A Canine Model of Tracheal Stenosis Using Nd-YAG Laser (Nd-YAG laser를 이용한 기관협착 동물모델의 개발)

  • Background: Tracheal stenosis is an urgent but uncommon disease. Therefore, primary care clinicians have limited clinical experience. Animal models of a tracheal stenosis can be used conveniently for the learning, teaching, and developing new diagnostic and therapeutic modalities for tracheal stenosis. Recently, a canine model of a tracheal stenosis was developed using a Nd-YAG laser. To describe the methods and results of developed animal model, we performed this study. Methods : Six Mongrel dogs were generally anesthetized and the anterior 180 degree of tracheal cartilage of the animal was photo-coagulated using a Nd-YAG laser. The animals were bronchoscopically evaluated every week for 4 weeks and a pathologic evaluation was also made. Results : Two weeks after the laser coagulation, the trachea began to stenose and the stenosis progressed through 4 weeks. All animals suffered from shortness of breath, wheezing, and weight loss in the 3 weeks after the laser treatment, and two died of respiratory failure just before the fourth week. The gross pathologic findings showed the loss of cartilage and a dense fibrosis, which resulted in a fibrous stricture of the trachea. Microscopy also showed that the fibrous granulation tissue replaced destroyed cartilage. Conclusion : The canine model can assist in the understanding and development of new diagnostic and therapeutic modalities for tracheal stenosis.

Application of spatiotemporal transformer model to improve prediction performance of particulate matter concentration (미세먼지 예측 성능 개선을 위한 시공간 트랜스포머 모델의 적용)

  • It is reported that particulate matter(PM) penetrates the lungs and blood vessels and causes various heart diseases and respiratory diseases such as lung cancer. The subway is a means of transportation used by an average of 10 million people a day, and although it is important to create a clean and comfortable environment, the level of particulate matter pollution is shown to be high. It is because the subways run through an underground tunnel and the particulate matter trapped in the tunnel moves to the underground station due to the train wind. The Ministry of Environment and the Seoul Metropolitan Government are making various efforts to reduce PM concentration by establishing measures to improve air quality at underground stations. The smart air quality management system is a system that manages air quality in advance by collecting air quality data, analyzing and predicting the PM concentration. The prediction model of the PM concentration is an important component of this system. Various studies on time series data prediction are being conducted, but in relation to the PM prediction in subway stations, it is limited to statistical or recurrent neural network-based deep learning model researches. Therefore, in this study, we propose four transformer-based models including spatiotemporal transformers. As a result of performing PM concentration prediction experiments in the waiting rooms of subway stations in Seoul, it was confirmed that the performance of the transformer-based models was superior to that of the existing ARIMA, LSTM, and Seq2Seq models. Among the transformer-based models, the performance of the spatiotemporal transformers was the best. The smart air quality management system operated through data-based prediction becomes more effective and energy efficient as the accuracy of PM prediction improves. The results of this study are expected to contribute to the efficient operation of the smart air quality management system.

Estimation of Personal Exposure on Nitrogen Dioxide Using Time Activity - Comparative Study between Seoul, Korea and Brisbane, Australia - (시간활동도를 이용한 이산화질소 개인노출 예측 - 한국의 서울과 호주의 브리스베인의 비교 연구 -)

  • 현대 생활에서 대부분의 사람들은 90%이상을 실내(가정, 일반사무실, 실내작업장, 공공건물, 지하시설물, 상가, 음식점, 자동차, 지하철 등)에서 생활하기 때문에 실내공기질(indoor air quality)은 개인이 오염물질에 노출되는 주요한 요인이다. 이산화질소($NO_2$)는 고온의 연소과정에서 발생되는 부산물로써 차량, 발전소와 산업장 등에서 발생되고 있다. 실내에서 이산화질소의 농도는 가스레인지, 케로센(kerosene) 난방기, 흡연에 주로 영향을 받는다. $NO_2$는 호흡기 증상과 관련된 각종 질환을 유발시키는 것으로 보고되고 있다. 본 연구는 한국의 서울에서 직장인 95명의 시간활동도가 조사되었으며, 호주 브리스베인에서 직장인 57명의 시간활동도와 동시에 각 가정의 실내.외 및 직장의 $NO_2$ 농도를 측정하였다. 또한 개인 $NO_2$ 노출을 예상하여 각 도시의 빈도분포를 예상하였다. 본 연구의 결과를 보면 다음과 같다. 1. 서울의 95명의 직장인들은 실내에서 약 83.8%의 시간을 보냈으며, 브리스베인의 57명의 직장인들은 실내에서 약 88.3%의 시간을 보냈다. 2. 브리스베인에서 측정된 실내의 NO2 평균농도는 10.5ppb(${\pm}5.6$), 실외의 NO2 평균농도는 14.5ppb(${\pm}5.8$), 직장에서의 $NO_2$ 평균농도는 18.2ppb(${\pm}5.0$)였다. 개인의 $NO_2$ 노출은 평균 15.0ppb(${\pm}5.2$)였다. 개인의 $NO_2$ 노출은 실외의 $NO_2$ 농도(r=0.42)보다 실내의 $NO_2$ 농도(r=0.42)보다 실내의 NO2 농도(r=0.49)에 상관성이 더 높았다. 3. 시간 가중치 모델을 이용한 개인 $NO_2$ 노출은 측정된 개인 NO2 노출과 통계학적으로 상관성을 가지고 있었다(r=0.58). 예측된 개인 $NO_2$ 노출은 측정된 $NO_2$ 노출보다 낮게 나타났으며, 이것은 출퇴근 등에 의한 교통의 이동에 따른 노출 때문인 것으로 생각되었다. 4. $NO_2$ 농도 분포를 log-normal 분포, 시간활동도를 Normal 분포로 가정하고 Monte-Carlo 시뮬레이션을 했을 때 서울의 직장인의 개인 노출은 평균 36.7ppb(${\pm}10.9$)였으며, 브리스베인의 직장인의 개인 노출은 평균 13.7ppb(${\pm}4.1$)였다.

Air Pollution Risk Prediction System Utilizing Deep Learning Focused on Cardiovascular Disease

  • This paper proposed a Deep Neural Network Model system utilizing Keras for predicting air pollution risk of the cardiovascular disease through the effect of each component of air on the harmful virus using past air information, with analyzing 18,000 data sets of the Seoul Open Data Plaza. By experiments, the model performed tasks with higher accuracy when using methods of sigmoid, binary_crossentropy, adam, and accuracy through 3 hidden layers with each 8 nodes, resulting in 88.92% accuracy. It is meaningful in that any respiratory disease can utilize the risk prediction system if there are data on the effects of each component of air pollution and fine dust on oil-borne diseases. It can be further developed to provide useful information to companies that produce masks and air purification products.

  • 국내 산업재해 사고 사망자의 상당수가 건설업에서 발생하고 있다. 건설 현장에는 굴삭기, 크레인과 같은 중장비가 많고 높은 곳에서 작업하는 경우가 흔해 위험 요소에 노출될 가능성이 높다. 물리적 사고 외에도 작업 중 발생하는 미세먼지에는 여러 유해 인자가 존재하여 건설근로자들에게 호흡기질환과 같은 직업병을 유발한다. 정부에서는 산업현장 안전 관리의 중요성이 증가함에 따라 각종 산업재해로부터 근로자를 보호하기 위한 법안을 마련하였다. 따라서 건설 현장의 경우 산업재해를 방지하기 위해서 위험요소를 사전에 인지하고 즉각 대응할 수 있는 기술이 필요하다. 본 연구에서는 인공지능(AI)과 사물인터넷(IoT)을 통한 자동화 기술을 활용하여 24시간 안전 관리 시스템을 제안한다. 제안하는 IoT 기반 통합안전 관리 시스템은 AI를 적용한 CCTV를 통해 산업 현장을 모니터링하고, 다수의 IoT 센서가 측정한 수치를 근로자 및 관리자가 실시간으로 확인할 수 있게 하여 산업 현장 내 안전사고를 예방한다. 구체적으로 어플리케이션을 통해 미세먼지 농도, 가스 농도, 온도, 습도, 안전모 착용 여부 등을 모니터링할 수 있다. 모니터링 중에 유해물질의 농도가 일정 수치를 넘기거나 안전모를 착용하지 않은 근로자가 발견될 경우 근로자 및 관리자에게 경고 알림을 발송한다. 유해물질 농도는 IoT 센서를 통해 측정하며 안전모 착용 여부는 카메라 센서에 딥러닝 모델을 적용하여 인식하였다. 본 연구에서 제시한 통합안전관리시스템을 통해 건설현장을 비롯한 산업현장의 산업재해 감소와 근로자 안전 증진에 기여할 수 있을 것으로 기대한다.

Review on the Effects of Herbal Medicine on Respiratory Diseases in In Vivo Particulate Matter Models (미세먼지 in vivo 모델에서 호흡기 질환에 대한 한약의 효과에 관한 연구 동향 분석)

  • Objective: This study was conducted to review the effects of herbal medicine on respiratory diseases induced by the treatment of particulate matter in in vivo animal models. Methods: Literature searches were performed in seven databases (Pubmed, Embase, Cochrane Library, KISS, KTKP, OASIS, and ScienceON). After the searched studies were screened based on the inclusion/exclusion criteria, the publication date, origin, used animals, induction of particulate matter models, herbal medicine used for intervention, study design, outcome measure, and results of studies were analyzed. Results: Among a total of 972 studies primarily searched, 34 studies were finally included in our study. Of this number, 29 studies induced animal models by using only particulate matter, and 5 studies induced animal models with respiratory diseases, such as asthma and chronic obstructive pulmonary disease, by using particulate matter and other materials. In the selected studies, the treatments of herbal medicine in particulate matter models suppressed oxidative stress and inflammation in lung tissue, bronchoalveolar lavage fluid, and blood as well as lung injury in histological analysis. Conclusion: The results of this study suggest that herbal medicine is effective in treating respiratory diseases induced by particulate matter. These results are also expected to be useful data for designing further studies. However, more systematically designed in vivo studies related to particulate matter are needed.

Factors Associated with Health Service Utilization of the Disabled Elderly in Korea (장애노인의 의료이용에 영향을 미치는 요인)

  • The purpose of this study is to examine the factors associated with the probability and levels of the health service utilization among the disabled elderly in Korea. The sample includes 2,111 people older than 65 who are extracted from the 2008 National Survey on People with Disabilities. More than half (54.3%) of the sample experienced at least 1 outpatient physician visit within two weeks and 26.7% were hospitalized within a year. The key factors associated with the outpatient visits were health insurance status, the existence of chronic disease, self-rated health, the Activities of Daily Living (ADLs), as well as renal impairment. Similarly, the utilization of inpatient care was related to health insurance status along with the existence of the internal organ disabilities such as cardiac or respiratory disorders. The study implies the need for the health care policies regarding the prevention of chronic diseases, dependency for daily activities of the elderly, and a management system that specifically targets those with internal organ disabilities. Moreover, the study suggests that financial supports for the low-income group would be helpful to increase their access to health service utilization.

Anti-inflammatory Effects of Haepyoijin-tang in Aspergillus Oryzae Protease Induced Respiratory Inflammation Model (Aspergillus oryzae protease 유도 호흡기 염증모델에서 해표이진탕(解表二陳湯)의 항염증 효과)

  • Haepyoijin-tang and its main components have been used for phlegm, cough and dyspnea. Using a respiratory inflammation model, we intend to reveal the anti-inflammatory effect and pharmacological mechanism of Haepyoijin-tang. We induced the respiratory inflammation model by Aspergillus oryzae protease and ovalbumin administration. Female Balb/c mice (8 weeks old) were classified into four groups as follows: saline control group, aspergillus oryzae protease and ovalbumin induced respiratory inflammation group (vehicle), inflammation with Haepyoijin-tang (200 mg/kg) administration group, inflammation with dexamethasone (5 mg/kg) administration group (n=7). To identify the anti-inflammatory effects of Haepyoijin-tang water extracts, we measured the inflammatory cell number in bronchoalveolar lavage fluid (BALF) and total live lung cell number. In addition, we checked eosinophil ratio and number in BALF. And Interleukin (IL)-5 level was also measured in lung cell culture supernatant. To confirm the mechanism of anti-inflammatory effects, we analyzed the activated helper T cell (CD4+CD25+ cell) and Th2 cell (CD4+GATA3+ cell) ratio and number in lung by using flow cytometry. Finally, we attempted to confirm the immune mechanism by measuring the ratio and number of regulatory T cells (CD4+Foxp3+ cell). Haepyoijin-tang extracts treatment diminished inflammatory cell, especially, eosinophil number in BALF and total live lung cell number. Moreover, IL-5 level was reduced in Haepyoijin-tang treated group. Surprisingly, Haepyoijin-tang extracts administration not only decreased the activated helper T cell but also Th2 cell population in lung. Additionally, regulatory T cell population was increased in Haepyoijin-tang administration group. Our findings proved that Haepyoijin-tang extract have anti-inflammatory efficacy by suppressing Th2 cell activation and promoting regulatory T cell population.

Standard Chemotherapy with Excluding Isoniazid in a Murine Model of Tuberculosis (마우스 결핵 모델에서 Isoniazid를 제외한 표준치료의 예비 연구)

  • Background: Isoniazid (INH, H) is a key drug of the standard first-line regimen for the treatment of tuberculosis (TB), yet some reports have suggested that treatment efficacy was maintained even though INH was omitted from the treatment regimen. Methods: One hundred forty C57BL/6 mice were infected with the H37Rv strain of M. tuberculosis with using a Glas-Col aerosol generation device, and this resulted in depositing about 100 bacilli in the lung. Four weeks after infection, anti-TB treatment was initiated with varying regimens for 4-8 weeks; Group 1: no treatment (control), Group 2 (4HREZ): 4 weeks of INH, rifampicin (R), pyrazinamide (Z) and ethambutol (E), Group 3: 1HREZ/3REZ, Group 4: 4REZ, Group 5: 4HREZ/4HRE, Group 6: 1HREZ/3REZ/4RE, and Group 7: 4REZ/4RE. The lungs and spleens were harvested at several time points until 28 weeks after infection, and the colony-forming unit (CFU) counts were determined. Results: The CFU counts increased steadily after infection in the control group. In the 4-week treatment groups (Group 2-4), even though the culture was negative at treatment completion, the bacilli grew again at the 12-week and 20-week time points after completion of treatment. In the 8-week treatment groups (Groups 5-7), the bacilli did not grow in the lung at 4 weeks after treatment initiation and thereafter. In the spleens of Group 7 in which INH was omitted from the treatment regimen, the culture was negative at 4-weeks after treatment initiation and thereafter. However, in Groups 5 and 6 in which INH was taken continuously or intermittently, the bacilli grew in the spleen at some time points after completion of treatment. Conclusion: TThe exclusion of INH from the standard first-line regimen did not affect the treatment outcome in a murine model of TB in the early stage of disease. Further studies using a murine model of chronic TB are necessary to clarify the role of INH in the standard first-line regimen for treating TB.

The Preventive Effect of Allergic Inflammation by Induction of Oral Tolerance in a Mouse Model of Chronic Asthma (마우스 만성천식모델에서 경구면역관용 유도에 의한 알레르기 염증의 예방효과)

  • Background : Induction of oral tolerance (OT) has been known to prevent allergic inflammation in acute asthma model within 4 weeks. However it is remained whether induction of OT may effectively prevent allergic inflammation in chronic asthma model over 4 weeks. We observed the effect of induction of OT on allergic inflammation and airway remodeling in chronic asthma model up to 8 weeks. Methods : 5-week-old female BALB/c mice divided into 4 groups-control group, asthma group, low dose OT group, and high dose OT group. To induce oral tolerance mice were fed ovalbumin (OVA) before sensitization with OVA and aluminum hydroxide-1 mg for 6 consecutive days in the low dose OT group and 25 mg once in the high dose OT group. Mice in the asthma group were fed phosphate buffered saline instead of OVA. After sensitization followed by repeated challenge with aerosolized 1% OVA during 6 weeks, enhanced pause (Penh), inflammatory cells, IL-13, and IFN-${\gamma}$ levels in bronchoalveolar lavage (BAL) fluids as well as OVA-specific IgE, IgG1, and IgG2a levels in serum were measured. In addition the degree of goblet cell hyperplasia and peribronchial fibrosis were observed from lung tissues by PAS and Masson's trichrome stain. Results : Both OT groups showed a significant decrease in Penh, inflammatory cells, IL-13, and IFN-${\gamma}$ levels in BAL fluids as well as OVA-specific IgE, IgG1, and IgG2a levels in serum compared with the asthma group (P<0.05). In addition, the degree of goblet cell hyperplasia and peribronchial fibrosis were significantly attenuated in both OT groups compared with the asthma group (P<0.01). Conclusion : These results suggest that induction of OT may effectively prevent allergic inflammation as well as airway remodeling even in chronic asthma model up to 8 weeks.
