest an anti-aging health care plan through a literature analysis of the life activity system of Oriental Medicine. The source of normal maintenace, development, and change of the natural world and the human body's life activity process is the dynamical cooperative equilibrium of the internal Shen-Ji's discipline that creates, grows, changes, makes differences and stores(生长化收藏) and external Qi-Li's process of change from birth to growth, maturity to old age and ending of life(生長壯老已). Therefore, it was analyzed that the natural and human life activity processes are normally maintained, developed, and transformed. As a result, when the natural healing power of Genuine-Qi, the source of the human vitality, rises with the 'breathing massage' that combines a static breathing method which is anti-aging health care method applying the lifting entering movement of Qi and a dynamic abdominal massage, and it harmonizes the Shen-Ji which is the life process system and Qi-Li so that it is believed to be helpful in prolonging a healthy lifespan.

Current Status of Respiratory Care in Korean Intensive Care Units (국내 중환자실내 호흡치료의 현황)

  • Backgrounds : Respiratory care for patients in intensive care units (ICUs) has been performed mainly by nurses in Korea. However, the current status of respiratory care in the Korea ICUs is not well known. Respiratory care and the methods of delivery in ICUs were surveyed. Method : A confidential questionnaire was distributed to the head nurses working the ICUs at 117 hospitals in Korea. One hundred hospitals returned the questionnaires, for a response rate of 85%. The hospitals were divided into three groups : Main university hospitals (MUH), university associated hospitals (UAH), and general hospitals (GH) Result : Eighteen units of 66 units in MUH and 35 units of 58 units in GH were organized as a general ICUs. The percentage of ICUs with full-time doctors was 47.1%. The nurses usually delivered respiratory care spending from 1 to 4 h during their 8 h of working time. Although the respondents felt that respiratory care should be delivered by trained respiratory therapists, these therapists were not found at the hospitals. Most of the units performed percussion, tracheal suctioning, and positional changes. However, vibration and IPPB were less frequently performed in GH. Among oxygen supply apparatus, venturi mask and T-piece were not frequently used in GH. GH applied a noninvasive ventilator mode less frequently than MUH and UAH. The percentage of Swan-Ganz catheter monitoring was only 21.4% in GH. Conclusion : Respiratory care for patients in the Korean ICUs was provided by nurses on the whole. In addition, there were many differences in the level of respiratory care according to the type of hospital. To overcome the current problems revealed, an effective in-hospital training program for the development of full-time respiratory care therapists should be established urgently in Korea.

  • In August 2000, the American Heart Association and the European Resuscitation Council published the conclusions of tile International Guidelines 2000 Conference on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care which contains both the new recommendations and an in-depth review. The most important changes in the recommendations according to the authors are discontinuation of the pulse-check for lay people, 500 ml instead of 800∼1,200 ml tidal volume during bag-valve-mask ventilation (FiO2 > 0.4) of a patient with an unprotected airway, unifying correct endotracheal intubation size as 8.0 mm, vasopressin (40 units) and epinephrine (1 mg) as comparable drugs to treat patients with ventricular fibrillation, early prehospital survey and intravenous lysis for patients who have suffered coronary artery syndrome and stroke.

  • Ischemic ventricular rupture is one of the most fatal complications following myocardial infarction, and this requires prompt diagnosis and operation. A 75-year-old female was admitted to the ER in a semicomatous mentality with cyanotic extremities. Cardiac echography was carried out in the ER, and a $1.5{\sim}2\;cm$ thickness of effusion in the pericardium was seen. Because the patient's heart had declined to 35 times per min, an emergency operation was started while giving cardiac massage. After observing a 1 cm rupture on the right ventricular wall and a necrotic hemorrhagic scar with a rupture on the left ventricular apical wall, repair of the ruptured areas with a large Satinsky clamp was carried out to control bleeding without cardiopulmonary bypass. On the 28th day after surgery, she was discharged home with a minimal degree of dyspnea.

Palliative Care Provided for Older Patients with Terminal Stage of Cardiopulmonary Disease Before and After Life-Sustaining Treatment Decisions (만성 심폐질환을 가진 말기 노인환자의 연명의료결정 전후 완화간호의 제공)

  • This study was a retrospective descriptive study to identify frequency and change of palliative care provided for older patients with terminal stage of cardiopulmonary disease before and after life-sustaining treatment (LST) decision making. As a result of chart review of 124 older patients in a university hospital, oral analgesics medication, cold and hot therapy for pain management, antibiotics medication and urine culture for urinary infection, oral care, hair wash, and partial bath were provided significantly less after LST decision making. Provision of praying and relaxation therapy for pain control, oral and nasal care, and emotional care were not changed before and after LST decision making. Spiritual care was the least provided care. Therefore, non-pharmacological pain management, emotional care, and spiritual care need to be improved for older patients with terminal cardiopulmonary disease at the end of life.
