The Long-Term Stability of the Lower Incisor Axis in Class II division 2 Malocclusions (제II급 2류 부정교합에서 하악 절치 치축의 장기적인 안정성에 관한 연구)

  • The purpose of this study was to evaluate the post-retention stability of the lower incisor axis in Class II division 2 malocclusions. The dental casts and lateral cephalograms from before (T1) and after (T2) orthodontic treatment and long-term post-retention (T3) in 62 Class II division 2 malocclusion cases were included in this study. After several linear and angular measurements at each time were taken, the significance in the amount of change of the lower incisor axis for each gender and extraction versus non-extraction was evaluated. The results showed that the lower incisors that inclined labially during treatment were unstable and relapsed to the original lingual position in Class II division 2 malocclusions (p<0.001). There was no significant difference between extraction and non-extraction groups for the amount of lingual relapse of the lower incisors (p>0.05). There was no significant difference between male and female groups for the axial change of the lower incisors (p>0.05). As a result of multiple regression analysis, the cephalometric measurement best predicting the lower incisor position to the A-Pog line post-retention was pre-treatment L1-Apog(mm) and pre-treatment SNGoMe$(^{\circ})$. Because of the instability of labially inclined lower incisors after orthodontic treatment, the treatment goal should be the pre-treatment incisor axial position.

  • Supernumerary teeth are defined as an excess in the number of teeth when compared to the normal dental formula. They are more prevalent in the permanent dentition than the primary dentition. Supernumerary teeth can occur in the maxilla, mandible, or both. But the majority are found in the maxilla and most of it is found in the premaxilla region The present cases documents about the uncommon cases of supernumarary teeth on maxillarty premolar area and mandibular incisal area.

Relationship between mesiodistal width and enamel thickness in mandibular incisors (하악 절치 근원심폭경과 법랑질 두께의 관계)

  • Objective: The purpose of this study was to investigate the relationship between the enamel thickness of proximal surfaces and the morphologic features of mandibular incisors. Methods: Mesiodistal/faciolingual (MD/FL) index, MD width, and height of contour width/cervical width ratio were measured in 40 incisors extracted from Koreans. For determining the height of contour width/cervical width ratio, the cervical width was measured as the distance between proximal cementoenamel junctions. Then, the labial surface was ground to the height of the contour level to measure enamel thickness. Pearson correlation analysis was used to investigate the correlation between enamel thickness and morphologic features. Results: Enamel thickness was $0.75{\pm}0.07mm$ per side, and MD width was $5.56{\pm}0.40mm$. Enamel thickness and MD width were significantly correlated. However, a significant relationship was not observed between enamel thickness and MD/FL index or the height of contour width/cervical width ratio. Conclusions: The results suggest that enamel thickness is affected only by MD width. Therefore, if the MD width is the same for mandibular incisors with a large MD/FL index or triangular shape and mandibular incisors with normal shape, then the limit of enamel reduction for reproximation will be the same.

Postretention stability after mandibular incisor extractions assessed by cast and radiograph - The American Board of Orthodontics cast and radiographic evaluation (모형과 방사선 사진을 이용한 하악 전치 발치 치료의 안정성 평가 - 미국교정학회에서 제시한 방법)

  • Purpose: The purpose of this study was to evaluate the improvement and posttreatment stability of patients treated with extraction of lower incisors. Materials and Methods: The total of 20 patients with extracted lower incisors were analyzed by means of diagnostic models and panoramic x-rays at the time of initial, final and after 2 years of retention period of the treatment. Irregularity index, overjet, overbite, tooth size-arch length discrepancy (TSALD), intercanine width, intermolar width and American Board of Orthodontics cast/radiographic evaluation (ABO-CRE) were analyzed. Statistical analysis was performed using Wilcoxon signed-rank test. Results: After treatment, irregularity index showed significant decrease (P = 0.000). TSALD showed significant increase (P = 0.028). During retention period, irregularity index showed significant increase (P = 0.001). For ABO-CRE, total score showed significant decrease after treatment (P = 0.000) and showed average decreased which was not significant result (P = 0.053). Conclusion: Through evaluation of stability of extraction of lower incisors by means of diagnostic models and panoramic x-rays, it can be concluded that lower incisor extraction treatment had been stable for 2 years after treatment.

Clinical Implications of the Premature Loss of the Mandibular Primary Canine (하악 유견치 조기탈락은 무엇을 의미할까?)

  • The premature loss of the mandibular primary canine is relatively frequent and a sign of the upcoming space problems in the transitional period. This situation is caused by the permanent lateral incisor resorbing the root of the primary canine during its eruption. Bilateral loss of primary canines in a crowded arch leads to the lingual tipping of the permanent incisors, with the consequent reduction in the arch perimeter and increase in overbite. When the loss of a primary canine is unilateral, tipping of the adjacent incisors occurs toward the space, resulting in midline deviation. In these situations, treatment possibilities, such as extraction of the antimeric tooth or placement of a passive lingual arch, can be applied; although there are some controversies concerning this. Most space problems with less than 4 mm can be resolved through preservation of the leeway space using sequential disking of the primary teeth and a passive lingual arch, regaining space or limited arch expansion in the late mixed dentition. In cases with 4~6 mm of space problems, arch expansion (A-P or transverse) can be applied. Space problems with more than 6 mm should be treated through diagnosis and treatment planning. Most of these cases will require extraction of permanent teeth including serial extraction (guidance of eruption).

Estimated Time of Biomineralization in Developing Rat Incisors (발생 중인 쥐 절치의 생물학적 광화 소요 시간)

  • The aim of this study was to estimate time of biomineralization in developmental stages of rat lower incisors. Eruption length was measured. Four stages of incisor development were identified on histologic and microscopic computerized tomography (micro-CT) sections: (1) preodontoblast, (2) dentin matrix secretion, (3) enamel matrix secretion, and (4) enamel calcification. The overall eruption rate of the rat lower incisor was $600{\pm}70{\mu}m/day$ ($mean{\pm}SD$; n = 12). The length of the enamel secretion was $4.59{\pm}0.75mm$ in histologic section, was $3.64{\pm}0.63mm$ in radiographic section, which converts to $180.4{\pm}30.0hours$, $145{\pm}25hours$ respectively (n = 24). These findings suggested that the four biomineralizing developmental stages of the rat incisor took only several days. The significance of this animal study was to provide understanding for the rapid biomineralization process of developing rat tooth germ by analysis of tooth forming period.

DENTAL COMPLICATIONS AND MANAGEMENT OF A PATIENT WITH NEUROBLASTOMA : SPLINT OF MANDIBULAR INCISORS WITH ROOT ANOMALIES USING MINI-SCREW (신경모세포종의 의학적 치료에 따른 치과적 합병증과 관리 : Mini-screw를 이용한 치근이형성 하악 절치의 고정술)

  • Neuroblastoma is a common malignant tumor of the sympathetic nervous system in childhood, arises from embryonic neural crest cells. The period of tooth development is matched with peak times of diagnosis and treatment of neuroblastoma. The intensive multimodality treatment including radiotherapy and chemotherapy is used in patients with neuroblastoma has been shown to have late adverse effects and disturbances in dental development like tooth agenesis, microdontia, enamel hypoplasia and short roots. A 8-year old girl had been on medication and radiotherapy for neuroblastoma since she was 15 months old at Department Pediatrics, Chonnam National University Hospital. Oligodontia, microdontia, and short root formation was notable in clinical and radiological examination. Mobility of lower permanent incisor was detected and measured at about degree 2. Resin wire splint using mini-screw implantation on buccal alveolar bone was conducted for maintenance of mandibular incisors and alveolar bone. Excessive mobility has been eliminated and maintained well so far. Further treatment is planned for re-evaluation of mobility, preventing dental caries and regular oral hygiene management. Although we need further evaluation, this treatment could be one of alternative therapy for those who have similar dental anomalies.

Jaw movement analysis in subjects with implant-supported prosthesis (임플랜트 보철 환자의 하악 운동에 관한 연구)

  • 임플랜트 보철 환자의 하악 운동에 관한 연구를 위해 완전 고정성 보철 치료를 한 8명의 환자와 상하악 총의치를 장착한 8명의 환자, 그리고 5명의 자연치아 피검자를 대상으로 Sirognathograph(Siemens, Benshiem, Germany)와 BioPAK program(BioResearch, Milwaukee, U.S.A.)으로 한계운동과 당근시편에 대한 편측 저작운동에 대한 검사를 시행하였다. 편측 저작 운동은 전두면에서의 저작운동의 높이와 폭, 전두면에서의 저작각도, 시상면에서의 절치기준점의 최대 이동거리, 시상면에서의 저작운동의 전후방 이동거리, 최대개구속도, 최대폐구속도의 기준점으로 각 저작주기의 성적을 평균처리 하여 다음과 같은 결론을 얻었다. 1. 전두면에서의 저작운동의 높이는 자연치, 총의치, 임플랜트 군간에 유의성있는 차이가 없었다(p>0.05). 2. 전두면에서의 저작운동의 폭은 임플랜트 군이 자연치, 총의치 군에 비해 유의성있게 작은 수치를 보였다(p<0.05). 3. 전두면에서의 저작각도 분석시 임플랜트 군과 자연치 군, 임플랜트 군과 총의치 군간에 유의성있는 차이가 없었다(p>0.05). 4. 시상면에서의 절치기준점의 최대 이동거리 분석시 임플랜트 군과 자연치 군, 임플랜트 군과 총의치군간에 유의성 있는 차이가 없었다(p>0.05). 5. 시상면에서의 전후방이동거리는 자연치 군이 임플랜트 군, 총의치 군에 비해 유의성 있게 크게 나타났다(p<0.05). 6. 최대개구속도는 자연치 군이 총의치 군과 임플랜트 군에 비해 유의성 있게 작은 수치를 보였다(p<0.05). 7. 최대폐구속도는 각 군간에 유의성 있는 차이가 없었다(p>0.05). 8. 저작운동은 개인간의 차이가 커서 명확한 결론을 유추하기가 어려웠다.

Comparative Tooth Morphology and Qualitative Analysis on the Enamel Surface using Energy Dispersive X-ray Spectroscopy in the White-Toothed Shrew Crocidura lasiura and C. suaveolens and the Red-Toothed Shrew Sorex caecutiens from Korea (한국 땃쥐속 우수리땃쥐 Crocidura lasiura와 작은땃쥐 C. suaveolens, 뒤쥐속 뒤쥐 Sorex caecutiens의 치아형태와 Energy Dispersive X-ray Spectroscopy를 이용한 법랑질 표면의 정성적 분석 비교)

  • The tooth morphology and qualitative mineral contents on enamel surface using energy dispersive X-ray spectroscopy, (EDX) were examined in the white-toothed shrew (genus Crocidura ) Crocidura lasiura and C. suaveolens and the red-toothed shrew (genus Sorex) Sorex caecutiens. In the case of C. lasiura and C. suaveolens, dental formula was found I 3/1 C1/1 P1/1 M3/3=28. The upper 1st and 2nd molars had an unequal W-shape formed by 5 cusps on the crown. The 3rd molar was found one-third the size of those of 1st and 2nd molars. The upper 1st incisor had two different sized hook-shapes and the lower 1st incisor was even. In the case of S. caecutiens, dental formula was found to be I3/1 C1/1 P3/1 M3/3=32. The upper 1st and 2nd molars had an equal W-shape on crown. The upper 3rd molar was half the size of those of the other molars. The upper 1st incisor possessed two similar sized hook-shapes and the lower 1st incisor had an uneven and serrated form. A comparison with the dental and cranial measurements revealed C. lasiura to be the largest of the three species (p<0.001) and C. suaveolens and S. caecutiens were similar in size (p>0.05). A qualitative analysis of mineral contents on enamel surface of the lower 1st incisor and lower 1st molar using EDX revealed C, O, P, Ca and Cu in all specimens and Pb was detected in several enamel specimens. No significant differences in the mineral contents (% weight) were observed among the three species (p>0.05). Fe was only detected on enamel surface of S. caecutiens with red pigmented teeth. Therefore, Fe is responsible for the red tip of the teeth. These results suggest that tooth morphological characteristics including the color of the tooth tip might be used as the key classifying species belonging to Crocidura and Sorex.

  • Extracting mandibular incisors for orthodontic treatment may adversely affect the occlusion. However, when properly used, extraction of mandibular inciors is a selection for the correction of the malocclusion. Generally, treatment for crowding needs to select between nonextraction and four premolar extraction. Approaches for crowded mandibular incisors include distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Extraction of incisors is used in case of crowding, anterior tooth size discrepancy, absent of maxillary lateral incisors, and ectopic eruption. But severe overjet. overbite, and space are the contraindication of it. A patient had severe crowding on upper anterior teeth, impacted upper left lateral incisor, palatal ectopic eruption of upper right incisor and severe crowding on lower anterior teeth. Lower lateral incisors are extracted for space availability and facial esthetics. We report the case of orthodontic treatment of upper and lower anterior crowding through extraction of lateral incisor.
