The prevalence of temporomandibular disorders in $16\~18$ aged subjects at Yongin, Kyungkido, Korea (경기도 용인지역 $16\~18$세 연령자의 측두하악장애 유병율에 관한 연구)

  • An epidemiological investigation was carried out at Yongin, Kyungkido, South Korea to determine the prevalence of symptoms of temporomandibular disorders and parafunctional habits in adolescent aged 16, 17 and 18 years. 2,098 students(male 507, female 1,591) were randomly selected and investigated with the questionnaire. The results were obtained as follows, 1. The prevalence of symptoms of temporomandibular disorders was $80.9\%$ total, $77.3\%$ for male and $82.0\%$ for female with significance between male and female(P<0.05). 2. The prevalence of symptoms of temporomandibular disorders was gradually increased according to increasing age with significant difference among ages(P<0.001) 3. The most frequent symptom in the temporomandibular disorders is pain on chewing($65.9\%$), and then clicking($50.7\%$), Pain on mouth opening($41.8\%$), and pain on TMJ($36.8\%$) 4. There was little corelationship between clenching habits and symptoms of temporomandibular disorders(r=0.166), and also between bruxing habits and those(r=0.057).

The Effectiveness of Age Estimation Method by Occlusal Tooth Wear in Temporomandibular Disorder(TMD) Patients (측두하악장애 환자에서 치아 교모도에 의한 연령감정의 유효성)

  • The purpose of this study was to estimate occlusal tooth wear and bruxism severity in TMD patients, and evaluate the effectiveness of the present age estimation method by occlusal tooth wear in TMD patients. Takei's age estimation method was applied to 163 subjects(56 controls, 107 TMD patients). The author analyzed the degree of occlusal tooth wear from the difference between estimated age and actual age. The obtained results were as follows: 1. In all age group, occlusal tooth wear in TMD patients is higher than those in asymptomatic controls. In 20's age group, a statistically significant difference was found. 2. In both gender, occlusal tooth wear in TMD patients is higher than those in asymptomatic control. In male, a significant difference was found. 3. Occlusal tooth wear in TMD patients with bruxism is significantly higher than that in control. 4. We found no significant difference in bruxism severity and occlusal tooth wear among the RDC/TMD subgroups. 5. Application of Takei's method would be still useful for age estimation in Korean. In case of age estimation of TMD patients using occlusal tooth wear, evaluation of the bruxism severity and appropriate correction according to age, sex and difference of geographical location should be considered.

MECHANO THERAPY OF PEDIATRIC CONDYLAR FRACTURES USING BENOIST'S APPLIANCCE : A CASE REPORT (Benoist씨 장치를 이용한 소아의 하악 과두 골절의 치험례)

  • Mandibular fracture is less common in children than in adults. However, children are more susceptible to ankylosis and developmental disorders, and don't respond as well to intermaxillary fixation compared to adults. On the other hand, bone fracture is healed more quickly in children and complications are scarce. Mandibular fracture in children is usually treated successfully with acrylic splint therapy with or without the use of eyelet wires and intermaxillary fixation. Severe complications that include ankylosis and developmental disorders may occur. The frequency and severity of such complications can be mitigated with a shorter duration of intermaxillary fixation and good post-operative care. Encouraging mandibular physical therapy by increasing patient motivation may be necessary in such cases where the patient's response is poor and the duration of intermaxillary fixation increases; when the patient is unable to undergo physical therapy, or when intermaxillary fixation is not necessary with the patient showing only minor symptoms such as trismus. In this case report, a 6 year-old girl with bilateral condylar fracture was treated with elastic in both the upper and lower jaws to allow mandibular physical therapy using a Benoist's appliance, which allows opening, lateral, and protrusive retrusive movements of the mandible. A 7-month follow-up showed beneficial therapeutic effects such as increased mandibular movement and prevention of condylar ankylosis.

ERUPTION PATTERN OF THE MANDIBULAR FIRST MOLAR USING THE CONE BEAM CT (Cone Beam CT를 이용한 하악 제 1대구치 맹출 양상에 관한 연구)

  • The purpose of this study was to investigate the eruption pattern of the mandibular first molar in sagittal, frontal and horizontal views using the cone beam CT scanning. CT images were obtained from healthy 83 children (42 boys, 41 girls) between 3 to 10 years of age with a normal dentition according to Nolla stage. 1. In the frontal and horizontal view, the intermolar width decreased continuously with stage and slightly increased at the last stage. 2. In the sagittal and frontal view, eruption distances from occlusal plane were observed the largest change between stage 5 and 7. 3. In the horizontal and sagittal view, mandibular first molar from distal surface of primary second molar moved distally between stage 4 and 6. 4. In the sagittal view, angle from occlusal plane to mesio-distal axis increased between stage 4 and 8. 5. In the frontal view, angle from occlusal plane to bucco-lingual axis increased continuously during all stage. 6. In the horizontal view, angle from midsagittal plane to long axis increased between stage 5 and 8.

Clinical Assessment of Patients with Mandibular Condyle hypoplasia (하악 과두저형성증 환자의 임상적 평가)

  • Objective : Condyle hypoplasia in temporomandibular joint(TMJ) is often observed in several radiographic views. Mandibular Condyle hypoplasia is frequently confused with osteoarthritis with bony changes in TMJ. This paper investigated clinical characteristics of mandibular condyle hypoplasia as compared with TMJ osteoarthritis. Material and method : 276 patients with TMD were taken clinical and radiological examination and were divided into study group, 189 patients diagnosed with mandibular condyle hypoplasia, and control group, 87 patients diagnosed with TMJ osteoarthritis. And clinical features(Onset, Overjet, Overbite, Noise, Locking, NAS of noise, LOM, pain, MCO, and site of diagnosis and pain)of the two groups were compared. Results : 1. Mandibular condyle hypoplasia and TMJ osteoarthritis were similar in many of the clinical features. 2. Mandibular condyle hypoplasia concordance rates of the radiographic diagnosis site and the pain site was significantly lower than TMJ osteoarthritis. 3. Bilateral mandibular condyle hypoplasia group had more occlusal discomfort, and clenching habits than unilateral mandibular condyle hypoplasia group. 4. Unilateral mandibular condyle hypoplasia group had more unilateral chewing habits and LOM than unilateral TMJ osteoarthritis group. Unilateral TMJ osteoarthritis group had more morning stiffness and higher concordance rates of the radiographic diagnosis site and the click sound site than unilateral mandibular condyle hypoplasia group. 5. Bilateral mandibular condyle hypoplasia group had more usual headaches and overjet than bilateral TMJ osteoarthritis group. Conclusion : Mandibular condyle hypoplasia has somewhat distinguishing clinical characteristics as compared with TMJ osteoarthritis.

EFFECT OF PULSING ELECTROMAGNETIC FIELDS COMBINED WITH ANTERIOR MANDIBULAR DISPLACEMENT ON CONDYLAR GROWTH IN THE RAT (맥동 전자기장과 하악골 전방이동이 백서의 하악과두 성장에 미치는 영향에 관한 실험적 연구)

  • 전기적 자극에 의한 골성장기전의 개념을 이용하여 임상적 효율성을 증진시키기 위한 연구는 현재 교정학을 비롯한 치과영역에서 활발히 진행되고 있는 분야 중의 하나이다. 전기적 자극의 여러 형태 중의 하나인 전자기장과 하악의 기능적 전방 이동을 유도하는 악기능교정장치가 백서의 하악과두 성장에 미치는 영향을 구명하기 위하여 본 연구를 시행하였다. 생후 4주된 Sprague Dawley계 백서 48마리를 대조군 12마리, 실험군 36마리로 나누고, 실험군은 다시 전자기장을 가한 군, 하악골 전방이동 장치를 장착시킨 군, 전자기장과 하악골 전방이동 장치를 병용시킨 군으로 분류하여 각각 12마리씩 실험동물을 배정하였다. 각 군의 실험동물은 15 HZ의 특수 전자기장이나 하악전방이동 자치가 하루10시간씩 작용되도록 특별히 제작한 실험장치 속에 넣어 1주간, 4주간씩 사육하여 희생시킨 후 하악골을 분리하고 연조직을 박리한 후 $10\%$ formalin에 보관하였다. 하악골 길이를 측정하기 위해 0.05mm까지 계측 가능한 캘리퍼를 이용하여 하악과두의 후연에서 이공까지의 거리를 계측하였고, 하악과두를 절제하여 0.5M EDTA에 탈회시켜 파라핀 포매를 하였다. 표본의 절단방향은 시상평면에 평행하게 하여 $6{\mu}m$두께로 연속절단 하였으며, 그 중 중심의 3절편을 취하여 통법에 의한 H-E 중염색을 시행하였다. 하악골 계측과 H-E 중염색 표본을 통한 조직학적 관찰을 통해 다음과 같은 결론을 얻었다. 1. 4주군에서 전자기장만에 노출된 실험군은 대조군에 비해 하악골 길이가 유익성 있게 증가되었다. 2. 전자기장과 하악골 전방이동 장치를 병용한 실험군은 하악골 전방이동장치만을 사용한 실험군에 비하여 하악골 길이가 증가되었다. 3. 전자기장에 노출된 실험군은 전구 연골아세포(prechondroblast)의 증식, 비대연골 세포층의 세포간질 및 연골내 골화층의 석회화가 모두 증가되었다. 4. 본 실험에 사용한 15 HZ전자기장의 주요작용부위는 백서의 하악과두 성장지역 중 연골내 골화의 석회화 지역이며, 또한 이는 하악골 전방이동 장치와 병용시 하악과두 성장을 촉진시킬 수 있음이 관찰되었다.

A comparative study of initial lateral cephalometric characteristics: mandibular setback surgery only versus mandibular setback surgery with advancement genioplasty (하악 후퇴술과 전진 이부성형술이 시행된 III급 부정교합자의 초진 시 측모 두부 방사선사진 특성에 관한 연구)

  • Objective: The aim of this study was to compare the initial lateral cephalometric characteristics in two groups of patients: those that had mandibular setback surgery only and those that had mandibular setback surgery with advancement genioplasty. Methods: The lateral cephalograms of thirty-one patients were studied. Twenty-one Class III patients (group A) had only madibular setback surgery Twelve Class III patients (group B) had mandibular setback surgery with advancement genioplasty. Results: Differences between two groups were found in N-Me, ANS-Me, Occlusal Plane angle, Palatal Plane to U1, Mandibular Plane to L1, Mandibular Plane to L6, SN to U1, Sn-Stms, and Pog' projection. Compared to group A, group B showed more linguoversion and extrusion of upper incisors, more extrusion of lower incisors and lower first molar, and more steepness of the occlusal plane. N-Me, ANS-Me, and Sn-Stms were also longer in group B. But Pog' projection was shorter than group A. Conclusion: We conclude that certain initial lateral cephalometric characteristics may help indicate the inclusion of advancement genioplasty when mandibular setback surgery is planned in skeletal Class III patients.

ANALYSIS OF FACIAL SOFT-TISSUE CHANGES AFTER MANDIBULAR SAGITTAL SPLIT RAMUS OSTEOTOMY (하악 전돌증 환자의 하악지 시상 골절단술후 경조직 변화에 따른 안모 연조직 변화 분석)

  • This study was intended to perform cephalometric analysis of the facial soft tissue profile after surgical correction of skeletal Class III malocclusion after SSRO in 29 patients (Males 12, females 17). Lateral cephalograms were taken in centric occlusion before and immediate, long term after surgeries. 1. Counter-clockwise rotation of mandible was observed after the surgery, average relapses of mandibular set back were 1.23-1.28mm. The net effects of the mandibular set-back after surgeries were 81.7-82.2%. Because these relapse tendencies may reduce the effects of the surgical outcomes, surgeon must consider these net before the surgical treatment planning. 2. The ratio of horizontal changes of hard tissue to soft tissue at lower lip, mentolabial sulcus, pogonion were 72.7-93.7%, 100.3%, 99.1-102.1% respectively. There were little changes at upper lip position anteroposteriorly. 3. The relationship of upper and lower lips were improved after surgery. Lower lip was posteriorly repostioned and upper lip was flattend and elongated in conjunction with deepening of inferior lobial sulcus. But profile of chin was still prominent after surgery. 4. Hard tissue horizontal changes and tissue vertical changes were significantly correlated with each other and there were reverse correlations with hard tissue vertical changes and soft tissue horizontal changes.
