설페이티드 다당류의 실험실적 효능 검색과 동물내에서의 비교

  • 헤파린은 혈액응고계에서 antithrombin III 존재하에서 thrombin과 factor Xa의 작용을 억제함으로써 항응고제로서 작용을 한다. 심정맥혈전증 등 임상에 응용성이 높지만 장기적 사용시 혈소판 감소효과, 출혈, 골다공증 등의 부작용이 나타나고 있다. 본연구는 식물성 생약으로부터 당을 분리 정제하여 화학적으로 sulfation시켜 in vitro와 ex vivo에서 항응고활성을 비교하였다. 우선적으로 aPTT를 측정하여 응고시간의 연장을 시키는 다섯종류의 식물생약을 선택하였고 이 중에서 청호(Artemisiae apiaceae)로부터 산성당을 분리하여 pyridine과 chlorosulfonic산으로 sulfation 시켰율 때 in vitro상에서 항응고활성은 sulfation전에 비해 두드러지게 증가하였다. 농도를 달리 하여 실험동물에 투여시 응고시간의 연장 역시 비슷한 양상을 보여주었다. Thrombin 억제는 발견되지 않았지만 sulfate기와 항응고 활성과는 관계가 있는 것처럼 보였다.

Iliacus Muscle Rupture with Associated Partial Femoral Nerve Palsy during Soccer Game - Case Report - (축구 경기 중 발생한 장골근 파열과 부분 대퇴 신경 마비 - 증례보고 -)

  • Iliacus muscle tears are a rare injury seen after the high-energy trauma or as a result of low-energy injuries in patients with a bleeding diathesis as coagulopathy, receiving anticoagulation therapy and hemophiliac. Femoral nerve palsy due to compression from a hematoma by iliacus muscle rupture are rarely reported. Routine evaluation includes MRI to confirm and define the pathologic abnormality supplemented by EMG and nerve conduction studies to evaluate patterns and extent of femoral nerve injury. Hematologic evaluation for bleeding diathesis may preceded, if suspicion of coagulopathy is present. We report the case of a healthy 32-year-old male with iliacus rupture and concomitant femoral nerve palsy sustained by kicking motion during soccer game. After 6 months of observation with non-operative treatment regimen, satisfactory results were obtained, so we report it with a review of the literatures.

Effect of Seatangle Oligosaccharide Drink on Oxidation of Serum Lipid and Bleeding and Plasma Clotting Time in Rats Fed a Hyperlipidemic Diet (해조 올리고당 음료가 고지혈증 유도 쥐에 있어서 혈액 지질 산화 및 혈액ㆍ혈장 응고에 미치는 영향)

  • We investigated the effect of seatangle drink and seatangle extract on lipid oxidation, blood coagulation and intestinal movement in rats fed a hyperlipidemic diet. In the dietary hyperlipidemic induced group, the serum superoxide dismutase activity decreased and formation of hydroxy radical increased when compared to normal group, but these were controlled by seatangle drink treatment. The decreased of bleeding time and increased of tissue factor in the dietary hyperlipidemic rats were regulated by seatangle drink and seatangle extract, and especially the activity of tissue factor was remarkably decreased. Seatangle drink and seatangle extract were increased contraction on intestinal movement. Therefore, it can be concluded that seatangle drink or seatangle extract treatment depresses changing in absorption of gastrointestinal track in rats fed a hyperlipidemic diet.

Evaluation of the Anti-thrombosis Activities of the Aerial Parts of Sageretia thea (상동나무 지상부의 항혈전 활성)

  • Recently, thrombotic diseases have become rapidly more prevalent due to Westernized lifestyles and high-fat diets. In this study, the anti-thrombosis activities of the aerial parts of Sageretia thea were evaluated using ethanol extracts of the leaf (ST-L), branch (ST-B), and fruit (ST-F), and their anti-coagulation, platelet aggregation inhibition, and hemolytic toxicity were assessed. In comparison to the ST-F extract, the ST-B exhibited 6.7 times more polyphenol content, and the ST-L had 2.7 times more total flavonoid content. The ST-L and ST-B extracts showed stronger inhibitions of thrombin, prothrombin, and blood coagulation factors than aspirin, berry extracts, or commercial oriental herbs. Furthermore, ST-L and ST-B showed superior platelet aggregation-inhibitory activities than aspirin. The ST-F extract demonstrated only minor anti-thrombosis effects, and none of the extracts showed hemolysis against red blood cells up to 1 mg/ml. Phenolic acid and flavonoid analysis of the ST-L and ST-B extracts showed abundant rutin, isoquercitrin, and astragalin as the major active compounds. Further research on the anti-thrombotic activity of isoquercitrin, a rare flavonoid from quercetin, is necessary. This is the first report of isoquercitrin in Sageretia thea, and our results suggest that ST-L and ST-B extracts could therefore developed as anti-thrombosis agents.

Blood Compatibility of Polyurethane-poly(vinyl alcohol) Polymer Blends (폴리우레탄-폴리비닐알콜 블렌드의 혈액적합성)

  • The blood compatibilities of PU/PVA polymer blends with different mixing ratios were evaluated using various methods, such as fibrinogen adsorption, plasma recalcification time, platelet adhesion, whole blood clotting time, and complement activation. In addition, PVA on the surface of the polymer blends was crosslinked by glutaraldehyde to restrain the mobility of PVA molecules for characterizing the effect of PVA in the polymer blends on blood compatibility. The fibrinogen adsorption on the polymer blends decreased with the increase of PVA amount in the polymer blends. The plasma recalcification times of the polymer blends with 10-50 wt% PVA were longer than those of PU, PVA, and polymer blends with higher amount of PVA. The morphological changes and adhesion of platelets on the polymer blends with 30-50 wt% PVA were less than those on the other materials. The blood clotting times and complement activation on the polymer blends with 30-50 wt% PVA were reduced, compared to the other materials. On the other hand, the blood compatibility of the crosslinked polymer blends was relatively decreased, compared to the non-crosslinked ones. According to these experimental results, the blood compatibility of the polymer blends with 30-50 wt% PVA was better than that of the other materials and such a blood compatibility of the polymer blends might be related to the mobility of PVA molecules on the surface.

Polycythemia vera combined with coagulation disorder -A case report- (혈액응고 장애가 동반된 진성다혈증 1례)

  • We report a case of polycythemia vera combined with coagulation disorder. The patient was 54 years old man who complained of continuous bleeding after incision of skin abscess 20days ago. Laboratory tests were revealed prolonged aPTT and slightly prolonged PT. Coagulation factor, I, VIII, IX, XI and fibrinogen decreased, however FDP did not increased. It appears that patient with polycythemia vera have chronic activation of coagulation system, probably initiated by activation of factor XII. Platelet aggregation test to ADP, collagen, epinephrine was also revealed poor response.

Joint model of longitudinal data with informative observation time and competing risk (결시적 자료에서 관측 중단을 모형화하기 위해 사용되는 경쟁 위험의 적용과 결합 모형)

  • Longitudinal data often occur in prospective follow-up studies. Joint model for longitudinal data and failure time has been applied on several works. In this paper, we extend it to the case where longitudinal data involve informative observation time process as well as competing risks survival times. We use a likelihood approach and derive an EM algorithm to obtain maximum likelihood estimate of parameters. A suggested joint model allows us to make inferences for three components: longitudinal outcome, observation time process and competing risk failure time. In addition, we can test the association among these components. In this paper, liver cirrhosis patients' data is analyzed. The relationship between prothrombin times measured at irregular visiting times and drop outs is investigated with a joint model.
