Surgical Resection of the Pulmonary Mucormycosis in the Diabetic Patient -1 case report- (당뇨병환자에서 발생한 폐모균증의 외과적 절제)

  • 폐모균증은 드물지만 치명적인 기회감염성 진균증으로 혈액종양, 신부전, 또는 당뇨와 같이 면역기능이 저하된 환자에서 주로 발생한다. 환자는 3년전부터 당뇨병 및 갑상선기능 항진증을 앓아온 32세 남자로 1개월간의 기침, 발열 및 좌측 흉통을 주소로 내원하였다. 기관지 내시경하 경기관지 폐생검상 폐모균증 진단되어 좌폐하엽절제술을 시행하였다. 술후 21일째 흉부방사선 사진 소견상 다수의 국소적인 흉막삼출로 흉강경을 이용한 배약술을 시행하였다. 환자는 술후합병증이 없이 흉강경배액술후 23일째 퇴원하였다. 기왕의 문헌보고에 의하면, 한쪽폐에 국한된 모균증을 가진 수술가능한 환자에서는 조기에 과감한 수술적 처치가 생존율을 높이는 것으로 제안하고 있다.

Illness Experience of Adolescents with Hematologic Malignancies (혈액종양 청소년의 질병 경험)

  • Purpose: The purpose of this study was to describe the experience process of adolescents with hematologic malignancies. The question for the study was "What is the experience of adolescents with hematologic malignancies like?" Methods: The grounded theory methodology was used for this study. The data were collected through in-depth interview from 10 adolescents with hematologic malignancies. Data collection was done from January to June 2007. Theoretical sampling was used until the data reached saturation. Results: As a result of the analysis, "Reconstructing self-image from deviated and suspended life" was identified as the core category. And 11 subcategories were identified and they were integrated to the core category. 'Establishment of expanded and matured self' was identified as the consequence. Conclusion: The results of the study provide a frame for effective individualized nursing intervention strategies in helping adjustment of the adolescents with hematologic malignancies.

  • Purpose: The main purpose of this study was to observe the adaptation experience process of adolescents with hematologic malignancies. Methods: The Grounded Theory Method, developed by Strauss & Corbin (1998), was used in this study. The data were collected through in-depth. interviews with 10 adolescents with cancer. Data collection and analysis occurred concurrently. Theoretical sampling technique was used until the data reached saturation. Results: "A deviated life" was verified as the central phenomenon for adolescents with cancer. The adaptation experience process was divided into 3 steps: "Self-confusion", "Reinterpretation", and "Regeneration of self". Through these 3 steps, the adolescents with hematologic malignancies fitted a new life by self-integration. Conclusion: The results of this study provide a frame for individualized nursing intervention strategies in helping with the psychosocial adaptation of adolescents with hematologic malignancies.

  • 건강검진에서 혈압, 혈당과 더불어 빠지지 않는 항목이 바로 간 수치다. 흔히 간 수치가 높다 낮다 등으로 표현하는데, 정확한 의미를 아는 사람은 많지 않다. 간 수차(간 효소 수치)는 간이 얼마나 튼튼한지를 나타내는 척도다. 이 같은 간 수치를 알아보는 간 기능 검사는 1차적으로는 소변을 받고 혈액을 뽑으면 된다. 따라서 검사 전 금식은 필수다. 만약 1차 검사에서 이상이 발견되면 2차 혈액 검사를 진행하거나 종양표지자, 복부초음파, CT, MRI, 복강경, 복부혈관조영 등 다양한 방법으로 심층적인 검시를 진행한다. 간은 2/3가 손상돼도 정상기능을 유지하는 반면, 기능을 상실하기 전까지 이렇다 할 증상이 없어 질환을 알아차리기가 쉽지 않다. 그래서 간을 '침묵의 장기'라는 고약한 별칭이 따라붙는 이유다. 간 기능 검사를 통해 수시로 점검하는 길만이 최선이다.

Biodistribution and Scintigraphy of Iodine-131-Iododeoxyadenosine in Rats Bearing Breast Cancer (흰쥐에서 Iodine-131-Iododeoxyadenosine의 생체분포 및 유방암 영상화에 관한 연구)

  • Purpose: I-131 labeled (2'-deoxy-2'-iodo-${\beta}$-D-arabinofuranosyl) adenine (IAD) may be involved in DNA synthesis during active proliferation of tumor cells. We conducted this study to find out the biodistribution of IAD and it's feasibility for scintigraphic tumor imaging. Materials and Methods: Tosyl acetyl-adenosine was dissolved in acetonitrile, and I-131-NaI was added and heated to synthesize IAD. Female Fisher 344 rats innoculated with breast tumor cells were injected with 0.27 MBq of IAD. Rats were sacrificed at 0.5, 1, 2, 4, 24h and the % of injected dose per gram of tissue (%ID/g) was determined. For scintigraphy, rats bearing breast cancer were administered with 1.11 MBq of IAD and imaging was performed after 2 and 24h. Then, rat body was fixed and microtomized slice was placed on radiographic film for autoradiography. Results: %ID/g of tumor was 0.74 (0.5h),0.73 (1h), 0.55 (2h), 0.38 (4h), and 0.05 (24h), respectively. At 1h after injection, %ID/g of tumor was higher than that of heart (0.34), liver (0.42), spleen (0.47), kidney (0.69), muscle (0.14), bone (0.33) and intestine (0.51). However, %ID/g of tumor was lower than blood (1.06), lung (0.77), and thyroid (177.71). At 4h, %ID/g of tumor in comparison with other tissue did not change. Tumor contrast expressed by tumor to blood ratio was 0.69 and tumor to muscle ratio was 5.11 at 1h. However, these ratios did not improve through 24h. On autoradiogram and scintigraphy at 2 and 24 hour, the tumor was well visualized. Conclusion: This results suggest that IAD may have a potential for tumor scintigraphy. However, further work is needed to improve localization in tumor tissue.

  • Intraocular melanoma is the most common primary occular malignancy in adult above the age of 20. Before treatment of intraocular melanoma, it is essential to do complete work-ups including LFT (LDH, SGPT), chest X-ray, whole body bone scan. Also, liver-spleen and brain scan will be done if clinically indicated. Though, malignant melanoma is radioresistant, however, show rapid tumor shrinkage after irradiation with total tumor dose of 7,000 to 8,000 rads in 7-8 weeks.

  • Primary xanthoma of the bone is a rare benign neoplasm, and it is extremely rare to find this in the ribs. It is most commonly reported in soft tissue and it associated with hyperlipoproteinemia. A 54-years-old male who complained of left chest pain had an X-ray taken. It revealed a left 3rd rib tumor. The blood examinations for lipid and protein were normal. A resection was done for tissue examination. The mass was histolopathologically diagnosed as a xanthoma.

  • Thyroid cancer refers to various cancers arising from thyroid gland. Differentiated thyroid cancers (DTCs) include papillary, follicular, and Hurthle cell carcinomas and represent cancers retain normal thyroid functions such as iodine uptake. Radioactive iodine (RAI) is generally used for upfront treatment of metastatic DTCs, but RAI refractory DTCs remain to be clinical challenges. Sorafenib and lenvatinib were approved for the treatment of RAI refractory DTCs and more recently, genomics-based targeted therapies have been developed for NTRK and RET gene fusion-positive DTCs. Poorly differentiated and anaplastic thyroid cancers (ATCs) are extremely challenging diseases with aggressive courses. BRAF/MEK inhibition has been proven to be highly effective in BRAF V600E mutation-positive ATCs and immune checkpoint inhibitors have shown promising activities. Medullary thyroid cancers, which arise from parafollicular cells of thyroid, represent a unique subset of thyroid cancer and mainly driven by RET mutation. In addition to vandetanib and cabozantinib, highly specific RET inhibitors such as selpercatinib and pralsetinib have demonstrated impressive activity and are in clinical use.

Leukoencephalopathy after CNS Prophylactic Therapy in Pediatric Hematologic Malignancy (소아 혈액종양 환자에서 중추신경계 예방적 치료 후 발생한 백질뇌병증)

  • Purpose : Leukoencephalopathy(LE) is one of the most serious complications in children with hematologic malignancies during the course of treatment. Early recognition is important to reduce the impact and sequelae from LE. We therefore investigated the clinical features of LE following central nervous system(CNS) prophylaxis in children with hematologic malignancies and evaluated the significance of regular check-ups of brain MRI. Methods : We retrospectively reviewed children with hematologic malignancies who had CNS prophylaxis including intrathecal(IT) methotrexate(MTX) and/or cranial irradiation at the Department of Pediatrics, Kyungpook National University Hospital from Oct. 1995 to May 2002. Fifteen cases of acute leukemia and one case of lymphoma who experienced LE following CNS prophylaxis were included in the study. Clinical data were analyzed from the medical records and brain MRIs were reviewed by neuroradiologists. Results : The ages ranged from 1 to 13 years(median age=5.2 years), and the male to female ratio was 3 : 1. The time interval from the beginning of chemotherapy to the time of diagnosis of LE ranged from 2 to 17 months. They all had IT MTX two to 15 times and ten underwent cranial irradiation(1,800 rads). At the time of diagnosis, ten of them had neuropsychiatric symptoms including seizures, personality changes, headache, etc. After the change of treatment modality, four cases showed significant improvement on follow-up MRIs, six cases had no significant changes and two had worsening of LE. Four patients died of infection and bone marrow relapse. Conclusion : CNS prophylaxis with IT therapy and cranial irradiation may cause leukoencephalopathy during the course of treatment. As a result, regular brain MRI check-up is recommended for the early detection and reducing the incidence of LE, along with changes in the treatment modality.
