Objectives The purpose of this study is to improve the accessibility of Korean medicine by standardizing managements, improving quality of medical services, and reducing medical costs in ankle sprain by develop clinical pathway (CP). Methods The development of CP in this study is based on clinical practice guideline (CPG) for ankle sprain, and aims to maximize the quality of treatment, such as reducing treatment time and medical costs, and increasing patient satisfaction through standardized pathway. The CP was revised after consultation and review by the advisory committee. The advisory committee is consisted of a stakeholder group applying the CP. Results In previous research studies, there were no Korean medicine CP studies on ankle sprain. Based on CPG for ankle sprain and analysis of medical records, 6 types of time task matrix type CP (for Korean medicine doctors, medical assistant, patients) and 4 types of algorithm type CP (for Korean medicine clinics, Korean medicine hospitals, and cooperative practicing hospitals, public medical centers) were derived as a result. Conclusions Ankle sprain CP is expected to not only increase patient satisfaction and maximize the quality of treatment, but also reduce the financial burden of health insurance by reducing medical costs.

한국의 전통의학은 유구한 역사를 가지고 있지만, 20세기초 일제의 강제합병과 서구문물의 무분별한 유입과정을 거치면서 단절된 역사를 가지고 있다. 명칭에 관해서도 원래 의학(醫學)으로 통칭되던 것이 동의학(東醫學), 한의학(漢醫學)으로 불리기도 하였고 1985년에는 한국적인 특색을 살린 '한의학(韓醫學)'으로 정식 개칭되어 지금까지 사용되고 있다. 본고는 일제시대이후 한의학의 변천과정을 조선말일제강점기이후부터 한의학의 변천을 시대별로 중요사안별로 정리한 연구이며, 현대 한의학의 교육과정의 일람을 통해 현재 한의학의 특색을 규명하였다.

This study is to show the clinical effect of additional CP Pharmacopuncture on hypoesthesia of lower limb which was occurred by laminectomy. The patients were treated using CP Pharmacopuncture, and other treatments including acupunture, moxibustion and herbal medication. The effect of treatment was measured by Numeric Rating Scale(NRS) and American Spinal Injury Association(ASIA) scale. After treatment, NRS was gradully reduced and ASIA scale's score was incresed. These results suggest that CP Pharmacopuncture might be a possible therapeutic option for hypoesthesia of lower limb which was occurred by laminectomy.

In the field of Korean medicine (KM), KM terms has been mixed used for a long period. After the Society of KM had systematically organized the KM terms used in our country, the book for the Korean medical standard terminologies was published. In this study, we developed an online service based on the book. The online service provides various useful functions for searching the terms and their commentary and collecting users' opinions. Users can also compare the standard terms with their semantic search results and naver's knowledge search results within our system. By analyzing logs of our system, we suggested several directions for future development, including the improvement of search functions, advertisements, and the provision of open APIs. In the future, our online service might provide the important information on clinics, academic researches, and international standards.

의료 분야의 정보화 움직임으로 인해 단순한 정보저장과 검색시스템에서 벗어나 지능화된 서비스를 제공해주는 시멘틱 웹 기반의 의료 시스템이 요구되고 있다. 이에 한의학 분야도 한의사의 진단을 보조할 수 있는 지식기반시스템에 대한 요구가 증대되고 있다. 온톨로지는 시멘틱 웹의 핵심적인 지식체계로 지식의 처리와 추론이 가능하므로 질의 및 논리 추론을 통하여 진단을 내리는 한의학 지식 데이터베이스 구축에 적합하다. 따라서 본 연구에서는 한의학 진단의 기반이 되는 환자의 질병, 증상, 원인, 처방 등의 관계를 온톨로지로 구축하여 한의학 분야의 지식을 표현하기 위한 기본 지식 체계를 제시하고 구축된 한의학 온톨로지를 기반으로 한의학 진단보조시스템을 개발한다. 구축 된 시스템은 한의사 진단을 보조하여 줄뿐만 아니라 한의학의 진단 결과를 객관적으로 점검할 수 있게 해준다. 또한, 진단보조시스템과 온톨로지 관리시스템을 결합함으로써 컴퓨터 비전문가에 의한 한의학 온톨로지의 확장 및 수정이 가능하도록 한다.

Objectives : Ukgan-san plus Citri Pericarpium and Pinelliae Rhizoma (UCP) is used as a traditional herbal formula in Korea and Japan for treatment of fever, fever-induced convulsions, and liver dysfunction and so on. In this study, we investigated the cytoprotective effect and underlying mechanism of UCP against oxidative stress induced by cotreatment of arachidonic acid (AA) and iron. Methods : To evaluate the hepatoprotective effects of UCP against AA + iron-induced oxidative stress in HepG2 cell, cell viability and changes on apoptosis-related proteins were assessed by MTT and immunoblot analyses. The changes in intracellular reactive oxygen species (ROS), glutathione (GSH), and mitochondrial membrane permeability (MMP) were investigated against to the oxidative stress. Furthermore, to verify underlying molecular mechanism, NF-E2-related factor 2 (Nrf2) and its downstream target genes were examined by immunoblot analysis. Results : Treatment of UCP increased the cell viability and altered the expression levels of apoptosis-related proteins such as PARP, caspase-9, caspase-3, Bcl-2. UCP also inhibited the GSH depletion, excessive ROS production and mitochondrial dysfunction induced by AA + iron. In addition, the Nrf2 and the Nrf2 target genes activation were increased by UCP. Conclusions : These results indicated that UCP has the ability to protect against oxidative stress-induced hepatocyte damage, which may be mediated with Nrf2 pathway.

Background: The aim of this study was to report the effect of acupotomy for patients with headache. Methods: Four patients with headache were treated with acupotomy. The improvement of symptoms was evaluated by numeric rating scale(NRS), Headache impact test-6(HIT-6) and headache attack frequency. Results: The numeric rating scale score and headache attack frequency decreased throughout the treatment period. No side effects were observed during treatment. Conclusion: Acupotomy had shown a positive clinical effect in the treatment of a headache in this case series. Further studies are required for its application on various headache.
