  • 저자들은 1994년 1월부터 1996년 8월까지 과대비성(hypernasality)을 주소로 영동세브란스병원 이비인후과를 내원하여 상저부 인두피판(superior based pharyngeal flap) 혹은 후인두벽 증대(posterior pharyngeal wall augmentation) 시행받은 환자 20명을 대상으로 수술 전후의 치료결과를 판정하였다. 수술 전후의 치료결과는 2명의 이비인후과의사와 1명의 언어치료사가 구강을 통한 내시경 적검사와 음성평가를 동시에 시행함으로써 판정하였다. (중략)

Trough방법에 의한 광범위 기관 협착의 치료

  • 일반적으로 기관협착의 치료는 기관절재후 단단문합술로 가능하나 광범위한 기관협착이 발생하였을 때는 그 치료가 매우 어렵다. 이경우는 기도전벽을 개방하여 경부 피부와 기관연을 봉합하는 Trough를 형성한 뒤 이차적으로 연골이식이나 경부 피부를 이용한 피판을 만들어서 기관을 재건하는 Trough방법이 주로 적용되고 있다. 이 때 이식되는 연골은 늑연골 이나 비중격 연골이며 측벽의 재건에는 늑연골이 좋고 전벽의 재건에는 점막이 붙어 있는 비중격 연골의 복합이식편이 사용될 수 있다. 본 교실에서는 기관 절제 후 단단문합술이 불가능하였던 5명의 환자에서 Trough방법을 이용하여 기관 재건에 성공하였기에 그 결과를 보고하고자 한다.

  • 후두 또는 하인두 암종에 대한 광범위 절제 후 효율적인 호흡을 위하여 영구기공(permanant tracheostome)을 만들어 주게 되는데, 여러 가지 원인에 의하여 기공 협착이 발생하면 가피형성, 혈담, 호흡곤란 등을 초래하게 된다. 현재까지 기공협착의 치료는 Z형 성형술, 국소 스테로이드요법, 스텐트 삽입법 등 다양하게 이용되고 있으나 만족할 만한 결과를 얻는데 한계가 많았다. 이러한 기공협착의 치료에 도움을 얻고자, 저자들이 경험한 12례의 기공협착 환자에 대하여 기관전진점막피판(tracheal advancement flap)을 이용하여 성공적으로 치료하였기에 요약 보고하고자 한다.

  • Reconstruction techniques of orofacial defects caused by wide excision of the intraoral malignant lesions are various. Although radial forearm free flap is a common donor site on reconstruction of soft tissue defect, anterolateral thigh (ALT) free flap also has an established site in orofacial soft tissue reconstruction as the favored donor flap with recent progress of the microsurgical technique. A 59-year-old female complained of hyperplastic mass on the right retromolar and buccal cheek, which was diagnosed as a squamous cell carcinoma (SCC) by an incisional biopsy. Before the operation, we planned a wide excision of the SCC lesion, supraomohyoid neck dissection, reconstruction with radial forearm free flap (RFFF), and split thickness skin graft. We accidentally found an arterial variation of the forearm area during elevation of RFFF, and changed the plan of reconstruction operation to reconstruction with ALT free flap. Operative sites was healed well during the post-operative period, and we referred to the department of radiation oncology for post-operative radiotherapy.

  • advancement in the treatment of esophageal perforation due to the development of ICU care and antibiotics. There were controversies in the treatment of esophageal perforation when diagnosed after 24hrs. From 1995 to 2000, we performed a buttressed primary repair and mediastinal drainage in 6 Boerhaave\`s syndrome patients among 13 esophageal perforation patients. Two patients died(33%). They died because of pneumonia, ARDS and sepsis on 38th, 39th post-operative day respectively. Two patients had leak at the site of repair which was treated completely with conservative treatment. We report on the result of a buttressed primary repair and mediastinal drainage for 6 Boerhavve\`s syndrome patients.

  • Microvascular tissue transfers have facilitated primary closure of various complex defects after radical ablation of head and neck cancers. From Oct 1991 to Feb 1992, we used forearm free flap in two patients and delto-pectoral flap in one patient who had preoperative irradiation for pharyngoesophageal reconstruction. The stricture and fistular formation were most troublesome complication in forearm free flap, so we designed as lazy S shape in distal flap margin to prevent circular contraction and longitudinal margin was deepithelized(5mm) and sutured double layer to withstand fistular formation and this can be considered useful in place of a free jejunal transfer.

  • The use of the autogenous free fat is a well-known procedure to fill in superficial depressions resulting from the traumatic or congenital defects. The major donor site for this procedure was the abdominal subcutaneous fat or buttocks. In 1977, Egyedi was the first to report the use of the buccal fat pad as a pedicled graft. The buccal fat pad is a structure usually considered to be a nuisance when encountered in intraoral procedures such as facial bone osteotomies, elevation of buccal falp, or procedures on Stensen's duct. In these operations, appearance of the buccal fat pad complicates surgical exposure. The buccal fat pad is a lobulated convex mass of fatty tissue covered by a very delicate membrane, and is described as having a body from which four processes extend. These projection serve as a filling material between the various muscular structures in the area. Recently malar depression was augmented with the pedicled buccal fat pad in 3 cases, and it was used for the reconstruction of the nasolabial fold in one case.

  • Background and Objectives: The goals of laryngeal reconstruction have been prevention of aspiration, production of a functional voice, and maintenance of an adequate airway for decannulation. It is generally believed that the reconstruction of the glottic region after vertical partial laryngectomy (VPL) can improve laryngeal function. The objective of this study is to evaluate of voice function after VPL with mucosal flap and fat graft reconstruction. Materials and Methods: From 1994 to 2006, 13 patients, who had been treated with VPL with mucosal flap and fat graft reconstruction. The voice characteristics, acoustic, aerodynamic parameter were measured in 13 patients after vertical partial laryngectomy with mucosal flap and fat graft reconstruction. Acoustic analysis was carried out using Computerized Speech Lab (CSL) and aerodynamic analysis were carried out using Aerophon II,3 months and 12 months after surgery. Results: The GRBAS scale, jitter, shimmer, NHR were improved as time goes on after surgery. But, maximum phonation time was shortened after surgery and there is no significant differences between before and after surgery in mean flow rate. Conclusion: The voice function of the mucosal flap and fat graft reconstruction after VPL were satisfactory. This can be an excellent reconstruction method after vertical partial laryngectomy.

  • A flathead fish, collected from the water off Cheju lsland is tentatively identified as Inegocia guttata of the family Platycephalidae for the first time from Korea. Inegocia guttata is similar to Cymbacephalus beauforti in morphological characters, but differs from C. beauforti in lacking dermal flaps on eyes and having pored scales much fewer than oblique scales slanting backward and downward above the lateral line, single and tongue - like interopecular flap, nasal spines and circled blackish spots in the pectoral and ventral fins. A new Korean name, "Akoyangtae", is proposed for the I. guttata.
