Left Ventricular Inflow Obstruction Caused by a Persistent Left Superior Vena Cava and a Dilated Coronary Sinus - A case report - (관상정맥동 확장에 의한 좌심실 유입로 폐쇄 - 1예 보고 -)

  • Left ventricular inflow obstruction can be caused by a persistent left superior vena cava (SVC) and a dilated coronary sinus. A 31-day-old male infant with secondum atrial septal defect (ASD) and bilateral SVC underwent an operation for treating his uncontrollable congestive heart failure. The preoperative 2-dimensional echocardiography showed a normally sized mitral valve shrouded by a dilated coronary sinus. The operation consisted of pericardial patch closure of the ASD, coronary sinus unroofing and left SVC transfer to the right atrial auricle. The postoperative course was complicated by persistent chylothorax, which was controlled by thoracic duct ligation, He was discharged to home at the postoperative day 39. He has been followed up for 9 months and has displayed normal development.

Partial Left Ventriculectomy in the Pediatric Patient with Dilated Cardiomyopathy (확장성 심근증 환아에서의 부분 심실 절제술의 적용 -1례 보고-)

  • Heart transplantation was planned for a 10-year old boy who had dilated cardiomyopathy with severe congestive heart failure and had been on dopamine for 1month. However, partial left ventriculectomy and mitral annuloplasty were performed instead, because there was no donor heart of the adequate size and the symptoms were aggravated. The clinical symptoms were markedly improved after the surgery. Comparing the postoperative echocardiographic results with the preoperative results, there were remarkable changes in the left ventricular ejection fraction(preoperative LV EF 17% to postoperative 3 months 29%, 6 months 35%, 1 year 36%) and the left ventricular end-diastolic dimension(preoperative 72 mm to postoperative 3 months 59 mm, 6 months 61 mm, 1 year 61 mm). Partial left ventriculectomy and mitral annuloplasty reduced the cardiac loading in the dilated cardiomyopathy. Partial left ventriculectomy and mitral annuloplasty may be considered as one of the alternative surgical metho s to carry over until a heart transplantation can be performed, especially for children.

  • We experienced tracheobronchial stenosis caused by malrotation of the heart in a 3-year-old girl. Malrotation of the heart is induced by the decreased right lung volume, which was the result of right lung hypoplasia and herniation of the left thoracic cavity. We corrected the right lung volume and location of the heart to treat tracheobronchial stenosis.

  • Pleomorphic hyalinizing angiectatic tumor (PHAT) is a rare, low grade neoplasm which usually occurs in subcutaneous soft tissue. Histologically, it features ectatic blood filled vessels surrounded by hyalinized stroma and pleomorphic spindle cells. Clinically and histologically, PHAT could be misdiagnosed as such tumor as neurogenic tumor, malignant fibrous histiocytoma. About 100 cases of PHAT have been reported so far, and it is even rarer in head and neck area. We experienced a case of PHAT in 41-year-old male with several months of history of gradually enlarging neck mass which was surgically removed by wide excision.

  • 병변의 양상에 따른 감별진단 재발성 이하선 종대를 보이는 경우 타액선조영술이 영상진단방법중 1차적인 선택이 된다. Stensen 씨관이나 중심선관(central glandular duct)의 확장을 보이는 경우 만성 타액선염으로 진단할 수 있다. 소아에서의 단일성 양성종괴의 감별진단은 임파절, 혈관종, 양성혼합종양, 저급점액상피암, 임파관종 등이다. 성인에서의 단일성, 양성으로 관찰되는 종괴는 양성혼합종양, Warthin씨 종양, 저급점액상피암, 선상낭성암, 소포상세포암 등이다. 여러개의 종괴를 보이는 경우 Warthin씨 종양, 소포상세포암, 임파종, 육아종, 전이암 등이며 단일성 낭성종괴의 경우는 branchial cleft cyst,, Warthin씨 종양, 상피낭포 등이다.

Clinical evaluation of bronchiectasis (기관지확장증의 임상적 고찰)

  • We experienced 48 operations in 46 surgical patients of bronchiectasis admitted to the Department of Thoracic and Cardiovascular Surgery of Jeonbug National University Hospital from January, 1975 to August, 1982. Among 46 patients, 27 patients [59%] was age group between 21 to 30 years. Common symptoms were cough with sputum, hemoptysis, dyspnea, fever and chilliness, and chest pain. The duration of the symptoms was variable between below one year and above 10 years. The most frequent associated disease, probably the cause of the bronchiectasis, was secondary bacterial infection after viral infection. The left lower lobe and lingular segment was involved most frequently, and the most frequent pathologic type was mixed type [40%]. Single lobectomy, and combined lobectomy and segmentectomy were performed in 77% of the patients. Bilateral resection was performed in three patients with good result. In those patients, the isolated pulmonary function test on each side of the lung performed 2 month later primary lung resection could make them be prevented from pulmonary insufficiency after secondary lung resection. The results were good except two patients who developed pulmonary insufficiency and chronic empyema with bronchopleural fistula.

  • During the past fifteen and one half years, a total of 64 cases of bronchiectasis were treated by pulmonary resection. The diagnosis of bronchiectasis was made relatively easily and accurately with bronchography. The average age of onset in the present series was 21 years old. Preoperative bronchograms were taken in all cases and revealed involvement of the left lower lobe in 14 cases, the right lower lobe in one case, the right upper lobe in 3 cases, the left lower lobe anal lingular segment in 9 cases, and multilobar involvement in cases. Bronchographically 18 tubular and II saccular and 2 cystic and 2 saccular and tubular forms were noted. Various types of pulmonary resection were performed on 59 cases. Complication developed in two cases. One showed pulmonary vein bleeding and was treated completely by reoperation; the other case developed empyema.

  • This study based on the clinical analysis of 42 cases of bronchiectasis treated by surgical resection at the Department of Thoracic and Cardiovascular Surgery, Chosun University Hospital, during 6 years period from 1986 to 1991. The 42 patients with bronchiectasis were composed of 33 males and 9 females, and the peak incidence of age was in the 3rd decade. The clinical symptoms were cough, purulent sputum, hemoptysis and dyspnea. Based on the morphological classification by bronchograpgy, the common type was present in 19 cases[45%] of cylindrical type. The frequently involved site was left lower lobe, and so left lower lobectomy was the most frequently performed method of surgical resection. Postoperative complications were occurred in 8 cases[18%], such as wound infection, atelectasis, pleural effusion, hemothorax, but there were no fatal cases.

Surgical Angioplasty of Isolated Left Coronary Ostial Stenosis (좌관상동맥 입구협착의 외과적 확장술)

  • The isolated coronary ostial stenosis is rare and a critical lesion which requires urgent surgical intervention. Recently direct angioplasty is assumed as a preferable approach to conventional bypass grafting. From Mar. 1990 to Aug. 1993, six patients underwent direct angioplasty in Sejong Heart Institute. The mean age of 6 patients was 48 years [range 37 to 63 and they consisted with 5 females and one male. All had severe angina [class III or IV of short duration [mean 5.3 months and a low incidence of risk factors. Despite the crucial location of the lesion, most patients had well preserved left ventricular function and normal wall motion. We performed direct angioplasty with autologous pericardium via anterior approach except one patient who underwent direct angioplasty and CABG. One patient died 4 hours after angioplasty probably due to acute coronary dissection. The survived 5 patients maintain normal life without symptoms during 26.2 months follow up [range 5 to 47 months .Our preliminary results suggest that angioplasty of isolated coronary ostial stenosis in highly selected patient can be carried out with good results and relatively low operative risks.

  • Postpneumonectomy syndrome is a rare condition characterized by dyspnea due to an extreme mediastinal shift and bronchial compression of the residual lung after a right pneumonectomy or a left pneumonectomy with the right aortic arch, Severe fibrosis of the lung such as tuberculsosis (TB) - destroyed lung can cause similar clinical features in the absence of pneumonectomy. We report a unique case of postpneumonectomy syndrome without pneumonectomy, which was successfully treated with pneumonectomy and mediastinal repositioning with tissue expanders.
