Maxillary overdenture restoration using functionally generated path technique: a case report (Functionally generated path technique을 이용한 편악 피개의치 수복 증례)

  • The functionally generated path (FGP) technique, first described by Meyer in 1933, is a method in the oral cavity to use the movement pathway formed by the opposing cusps within the border movement of the mandible. Using this method, an appropriate occlusal shape can be given to the patient. In this case, the FGP technique was selected to provide a bilateral balanced occlusion when restoring the edentulous maxilla that opposes the natural mandibular teeth with irregular arrangement with overdentures. In addition, in order to precisely form the occlusal surface of the posterior region with the FGP technique and to reduce the attrition of denture teeth, zirconia denture teeth, not conventional resin artificial teeth, were individually manufactured. After treatment with these materials and methods, satisfactory results were obtained for both the operator and the patient.

National Oral Health Screening for Infants and Children: A Survey on its Perception, Requirements and Accuracy of Parents and Dentists (영유아 구강검진에 대한 부모와 치과의사의 인식과 요구도 및 정확도 조사)

  • This study aimed to investigate parents' and dentists' perceptions of the national oral health screening for infants and children (OHS), and evaluate the dentists' accuracy of analyzing the results of OHS. A questionnaire survey was conducted, face-to-face or online, for 90 parents of children who received OHS in the Department of Pediatric Dentistry at Yonsei University Dental Hospital and 100 dentists working at local clinic or university dental hospital from May to October 2021. Most parents and dentists were aware of the importance of OHS, and approximately 96.7% of parents were satisfied with OHS. The requirements of parents and dentists about OHS were different. The reasons for having difficulties in explaining parents after OHS and the opinions on appropriate period and number of OHS were disagreed between pediatric dentists and general dentists. Regardless of dentists' major, work experience, elapsed period after taking online education program, the accuracy of the examination result was low. In this study, various opinions of parents and dentists on OHS were collected. Efforts should be made to enhance the oral health of infants and children by considering the requirements of parents and dentists and improving the accuracy of examination results.

Relationship between usage of removable denture and depression in Korean adults with loss of multiple teeth: A cross-sectional study using the seventh Korea National Health and Nutrition Examination Survey (다수 치아를 상실한 한국 성인에서 가철성 의치의 장착 여부와 우울증의 연관성: 국민건강영양조사 제 7기 자료를 활용한 단면 연구)

  • Purpose: The purpose of this study is to evaluate the relationship between removable denture and depression. This study aims to investigate the relationship between depression and each group by dividing them into a group that were using removable dentures and a group that needed removable dentures but were not using removable dentures. Materials and methods: This study was conducted using raw data from the 7th Korea National Health and Nutrition Examination Survey (KNHNES). By using variables related to the state of the prosthesis and the need for prosthesis among oral examination data, the oral examination data were classified into two groups; group using removable denture and group in need of removable dentures. In addition, the variables of depression were classified into normal (0-4), mild depression (5-9), moderate depression (10-19), and severe depression (20-27) using the values of PHQ-9. For statistical analysis, logistic regression analysis was performed using SAS 9.4. Results: In the group in need of removable dentures, the risk of severe depression was statistically and significantly higher than the group using removable dentures. Conclusion: In patient who have lost multiple teeth, rehabilitation of the missing area with removable dental prosthesis is expected to decrease the occurrence of severe depression.

Effects of various root canal sealers on tooth discoloration and internal bleaching (근관치료용 실러가 치아변색과 실활치 미백에 미치는 영향)

  • There are several causes of tooth discoloration following root canal treatment. In this study, we evaluated the effects of sealers on tooth discoloration and internal bleaching. Twenty-four teeth were divided into 4 groups: control group, AH plus, Endosequece BC, and MTA fillapex group. Root canal filling was performed using each sealer conventionally and non-vital bleaching was performed with sodium perborate. The L, a, and b values were measured using Vita easyshade. Tooth discoloration after root canal treatment occurs irrespective of the type of sealers and may cause discoloration with only gutta-percha cone. The effect of non-vital bleaching following the use of calcium silicate-based sealers such as Endosequece BC and MTA fillapex was higher than that of AH plus. Therefore, it needs careful use of sealers in endodontics and calcium silicate-based sealers have advantages of bleaching in case of discolored tooth.

  • The CAD-CAM technique is a rapidly developing field in the dental field and is applied to various fields of prosthetic treatment. Among them, the manufacturing of dentures using the milling technique classified as subtractive manufacturing is one of the commercialized digital full denture manufacturing methods. At the same time, it is possible to more efficiently manufacture a metal framework for implant overdenture by selective laser sintering or melting technique classified as an additive manufacturing method. The purpose of this article is to describe the fabrication of CAD-CAM maxillary complete denture and mandibular implant overdenture as well as its features.

  • Panoramic radiography is widely used as a primary radiological diagnosis in dentistry. However, due to the overlapping of anatomical structures, diagnostic capability of panoramic radiography is limited. Recently, panoramic machine equipped with various image processing software has been introduced to improve the diagnostic capability. This study aims to introduce panoramic equipment with multi-layer imaging software that can provide 41 images from anterior to posterior in region of interest, and to report in various clinical applications.

  • Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an X-linked recessive disorder and is the most common enzyme deficiency worldwide. Although this disease is rare in Korea, its prevalence may increase due to an increase of multicultural families. Patients with this deficiency are prone to hemolytic anemia provoked by specific drugs commonly used or prescribed in pediatric dentistry. It is necessary for pediatric dentists to have accurate knowledge of a patient's medical history and to consult with a pediatric hematologist to avoid oxidative stress that can lead to hemolysis. The most effective treatment is prevention of exposure to factors that may trigger anemia. Appropriate education regarding risk factors is essential for patients diagnosed with this disease. This case report aimed to discuss the drugs and dental management methods that should be cautious in children with G6PD deficiency.

  • Jaw motion tracking, which is introduced in recent case reports, is a method which records the patient's individualized pathway of the mandibular movements along with facebow transfer, and reproduces the information in the virtual space of computer-aided-design/computer-aided-manufacturing (CAD-CAM) software. In this present case, a collapse of the occlusal plane was observed, due the loss of posterior teeth for a long period. Full-mouth rehabilitation with an increase in the occlusal vertical dimension was planned. First, the patient's mandibular movements were recorded on the newly established jaw relation by jaw tracking, and this information was assembled with the patient's intraoral data to create a virtual patient. Implant planning and diagnostic wax-up was done on the virtual patient, leading the fabrication of the provisional prosthesis. On the newly established jaw relation with an increase in the occlusal vertical dimension, canine guidance of the provisional prosthesis was checked. Finally, the provisional prosthesis was carried out to the definitive prosthesis. Using the advantages of the technologies in the digital dentistry, the patient was satisfied with the function and the esthetics after the treatment.

  • In order to manufacture functional and esthetic prostheses, it is essential to accurately transmit information about the patient's occlusal plane. In particular, in the case of a completely edentulous patient, the occlusal plane is very important to correctly support the soft tissue, to achieve aesthetic harmony with the facial appearance, and to properly pronounce it, and to form a balanced occlusal relationship for stable mastication. In the conventional method, various facebow systems were used to transmit patient's information from the clinic to the laboratory, but there were several limitations in the process of transferring them to CAD. To simplify this process, a prefabricated POP (PNUD Occlusal Plane) Bow system was recently developed. In this case, a CAD-CAM (Computer-aided design-computer-aided manufacturing) treatment dentures reflecting the patient's occlusal plane information was manufactured using the POP Bow system during the treatment of a completely edentulous patient, and aesthetic and functional satisfaction was obtained.
