Stenosis of Esophageal Reconstruction by Abscess (농양에 의한 재건식도 협착증 -치험 1례 보고-)

  • We experienced a case of unusual complication following esophageal reconstruction. In 1969, accidentally the patient swallowed Aye and was developed benign esophageal stricture one year later. In 1972, esophageal reconstruction with right colon was done but pus was drained out of the abdominal wound. After then wound disruption and healing were repeated. In 1996, stenosis of colonic graft was found and resection of stenotic area and end to end anastomosis was done. We concluded that it was developed inflammatory change of graft by intraoper tive infection.

Heart Valve Stenosis Region Detection Algorithm on Heart Sounds (심음에서의 심장판막협착 영역 검출 알고리듬)

  • In this paper, a new algorithm is proposed for the heart valves stenosis region detection using heart sounds. Many researches for detecting primary components or removing heart murmurs have been studied, but their performances are degraded at abnormal heart sounds such as aortic stenosis and mitral stenosis because of large heart murmurs. In this paper, heart murmur detection method is proposed based on noise intensity function. The proposed noise intensity function detect the primary components S1, S2, then set session up using S1, S2. And then noise intensity function was computed using autocorrelation value of each session. The proposed noise intensity function estimated noise intensity of each sessions and detected heart murmurs. According to simulation results, the proposed algorithm has better performance than former study for detecting heart valve stenosis region.

  • Once it is diagnosed, immediate surgical extirpation is desirable for treating left ventricle myxoma that's accompanied with stenosis of the left ventricle outflow tract. This is because this condition may potentially induce fatal complications such as cerebral infarction or myocardial infarction that's triggered by myxoma embolus, or even sudden death due to coronary malperfusion. An 18-year-old male with the chief complaint of NYHA class II exertional dyspnea was found to have a $4{\times}3\;cm^2$ sized mass on transthoracic ultrasonography, which was shown to move down the left ventricle outflow tract on the systolic phase. The mass was immediately extirpated by incision of the left ventricle; the mass was finally diagnosed as a myxoma. The patient was discharged on at the 10th day postoperatively without any complications. On the 22-month follow-up observation made at the out-patient clinic after discharge, there have been no noticeable, significant changes seen on physical examination or the cardiac ultrasonography.

  • Diffculty decannulation is one of serious and troublesome complication in management. The etiologic factors were deviation of tracheal wall, growing of granulation tissue, subglottic edema, cicatricial stenosis of trachea and functional factor. We observed 9 cases of diffculty decannlation after tracheostomy and we report with literature review on these cases.

Factors Associated with Conversion from Conservative to Surgical Treatment in Single-Level Lumbar Spinal Stenosis Patients (보존적 치료 중인 단분절 요추관 협착증 환자에서 수술적 치료로 전환과 관련된 연관 인자)

  • Study Design: Retrospective study of prospectively-collected data. Objectives: To determine the factors associated with conversion from conservative to surgical treatment in single-level lumbar spinal stenosis patients. Summary of Literature Review: Various reports have presented clinical outcomes after the surgical and nonsurgical treatment of spinal stenosis. However, few reports have investigated factors predicting conversion to surgery during the course of conservative treatment. Materials and Methods: We analyzed 40 patients who visited our hospital from May 2010 to May 2015 and were traceable for at least 3 years after being advised to undergo surgery following 3 months of conservative treatment. Of these patients, 20 underwent surgery and 20 did not. We then investigated the factors associated with conversion to surgical treatment. Clinical assessments were conducted using a questionnaire, and the overall area of the spinal canal and the muscle area within the spinal canal were measured using magnetic resonance imaging. Results: The average area of the spinal canal was $81.40{\pm}53.61mm^2$ in the surgical group, compared to $127.75{\pm}82.55mm^2$ in the nonsurgical group (p=0.042). The muscle area in the spinal canal was $5.17{\pm}1.30cm^2$ in the surgical group, whereas it was $6.40{\pm}1.56cm^2$ in the nonsurgical group (p=0.010). The patients in the surgical group were more likely to have experienced repetitive strain and to have frequently visited health clubs (p=0.047, p=0.037, respectively). However, regular stretching was more common in the nonsurgical group (p=0.028). Conclusions: The factors associated with conversion to surgical treatment were a narrow spinal canal, a small muscle area within the spinal canal, visiting health clubs, repetitive sprain, and not stretching. A small muscle area within the spinal canal can be considered as a key factor related to surgical conversion.

Preservation of the Posterior Ligaments for Preventing Postoperative Spinal Instability in Posterior Decompression of Lumbar Spinal Stenosis: Comparative Study between Port-Hole Decompression and Subtotal Laminectomy (요추부 척추관 협착증의 후방 감압술에서 후방 인대의 보존 여부와 술 후 척추 불안정성과의 연관성: 포트홀(Port-Hole) 감압술과 후궁 아전절제술 간 비교 연구)

  • Purpose: To determine if sparing the interspinous and supraspinous ligaments during posterior decompression for lumbar spinal stenosis is significant in preventing postoperative spinal instability. Materials and Methods: A total of 83 patients who underwent posterior decompression for lumbar spinal stenosis between March 2014 and March 2017 with a minimum one-year follow-up period, were studied retrospectively. The subjects were divided into two groups according to the type of surgery. Fifty-six patients who underwent posterior decompression by the port-hole technique were grouped as A, while 27 patients who underwent posterior decompression by a subtotal laminectomy grouped as B. To evaluate the clinical results, the Oswestry disability index (ODI), visual analogue scale (VAS) for both back pain (VAS-B) and radiating pain (VAS-R), and the walking distance of neurogenic intermittent claudication (NIC) were checked pre- and postoperatively, while simple radiographs of the lateral and flexion-extension view in the standing position were taken preoperatively and then every six months after to measure anteroposterior slippage (slip percentage), the difference in anteroposterior slippage between flexion and extension (dynamic slip percentage), angular displacement, and the difference in angular displacement between flexion and extension (dynamic angular displacement) to evaluate the radiological results. Results: The ODI (from 28.1 to 12.8 in group A, from 27.3 to 12.3 in group B), VAS-B (from 7.0 to 2.6 in group A, from 7.7 to 3.2 in group B), VAS-R (from 8.5 to 2.8 in group A, from 8.7 to 2.9 in group B), and walking distance of NIC (from 118.4 m to 1,496.2 m in group A, from 127.6 m to 1,481.6 m in group B) were improved in both groups. On the other hand, while the other radiologic results showed no differences, the dynamic angular displacement between both groups showed a significant difference postoperatively (group A from 6.2° to 6.7°, group B from 6.5° to 8.4°, p-value=0.019). Conclusion: Removal of the posterior ligaments, including the interspinous and supraspinous ligaments, during posterior decompression of lumbar spinal stenosis can cause a postoperative increase in dynamic angular displacement, which can be prevented by the port-hole technique, which spares these posterior ligaments.

Cor Triatriatum Dexter Complicated with Pulmonic Stenosis and Patent Foramen Ovale in A Pitbull Terrier Puppy (핏불테리어종 자견에서 폐동맥 협착과 난원공 개존증이 합병된우측 삼중심방증)

  • A 2-month-old intact male Pitbull terrier (weighing 1.01 kg) were referred with primary complaints of severe abdominal distension, exercise intolerance, retarded growth, yellow nasal discharge and anorexia. Diagnostic imaging studies found enlarged right atrium partitioned by abnormal membrane, severe pulmonic systolic jets (5.66 m/s of peak velocity) and right-to-left blood shunt at the inter-atrial septum. Based on clinical and diagnostic findings, the case was diagnosed as cor triatriatum dexter complicated with severe pulmonic stenosis and right-to-left shunted patent foramen ovale. Either surgical or interventional therapy has not been attempted, because of unstable patient's condition. Using blood transfusion, oxygen supply and cardiac medications (i.e., sildenafil, spironolactone, enalapril), the dog was recovered.

Double-chambered Right Ventricle with Intact Ventricular Septum in a Maltese Dog (말티즈 견에서 발생한 우심실양분증)

  • A 6-year-old female Maltese (body weight, 3.1 kg) without clinical signs was referred for further evaluation of the cause of cardiac murmur. Thoracic radiography revealed right-sided cardiomegaly. Echocardiography showed marked hypertrophic remodeling of the right ventricular free wall and an anomalous muscular bundle and fibrous nodule near the subinfundibular portion of the right ventricular outflow tract (RVOT), indicating a double-chambered right ventricle (DCRV). The turbulent flow from the anomalous muscular bundle to the main pulmonary artery was 4.6 m/sec, in addition to the tricuspid valvular regurgitation of 4.4 m/sec and main pulmonary artery flow of 1.1 m/sec. The dog is receiving atenolol (0.5 mg/kg) to minimize the deleterious cardiac effects of the high afterload, even though she remains asymptomatic. This report describes a case of DCRV, a rare congenital heart disease in dogs in South Korea.

Trilogy of Fallot in a Dog (개에서 발생한 팔로삼징)

  • A 3 years-old female mixed dog (weighing 5.3 kg) was referred to veterinary teaching hospital of Kangwon National University with primary complaints of syncope, severe exercise intolerance, depression and lethargy. Diagnostic studies revealed polycythemia, right sided cardiac enlargement on thoracic radiography and right-to left atrial septal defect, severe pulmonary stenosis (~5 m/s of peak velocity) and right ventricular hypertrophy. Based on diagnostic findings, the dog was diagnosed as trilogy of Fallot. To improve clinical condition of this dog, diltiazem and enalapril were prescribed with weekly phlebotomy. To author's best knowledge, this is the first case of trilogy of Fallot in Korea.

Anterior Tracheoplasty -Two Cases Report- (전방 기관성형술 -2례 보고-)

  • We describe here two cases of anterior tracheoplasty utilizing an autologous pericardial patch. One patient was a 9 year-old female who had a congenital long tracheal stenosis associated with major vascular anomalies including pulmonary artery sling. One-stage correction was done under the support of an extracorporeal membrane oxygenation system. She required a prolonged ventilation support for 10 days postoperatively until the implanted pericardium was fixed to the mediastinal structures. The other patient was a 8 year-old male who had acquired tracheal stenosis following a complicated tracheostomy. By applying additional support over the pericardial patch with the costal cartilage, an endotracheal tube could be removed immediately after the operation. Both patients have been doing well in a postoperative follow-up of over a year, and there have been evidences of growth in the reconstructed trachea.
