Determination of Weight of environmental/ecological assessment Factors of Environmental Conservation Value Assessment Map (ECVAM) in Korea using Artificial Neural Network and GIS (인공신경망 및 GIS를 이용한 국토환경성평가지도 환경.생태적항목 가중치 분석)

  • 본 연구의 목적은 인공신경망 기법을 이용하여 2006년 전국을 대상으로 구축된 국토환경성평가지도의 환경 생태적항목에 대한 각 항목별 가중치를 결정하는 것이다. 기본 분석 도구로 지리정보시스템(GIS)가 사용되었다. 국토환경성평가지도의 환경 생태적항목은 다양성(생태자연도), 자연성(임상도, 녹지자연도, 생태자연도), 풍부도(생태계변화관찰 지역도), 희귀성(생태자연도), 허약성(수치지형도, 토지피복도), 군집구조의 안정성(임상도)등이 활용되어 구축되었다. 본 연구는 기 구축되어 사용되고 있는 국토환경성평가 지도의 환경 생태적항목을 공간 데이터베이스를 이용하고, 인공신경망 기법을 적용하여 각 평가항목간의 상대적 가중치를 구하였다. 인공신경망의 훈련 지역은 환경 생태적항목중 환경성이 높은 1등급 지역 및 환경성이 낮은 5등급 지역을 추출하였다. 그 결과 50번의 가중치를 산정하였을 경우 허약성이 다른 항목들에 비해 1.58배 정도 높은 상대적 가중치를 나타냈다. 이러한 가중치는 국토환경성평가지도 환경 생태적 항목의 취약성도를 작성하는데 활용될 수 있다.

Frailty and its related Factors in Vulnerable Elderly Population by Age Groups (취약계층 노인의 연령별 허약정도와 관련 요인)

  • Purpose: This study aimed to investigate factors affecting frailty by age groups among vulnerable elders in Korea. Methods: In this secondary analysis, data were collected from records for 22,868 elders registered in the Visiting Health Management program of Public Health Centers in 2012. Health behaviors, clinically diagnosed disease, frailty, depression and cognitive condition were assessed. Data were analyzed using stepwise regression to determine the associated factors of frailty by age group. Results: Alcohol consumption, physical activity, number of diseases, DM, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors significantly associated with frailty among the elders aged 65~74 (F=135.66, p <.001). Alcohol consumption, physical activity, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors associated with frailty in the elders aged 75~84 (F=245.40, p <.001). Physical activity, CVA, arthritis, depression and cognitive condition were factors associated with frailty in the elders over 85 years of age (F=96.48, p <.001). Conclusion: The findings show that frailty of elders and associated factors were different by age group, and common factors affecting frailty were physical activity, CVA, arthritis, depression and cognitive condition. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to age group.

Quality of Life, Frailty and Depression in Elderly in Rural Area (일부 농촌 지역 노인의 허약수준, 우울, 건강 관련 삶의 질)

  • Purpose: The purpose of this study is to identify health-related factors, especially for the elderly who are subject to visiting health care at vulnerable populations. Methods: Tools were Guide to Community Integrated Health Promotion Project 2016, Visit Health Care Health Interview Survey, measures of physical function, motor skills, composite mobility, BMI, and subjective fitness levels. Depression was measured with the Short Results: Older elders living alone were more vulnerable than those with living others. Elders with less education showed greater weakness but the difference was not significant. Average scores for frailty were 2.21 (healthy group), 7.66 (high-risk group) and 15.69 (frail group). Scores based on weakness level differed significantly with the exception of nutrition. Nine out of 10 elders in disadvantaged areas were in the frail group or at high risk. Conclusion: Results support the goal to maintain/improve physical/mental functions through individual management of high-risk/frail older adults at risk of becoming infirm. It is imperative to implement a public health care delivery system to ensure programs are operated effectively and personalized.

The Evaluation of Feasibility and Predictive Validity of Comprehensive Korean Frailty Instrument: Using the 2008 and 2011 Living Profiles of Older People Survey in Korea (포괄적 한국 노인 허약사정 도구의 적용가능성과 예측타당도 평가: 2008, 2011년 노인실태조사 자료 이용)

  • Purpose: This study aimed to verify the predictive validity of Comprehensive Korean Frailty Instrument (CKFI) among older adults. Methods: A secondary analysis of data from a prospective cohort study was conducted. Frailty was determined in older adults (N=9,188) according to the data in 2008 and the effects of frailty on adverse outcomes (such as institutionalization and death) were evaluated according to the data in 2011. The Cardiovascular Health Study (CHS) index was used to compare with the predictive validity of CKFI. Results: The prevalence of frailty was 26.3%. With the CKFI, the frail group had a higher risk of negative health outcomes compared to the robust and pre-frail groups after three years. The two of the highest risks identified using the CKFI and CHS index were institutionalization (5.522 times higher) and mortality (3.210 times higher). For both instruments, the survival analysis revealed that the risk of death increased as the degree of frailty increased. Conclusion: The CKFI consisting of self-report items and multidimensional aspects of frailty can be used as a simple instrument for assessing the frailty of older adults residing in a local community in Korea.

Educational needs for the development of a simulation module of home visiting care for the frail elderly (시뮬레이션 기반 허약노인 방문간호 교육 요구도)

  • Purpose: This study aimed to identify the educational needs of nurses and nursing students for the development of a simulation module of home visiting care for frail, elderly people. Methods: Focus group interviews were conducted with 15 home visiting nurses working in public health centers and 14 nursing students who experienced home visiting from September 10 to October 10, 2018. Results: Bloom's taxonomy of learning objectives, namely, cognitive, affective, and psychomotor domains was used as a framework for data analysis. The defined educational needs for each domain were as follows: "understanding frail, elderly people" for the cognitive domain; "intervention for mental health" and "building a therapeutic relationship" for the affective domain; and "nursing skills", "health education for healthy lifestyles", "referral to the community resource connection", "protection for visiting nurses" for the psychomotor domain. Conclusion: Based on the findings of this study, a simulation module of home visiting care for frail, elderly people can be developed and used for nursing students and nurses to strengthen the capacity for home visiting care.

Community Resource Linkage to Revitalize Frailty Prevention Programs for Vulnerable Seniors: Persons Receiving Care from Living Support Workers in the Elderly Customized Care Project (취약계층 노인의 허약예방 프로그램 활성화를 위한 지역사회자원연계 사례: 노인맞춤돌봄서비스 생활지원사의 돌봄대상자)

  • Purpose: This study evaluates the effectiveness of providing frailty prevention services by living support workers through a case of community resource connection centered on living support workers to revitalize frailty prevention programs for vulnerable elderly people. Methods: This is a research study using secondary data from a neighborhood health-sharing project among the integrated health promotion projects of one public health center in Daegu Metropolitan City. To assess frailty effects pre-assessments were conducted in August, and post-assessments were conducted in November. Frailty was measured using a 20-item frailty instrument used in home healthcare projects. Data were analyzed using the chi-square, independent t-test, and paired t-test. Results: Preliminary measurements showed that older elderly had higher frailty scores than younger elderly. However, among the elderly aged 75 or older the total frailty score decreased statistically significantly from 5.97 points to 5.30 points (t=3.03, p=.003). Conclusion: The older elderly showed greater effect of frailty prevention than the younger elderly.

The effects of a personalized nutrition intervention program on food security, health and nutritional status of low-income older adults in Seoul city (서울시 일부 취약계층 노인에서 맞춤형 영양중재 프로그램에 따른 식품안정성 확보 및 건강·영양상태 개선 효과)

  • Purpose: This study was conducted to assess the effects of a personalized nutritional intervention program on food security and health and nutritional status of elderly people in the city of Seoul. Methods: A total of 372 elderly adults aged 65 years or above who resided in Seoul were enrolled in this study. Personalized supplementary food supply and nutritional education based on chronic disease status, disability and cooking ability were implemented for 4 months. To evaluate the effectiveness of the program, nutrient intake, food security status, anemia status, chronic disease management, and frailty status, and prevalence of malnutrition (Mini Nutritional Assessment) were examined. Results: After the program, all subjects displayed significantly increased nutrient intake. Before the intervention, all subjects were in a state of food insecurity; however, after the intervention, 37.1% of the subjects were food secure. Moreover, the rates of being at risk of malnutrition and malnutrition in subjects were decreased and instead rate of those who improved to normal increased to 29.8% from 0% of normal rate before the prevention. The rate of subjects without anemia increased from 18.7% to 28.5% after the intervention. In addition, the rate of subjects with intensive or periodic management of chronic diseases decreased, while those with occasional management of chronic diseases increased from 0% to 4.6%. Furthermore, the rates of being at risk of frailty and frailty were decreased and the normal rate increased from 0% to 9.7% instead. Age group-based analysis showed that elderly people over 80 years showed less improvement in the management of the chronic disease status and the frailty status. Conclusion: Personalized supplementary food supplies and nutritional education improved not only the nutritional status but also disease status in vulnerable older adults, and the effects were more significant in adults aged less than 80 years.

Relations among Fall Efficacy, Perception of Fall Risk and Fall Prevention Behavior in the Frail Elderly at Home (재가 허약노인의 낙상효능감, 낙상위험지각 및 낙상예방행위와의 관계)

  • The purpose of this study is to identify the relationship between falls efficacy, perception of fall risk and fall prevention behavior in frail elderly. The data was collected from 300 elderly through the questionnaire. 1)the mean score of fall efficacy was $34.3{\pm}11.5$, perception of fall risk was $34.3{\pm}11.5$, and fall prevention behavior was $17.7{\pm}2.75$. 2)Fall efficacy was significantly different according to falls experience(t=2.905, p<.05). Perception of fall risk was significantly different according to falls experience (t=2.704, p<.05) and Fall prevention behavior was significantly different according to falls experience (t=2.706, p<.05). 3) It showed the negative relation between fall efficacy and perception of fall risk. It showed the positive relation between falls efficacy and falls prevention behaviors, and falls prevention behaviors and perception of fall risk. According to this study, it showed relation between fall efficacy, perception of fall risk and falls prevention behaviors. Thus it is highly recommended to include reinforce of efficacy and perception of fall risk in an effective fall-prevention program.

Development and evaluation of physical activity promotion program in the resident elderly (재가노인을 위한 신체활동증진 프로그램의 개발 및 효과)

  • The purpose of this study was to identify the effects of a visiting exercise program for physical activity promotion on physical (grip strength, static balance) and psychological (depression, perceived health status) function of frail elderly patients receiving home visiting service care. Subjects included a total of 49 patients; 24 in the control group and 25 in the experimental group. The visiting exercise program was carried out 16 times for 8 weeks. The collected data were analyzed by ${\chi}^2-test$ and t-test using SPSS/WIN 21.0 program. There was a significant improvement in the left grip strength (t=2.19, p<.000), right grip strength (t=5.07, p=.032), static balance (t=3.98, p=.050), and depression (t=-8.83, p=.009) in the experimental group compared with the control group. The visiting exercise program for physical activity promotion was effective on physical and psychological functions for frail elderly patients receiving home visiting service care. Further research is needed in a longitudinal study incorporating various centers to examine the maintenance of such effects.

Factors Influencing the Health-Related Quality of Life by Age among Vulnerable Elderly Women (저소득 여성노인의 연령별 건강관련 삶의 질에 영향을 미치는 요인)

  • This study was performed to investigate the factors which impact on the health-related quality of life of young-old(65-69 yr), old-old(70-79 yr), and oldest-old(80 yr or above) women in vulnerable elderly received home care service from public health centers in B city. The data were collected from 383 elderly women using structured questionnaires from September to November, 2010. Multiple regression with the SPSS WIN 18.0 program were used to analyze the data. There were statistically significant differences among young-old, old-old, and oldest-old women regarding the health-related quality of life, life satisfaction, cognitive function, frail condition. The models including life satisfaction, frail condition, cognitive function, perceived health status, number of chronic diseases were explained variance of the health-related quality of life elderly women differently like 42.8% of young-old, 28.9% of old-old, and 31.5% of oldest-old. Finally, frail condition and life satisfaction were predictors in explaining the level of health-related quality of life among vulnerable old women regardless of age. Based on the findings of the study, health promotion programs should be developed to improve health-related quality of life of vulnerable aged women according to age differences.
