The purposed of this study was to identify cardiocerebrovascular disease risk assessment and the factors that affect cardiocerebrovascular disease risk. The subjects of this study were 1340 middle age women worker in one shipyard. This study was base on worker health check up data. Data were analyzed using SPSS18.0 program. "healthy group" were 1078(80.4%), "low risk group" were 45(3.4%), "moderate risk group" were 150(11.2%), "high risk group" were 67(5.0%). Age, HDL, LDL, smoking, family history of stroke, hypertension were the factors that affected the risk cardiocerebrovascular disease. Results of this study can be used as fundamental data for developing cardiocerebrovascular disease preventive program in middle aged women.
We have experienced 25 cases of aortic aneurysm from October 1987 to January 1996. Patients ranged in age from 26yrs to 73yrs(mean age 52. Syrs). There were 13 males and 12 females. Eighteen cases were thoracic aneurysm and seven were abdom nal aneurysm. The cause of aneurysm were dissecting in 16cases aneurysms and non-dissecting in 9 cases. Risk factors of aortic aneurysm were hypertension, hypercholesterolemia, Marfan's syndrome. In thoracic aneurysm patients, 1'S cases of dissecting aneurysm underwent aneurysmectomy and replacement of vessel interposition graft with or without coronary artery implantation on the graft. 6 cases of non-dissecting aneurysm underwent operation with same policy as dissecting aneurysm. In 7 case of abdominal aneurysm,all patients underwent aneurysmectomy and graft interposition with straight i)r Y graft. Thcre were 5 postoperative death(mortality 20%). Several cases of complications were improved with proper managements. All survivors showed improvement in clinical symptom and sign and discharged without specific complications.
Arteriosclerosis is a disease in which blood circulation is impaired due to loss of elasticity as blood vessels become narrower, and is a potential cause of recently increasing cardiovascular and cerebrovascular diseases. Carotid ultrasound is used as a predictor of cardiovascular and cerebrovascular diseases by evaluating the degree of atherosclerosis. Therefore, this paper attempted to investigate the correlation between the increase in the thickness of the inner media and various clinical variables in carotid ultrasound. Patients with carotid ultrasound findings were classified into three stages: mild intima thickening, sclerosis, and significant stenosis. CAVI (Carotid Ankle Vascular Index: vascular age) data measured in the degree and physical characteristics of the carotid artery(gender, age, body mass index, blood pressure), blood test(total cholesterol, triglycerides, high-density lipoprotein, low-density lipoprotein, creatine phosphokinase, fasting blood sugar), and arteriosclerosis test were collected. It was confirmed that the carotid intima thickness was correlated with variables such as hypertension, hyperlipidemia, and BMI, and also correlated with the risk factors of cardiovascular disease as CAVI increased.
This study aims to identify factors to affect regular utilization status of medical care in cardio-cerebrovascular patients. The research selected 770 cardio-cerebrovascular patients among surveyees from the Korea Health Panel 2010. We analyzed states of medical care utilization using descriptive statistics. Logistic regression analysis was used to examine the main factors associated with regular utilization status of medical care in cardio-cerebrovascular patients. In result, the significant factors associated with regular utilization status of medical care in cardio-cerebrovascular patients were age, education level, household income level. CCI, presence or absence of high risk drinking, and presence or absence of obesity. There's a high probability that patients aged between 60 and 69, equal to and higher than those of high school graduate in education level, upper middle class in household income, the higher CCI, absence of high risk drinking, presence of obesity utilize medical care services more regularly. Therefore, it is necessary to develop effective program and individualized approach for patients using lesser periodical medical care and patients with high risk drinking problem. In the future, these findings can be used an important data for healthcare policy and assessment.
Background: Major vascular injuries can jeopardize a patient's life or imperil limb survival. We performed this study to establish an optimal management plan for vascular injuries. Material and Method: We retrospectively reviewed 26 cases of vascular injury that were treated at Pusan National University Hospital from May, 1999 to September, 2004. The age and sex distribution, the locations and causes of vascular injury, the diagnostic tools, the degree of injuries, clinical manifestations, the treatment modality and complications were reviewed. Result: The mean age was 39.5 years (range: $12{\sim}86$) and the male to female ratio was 22 : 4. The injuries were in 6 descending thoracic aortas, 4 femoral arteries, 4 popliteal veins and so on. The causes of injury were iatrogenic in 8 cases, traffic accident in 7, stab injury in 6 and industrial accident in 5. The most commonly used diagnostic tools were CT and angiography. The degrees of arterial injury were pseudoaneurysm in 10 cases, partial severance in 5, complete severance in 3 and thrombosis in 3. The degrees of venous injury were partial severance in 6 cases, complete severance in 2 and arteriovenous fistula in 2. The clinical manifestations were absence of pulse in 8 cases, coldness in 7, chest pain in 6, swelling in 5, bleeding in 5 and so on. The most frequently used type of revascularization was graft interposition in 11 cases. Two arteriovenous fistulae were repaired by endovascular procedure. There was one case of mortality due to multi-organ failure after hemorrhagic shock, There were three major amputations, and two of them were due to delayed diagnosis and treatment. Conclusion: A system for the early diagnosis and treatment is essential for improving limb salvage and patient mortality. As a consequence of the widespread application of endovascular procedures, the incidence of iatrogenic injuries has recently increased. Educating physicians is important for the prevention of iatrogenic injury. Easy communication and cooperation for earlier involvement of a vascular surgeon is also an important factor.
저자들은 1977년부터 1986년까지 10년동안 서울대학병원 이비인후과에서 유년기성 비인강 혈관섬유종의 진단하에 치료받았던 27명의 환자에 대해 임상적 조사를 시행하여 다음과 같은 결과를 얻었다. 1) 남녀비율은 전례에서 남성이었으며 연령은 9세∼22세의 분포를 보였고 16∼17세가 가장 많았다. 2) 주증상으로는 전례에서 비출혈을 동반하는 비폐색증을 보였으며 편측인 례가 7례였다. 3) 종양의 크기는 21례에서 비인강 및 비강내에 국한되어 있었고, 4례에서 익상상악와, 2례에서 두개내 침습을 보였다. 4) 종양의 혈액공급은 동측의 내상악동맥에서 이루어지는 경우가 27례, 동측의 상행인두동맥이 13례, 양측에서 이루어지는 경우가 9례 있었다. 5) 치료는 23례에서 수술적 요법을, 4례에서 방사선치료를 시행하였다. 수술적 요법 23례중 11례 에서 경반구개제거술(hemipalatal approach), 8례에서 경전구개제거술(transpalatal appoach), 2례에서 경전구개 및 경상악동제거술(transantral approach), 2례에서 외비절제술(lateral rhinotomy approach)에 의해서 종양을 적출하였다.
The changes in the blood flow in the peripheral vascular system under strong pulsed magnetic fields (pMF) were studied by digital infrared thermal imaging (DITI). After pMF stimulus temperatures in stimulated area were commonly increased in both groups of age and gender. In order to reduce heat generated from coil in pMF stimulus system plastic moldings were fabricated, so that certain distance was kept between stimulus system and the skin and to prevent direct contact to the skin. It is believed that skin temperature is increased by internal electromagnetic energy stimulated the peripheral vascular system by non-contact method.
Congenital esophago-respiratory fistulae in adults have rarely been reported. Moreover, most of those are the cases of bronchoesophageal fistulae, that is to say esophago-lower respiratory fistulae. We experienced case of a congenital tracheoesophageal fistula in an adult, not a bronchoesophageal fistula. At our hospital, a 20-year-old male with recurrent episodes of a paroxysmal(especially postprandial) cough, respiratory infection and relative growth retardation had been diagnosed by using esophagography and esphagoscopy as having a congenital tracheoesophageal fistula with a concomittant esophageal diverticulum. The surgical correction was done successfully. We are excited to report a case of a congenital tracheoesophageal fistula in an adult, which is believed to be the first case of its kind in Korea.
Primary cardiac tumors are rare disease and they present nonspecific symptom. They are divided in benign and malignant tumors and require surgical therapy and/or additional therapy. From March 1995 to March 2003, twenty one patients were diagnosed as having primary cardiac tumors. We analysed them retrospectively in terms of various perioperative factors and early and late results. 6 men and 15 women and their average age was 45.44$\pm$18.76. Pathology revealed eighteen benign (fourteen myxoma, two fibroelastoma, one hemangioma and one paraganglioma) disease and three malignant (one angiosarcoma, one mesothelioma and one myxofibrosarcoma) disease. There was one (myxoma) operative mortality and three late death (hemangioma, angiosarcoma and mesothelioma) during additional therapy and follow up. Surgical treatment of primary cardiac tumor is important and sometimes additional therapy is required but the prognosis of malignant cardiac tumor is still very poor.

Recently the prevalence rate of diabetes in Korea has been increasing rapidly due to high growth of economy and changes in dietary lifestyle. Vascular complication is one of diabetic complications which have frequently occurred by obesity, hyperglycemia and impaired glucose metabolism. Photoplethysmogram(PPG) measured from finger and toe is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, peripheral vascular compliance study for diabetic patient was never evaluated by using second derivative of PPG. Therefore, we aimed to objectively compare and to assess normal(n=850) and diabetic(n=50) groups vascular compliance using the second derivative of PPG waveform in this study. The evaluated factors of the second derivative of PPG are 'a', 'b', 'c', 'd', 'e' and b/a rapresents vascular compliance. This study found out that when vascular compliance is decreased, the absolute value of b/a is decreased. The average vascular compliance of 50 diabetic patients with neuropathy, $-1.09{\pm}0.14$ is statistically lower than the normal group, $-0.81{\pm}0.09$ (p<0.05). In conclusion, we suggest an objective evaluation of peripheral vascular compliance for diabetic patients and prevention of vascular complication.
